Clinical Usefulness of the Jones Jig Appliance for Alignment of Premolars and Molars: Case Reports

  • Early loss of the primary maxillary second molar can lead to complications in which mesial drift of the adjacent first molar (M1) can disturb eruption of the succedaneous second premolar (P2). This study reports two cases of space loss for P2 caused by early exfoliation of its predecessor. After the eruption of the first premolar, the Jones jig appliance was used to distalize M1 and regain space for the eruption of P2. The appliance was further utilized to align the palatally erupted P2 into the dental arch. In both cases, the space and corrected position of P2 were well maintained. Early exfoliation of the primary second molar caused by mesial encroachment of M1 is a common phenomenon, and pediatric dentists should attend to this during routine examinations. An appropriate intervention should be initiated when the primary second molar is lost during the mixed dentition period. If used with careful anchorage control, the Jones jig appliance can effectively resolve this problem.

  • A physician assumes toward his patient the obligation to use such reasonable care and skill as is commonly possessed and exercised by physicians in the same general line of practice in the same or similar localities and to use his best judgment at the times. Medical disputes between physicians and patients are, ever more increased in these days as human body, happens to cause a variety of changes in body unlike the function of machine. Such increased trends of medical disputes became a problem in common across the word under the influence of affluent living standard, high consciousness of life value and right by today's people. The aim of this dissertation is oriented to forming a physician's responsibilities in medicalcare accidents arising between physicians and patients. A general physician, for example, has not been negligent merely because, a specialist might have treated the patient with greater skill and knowledge. However, the fact that a physician may have acted to the best of his ability will not avoid legal problems for damages resulting from substandard treatment, that is the degree of care and skill which is to be expected of the ordinary practitioner in his field of practice. The duty of a physician who is, or holds himself out to be, a specialist is greater in the field of his specialty than one who is a general physician. A patient's consent to routine medical procedures is implied from the fact that patient comes to the physician with a medical problem and voluntarily submits to the procedures. For the more serious medical procedures and for major operations, however, it is preferable for the physician to have the patient's consent in writing, to facilitate proof of the consent in the event of a dispute or litigation. Suppose that mistakes on the part of physicians are likely to be blamed in all cases of malpractice. Then it will create a sort of shrinkage in activities of medical treatment. There should be some limitation on excessive application of 'The thing speaks for itself' on mistakes by physicians and availablity of cause and effect. It is a matter of complicity as well as a matter of importance to draw a definite boundary on responsibilities of physician. A series of further research on this particular aspect is strongly urged.

  • I try to solve the problem of the usage of the general software interrupt with the nested call of the software interrupt and the effective passing way of the parameters. The software interrupt should be protected against the indiscriminate access because it is used to call the system functions or to use the system resources by generating a software interrupt. But, it is difficult to effectively handle the SWI instruction because of its limited usage. I designed and implemented nested call of the software interrupt and the effective way that handle the parameters in the software interrupt service routine to solve this problem in this paper. In other words, from the single SWI call to the nested SWI call, I improved the software interrupt use all the more flexibly, and I compared and analyzed the strong and weak points of the two passing ways of the parameters. The main differences between these two ways are speed and readability. The stack pointer getting way incurred a lot of overhead although it has a very great readability. But, the stack pointer passing way producted 19% of the effectivity in speed by reduce overhead.

  • Purpose: The purpose of this study was to identify levels of knowledge, perception, and practice of eye care as part of nursing care in the ICU. Method: A descriptive, cross-sectional study design was used. The participants in this study were 269 registered nurses working in the ICUs of five university hospitals in Seoul, Korea. Between November 2005 and December 2005, data were collected using a semi-structured questionnaire developed by the researchers, and analyzed using the SPSS Program. Results: In general, levels of knowledge of nursing care for the eyes were moderate among ICU nurses, but levels of perception of nursing care for the eyes were very high. Of the respondents, 61.7% reported that they would consult a doctor whenever they found an eye problem. Only 42.4% nurses answered that they provide eye care as part of the daily routine and 43.9% nurses responded that they cleanse the eye lids with wet saline gauze. In regression analysis, the practice of eye care as part of nursing was significantly influenced by perception of nursing care for the eyes, and knowledge of interventions for nursing care for the eyes. Conclusion: These results suggest that education on care of the eyes for ICU nurses and the development of a standardized eye care protocol should be done to improve quality of nursing care in the ICU.

