국내 양돈장에서 분리한 G9형 돼지로타바이러스의 특성 (Characterization of G9 genotype porcine rotavirus isolated in Korea)

  • Porcine rotaviruses are the most common causes of viral gastroenteritis in piglets around the world. The major G genotypes of porcine rotaviruses causing diarrhea were G4, G5 and G11 genotypes. Recently, G9 genotype rotaviruses were problemed at swine farms and frequently recognized from diarrheic piglets. In this study, a porcine rotavirus (PoRV-1) was isolated from piglet showing diarrhea using MA104 cells and confirmed as rotavirus by electron microscopy, genomic RNA electropherotyping and indirect immunofluorescence antibody tests. The nucleotide sequence of the VP7 gene of PoRV-1 was determined and compared with those of other genotype rotavirus strains from other parts of the world. Also, the nucleotide sequences of VP4, VP6 and NSP4 genes of PoRV-1 were determined and compared with those of other rotavirus strains from other countries. The results showed that the PoRV-1 isolate belonged to the G9 genotype and the P, I and E genotypes of PoRV-1 were P[23], I5 and E1, respectively. The Korean G9 PoRV-1 isolate and its nucleotide sequence data would be usefully used for the development of porcine rotavirus vaccines in near future.

조류 로타바이러스의 NSP4 유전자 염기서열분석 및 발현 (Nucleotide sequence analysis and expression of NSP4 gene of avian rotavirus)

  • The nonstructural protein 4 (NSP4) of rotavirus encoded by gene 10, plays an important role in rotavirus pathogenicity. In this study, NSP4 gene of avian rotavirus (AvRV-1, AvRV-2) was analyzed and expressed using baculovirus expression system. The sequence data indicated that the NSP4 gene of AvRV-1 and AvRV-2 were 727 bases in length, encoded one open reading frame of 169 amino acids beginning at base 41 and terminating at base 550, and had two glycosylation sites. Nucleotide sequences of NSP4 gene of AvRV-1 and AvRV-2 exhibited a high degree of homology ($88.1{\pm}7.6%$) with avian rotaviruses, namely Ty1, Ty3 and PO-13. Phylogenetic analysis showed that AvRV-1 and AvRV-2 belonged to genotype NSP4[E], which is widely found in group A avian rotaviruses. The baculovirus-expressed NSP4 migrated at 20-28 kDa and reacted with NSP4-specific antiserum by FA and Western blot. Furthermore, it was found to be a glycoprotein by using tunicamycin, which is a specific inhibitor of N-linked glycosylation.

Molecular characterization of avian rotavirus isolated in Korea

  • An avian rotavirus (AvRV-2) was isolated from feces of broilers suffering from acute gastroenteritis in 2011. It was the first avian rotavirus isolated in Korea. To investigate the molecular characteristics of AvRV-2, the VP4, VP6, VP7 and NSP4 gene nucleotide sequences were determined and compared with those of rotavirus strains available in the GenBank database. The phylogenetic tree of VP7 gene showed that AvRV-2 had a high degree of nucleotide sequence homology (93.4% to 94.7%) with those of rotaviruses belonging to genotype G19 cluster. The phylogenetic tree of the VP4 gene revealed a high degree of nucleotide sequence homology (95.8% to 95.9%) with genotype P[30] rotaviruses isolated from chickens. The VP6 and NSP4 gene nucleotide sequences showed the highest identities with those of avian strains with 95.3% to 96.4% and 90.3% to 92.2%, respectively. Genetic characterization of the VP4, VP6, VP7 and NSP4 showed that AvRV-2 strain was most closely related to chicken rotavirus strains from Germany and Japan. Comparative nucleotide sequences and phylogenetic analysis indicated that avian rotavirus isolated from broilers belonged to genotype G19P[30] and it was the first report on avian rotavirus infection in Korea.

