Background : Among 'structure', 'process' and 'outcome' approaches, outcome evaluation is considered as the most direct and best approach to assess the quality of health care providers. Risk-adjustment is an essential method to compare outcome across providers. This study has aims to judge performance of hospitals by severity adjusted mortality rates of coronary artery bypass graft (CABG) surgery. Methods : Medical records of 584 patients who got the CABG surgery in 6 general hospitals during 1996 and 1997 were reviewed by trained nurses. The MedisGroups was used to quantify severity of patients. The predictive probability of death was calculated for each patient in the sample from a multivariate logistic regression model including the severity score, age and sex. For evaluation of hospital performance, we calculated ratio of observed number to expected number of deaths and z score [(observed number of deaths - expected number of deaths)/square root of the variance in the number of deaths], and compared observed mortality rate with confidence interval of adjusted mortality rate for each hospital. Results : The overall in-hospital mortality was 7.0%, ranged from 2.7% to 15.7% by hospital. After severity adjustment the mortality by hospital was from 2.7% to 10.7%. One hospital with poor performance was distinctly divided from others with good performance. Conclusion : In conclusion, severity-adjusted mortality rate of CABG surgery might be applied as an indicator for hospital performance evaluation in Korea. But more pilot studies and improvement of methodologies has to be done to use it as quality indicator.
Methicillin-resistant Staphylococcus aureus (MRSA) is a bacterium responsible for a number of infections in humans that are difficult to treat, and as a result, is a substantial contributor to morbidity and mortality. In the present study, in search of natural products capable of inhibiting this multidrug-resistant bacterium, we investigated the antimicrobial activity of Lysimachia clethroides Duby root. The antibacterial activities of EtOH extract of Lysimachia clethroides Duby root and its n-hexane, EtOAc, n-BuOH and water fractions were evaluated against 15 strains of methicillin-resistant staphylococcus aureus (MRSA) and 1 standard methicillin-susceptible S. aureus (MSSA) strain by using the minimal inhibitory concentrations (MICs) assay, colorimetric assay using MTT test, checkerboard dilution test. Antimicrobial activity of n-hexane fraction of Lysimachia clethroides Duby root was remarkable. Against the 16 strains, the minimum inhibitory concentrations (MICs) were in the range of $31.25-62.5{\mu}g/ml$ and FICI values for n-hexane fraction of Lysimachia clethroides Duby root+AM and n-hexane fraction of Lysimachia clethroides Duby root+OX were checkerboard method performed using the MRSA, MSSA and one clinical isolate strains via MICI 0.12-1 and 0.25-0.75, showing the increase of synergistic effect. When combined together, these antibiotic effects were dramatically increased. These effective combinations could be new promising agents in the management of MRSA.

Reforestation is one of the most important ways to reduce erosion soil. The objective of this study was to determine the effect of jellyfish soil amendment on seedling growth for reforestation. 100g jellyfish soil amendment was applied into planting hole for the purpose of improvement of the Chamaecyparis obtusa seedling growth. Results showed that during growing period, mortality were 4.4% for the fertilized group and 8.3% for control group. The seedling average height and root diameter were 95.0 cm and 1.07 cm in fertilized group and 40.6 cm and 0.74 cm in control group. The fertilized group was found to be superior (by 1 to 5%) to control group in terms of mortality rate, seedlings height, and root diameter. The positive growth of the fertilized group can be due the increase in soil moisture and the higher availability of nutrients to the plants from jellyfish fertilizer.
Soil-borne diseases are the major problems in mono cropping. A mixture (designated LTM-m) composed of agricultural wastes and a beneficial microorganism Streptomyces saraceticus SS31 was used as soil amendments to evaluate its efficacy for managing Rhizoctonia solani and root knot nematode (Meloidogyne incognita). In vitro antagonistic assays revealed that SS31 spore suspensions and culture broths effectively suppressed the growth of R. solani, reduced nematode egg hatching, and increased juvenile mortality. Assays using two Petri dishes revealed that LTM-m produced volatile compounds to inhibit the growth of R. solani and cause mortality to the root knot nematode eggs and juveniles. Pot and greenhouse tests showed that application of 0.08% LTM-m could achieve a great reduction of both diseases and significantly increase plant fresh weight. Greenhouse trials revealed that application of LTM-m could change soil properties, including soil pH value, electric conductivity, and soil organic matter. Our results indicate that application of LTM-m bio-organic amendments could effectively manage soil-borne pathogens.
Bacterial endocarditis of the native aortic valve is associated with significant morbidity and mortality despite aggressive medical and surgical treatment, especially when perivalvular tissue was invaded and destructed. The pulmonary autograft is full viable and immune compatible tissue. This paper describes successful Ross operation as total root replacement in 38 years old native valve endocarditis patient with aortic root abscess.
In vitro organogenesis system of the valuable ornamental species, Prunus yedoensis which is native to Korea, was established through callus culture derived from root tissues. Callus were induced on the medium supplemented with 2,4-D and BA or NAA and kinetin. Organogenesis was differ from the callus type, and NAA and kinetin combination was effective to induce organogenic callus. Growth of callus was influenced by sucrose concentrations. High level of sucrose (over 5%) had adverse effects such as decreased fresh weight and increased mortality of callus. Shoots developed from the callus when $GA_3$ was treated with BA in the medium. Results showed that $GA_3$ is essential for shoot development and elongation from callus in this species.

