IEEE 1149.1의 실시간 신호 시험 구조 설계 (Design of Run-time signal test architecture in IEEE 1149.1)

  • 보드에 장착된 소자들을 테스트하기위해 제안된 IEEE 1149.1 시험 구조는 입력으로 TDI 핀을 사용하고 출력으로 TDO 핀을 사용하는 커다란 직렬 쉬프트 레지스터이다. IEEE 1149.1은 보드 수준에서의 테스트는 완벽하게 수행하지만 보드가 시스템에 장착되고 난 후의 수행 중인 시스템 수준에서의 실시간 동작클럭 속도로의 테스트에는 문제가 있다. 즉시험대상 핀의 실시간 동작신호를 시험하기 위하여 직렬 시프트 레지스트 체인들의 출력속도를 동작 클럭의 쉬프트레지스터 배수 이상의 속도로 작동 하여야 한다. 본 논문에서는 시스템 클럭과 동일한 속도로 실시간 신호를 캡쳐하기 위한 실시간 신호 시험 구조를 설계하고 시험 절차를 제안하였다. 제안한 실시간 신호 시험 구조를 Altera의 Max+Plus 10.0을 사용하여 제안한 시험 절차에 따라 시뮬레이션을 수행하였으며, 이를 통해 제안한 시험구조가 정확히 동작함을 확인하였다.

Overexpression of Long Non-Coding RNA MIR22HG Represses Proliferation and Enhances Apoptosis via miR-629-5p/TET3 Axis in Osteosarcoma Cells

  • In this study, we evaluated the mechanism of long non-coding RNA MIR22 host gene (LncRNA MIR22HG) in osteosarcoma cells. Forty-eight paired osteosarcoma and adjacent tissues samples were collected and the bioinformatic analyses were performed. Target genes and potential binding sites of MIR22HG, microRNA (miR)-629-5p and tet methylcytosine dioxygenase 3 (TET3) were predicted by Starbase and TargetScan V7.2 and confirmed by dual-luciferase reporter assay. Cell Counting Kit-8, colony formation and flow cytometry assays were utilized to determine the viability, proliferation and apoptosis of transfected osteosarcoma cells. Pearson's analysis was introduced for the correlation analysis between MIR22HG and miR-629-5p in osteosarcoma tissue. Relative expressions of MIR22HG, miR-629-5p and TET3 were measured by quantitative real-time polymerase chain reaction or Western blot. MiR-629-5p could competitively bind with and was negatively correlated with MIR22HG, the latter of which was evidenced by the high expression of miR-629-5p and low expression of MIR22HG in osteosarcoma tissues. Overexpressed MIR22HG repressed the viability and proliferation but enhanced apoptosis of osteosarcoma cells, which was reversed by miR-629-5p upregulation. TET3 was the target gene of miR-629-5p, and the promotive effects of upregulated miR-629-5p on the viability and proliferation as well as its repressive effect on apoptosis were abrogated via overexpressed TET3. To sum up, overexpressed MIR22HG inhibits the viability and proliferation of osteosarcoma cells, which was achieved via regulation of the miR-629-5p/TET3 axis.

Radiation Dose from Computed Tomography Scans for Korean Pediatric and Adult Patients

  • Background: In recent events of the coronavirus disease 2019 (COVID-19) pandemic, computed tomography (CT) scans are being globally used as a complement to the reverse-transcription polymerase chain reaction (RT-PCR) tests. It will be important to be aware of major organ dose levels, which are more relevant quantity to derive potential long-term adverse effect, for Korean pediatric and adult patients undergoing CT for COVID-19. Materials and Methods: We calculated organ dose conversion coefficients for Korean pediatric and adult CT patients directly from Korean pediatric and adult computational phantoms combined with Monte Carlo radiation transport techniques. We then estimated major organ doses delivered to the Korean child and adult patients undergoing CT for COVID-19 combining the dose conversion coefficients and the international survey data. We also compared our Korean dose conversion coefficients with those from Caucasian reference pediatric and adult phantoms. Results and Discussion: Based on the dose conversion coefficients we established in this study and the international survey data of COVID-19-related CT scans, we found that Korean 7-year-old child and adult males may receive about 4-32 mGy and 3-21 mGy of lung dose, respectively. We learned that the lung dose conversion coefficient for the Korean child phantom was up to 1.5-fold greater than that for the Korean adult phantom. We also found no substantial difference in dose conversion coefficients between Korean and Caucasian phantoms. Conclusion: We estimated radiation dose delivered to the Korean child and adult phantoms undergoing COVID-19-related CT examinations. The dose conversion coefficients derived for different CT scan types can be also used universally for other dosimetry studies concerning Korean CT scans. We also confirmed that the Caucasian-based CT organ dose calculation tools may be used for the Korean population with reasonable accuracy.

