  • Combination therapy of antibiotics is leading to improved efficacy or safety profiles with decrease emergence of bacterial resistance. Because of this benefit, many of antibacterial combinations have been used in veterinary practice for the past few decades. The purpose of this study was to examine the in vitro activity of an amoxicillin alone and in combination with other antibiotics against pathogenic bacteria of fish origin. Based on the fractional inhibitory concentration (FIC) index (FIC $$\leq_-$$ 0.5), a synergistic interaction was shown in combination of florfenicol with amoxicillin or cefuroxime. The combination of florfenicol and amoxillin showed higher antibacterial activity than that of florfenicol and cefuroxime. Ratio of amoxicillin and florfenicol in combination was 1 : 1, which showed the antibacterial activity against bacterial isolates of fish as compared with other ratios. A synergetic effect of the combination (amoxicillin and florfenicol) was further confirmed in the time-kill curve study. The study showed a better in vitro antibacterial activity of a 1 : 1 combination of amoxicillin and florfenicol than the individual antibacterial against bacterial isolates of fish. In conclusion, the combination of florfenicol and amoxicillin may serve as a potential antibacterial therapy in fishes infected pathogenic bacteria.

  • The rapid development of artificial intelligence (AI), including deep learning, has led to the development of technologies that may assist in the diagnosis and treatment of diseases, prediction of disease risk and prognosis, health index monitoring, drug development, and healthcare management and administration. However, in order for AI technology to improve the quality of medical care, technical problems and the efficacy of algorithms should be evaluated in real clinical environments rather than the environment in which algorithms are developed. Further consideration should be given to whether these models can improve the quality of medical care and clinical outcomes of patients. In addition, the development of regulatory systems to secure the safety of AI medical technology, the ethical and legal issues related to the proliferation of AI technology, and the impacts on the relationship with patients also need to be addressed. Systematic training of healthcare personnel is needed to enable adaption to the rapid changes in the healthcare environment. An overall review and revision of undergraduate medical curriculum is required to enable extraction of significant information from rapidly expanding medical information, data science literacy, empathy/compassion for patients, and communication among various healthcare providers. Specialized postgraduate AI education programs for each medical specialty are needed to develop proper utilization of AI models in clinical practice.

  • Purpose: This study was done to clarify effects of bispectral index monitoring sedative administration, compared to MOAA/S (Modified Observer's Assessment of Alertness and Sedation), on conscious sedation, physiological stability and recovery time for patients undergoing endoscopic submucosal dissection. Methods: Participants In this study were patients who underwent endoscopic submucosal dissection because of early gastric cancer. Participants were assigned randomly to an experimental group receiving sedatives based on bispectral index monitoring or to a control group with the MOAA/S instrument. Movements, belching, memory, pain, discomfort, physiological stability (MBP, PR, $SpO_2$), and recovery time were measured during the treatment and recovery. Data were analyzed using Spearman partial correlation coefficient analysis, Mixed model and Wilcoxon rank sum test to determine differences in the parameters. Results: There were no statistically significant differences between the two groups for conscious sedation(movement, belching, memory, pain, or discomfort), physiological stability and recovery time. Conclusion: The results of this study indicate that no differences were found between the two types of monitoring. Thus, use of a bispectral index monitor in clinical practice enabling medical staff to readily assess the conscious sedation of for these patients is expected to be increasingly used as an objective assessment tool for conscious sedation for patient safety.

  • The success of construction projects is heavily dependent on the work ability of the field managers who manage the execution of projects. The work ability of field managers is influenced by various elements such as job stress and individual characteristics. The purpose of this study is to reveal the factors influencing the work ability of construction field managers in Korea. Work ability is evaluated bases on the Work Ability Index(WAI) developed by the Finnish Institute of Occupational Health, while job stress is measured using the Korean Occupational Stress Questionnaire Short Form developed by the Korean Occupational Safety & Health Agency. The results showed that field managers under higher job stress have lower work abilities. In addition, it is revealed that exercise or religious practice have a positive influence on work ability. This study will help to increase the work ability and enable the efficient management of organization at building construction sites.

  • Objectives This study aims to establish a protocol for a systematic review to evaluate the effectiveness and safety of manual therapy (MT) for obstructive sleep apnea (OSA). Methods We will conduct a search for relevant randomized controlled trials using seven databases, including MEDLINE/PubMed, Embase, and the Cochrane Central Register of Controlled Trials. The study includes patients with OSA treated with MT. Comparators include all other treatments excluding MT. The primary outcome is the apnea-hypopnea index; secondary outcomes include mean peripheral oxygen saturation, snoring index, quality of sleep, quality of life, peak nasal inspiratory flow, and adverse events. Results Two independent researchers will select studies based on inclusion criteria and extract necessary data. Risk of bias (RoB) will be assessed using the Cochrane RoB 2.0 tool. Meta-analysis will be conducted if there are two or more studies with the same outcome measure; otherwise, a qualitative analysis will be performed. Subgroup analysis will be conducted based on the type of MT, and evidence certainty will be evaluated using the Grading of Recommendations Assessment, Development, and Evaluation approach. Conclusions This study will evaluate the effect of MT on OSA. By systematically reviewing various MTs, it aims to refine application methods in clinical practice and provide a foundation for future research.

