Risk analysis using PROMETHEE method In Building Construction MANAGEMENT

  • The building construction projects include a variety of risk factors due to uncertainties. To succeed in the projects, it is important how risks are managed. Risk management is composed of identification, analysis and response. Especially, the risk analysis is important to objectively calculate significance of risk factors. This paper evaluates a method to find priorities of risks using the AHP(Analytic Hierarchy Process). The method has some defects; (1) the consistency becomes weak as the number of pair-wise compared risks is large, and (2) the input and output procedures are complex when risks are added to or removed from a risk database. Thus the paper adopt the PROMETHEE(Preference Ranking Organization METHod Enrichment Evaluations) analysis process which is able to overcome the limitation of the AHP restricted to 9 risk factors. The PROMETHEE method makes the procedure of risk analysis simple, when the risk factors pull out and put in the risk database. The purpose of this study is to prove the possibility of the PROMETHEE analysis process by being compared with AHP.

  • Due to the decreasing domestic construction market since 2007, Korean construction companies are expanding overseas market. As a result, the international market share by Korea has been continuously increased and achieved 65.2 billion dollars in 2013. Despite of such visible results, profitability concerns are constantly arising. It is pointed out that the low-priced bid competition between Korean construction companies and various unpredictable risks are the most crucial factors which aggravate the profitability in the overseas projects. From this point of view, predicting the risks in advance and controling them could be the most important tasks to improve the profitability. This research proposed 202 risk factors with a hierarchy and relational database schema for power plant construction, which is based on the 24 risk classifications in previous research (Kim & Jung 2013). Proposed risk classification and relational database schema could be utilized as the basic data in risk management system.

  • Turn-key and CMF(CM for Fee) have been selected as procurement paths of large public projects in Korea. However, recently, researches and discussions on CMR(CM at Risk) have been popular as the Korean government seeks for alternative procurement paths to enhance the performance. In the CMR projects, the part who is responsible for construction management should predict and control the potential risks for guaranteeing benefits. In this research, CMR projects in which a general contractor plays the role as CMr is mainly analyzed. We give attentions to risk management during construction process given that subcontractors, main parts in this phase, work in various ways and thus more chances for faults exist. In this case, CMr manages risks to control the procedure of project. So, potential risk factors can constitute the database, and that database can be matched to following activity. And construction manager can forecast measures of risk using the result of matching. This chain of activities increase efficiency in roles of CMr on CMR(CM at Risk). Furthermore this study will propose one of the solution of Construction Management Module.

A predictive nomogram-based model for lower extremity compartment syndrome after trauma in the United States: a retrospective case-control study

  • Purpose: The aim of this study was to utilize the American College of Surgeons Trauma Quality Improvement Program (TQIP) database to identify risk factors associated with developing acute compartment syndrome (ACS) following lower extremity fractures. Specifically, a nomogram of variables was constructed in order to propose a risk calculator for ACS following lower extremity trauma. Methods: A large retrospective case-control study was conducted using the TQIP database to identify risk factors associated with developing ACS following lower extremity fractures. Multivariable regression was used to identify significant risk factors and subsequently, these variables were implemented in a nomogram to develop a predictive model for developing ACS. Results: Novel risk factors identified include venous thromboembolism prophylaxis type particularly unfractionated heparin (odds ratio [OR], 2.67; 95% confidence interval [CI], 2.33-3.05; P<0.001), blood product transfusions (blood per unit: OR 1.13 [95% CI, 1.09-1.18], P<0.001; platelets per unit: OR 1.16 [95% CI, 1.09-1.24], P<0.001; cryoprecipitate per unit: OR 1.13 [95% CI, 1.04-1.22], P=0.003). Conclusions: This study provides evidence to believe that heparin use and blood product transfusions may be additional risk factors to evaluate when considering methods of risk stratification of lower extremity ACS. We propose a risk calculator using previously elucidated risk factors, as well as the risk factors demonstrated in this study. Our nomogram-based risk calculator is a tool that will aid in screening for high-risk patients for ACS and help in clinical decision-making.

사업장의 기계재해 위험성평가에 관환 연구 (A study on quantitative risk assessment of mechanical accidents)

  • Recently the occurrence ratio of mechanical accidents in industrial disaster of korea is increased, but very little accident information has been given regarding the prevention of mechanical accidents. In this study, mechanical accidents by the analysis of industrial accident case was examined. And it was proposed the risk assessment method and building database for investigation of risk factors in mechanical accident. As a result, from mechanical accidents database, it was found that the occurrence ratio of mechanical accident by constriction and falling etc. is very high and death ratio by mechanical accident is larger than that of usual accident. And we applied the quantitative risk assessment method proposed by this study in the mechanical accident analysis of a domestic manufacturer. From the results, the risk factors due to constriction, curling, falling and scattering are principal causes of mechanical accidents. These result is similar to that of the analysis of mechanical accidents for recent 12 years in korea.

Prolonged Opioid Use Among Opioid-Naive Women Undergoing Breast Reconstructive Surgery

  • Background Patients that undergo mastectomy for breast cancer with reconstruction may be prone to prolonged opioid use. As risk factors are not well-established, this article sought to better understand the risk factors that may be associated with this. Methods Patients that underwent breast reconstruction between 2010 and 2018 were identified in PearlDiver, a national insurance claims database. Patient demographics and comorbidities were elucidated, and various complications were then identified. Descriptive statistics as well as a multivariate analysis was used to evaluate the association of risk factors and complications. Results Breast reconstruction patients of 24,765 were identified from this database. Obesity, tobacco use, benzodiazepine use, and anticonvulsant use were all associated with prolonged opioid prescriptions greater than 90 days after both alloplastic and autologous reconstruction. Conclusion Prolonged opioid use continues to remain a topic of concern, and particularly in cancer patients that undergo breast reconstruction. Providers should be aware of potential risk factors for this to reduce this chance following breast reconstruction surgery.