A source and phase identification study of the 10 December 2002 Cheolwon, Korea, earthquake of ML 3.6 (2002년 12월 10일 규모 3.6 철원지진의 진원요소 및 파상분석)

  • We analysed seismic phases recorded by the 10 December 2002 Cheolwon, Korea, earthquake of $M_{L}$ 3.6 and obtained source parameters such as hypocenter, origin time, earthquake magnitude. Velocity and acceleration records used in this study are from the KMA and KIGAM seismic networks. Due to the location of the epicenter in the north of the DMZ(Demilitarized Zone), direct Pg phases were recorded only at five stations in the area south of DMZ. Identification of refracted Pn phase as the first arrival is difficult in most stations. Therefore, the hypocenter determined by existing routine methods could be affected by a large error. In order to avoid the possibility of the problem, we employed a method of seismic phase analysis developed by Kim et al.. The direct, refracted, and reflected P and S phases were successfully identified using the method together with the travel time curve data. In order to improve the accuracy in determination of the hypocenter and origin time, we included PmP and SmS phases in the analysis in addition to the phases such as Pg, Pn, Sg and Sn. The epicenter, depth, and origin time of the Cheolwon earthquake determined based on data of 11 stations within 200km from the epicenter are $38.81^{\circ}$N, $127.22^{\circ}$E, 12.0km, and 7:42:51.4(local time), respectively. The average value of the local magnitude based on the Richter's definition from all the stations is 3.6 in $M_{L}$. This magnitude is smaller by 0.2 and 0.5 compared with magnitudes determined by KMA and KIGAM, respectively.

  • Dental caries and periodontal disease continue to present unique problems in the dental management of the persons with disabilities because the chronicity of oral diseases complicates the primary physical or mental disability. The increased prevalence of dental disease in most persons with disabilities is probably not due to any inherent proclivity for dental disease but more likely evolves because dental care receives less attention. Prosthetic dentistry procedures are not contraindicated for most patients with physical and mental disabilities. Fixed bridges may be feasible if the patient or care provider can maintain adequate oral hygiene and the patient's disability dose not preclude this type of prosthesis. Removable partial or full dentures may be indicated if the patient or care provider can easily remove the prosthesis and care for it. Although most persons with disabilities need no additional behavior management modalities to complete dental care, some persons require professionally recognized behavior management techniques during treatment, such as physical restraint, pharmacologic agents, or general anesthesia. Hospitalization and the use of general anesthesia are sometimes required to deal effectively with the extreme management problem patient. This patient with mild mental retardation was fearful of dental treatment. Routine restorative, surgical and prosthetic dentistry procedures were performed under general anesthesia.

A Baseline Study on the Choice of Optimal Screening Test Items among Workers with Abnormal Liver Function Tests on Workers' Periodic Health Examination (근로자 건강진단시 간기능 이상자의 정밀검사항목 개선을 위한 조사연구)

  • Workers' periodic health examination is the main tools used to manage the health problems of most workers in Korea. The most common health problem found in workers' periodic health examination is liver disorder. Liver disorder is also one of the most common health problems in general population and one of the leading causes of mortality in adult population. Regulation proposed by government (No. 207, Ministry of Labor, 1992) defines the criteria for selection of workers with the liver dysfunction for further evaluative examination and the examination items used for diagnosis of the workers with liver dysfunction. This study was designed to evaluate the proficiency of each examination items presently defined in Regulation and propose the optimal examination items for detection of the liver disorders found by workers' periodic health examination. Study subjects are 186 workers with abnormal liver function tests in screening examination of workers' periodic health examination. Questionnaire survey including past history of liver disorder, drinking history, height and weight was done. Physical examination by physician, routine test items defined by Regulation (SGOT, SGPT, $\gamma$-GTP, protein, albumin, total and direct bilirubin, alkaline phosphatase, $\alpha$-feto protein, HBsAg and anti-HBs), anti-HCV antibody test and liver ultrasonography were done. Results are as follows; 1. Result of evaluative examination utilizing only the items defined in Regulation was; There were 75 workers with suspected live. disorder(40.3%), 63 with no liver dysfunction (33.9%), 13 with suspected hepatitis B(7.0%), 10 workers with hepatitis B(5.4%), 10 workers with hepatitis B carrier state(5.4%), 10 with alcoholic liver disorders(5.4%), 5 with fatty liver(2.7%). When alternative diagnostic criteria applying additional examination items (drinking history, body mass index, anti-HCV antibody and ultrasonography) diagnosability of liver disorder was increased. When all four items were included, final results were; 23 workers (17.8%) with hepatitis B (10 carriers, 13 suspects and 10 hepatitis B), 10 (5.4%) with hepatitis C(4 carriers, 5 suspects and 1 hepatitis C), 13(7.0%) with alcoholic liver disorder, 45(24.2%) with fatty liver (40 suspects, 5 fatty liver), 410%) with suspected liver disorders and 44 (23.7%) with normal liver. 2. Of examination items defined by Regulation, only SGOT, SGPT, $\gamma$-GTP and HBsAg were significantly different in abnormal rate and mean value, and all other laboratory findings did not showed significant difference between two groups. Drinking history, body mass index and anti-HCV antibody test which are the items that authors included in this study, also showed significant difference between two groups. Utilization of body mass index (BMI) for abnormal liver function group in diagnosis of fatty liver had high specificity (97.6%) but sensitivity (22.3%) was low. Therefore we suggest that SGOT, SGPT, $\gamma$-GTP, HBsAg, alcohol drinking history, BMI and anti-HCV Ab were useful for diagnosis of liver disorders among worker's periodic health examination.