Correlation between Telomere Length and Chronic Obstructive Pulmonary Disease-Related Phenotypes: Results from the Chronic Obstructive Pulmonary Disease in Dusty Areas (CODA) Cohort

  • Background: Chronic obstructive pulmonary disease (COPD) is a common chronic respiratory disease with increased prevalence in the elderly. Telomeres are repetitive DNA sequences found at the end of the chromosome, which progressively shorten as cells divide. Telomere length is known to be a molecular marker of aging. This study aimed to assess the relationship between telomere length and the risk of COPD, lung function, respiratory symptoms, and emphysema index in Chronic Obstructive Pulmonary Disease in Dusty Areas (CODA) cohort. Methods: We extracted DNA from the peripheral blood samples of 446 participants, including 285 COPD patients and 161 control participants. We measured absolute telomere length using quantitative real-time polymerase chain reaction. All participants underwent spirometry and quantitative computed tomography scan. Questionnaires assessing respiratory symptoms and the COPD Assessment Test was filled by all the participants. Results: The mean age of participants at the baseline visit was 72.5±7.1 years. Males accounted for 72% (321 participants) of the all participants. The mean telomere length was lower in the COPD group compared to the non-COPD group (COPD, 16.81±13.90 kb; non-COPD, 21.97±14.43 kb). In COPD patients, 112 (75.7%) were distributed as tertile 1 (shortest), 91 (61.1%) as tertile 2 and 82 (55%) as tertile 3 (longest). We did not find significant associations between telomere length and lung function, exacerbation, airway wall thickness, and emphysema index after adjusting for sex, age, and smoking status. Conclusion: In this study, the relationship between various COPD phenotypes and telomere length was analyzed, but no significant statistical associations were shown.

Radiologic Abnormalities in Prolonged SARS-CoV-2 Infection: A Systematic Review

  • We systematically reviewed radiological abnormalities in patients with prolonged SARS-CoV-2 infection, defined as persistently positive polymerase chain reaction (PCR) results for SARS-CoV-2 for > 21 days, with either persistent or relapsed symptoms. We extracted data from 24 patients (median age, 54.5 [interquartile range, 44-64 years]) reported in the literature and analyzed their representative CT images based on the timing of the CT scan relative to the initial PCR positivity. Our analysis focused on the patterns and distribution of CT findings, severity scores of lung involvement on a scale of 0-4, and the presence of migration. All patients were immunocompromised, including 62.5% (15/24) with underlying lymphoma and 83.3% (20/24) who had received anti-CD20 therapy within one year. Median duration of infection was 90 days. Most patients exhibited typical CT appearance of coronavirus disease 19 (COVID-19), including ground-glass opacities with or without consolidation, throughout the follow-up period. Notably, CT severity scores were significantly lower during ≤ 21 days than during > 21 days (P < 0.001). Migration was observed on CT in 22.7% (5/22) of patients at ≤ 21 days and in 68.2% (15/22) to 87.5% (14/16) of patients at > 21 days, with rare instances of parenchymal bands in previously affected areas. Prolonged SARS-CoV-2 infection usually presents as migrating typical COVID-19 pneumonia in immunocompromised patients, especially those with impaired B-cell immunity.