로타바이러스 감염이 관련된 신생아 괴사성 장염 (Rotavirus-associated neonatal necrotizing enterocolitis)

  • 목적 : 본 연구에서는 로타바이러스와 관련된 신생아 괴사성 장염(RV+NEC)이 로타바이러스와 연관되지 않은 신생아 괴사성 장염(RV-NEC)과 서로 다른 경과 및 예후를 보이는지 알아보고자 하였다. 방 법 : modified Bell stage II 이상의 환아들을 monoclonal antibody-based enzyme immunoassay (EIA) 결과를 통해 8명의 RV+NEC군과 22명의 RV-NEC군으로 분류하였고 이 두군의 임상양상과 방사선소견 및 치료 결과를 후향적으로 조사하여 비교하였다. 결 과 : RV+NEC군에서 RV-NEC군보다 재태주령과 출생체중이 통계적으로 유의하게 더 높았다($33.5{\pm}3.3$ 주 vs $29.3{\pm}4.4$ 주; P=0.01). 두 군간의 C-반응성 단백질의 최고치, 혈소판 감소증, 장벽 내 공기와 장 천공의 비교에서는 차이가 없었으나 문맥 정맥 내 공기의 빈도는 RV+NEC군에서 유의하게 많았다(88% vs 9%; P<0.01). III 이상의 병기와 수술율 그리고 합병증과 사망률을 비교한 결과에서는 두 군 간에 차이를 보이지 않았다. 결 론 : 로타바이러스와 관련된 신생아 괴사성 장염은 로타바이러스와 연관되지 않은 신생아 괴사성 장염에 비해 더 높은 재태주령과 출생체중을 가지나 질환의 중증도와 결과 면에서는 차이가 없었다.

로타바이러스 백신 경제성평가 연구에 대한 체계적 문헌고찰 (A Systematic Review on Economic Evaluation of Rotavirus Vaccination)

  • Objectives: Rotavirus is one of the main causes of severe diarrhea in children under five. Two types of rotavirus vaccines [$Rotarix^{(R)}$ (RV1) and $Rotateq^{(R)}$ (RV5)] have been introduced and its administration was optional in South Korea. A systematic review (SR) on economic evaluation (EE) of RV was conducted to examine whether the introduction of rotavirus vaccine to national vaccine program (NIP) is cost-effective. Methods: Previous SR studies of EE for RV were searched in August 2017 through databases such as MEDLINE and EMBASE. Additional search was performed to include literatures published after or unincluded in the previous SR studies. Among the 11 SR studies identified, 2 studies were reviewed via inclusion/exclusion criteria. A previous SR study including 104 original articles was selected by A MeaSurement Tool to Assess systematic Reviews. Among the 36 original articles identified through additional search, 10 were selected, resulting in 114 studies included in our analysis. Results: RV1-only, RV5-only, and evaluating-both studies account for about 44%, 22%, and 33%, respectively. Among RV1-only, RV5-only, or evaluating-both studies, 90%, 64%, or 68% of the studies concluded RV as being cost-effective, respectively. RV5-only studies were usually executed in high-income countries (68%), whereas RV1-only studies were executed mostly in lower (32%) and upper (26%) middle-income countries. When classifying studies by their funding sources, RV1-only studies (82%; 28 of 34 studies specifying funding sources) were chiefly supported by non-profit organization, and 100% of these studies were concluded as being cost-effective. RV5-only studies were mostly supported by profit organization (68%; 13 of 19 studies specifying sources), and 92% of these studies concluded as being cost-effective. Conclusion: By reviewing global EE studies for RV, we have learned that about 70% of these studies was shown to be cost-effective and RV1 appeared to be more cost-effective than RV5.