  • Purpose : We assessed about the measles elimination in Korea according to analyzing national data on measles cases for 2002-2007. Methods : We estimated the effective reproduction number according to the proportion of imported cases and outbreak size. Results : Eleven cases, thirteen cases, six cases, six cases, 25 cases (60.0% were linked to 1 outbreak of 15 cases), and 180 cases (55.6% were linked 11 outbreaks of 2, 2, 2, 3, 3, 3, 5, 9, 9, 12, 50 cases) were reported in 2002, 2003, 2004, 2005, 2006 and 2007, respectively. In 2002-2007, R is 0.95 by imported case and 0.43 by outbreak sizes. In 2007, multiple chains of transmission occurred in hospital. Most of cases were below 4 ages and unvaccinated. We didn't find most of the infection source. Conclusion : We should prevent outbreak of susceptibility pocket like nosocomial infection, conduct reliable routine surveillance and epidemiological investigation to solve a problem of the measles elimination.

Frequency, Clinical Pattern and Outcome of Thrombosis in Cancer Patients in Saudi Arabia

  • Objectives: Thrombotic risk is increased in patients with cancer and there are important implications for those who suffer a venous thromboembolism (VTE). We undertook this study to determine the frequency, clinical patterns, and outcome of VTE in Saudi patients with cancer. Methods: Cancer (solid tumors and lymphoma) patients who developed VTE from January 2004 to January 2009 were studied retrospectively. Demographics and clinical characteristics related to thrombosis and cancer were evaluated. Results: A total of 701 patients with cancer were seen during the study period. VTE was diagnosed in 47 (6.7%) patients (median age 52, range 18-80 years). Lower limb DVT was the most common type, seen in 47% patients, followed by PE in 19%, and 19% patients had both DVT & PE. Thrombosis was symptomatic in 72% patients while it was an incidental finding on routine workup in 28%. Cancer and VTE were diagnosed at the same time in 38% of patients, and 47% patients developed VTE during the course of disease after the cancer diagnosis. The majority of VTE post cancer diagnoses occurred during the first year (median 4 months, range 1-14). Additional risk factors for VTE were present in 22 (47%) patients and 14 (30%) of these patients were receiving chemotherapy at the time of thrombosis. Only 5 (10.6%) patients were receiving thrombo-prophylaxis at the time of VTE diagnosis. Most common types of tumors associated with thrombosis were breast cancer, non-Hodgkin's lymphoma and lung cancer. The majority of the affected patients (79%) had advanced stage of cancer. After a median follow-up of 13 (range 0.5-60) months, 38 (81%) patients had died. There was no difference in the mortality of patients with symptomatic or asymptomatic thrombosis (82% vs 78.6%). Conclusions: Thrombotic complications can develop in a significant number of patients with cancer, and almost half of the patients have additional risk factors for VTE. Thrombosis is usually associated with advanced disease and can be asymptomatic in more than a quarter of cases. Thromboprophylaxis in cancer patients is under-utilized. Community based studies are needed to accurately define the extent of this problem and to develop effective prophylactic strategies.

A Study on Health Care of the Aged Live in Seoul (노령자의 건강관리에 관한 연구 -서울 지역을 중심으로-)

  • This study aims to research on the actual state of the ways that the aged people live in Seoul use intentionally or unintentionally to maintain in their health. The aged people selected randomly for the study are 541 men and 902 women who are all more than 65 years old. This survey carried out from April to May in 1980. The results of this study are summed up as follows: 1. The main means of making a living engage was agriculture with the per cent of 57. 3 and next was commerce (11.9%). 2. As for their staple foods, the majority of them eat mixed food of rice and barley, and the rest eat rice or minor-grain mixed rice. As subsidiary foods, vegetables are the most common side dishes (68.9%) and see-weeds (6.7%) are taken by the minority of them. 3. Most of them (74.6%) frequently or occasionally take special health foods to maintaing their health. The most common health food turned out to be tonic liquor to men, while honey to women. 4. The majority of them(23.2% of men and 45.5% women) refrain from drinking and smoking. 5. 39.5 per cent of men and 25.5 per cent of wome) take light exercise such as going up hill and walking. 6. The majority of them undergo routine health check with the per cent of 10.8 or occasioanal health check with the 42.9. As medical treatment, many of them(45.9%) take Chinese remedies, western remedies(39.1%) and folk remedies (15.0%). 7. Many of them (66.0%) feel subjective symptoms about some geriatric diseases such as neuralgia, hypertension and cough. 8. Many of them (63.2%) free from mental trouble, while 17.7 per cent of them appeal to no job to do, 10.0 per cent of alienation and 9.1 per cent of domestic troubles. 9. Their subjective judgements of their secrets of health and longevity attach more importance to leading a regular life. 10. Most of them want to be served appropriate medical care, rest place, health consultations, etc. All these results reveal that health and longevity of the aged people are maintained by necessary cares and efforts, not by accident nor by apathy. It is thus highly desired tha tmany studies, enlightments and nationwide support about this problem be carried out in future.
