Depending on the location of the lung tumor, the choice of treatment technique should be considered when treating the Cyberknife. The 4DCT images of 18 lung cancer patients were analyzed, and location error values were extracted through application program. The evaluation result was lower than the average position error only in the upper and the inner. These results suggest that the Vertebral tracking technique is effective when it is close to the pulmonary attachment or near the vertebral body, and the Synchrony technique is effective at other positions. In the future, we would like to study cyber knife treatment technique according to the position of the tumor as well as the volume of the lung and the respiratory cycle.

Purpose : The purpose of this study was to investigate the effect on lung capacity of healthy men and women in their twenties by performing an intervention using the chest extension exercise and the bridge exercise, which are respiratory muscle strengthening exercises. Methods : Thirty adult men and women in their 20s participated in this study. All subjects participated in the study after hearing the explanation of the purpose and method of the study, filling out a consent form. All subjects were randomly assigned to the chest extension exercise (CEE) group and the bridge exercise (BE) group of fifteen each. Each exercise was performed twice a week for 4 weeks. Lung capacity was measured by forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) using spirometry. Lung capacity was measured before and after exercise. The measured data were compared through the dependent t-test and the independent t-test. The statistical significance level was set at .05. Results : After the intervention, the CEE group showed a significant increase in FVC and FEV1 compared to before the intervention (p<.05). After the intervention, the BE group also had a significant increase in FVC and FEV1 compared to before the intervention (p<.05). However, there was no difference in FVC and FEV1 between groups before and after the intervention (p>.05). Conclusion : There was no difference between groups in lung capacity after exercise. However, both the chest extension exercise and the bridge exercise increased FVC and FEV1, which was thought to be because both exercise methods were effective in increasing lung capacity. Therefore, both chest extension exercises and bridge exercises can be effectively applied as a way to increase lung capacity.

Objectives: This study was performed to check whether the CRS (Chemical Ranking and Scoring) system is appropriate as a method to determine substances as candidates for substances subject to permission and to apply this system to the selection of candidates for substances subject to permission. Methods: A risk score was obtained by multiplying the hazard score and the exposure score and then ranking them. The hazard sub-indicators are carcinogenicity, germ cell mutagenicity, reproductive toxicity, specific target organ toxicity-repeated exposure, respiratory sensitization and endocrine disrupting chemicals. Exposure sub-indicators are persistence, bioaccumulation and emission volume. Sensitivity analysis was performed for missing values. Correlation analysis and multivariable linear regression analysis were performed among hazard, exposure and risk in order to confirm that CRS was an appropriate method. Results: As a result of the sensitivity analysis on missing values, it was confirmed that the effect on the risk ranking was not sensitive. Correlation and regression analysis confirmed that exposure had a greater effect on risk than hazard. Conclusions: The CRS system, which derives a risk score using a hazard and exposure score, is judged to be appropriate as a method for the selection of preliminary of candidates for substances subject to permission. Benzene, cadmium, nickel, and cobalt were selected as priority candidates for substances subject to permission.
Background: For the elderly, masticatory function is one of the most important oral functions and the masticatory ability is related to the wearing of dentures. Many older people wear dentures for their masticatory function, but a significant number of older people who use dentures have found that they feel uncomfortable when performing their daily activities, such as performing masticating functions or talking. Objects: The purpose of this study is to investigate how the forward head posture (FHP), respiratory function and thickness of masseter (MS), and sternocleidomastoid (SCM) are affected by the presence or absence of dentures in the elderly, and what kind of correlation there is between these variables. Methods: The study was conducted on 11 patients in the normal group and 13 in the denture group. The participant's cognitive ability was evaluated using Mini-Mental State Examination Korean (MMSE-K), and the FHP was evaluated by measuring the craniovertebral angle (CVA). The thickness of the MS and SCM muscles were measured using ultrasound, and respiration was measured with a spirometry. As for the statistical method, the correlation of each variable was investigated using Spearman's correlation coefficient. Results: In the normal group, there was a significant correlation between forced vital capacity (FVC) and forced expiratory volume in 1 second (FEV1) (p < 0.001), and in the denture group, FVC and FEV1 (p < 0.001), maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP) (p < 0.001), CVA and Lt. MS (p = 0.012). Conclusion: CVA and Lt. MS of the denture group have a high negative correlation, it is related that the thickness of MS may be thick when the elderly wearing dentures are FHP.