COVID-19 진단을 위한 CT 검사: 프로토콜, 방사선량에 대한 체계적 문헌고찰 및 진단을 위한 CT 검사량 (CT Examinations for COVID-19: A Systematic Review of Protocols, Radiation Dose, and Numbers Needed to Diagnose and Predict)

  • 목적 Coronavirus disease 2019 (이하 COVID-19) 폐렴에서 CT를 일차 진단 검사로 사용하고자 하는 논의가 있지만, 대규모 인구에게 CT 검사를 적용했을 때의 상황을 고찰한 연구는 없었다. 본 연구에서는 COVID-19 폐렴을 다룬 연구들에서 CT 프로토콜과 방사선량을 분석하고, CT 검사가 일차 진단 검사법으로 사용될 때 필요한 CT 검사량에 대해 알아보고자 한다. 대상과 방법 본 연구는 9개의 인용도가 높은 영상의학과 저널에서 COVID-19 폐렴의 CT 기반 진단을 다룬 문헌들을 검색하였다. 먼저, 연구에서 제시된 CT 프로토콜, 방사선량을 조사하여, 이를 해당 국가의 diagnostic reference level과 비교하였다. 추가로, COVID-19에 대한 CT 민감도 94%, 특이도 37%를 적용하여, 우한시와 뉴욕, 이탈리아의 초기 COVID-19 outbreak에서 polymerase chain reaction (이하 PCR) 검사 양성률에 기반한 number needed to diagnose (이하 NND)와 number needed to predict (이하 NNP)를 계산하였다. 결과 총 86개의 연구가 검색되었고, 그중 CT 프로토콜은 81개의 연구에서(94.2%), 방사선량은 17개의 연구에서(19.8%) 보고되었다. 저선량 흉부 CT는 표준선량 흉부 CT보다 2배 많은 연구에서 활용되었다(39.5% vs. 18.6%). 방사선량을 보고한 17개의 연구들 중, 15개의 연구에서 방사선량은 해당 국가의 diagnostic reference level 수치보다 낮았다(88.2%). COVID-19에 대한 CT 민감도 94%, 특이도 37%를 적용하였을 때, NND는 3.2회 CT scans으로 나타났다. 한편, PCR 검사 양성률 50%, 25%, 10%, 5%에서의 한 명의 COVID-19 환자를 진단 위한 CT 검사량을 나타내는 NNP는 각각 2.2, 3.6, 8.0, 15.5회의 CT scans로 나타났다. 우한 시에서는 최종 17365명의 COVID-19 환자를 진단하기 위하여 약 35418명에서(PCR 검사 양성률 58%) 44840명(PCR 검사 양성률 38%)의 사람들이 CT 검사를 받은 것으로 나타났다. 뉴욕시와 이탈리아의 초기 COVID-19 유행 10주간, PCR 검사 양성률에 따라 일 CT 검사량이 최대 5.4, 10.9배까지 변화하였다. 결론 CT를 COVID-19에 대한 일차적인 진단검사로 사용할 경우, PCR 검사 양성률에 따라 CT 검사량은 변동량이 크고, 이는 추후 판데믹 상황에서 고려되어야 할 것이다.

늑골 골수염에 동반된 흉벽 천공성 농흉 1례 (Tuberculous Empyema Necessitatis with Osteomyelitis, a Rare Case in the 21st Century)

  • 흉벽 천공성 농흉(Empyema necessitatis)은 농흉이 흉막 외의 공간으로 확장되어 나간 것을 말한다. 결핵성 흉벽 천공성 농흉은 결핵의 드문 합병증으로서 특히 소아에서는 더욱 드물다. 본 저자들은 결핵에 노출된 병력이 없던 21개월 남자아이에서 7번째 늑골의 골수염에 동반된 결핵성 흉벽천공성 농흉을 경험하여 이에 대해 보고한다. 우리는 환자의 진단과 치료를 위해 수술적 치료를 시행하였고, 조직을 이용한 PCR 및 분자생물학적 검사에서 M. tuberculosis complex를 확인하여 항결핵제를 이용해 합병증 없이 치료하였다.