Bovine Lactoferricin Induces Intestinal Epithelial Cell Activation through Phosphorylation of FAK and Paxillin and Prevents Rotavirus Infection

  • We investigated the effect of bovine lactoferricin (Lfcin-B), a peptide derived from bovine lactoferrin, on activation of intestinal epithelial cells in IEC-6 intestinal cell, and protection against in vivo rotavirus (RV) infection. Treatment with Lfcin-B significantly enhanced the growth of IEC-6 cells and increased their capacity for attachment and spreading in culture plates. Also, Lfcin-B synergistically augmented the binding of IEC-6 cells to laminin, a component of the extracellular matrix (ECM). In the analysis of the intracellular mechanism related to Lfcin-B-induced activation of IEC-6 cells, this peptide upregulated tyrosine-dependent phosphorylation of focal adhesion kinase (FAK) and paxillin, which are intracellular proteins associated with cell adhesion, spreading, and signal transduction during cell activation. An experiment using synthetic peptides with various sequences of amino acids revealed that a sequence of 9 amino acids (FKCRRWQWR) corresponding to 17-25 of the N-terminus of Lfcin-B is responsible for the epithelial cell activation. In an in vivo experiment, treatment with Lfcin-B one day before RV infection effectively prevented RV-induced diarrhea and significantly reduced RV titers in the bowels of infected mice. These results suggest that Lfcin-B plays meaningful roles in the maintenance and repair of intestinal mucosal tissues, as well as in protecting against intestinal infection by RV. Collectively, Lfcin-B is a promising candidate with potential applications in drugs or functional foods beneficial for intestinal health and mucosal immunity.

Ginsenoside-Rb2 and 20(S)-Ginsenoside-Rg3 from Korean Red Ginseng Prevent Rotavirus Infection in Newborn Mice

  • It is well known that Korean red ginseng has various biological activities. However, there is little knowledge about the antiviral activity of Korean red ginseng and its ginsenosides. In this study, we addressed whether oral administration of ginsenoside-Rb2 and -Rg3 is able to protect against rotavirus (RV) infection. The protective effect of ginsenosides against RV infection was examined using an in vivo experiment model in which newborn mice (10-day-old) were inoculated perorally (p.o.) with $1.5{\times}10^6$ plaque-forming units/mouse of RV strain SA11. When various dosages of ginsenoside-Rb2 (25-250 mg/kg) were administered 3days, 2 days, or 1 day before virus challenge, treatment with this ginsenoside at the dosage of 75 mg/kg 3days before virus infection most effectively reduced RV-induced diarrhea. In addition, consecutive administration of ginsenoside-Rb2 (75 mg/kg) at 3 days, 2 days, and 1 day before virus infection was more effective than single administration on day -3. The consecutive administration of ginsenoside-Rb2 also reduced virus titers in the bowels of RV-infected mice. In an experiment to compare the protective activity between ginsenoside-Rb2 and its two hydrolytic products (20(S)- and 20(R)-ginsenoside-Rg3), 20(S)-ginsenoside-Rg3, but not 20(R)-ginsenoside-Rg3, prevented RV infection. These results suggest that ginsenoside-Rb2 and its hydrolytic product, 20(S)-ginsenoside-Rg3, are promising candidates as an antiviral agent to protect against RV infection.

로타바이러스 백신 도입 후 소아 입원 환자에서의 로타바이러스 위장관염 발생 변화 및 로타바이러스 백신 효과 추정 (Changes in the Occurrence of Rotavirus Gastroenteritis before and after the Introduction of Rotavirus Vaccine among Hospitalized Pediatric Patients and Estimates of Rotavirus Vaccine Effectiveness)

  • 목적: 로타바이러스(rotavirus [RV]) 백신 도입에 따른 소아 입원 환자에서의 로타바이러스 장염(rotavirus gastroenteritis [RGE]) 발생 변화와 백신 효과를 추정하였다. 방법: 2014-2015년과 2004-2005년, 두 기간 동안 한양대학교 서울병원 소아청소년과에 급성 장염(acute gastroenteritis [AGE])으로 입원한 환자의 자료를 후향적으로 비교 분석하였다. 백신 효과는 환자 검사-음성 대조군 연구를 통해 추정하였다. 결과: AGE에서의 RGE 비율은 2004-2005년(22.7%)에 비해 2014-2015년(9.0%)에 유의하게 감소하였다(P<0.001). 2014-2015년 RV 백신 완전접종률은 66.0%, 불완전접종률은 6.2%로, 백신의 예방 효과는 완전접종자에서 83.3% (95% confidence interval [CI], 60.5%-92.9%), 불완전접종자에서는 27.4% (95% CI, -163.7%-80.0%)였다. 결론: RV 백신이 도입된 이후 RGE 발생이 뚜렷이 감소하였다. 백신접종률을 향상시킴으로써 RGE로 인한 질병 부담을 더욱 낮출 수 있을 것으로 기대된다.

