At a brewery, the base plate-to-shell weld seam of a $90-m^3$ vertical cylindrical steel tank failed catastrophically. The 4 ton tank "took off" like a rocket leaving its contents behind, and landed on a van, crushing it. The top of the tank reached a height of 30 m. The internal overpressure responsible for the failure was an estimated 60 kPa. A rupture disc rated at < 50 kPa provided overpressure protection and thus prevented the tank from being covered by the European Pressure Equipment Directive. This safeguard failed and it was later discovered that the rupture disc had been installed upside down. The organizational root cause of this incident may be a fundamental lack of appreciation of the hazards of large volumes of low-pressure compressed air or gas. A contributing factor may be that the standard piping and instrumentation diagram (P&ID) symbol for a rupture disc may confuse and lead to incorrect installation. Compressed air systems are ubiquitous. The medium is not toxic or flammable. Such systems however, when operated at "slight overpressure" can store a great deal of energy and thus constitute a hazard that ought to be addressed by safety managers.

Survey on the hypergolic igniter and its rupture discs for liquid rocket engines was performed. The patents of the igniters for US MC-1 and Russia RD-170 engines and the discs for NASA's space vehicle were analyzed. The types of discs and holders, characteristics related to rupture pressures and working fluids, and ASME standards of the discs were examined. Also, survey on structural analyses of the disc were performed. Typical design features and experimental results of the currently developing igniter by the authors were presented.

Objectives: This study is to observe the effect of oriental medicine treatment combined with Soyeom pharmacopuncture therapy on traumatic rupture of intervertebral disc patients caused by traffic accidents. Methods: The patients who have traumatic rupture in lumbar(L)-spine(case 1) and cervical(C)-spine(case 2) were treated by oriental medicine treatment in combination with Soyeom pharmacopunture therapy. VAS(visual analog scale), ROM(range of motion), and physical exam were checked on a daily basis while NDI(neck disability index) and ODI(Oswestry disability index) were examined for 3 times at 7 day interval. Results: 1. Fer 8-9 days from the admission day, the sole use of oriental medicine treatment did not make many improvements in the case of two patients' symptoms. After combining with Soyeom pharmacopuncture therapy, chief complaints were improved significantly. 2. In case 1, lumbago decreased from VAS 10 to VAS 5 and lumbar ROM got better than before. ODl score cropped from 42 to 27. 3. In case 2, left shoulder pain and nuchal pain lowered from VAS 10 to VAS 4, left upper limb numbness and weakness were improved, The patient showed nearly fun ROM. NDI score decreased from 26 to 19. Conclusions: Oriental medicine treatment in combination with Soyeom pharmacopuncture therapy is proved to be helpful to improve the symptoms of the traumatic rupture of intervertebral else patients caused by traffic accident.

The intradural lumbar disc herniation which was first described by Dandy in 1942 is unusual. The pathogenesis is obscure and preoperative diagnosis is not easy. The patient usually have more severe neurologic deficits than those found in the much more common extradural disc herniations. We experienced two cases of the intradural lumbar disc herniation who had previous disc operations and the symptoms and neurologic signs were improved after removal of the intradural sequestration of disc fragments.

Objectives : We defined that the ruptured disc is extruded trans ligamentous through the posterior longitudinal ligament or sequestrated. The authors have experienced 12 cases of ruptured disc that underwent the conservative treatment. We studied the effect and possibility of the conservative treatment for the ruptured disc. Methods : The patients with ruptured disc have underwent the conservative treatment. After the treatment, we compare pre-treatment with post treatment in the view of state of patients' ambulation and physical conditions. Results : In this study, 12 cases of ruptured disc have underwent the conservative oriental treatment. After the treatment, 75% of the cases were improved above good state Conclusion : The ruptured disc has been operated universally. Except the emergency state, the conservative treatment is helpful to improve the symptoms of the patients with ruptured disc.

Intradural lumbar disc herniation[ILDH] is a rare pathology. The pathogenesis of ILDH is not known with certainty. Adhesions between the ventral wall of the dura and the posterior longitudinal ligament[PLL] could act as a preconditioning factor. Diagnosis of ILDH is difficult and seldom suspected preoperatively. Prompt surgery is necessary because the neurologic prognosis appears to be closely related to preoperative duration of neurologic symptoms. Despite preoperatively significant neurological deficits, the prognosis following surgery is relatively good. We report on case of ILDH at L3/4 with differential diagnoses, and the possible pathogenic factors are discussed.

A hybrid rocket engine capable of thrust throttling and underwater-working was developed for the underwater high-speed vehicle propulsion system. The hybrid rocket engine was designed and made by two types of ground test motor and underwater working motors. An engine performance was verified by the ground tests with the ground test motor and in the case of underwater motors the ground tests and underwater tests were performed. For the underwater operation a two-stage ignition system was adopted and a rupture disc was installed at the end of nozzle for a water-tight just before an ignition. Successful ignition and propulsion were confirmed in the underwater test with the final selected double rupture disc.

Objective: The purpose of this study was to report the radiological and symptomatic changes in a patient with acute herniated lumbar disc due to coughing, treated with megadose pharmacopuncture and combined Korean medicine. Methods: MRI examination diagnosed an acute lumbar disc herniation by coughing. We performed a combination treatment of megadose pharmacopuncture and integrated Korean medicine. NRS, ODI, EQ-5D, SLR test, and big toe extension test were checked on admission, after two weeks, on discharge, and after four months to confirm symptom improvements. Results: In the acute stage of lumbar intervertebral disc rupture, megadose pharmacopuncture combined with Korean medicine treatment showed improvement in NRS, ODI, EQ-5D, SLRT, and big toe extension tests. In addition, absorption of the herniated intervertebral discs was observed through an MRI scan on discharge. Conclusions: For patients with herniated lumbar disc acutely deteriorated by coughing, a combination of megadose pharmacopuncture with Korean medicine treatment may be a solution.

The occurrence of neurologic symptom, for example cauda equina syndrome or disc rupture after Chuna treatment seems to be an uncommon event considering the large number of Chuna treatment performed. We experienced a woman who developed sudden complication after lumbar Chuna treatment. Patients in spinal hernation undergone Chuna treatment can be the causative factor in neurological symptom and must be considered in each case. Therefore, physicans and patients should be aware that neurological complication may occur as a result of lumba Chuna treatment.

The deterioration of neurologic symptoms, like disc rupture or cauda equina syndrome within short time caused by abdominal pressure, seems to be an uncommon event. We experienced a man who had a sudden deterioration to cauda equina syndrome for a day. We presume that the reason is the increase of abdominal pressure by coughing. He had also undergone to repetitive minute damage after degenerative condition. Therefore, doctors and patients should be aware about degenerative state, possibility of minute damage, and be careful to its cause like coughing.