  • This study was performed to investigate the differences in food choice, nutrition labeling perceptions, and prevalence of obesity due to meal skipping in Korean elementary school children. A national survey was performed in 2010 to collect data on food intake frequency, understanding of nutrition labeling, and body mass index from 2,335 fifth grade students in 118 elementary schools selected from 16 metropolitan local governments by stratified cluster sampling. The data were analyzed using the SAS 9.1 and SUDAAN 10.0 packages. Students who consumed three meals for 6-7 days during the past week were classified into the regular meal eating (RM) group (n = 1,476) and those who did not were placed into the meal skipping (MS) group (n = 859). The daily intake frequency of fruits, vegetables, kimchi, and milk was significantly lower in the MS group compared to that in the RM group (P < 0.001), whereas the daily intake frequency of soft drinks and instant noodles (ramyeon) was significantly higher in the MS group than that in the RM group (P < 0.05). The MS group demonstrated a significantly lower degree of understanding with regard to nutrition labeling and high calorie foods containing low nutritional value than that in the RM group. The distribution of obesity based on the percentile criteria using the Korean growth chart was different between the MS and RM groups. The MS group (8.97%) had a higher percentage of obese subjects than that in the RM group (5.38%). In conclusion, meal skipping was related to poor food choice, low perception of nutrition labeling, and a high prevalence of obesity in Korean fifth grade children.

  • When eccentric or inclined load acts on foundation of the port & harbor structures, partial safety factors of bearing capacity limit state were estimated using reliability analysis. Current Korean technical standards of port and harbor structures recommend to estimate the geotechnical bearing capacity using the simplified Bishop method. In practice, however, simple method of comparing ground reaction resistance with allowable bearing capacity has been mostly used by design engineers. While the simple method gives just one number fixed but somewhat convenient, it could not consider the uncertainty of soil properties depending on site by site. Thus, in this paper, partial safety factors for each design variable were determined so that designers do perform reliability-based level 1 design for bearing capacity limit state. For these, reliability index and their sensitivities were gained throughout the first order reliability method(FORM), and the variability of the random variables was also considered. In order to verify partial safety factors determined here, a comparison with foreign design codes was carried out and were found to be reasonable in practical design.

  • This study proposes a reliability based design criteria for the steel bridge (H-beam, plate-girder and composite-beam), which is most common type of steel bridge, and also proposes the theoretical bases of nominal safety factors as well as load and rasistance factors based on the reliability theory. Major 2nd moment reliability analysis and design theories including both Cornell's MFOSM (Mean First Order 2nd Moment) Methods and Lind-Hasofer's AFOSM(Advanced First Order 2nd Moment) Methods are summarized and compared, and it has been found that Lind-Hasofer's approximate and an approximate Log-normal type reliability formula are well suited for the proposed reliability study. A target reliability index (${\beta}_0=3.5$) is selected as an optimal value considering our practice based on the calibration with the safety pravisions of the current steel bridge design code. Galambo's theory is used for the derivation of the algorithm for the evaluation of uncertainties associated with resistences by LRFD Format and SGST Format, whereas the magnitude of the uncertainties associated with load effects are chosen primarily by considering our level of practice. It may be concluded that the proposed LRFD reliability based design provisions for the steel highway bridge give more rational design than the current standard code for steel highway bridge.

  • This study proposes a reliability based design criteria for the R.C. superstructures of highway bridges. Uncertainties associated with the resistance of T or rectangular sections are investigated, and a set of appropriate uncertainties associated with the bridge dead and traffic live loads are proposed by reflecting our level of practice. Major 2nd moment reliability analysis and design theories including both Cornell's MFOSM(Mean First Order 2nd Moment) Methods and Lind-Hasofer's AFOSM(Advanced First Order 2nd Moment) Methods are summarized and compared, and it has been found that Ellingwood's algorithm and an approximate log-normal type reliability formula are well suited for the proposed reliability study. A target reliability index (${\beta}_0=3.5$) is selected as an optimal value considering our practice based on the calibration with the current R.C. bridge design safety provisions. A set of load and resistance factors is derived by the proposed uncertainties and the methods corresponding to the target reliability. Furthermore, a set of nominal safety factors and allowable stresses are proposed for the current W.S.D. design provisions. It may be asserted that the proposed L.R.F.D. reliability based design criteria for the R.C. highway bridges may have to be incorporated into the current R.C. bridge design codes as a design provision corresponding to the U.S.D. provisions of the current R.C. design code.