2004-2012년 소아 로타바이러스 위장관염의 역학적 추세에 관한 단일 기관 연구 (The Epidemiological Trend of Rotavirus Gastroenteritis in Children in a Single Center from 2004 to 2012: A Retrospective Study)

  • 목적: 본 연구는 국내 로타바이러스 백신 도입 전후 시기의 지역사회 소아 로타바이러스 위장관염의 역학에 대하여 조사하고자 하였다. 방법: 2004년 9월부터 2013년 8월까지 한전병원에서 급성 위장염 환자 중 로타바이러스 분변 항원검사를 시행했던 1개월 이상 18세 이하 환자를 후향적으로 분석하였다. 전체 환자에서 백신 도입 전(2004-2006년) 및 도입 이후(2008-2012년) 시기로 구분하여 비교 분석하였고, 5세 미만 환자에서는 백신 전(2004-2006년), 이행(2007-2009년), 백신 후(2010-2012년) 시기로 구분하여 비교 분석하였다. 결과: 전체 대상 환자에서 로타바이러스 위장관염 비율은 백신 도입 전 시기 25.0% (337/1,346)에서 도입이후 시기 20.8% (459/2,210)로 유의한 감소를 보였다(비율비, 0.83, [95% CI, 0.73-093]; P=0.0029). 백신 도입 이후 시기의 로타바이러스 위장관염 환자의 중앙연령은 2.6세로 도입 전 시기 1.6세 보다 유의하게 높았다(P<0.0001). 급성 위장관염 입원 환자 중 로타바이러스 감염 비율은 6-23개월 환자들에서 유의한 감소를 보였다(비율비, 0.62, [95% CI, 0.51-0.75]; P<0.0001). 5세 미만 환자들에서 백신 전, 이행, 백신 후 시기의 로타바이러스 위장관염의 비율은 각각 26.9% (308/1,144), 22.7% (295/1,299), 20.6% (186/902)로 유의한 감소추세를 보였다(P =0.0007). 백신 도입 이후 각 연도별 추세에서, 6-11개월(P=0.0018)과 12-23개월(P=0.0152) 환자군에서 유의한 감소 추세를 보였다. 결론: 단일기관 연구에서 로타바이러스 백신 도입 이후 로타바이러스 위장염 환자의 비율 감소와 연령 상승을 확인하였다.

2009년부터 2013년까지 서울의 일개 대학병원에서 동정된 로타바이러스 유전형의 분포 (Distribution of Human Rotavirus Genotypes in a Tertiary Hospital, Seoul, Korea During 2009-2013)

  • 목적: 한국에 백신이 도입된 이후 장염으로 입원한 소아에서 확인된 로타바이러스 유전형의 분포를 알아보고자 하였다. 방법: 2009년 8월부터 2013년 6월 사이에 급성 위장관염으로 입원한 소아에게서 수집된 201개의 로타바이러스 양성 대변 검체를 대상으로 로타바이러스 유전형 분석이 시행되었다. 결과: G 유전형은 G9 (33.3%), G1 (22.4%), G3 (15.9%), G2 (6.0%), G4 (3.0%), G10 (1.5%)이었고 혼합형(15.4%) 및 분류불능(2.5%)이 관찰되었다. P 유전형은 P[4] (45.3%)과 P[8] (43.8%)가 주로 검출되었고 혼합형(10.4%)과 P[2] (0.5%)가 발견되었다. G9P[4] 유전형이 2010/2011년에 자주 검출되었으나 이후에 G1P[8] 유전형이 주로 검출되었다. 결론: 로타바이러스 백신 도입 이후 2009-2013년 동안 G1, G3 및 G9이 번갈아가며 주로 검출되는 것을 확인하였다.