  • 최근 건설 프로젝트는 규모의 대형화, 공정의 복잡함 등에 의한 리스크 인자의 발생이 증가하고 있다. 이로 인해 건설관리에서 리스크 관리의 중요성은 더욱 부각되고 있는 현실이다. 리스크 관리는 확인, 분석, 대응의 과정을 통해서 이루어지며, 분석과정에서 위험인자들의 중요도 및 우선순위를 객관적이고 정확하게 분석해내는 것이 리스크 관리의 성공여부를 좌우한다. 본 연구는 기존연구에서 건설 프로젝트의 공정 리스크 중요도 및 우선순위를 산출하는데 사용된 AHP 기법의 문제점을 보완하기 위하여 PROMETHEE를 활용한 정량적 분석 프로세스를 제시하였다. PROMETHEE를 사용함으로써 AHP 기법에서 분석대상 인자가 9개를 초과 할 때의 신뢰성 저하와 인자의 추가 또는 삭제 시 발생하는 복잡한 산출절차의 문제점을 보완할 수 있을 것으로 기대된다.

COVID-19 Risk Factors Among Health Workers: A Rapid Review

  • Coronavirus disease 2019 (Covid-19) poses an important occupational health risk to health workers (HWs) that has attracted global scrutiny. To date, several thousand HWs globally have been reported as infected with the severe acute respiratory syndrome coronavirus 2 virus that causes the disease. It is therefore a public health priority for policymakers to understand risk factors for this vulnerable group to avert occupational transmission. A rapid review was carried out on 20 April 2020 on Covid-19 risk factors among HWs in PubMed, Google Scholar, and EBSCOHost Web (Academic Search Complete, CINAHL Complete, MEDLINE with Full Text, CINAHL with Full Text, APA PsycInfo, Health Source-Consumer Edition, Health Source: Nursing/Academic Edition) and WHO Global Database. We also searched for preprints on the medRxiv database. We searched for reports, reviews, and primary observational studies (case control, case cross-over, cross-sectional, and cohort). The review included studies conducted among HWs with Covid-19 that reported risk factors irrespective of their sample size. Eleven studies met the inclusion criteria. Lack of personal protective equipment, exposure to infected patients, work overload, poor infection control, and preexisting medical conditions were identified as risk factors for Covid-19 among HWs. In the context of Covid-19, HWs face an unprecedented occupational risk of morbidity and mortality. There is need for rapid development of sustainable measures that protect HWs from the pandemic.

Determining the incidence and risk factors for short-term complications following distal biceps tendon repair

  • Background: Distal biceps rupture is a relatively uncommon injury that can significantly affect quality of life. Early complications following biceps tendon repair are not well described in the literature. This study utilizes a national surgical database to determine the incidence of and predictors for short-term complications following distal biceps tendon repair. Methods: The American College of Surgeons' National Surgical Quality Improvement Program database was used to identify patients undergoing distal biceps repair between January 1, 2011, and December 31, 2017. Patient demographic variables of sex, age, body mass index, American Society of Anesthesiologists class, functional status, and several comorbidities were collected for each patient, along with 30-day postoperative complications. Binary logistic regression was used to calculate risk ratios for these complications using patient predictor variables. Results: Early postoperative surgical complications (0.5%)-which were mostly infections (0.4%)-and medical complications (0.3%) were rare. A readmission risk factor was diabetes (risk ratio [RR], 4.238; 95% confidence interval [CI], 1.180-15.218). Non-home discharge risk factors were smoking (RR, 3.006; 95% CI, 1.123-8.044) and ≥60 years of age (RR, 4.150; 95% CI, 1.611-10.686). Maleness was protective for medical complications (RR, 0.024; 95% CI, 0.005-0.126). Surgical complication risk factors were obese class II (RR, 4.120; 95% CI, 1.123-15.120), chronic obstructive pulmonary disease (COPD; RR, 21.981; 95% CI, 3.719-129.924), and inpatient surgery (RR, 8.606; 95% CI, 2.266-32.689). Conclusions: Complication rates after distal biceps repair are low. Various patient demographics, medical comorbidities, and surgical factors were all predictive of short-term complications.

치매노인의 낙상위험요인에 관한 메타분석 (A Meta-analysis of the Risk Factors related to Falls among Elderly Patients with Dementia)

  • Purpose: The purpose of this study was to provide data about the risk factors related to falls among elderly patients with dementia using meta-analysis. Methods: Key words used for search through electronic database (CINAHL, PubMed, Ovid-MEDLINE, RISS, KISS, DBPIA, National Assembly Library) included 'dementia', 'Alzheimer', 'fall'. Twenty studies met the inclusion criteria for the meta-analysis and 'R' version 3.2.2 was used to analyze the correlated effect size. Results: Study results showed that risk factors related to falls were identified as the demographic (age, gender, education), dementia-related (disease duration, cognition), physical (body mass index, walking, balance, activity of daily living, use of walking aids, number of medications including psychotropic drugs, musculoskeletal problems, parkinsonism, comorbidity), psychological (neuropsychiatric symptom, depression), environmental (Physical environment), and fall-related (fall history, high risk group of fall) factors. The effect size of risk factors such as high risk group of fall (r=.35), use of walking aids (r=.33), depression (r=.31), psychotropic drugs (r=.27), Musculoskeletal problems (r=.25) were higher than the other risk factors. Conclusion: Based on the findings of this study, strategies to improve elderly patient's depression, intensive care for high risk group of fall, and adequate training with walking aids are needed for prevention of falls in elderly patients with dementia.