Root-knot nematodes (Meloidogyne spp.) are parasites that attack many field crops and orchard trees, and affect both the quantity and quality of the products. A root-colonizing bacterium, Pseudomonas chlororaphis O6, possesses beneficial traits including strong nematicidal activity. To determine the molecular mechanisms involved in the nematicidal activity of P. chlororaphis O6, we constructed two mutants; one lacking hydrogen cyanide production, and a second lacking an insecticidal toxin, FitD. Root drenching with wild-type P. chlororaphis O6 cells caused juvenile mortality in vitro and in planta. Efficacy was not altered in the fitD mutant compared to the wild-type but was reduced in both bioassays for the mutant lacking hydrogen cyanide production. The reduced number of galls on tomato plants caused by the wild-type strain was comparable to that of a standard chemical nematicide. These findings suggest that hydrogen cyanide-producing root colonizers, such as P. chlororaphis O6, could be formulated as "green" nematicides that are compatible with many crops and offer agricultural sustainability.

The root-knot nematode, Meloidogyne incognita caused a serious damage to many plants. The phenolic components of the leaves of Schinus terebinthifolius were investigated as potential nematicidal agents for M. incognita. Nine compounds were isolated and characterized as viz., 1,2,3,4,6-pentagalloyl glucose (1), kaempferol-3-O-${\alpha}$-L-rhamnoside (Afzelin) (2), quercetin-3-O-${\alpha}$-L-rhamnoside (Quercetrin) (3), myricetin (4), myricetin-3-O-${\alpha}$-L-rhamnoside (Myricetrin) (5), methylgallate (6), protocatechuic acid (7), quercetin (8), and gallic acid (9) using nuclear magnetic resonance (NMR) spectroscopy. Compound 1 showed pronounced nematicidal activity compared to Oxamyl as a positive control. It showed the lowest eggs-hatchability (34%) and the highest mortality in nematode population (21% after 72 hours of treatment) at a concentration of $200{\mu}g/mL$. It exhibited the best suppressed total nematode population, root galling and number of eggmasses in infected tomato plants. The total carbohydrates and proteins were also significantly induced by 1 with reduction in total phenolics and increase in defense-related proteins. Thus, compound 1 could be a promising, more safe and effective natural nematicidal agent for the control of root-knot nematodes.

Background: This study analyzed the drought responses of five forest tree species grown in Korean peninsula, Korean fir Abies koreana (Ak), eastern white pine Pinus strobus (Ps), keyaki Zelkova serrata (Zs), tulip tree Liriodendron tulipifera (Lt), and Japanese elm Ulmus japonica (Uj). Physiological (chlorophyll, root collar diameter [RCD]) and biochemical responses (non-structural carbohydrates, proline, lipid peroxidase and abscisic acid [ABA]) of the plants grown under mild (MD) and severe drought (SD) were compared. Results: In this study, three soil moisture regimes: control (100% precipitation), MD (60% reduction in precipitation) and SD (20% reduction in precipitation) were applied. Soil moisture content showed high water content in control site compared to MD and SD. A decline in RCD was found for Korean fir, keyaki, and tulip plants, with eastern white pine and Japanese elm showing no significant decline to the prolonged drought exposure (both MD and SD). Total chlorophyll showed a significant decline in Korean fir and tulip, with the sugar levels indicating a significant increase in Korean fir and keyaki species under SD compared to control plants. Non-significant decline in sugar level was noted for eastern white pine and Japanese elm. High accumulation of ABA, malondealdehyde and proline was noted in Korean fir, tulip, and keyaki under SD compared to control. Signs of tree mortality was only observed in Korean fir under MD (38%) and SD (43%). Conclusions: The observed findings indicate the drought responses of five tree species. The majority of the morpho-physiological (especially mortality) and biochemical variables assessed in our study indicate superior long-term drought resistance of Ps and Uj compared to the highly sensitive Ak, and moderately sensitive Lt and Zs. The results provided will help species selection for afforestation programs and establishment of sustainable forests, especially of drought-tolerant species, under increased frequency and intensity of spring and summer droughts.

Background: The long-term results of homografts used in systemic circulation are controversial. We assessed the long-term results of using a cryopreserved homograft for an aortic root or aorta and its branch replacement. Materials and Methods: From June 1995 to January 2010, 23 patients (male:female=15:8, $45.4{\pm}15.6$ years) underwent a homograft replacement in the aortic position. The surgical techniques used were aortic root replacement in 15 patients and aortic graft interposition in 8 patients. Indications for the use of a homograft were systemic vasculitis (n=15) and complicated infection (n=8). The duration of clinical follow-up was $65{\pm}58$ months. Results: Early mortality occurred in 2 patients (8.7%). Perioperative complications included atrial arrhythmia (n=3), acute renal failure (n=3), and low cardiac output syndrome (n=2). Late mortality occurred in 6 patients (26.1%). The overall survival rates at 5 and 10 years were 66.3% and 59.6%, respectively. Six patients (28.6%) suffered from homograft-related complications. Conclusion: Early results of homograft replacement in aortic position were favorable. However, close long-term follow-up is required due to the high rate of homograft-related events.
