  • Purpose: This study was conducted to assess the fall risk factors. Method: The subjects were 87 persons who were older than 65 years living at 28 nursing home in Seoul and Gyunggi province. Subjects were interviewed using RAFS II for intrinsic factor and the environmental factor were assessed using a structured questionnaire from Oct. to Dec. in 2004. The data were analyzed by SPSS(ver. 12.0) programs, using descriptive statistics, $x^2$-test, and t-test. Result: The 37.9% of the subjects experienced the fall, and its average number is $1.94\pm1.75$. The Women's experience of the fall was higher than that of the men, but it was no significant difference with gender. There was no significant difference with the age and duration of living. The mean of the intrinsic risk factor was 13.38 in total score 39 points marks on the RAFS II scales. The intrinsic risk factor score of the fallen group 15.71 was significantly higher than the non-fallen group 12.10. The variables of recent fall experience(t=4.72, p=0.000) and urinary dysfunction(t=2.64, p=0.010) was significantly higher than the non-fallen group. The highest variable of the intrinsic factor was the age and the variables of drug intake, balance, chronic disease, recent fall experience, urinary dysfunction were followed in order. The mean of the environmental risk factor was 0.24 points. No significant differences were shown in environmental risk factor between the fallen and the non-fallen groups. To the fallen group, the place of entrance was the highest risky environmental factor. To see in area dimension, the floor surfaces was the highest risky environmental factor and equipment and illumination factor was the following risky elements in order. In the total score of environmental risk factors based on the ares, the fallen group was 0.26 and the non-fallen group was 0.24, but there were no significant differences between the groups. Conclusion: This results suggested that visual protection strategy, set up the safety device in the place of entrance and inner stairway, bathroom and nonskid mat in the nursing home would be contribute to the prevention of the fall for the elderly.

  • Purpose: The purpose of this study was to identify the fall risk factors and to evaluate the effectiveness of the Morse Fall Scale(MFS) as an assessment tool among hospitalized inpatients. Methods: The medical records of a total of 294 patients who admitted to hospital from January 1 to December 31, 2010 were reviewed. One hundred forth seven patients who had experienced fall were matched with 147 patients who have never experienced fall. The fall information was obtained from electronic medical records and fall reports. Results: There were significant differences in visual disturbances, pain, emotional disturbances, sleep disorder, urination problems and elimination disorder at admission between fallers and non-fallers. Patients who had higher MFS scores at admission were more likely to fall as compared to the patients with lower MFS scores. When falls did occur, these occurred within five days following admission, in the patient room, among patients with alert mental status, and among patients who were ambulant with some assistance. Conclusion: The findings of this study support the need of using risk assessment tool for predicting risk for falls. This finding can be used as a useful resource to develop nursing intervention strategies for fall prevention at the hospital.

  • Purpose: Patients' perception of fall risk is a promising new indicator for fall prevention. Therefore, a fall risk perception questionnaire that can be used rapidly and repeatedly in acute care settings is required. This study aimed to develop a short version of the fall risk perception questionnaire (Short-FRPQ) for inpatients. Methods: For the psychometric measurements, 246 inpatients were recruited from an acute care hospital. The construct (using confirmatory factor analysis and discriminant validity of each item), convergent, and known-group validities were tested to determine the validity of the Short-FRPQ. McDonald's omega coefficient was used to examine the internal consistency of reliability. Results: In the confirmatory factor analysis, the fit indices of the Short-FRPQ, comprising 14 items and three factors, appeared to be satisfactory. The Short-FRPQ had a significantly positive correlation with the original scale, the Korean Falls Efficacy Scale-International, and the Morse Fall Scale. The risk of falls group, assessed using the Morse Fall Scale, had a higher score on the Short-FRPQ. McDonald's omega coefficient was .90. Conclusion: The Short-FRPQ presents good reliability and validity. As patient participation is essential in fall interventions, evaluating the fall risk perception of inpatients quickly and repeatedly using scales of acceptable validity and reliability is necessary.

  • Purpose: This study was conducted in order to evaluate the reliability, validity, sensitivity, and specificity of the Short Form of Bobath Memorial Hospital Fall Risk Assessment Scale (BMFRAS-SF). Methods: A validation study was conducted on 207 elderly patients aged over 65 who were admitted to Bobath Memorial Hospital. Fall risk scores of BMFRAS, composed of eight subscales (age, fall history, physical activity, consciousness level, communication, fall risk factors, underlying disease, and medications) were assessed from the electronic medical record. BMFRAS-SF was derived from eight subscales of the BMFRAS representing the significance between fallers and non-fallers (fall history, physical activity, fall risk factors, underlying disease, and medications). Internal consistency reliability and interrater reliability were assessed by Cronbach's alpha and kappa coefficient. Validity was assessed by Spearman correlation analysis, factor analysis. Sensitivity, specificity, positive predictive and negative predictive values, and a receiver-operating characteristic curve (ROC) were generated. Results: Fallers had significantly higher risk scores than non-fallers in fall history, physical activity, fall risk factors, underlying disease, and medication scales. The BMFRAS-SF demonstrated acceptable Cronbach's alpha (.706) and kappa coefficients of .95. The BMFRAS-SF subscales showed good convergent validity and construct validity. The BMFRAS-SF presented good sensitivity(86.7%), specificity(67.9%), positive predictive value(42.9%) and good negative predictive value(94.8%) at a cut-off score of 5. Areas under the ROC curves were .860 for the BMFRAS and .861 for the BMFRAS-SF. Conclusion: The BMFRAS-SF was proved to be reliable and valid. It could be used for time-saving assessment and evaluation of the high risks for falls in clinical practice settings.

  • Purpose: The purpose of the study was to develop a knowledge scale of risk factors for fall among community-dwelling older adults. Further, the validity and reliability of the scale was developed. Methods: A preliminary scale was developed through content validity by five experts and targeted subjects using the CVI (Content Validity Index). Following the establishment of content validity, the scale was used with 359 community-dwelling older adults to further establish both validity and reliability of the scale. Specifically, construct validity using known-group comparison technique, and reliability using Cronbach's ${\alpha}$ were established. Results: After content validity testing, 44 preliminary items were selected. Construct validity was established by known group-comparison, in which scores between the fall and no fall groups were compared. The Cronbach's ${\alpha}$ of the final scale was .83. Conclusion: Reliability and validity of the scale were confirmed. This tool may be used for measuring knowledge of fall risk factor for the community-dwelling older adults.

  • Falling is a serious problem associated with aging. Unintentional injury, which most often results from falling, is one of the leading causes of death in elderly people. The purpose of this study is to investigate the risk factors of falls and to compare characteristics of people who fall with that of non-fallers among the rural community-dwelling elderly of Korea. A sample of 201 people, living in the community, aged 60 years and over was taken from the members of a center for seniors located in Jecheon city. The mean age of the participants was 70.5 years of age. The participants are comprised of 151 women and 50 men. Eighty four of the 201 participants (41.8%) fell during the previous year. Twenty two of the fallers (26.2%) fell down more than two times. It was found that fallers had poorer eyesight, multiple chronic diseases and a more difficult time walking than non-fallers. In the logistic regression analysis of falls, only the difficulty of walking one kilometer (OR=2.4) and chronic diseases (OR=2.5) have shown an increased risk of falls. The risk of recurrent falls is, in addition, influenced by the difficulty of walking one kilometer. The result of our study shows that the impairment of mobility was the strongest risk factor of recurrent falling.