완치된 결핵환자에서 발생한 Mycobacterium szulgai 폐질환 1예 (A Case of Mycobacterium szulgai Lung Disease in Patient with Healed Tuberculosis)

  • Mycobacterium szulgai is a rare nontuberculous mycobacterium found in Korea. It is an opportunistic pathogen and is usually isolated from patients with a history of alcoholism, chronic pulmonary disease, or an immunocompromising condition. We present here a case of M. szulgai isolated from a patient with a history of pulmonary tuberculosis. A 54-year-old man was admitted with dyspnea and febrile sensation. He had a history of pulmonary tuberculosis which occurred 30 years earlier and treatment with anti-tuberculosis medication. His chest computed tomography scan showed cavitary consolidation in both upper lungs. A sputum acid-fast bacilli (AFB) smear was positive and anti-tuberculous medication was started. However, a polymerase chain reaction for mycobacterium tuberculosis was negative and anti-tuberculous medication was stopped. M. szulgai was isolated on 3 separate sputum and bronchial wash fluid AFB cultures. He was treated with clarithromycin, rifampicin, and ethambutol. After 1 month, a sputum AFB smear and culture became negative and no additional M. szulgai were isolated during a 16-month treatment.

한국인 남자에서 Synapsin III 유전자의 D22S280 표지자와 정신분열병의 연합연구 (No Associations between Schizophrenia and D22S280 Marker on Synapsin III Gene in Korean Males)

  • Objectives : Synapsin III near VCFS region on chromosome 22q affects. It could be an interesting candidate gene for schizophrenia. D22S280 is a highly polymorphic genetic marker residing in synapsin III. We examined association of D22S280 marker on synapsin III with Korean patients with schizophrenia. Methods : The subjects were 46 male Korean patients with schizophrenia and 60 male normal controls. Using polymerase chain reaction, gel electrophoresis, ABI 310 genetic analyzer, and GeneScan Collection 3.1 software, we confirmed genotypes of D22S280 marker. We examined Hardy-Weinberg equilibrium and case-control association using SAS/Genetic 9.1.3. Results : Genotypes of both schizophrenia and control groups were in Hardy-Weinberg equilibrium. We could not find any significant statistical differences in allele-wise(${\chi}^2$=10.4, df=6, p=0.098) and genotype-wise (${\chi}^2$=22.1 df=19, p=0.258) analyses of D22S280 marker between schizophrenia and normal controls. Individual allele analyses with df=1 showed significant differences in A1(p=0.025) and A7(p=0.034) allele, which were not significant following Bonferroni corrections(A1:p=0.177, A7:p=0.235). Conclusion : We couldn't find any association between schizophrenia and the synapsin III gene. Given the small number of subjects studied, further investigations are needed.

코리안 쇼트헤어 고양이에서 발생한 크립토코쿠스 감염증 의심 1증례 (A Case of Nasal Cryptococcosis in a Domestic Shorthair Cat)

  • 8세 중성화 암컷 고양이가 만성 비강 삼출과 호흡곤란으로 내원하였다. 신체검사에서 오른쪽 비강의 출혈농성 삼출이 관찰되었고, 흡기성 호흡곤란과 코골기 증상, 종대된 오른쪽 하악 림프절이 확인되었다. 전혈검사와 혈청화학 검사에서 미약하게 증가한 헤마토크리트 값과 고글로불린혈증이 나타났으며, 혈청학적 및 PCR 기법을 이용한FeLV, FIV, Chlamydophila felis, Feline Calicivirus, Herpesvirus, Bordetella, Mycoplasma felis, H1N1 influenza 검사에서는 모두 음성이었다. 방사선 검사에서는 오른쪽 비강의 연조직 밀도 상승이 관찰되었고, CT촬영에서는 비중격의 위축과 골 용해가 확인되었다. 추가 검사로 실시한 하악 림프절 세포학 검사에서는 다양한 두께의 염색이 안 되는 협막을 갖는 곰팡이가 관찰되었으며, narrow based budding을 보이는 곰팡이도 관찰되어 크립토코쿠스 감염증으로 잠정진단하였다. 혈청학적 검사에서 크립토코쿠스 항원가는 1 : 32,768로 매우 높게 나왔다. 검사결과에 기초해서 곰팡이 감염 치료를 위해 fluconazole, clindamycin, tocopherol투여를 실시했으며 약물 투여 후 3일 이내에 환자의 증상은 극적으로 개선되었다. 장기적인 관찰과 추가 항원역가 검사를 실시하고자 하였으나 환자는 증상 개선 후 퇴원하여 재 내원하지 않았다.