The change in the position of the abdominal organs due to movement by respiration is one of the reasons behind inaccurate irradiation of organs during radiotherapy (RT). Although studies in human medicine have revealed on the respiratory movements of abdominal organs, there is little information and no reference data for dogs. The purpose of this study was to establish the reference values of abdominal organs movement in various postures using computed tomography (CT), and to compare the movements of organs between dorsal recumbency and ventral, right and left lateral recumbency during respiration. CT images for kidney, adrenal gland, medial iliac lymph node, urinary bladder, gallbladder, liver, stomach, and thoracic and lumbar vertebral body of five beagle dogs were acquired. The movements of organs were evaluated by comparing the end-expiratory and end-inspiratory images. Movements of the organs were evaluated by dividing it into right-to-left, dorsal-to-ventral, and cranial-to-caudal directions. The movements of abdominal organs according to the change in postures and respiration were establish. The movement of the bilateral organs was the least when the organs were in the downward position (p < 0.017). The movement of cranial-to-caudal direction was greater than the movement of the other directions in most of the organs. Data obtained in this study may be useful in selecting the appropriate posture that can reduce the movements of organs to be treated with RT, and the data could be useful for setting the planning target volume to consider the movements of the abdominal organs by respiration.

Objectives: An epidemiological investigation was requested for a worker who developed COPD and IPF after long-term molding and firing at a domestic tile manufacturing site. We would like to share the results of the exposure assessment and the estimation of past work. Methods: The content of crystalline silica in four raw materials was analyzed, and the respirable fraction of crystalline silica and dust generated in the air from molding and firing workers and other processes were measured. The measurement and analysis method referred to the NIOSH method. Results: The crystalline silica content of the raw material was 24~47%. The concentration of crystalline silica in the molding and firing process workers and the surrounding area was at the level of the exposure standards set by the Ministry of Employment and Labor and ACGIH, and the respirable and total dust exposure levels were generally low. The crystalline silica concentration of the area samples measured to estimate past work was about twice as high as the exposure standard of the Ministry of Employment and Labor (0.05 mg/m3), and the exposure levels of respirable dust were also quite high at 0.903 and 1.332 mg/m3. Conclusions: It was confirmed that tile molding and firing workers are currently exposed to a fairly high level of crystalline silica, and a high level is also confirmed in area samples to estimate past work. In the past, it is judged that the level of exposure would have been much higher due to differences in production volume, working method, presence/absence of local ventilation facilities, and process layout. When working in such a working environment for a long time, respiratory diseases such as lung cancer, COPD, and IPF can occur.

Background : A spirometric reference equation was recently developed for the general population in Korea. The applicability of the new Korean equation to clinical practice was examined by comparing it with the Morris equation, which is one of the most popular reference equations used for interpreting the spirometric patterns and for grading the disease severity in Korea. Methods : Spirometry was performed on 926 men and 694 women, aged 20 years or older, in November 2004 at the Asan Medical Center, Seoul, Korea. The subjects' age, gender, height, weight, and spirometric values ($FEV_1$ [forced expiratory volume in one second], FVC [forced vital capacity], and $FEV_1/FVC$) were obtained. The spirometric patterns and disease severity were evaluated using both equations, and the results of the Korean equation were compared with the Morris equation. The spirometric patterns were defined as normal, restrictive, obstructive, and undetermined according to the level of $FEV_1/FVC$ and FVC. The disease severity was defined according to the level of $FEV_1$ level for subjects with an airflow limitation, and according to the FVC level for those subjects without an airflow limitation. Results : Spirometric patterns were differently interpreted in 22.5% (208/926) of the men and 24.8% (172/694) of the women after the application of the Korean equation compared with the Morris equation. Of the subjects with airflow limitation, disease severity was differently graded in 30.2% (114/378) of the men and 39.4% (37/94) of the women after the application of the Korean equation. Of the subjects without airflow limitation, disease severity was differently graded in 27.9% (153/548) of the men and 30.2% (181/600) of the women after the application of the Korean equation. Conclusion : Achange in the reference equation for spirometry could have an effect on the interpretation of spirometric patterns and on the grading of disease severity.
Background: Panting method for airway resistance measurement has the disadvantages of departing from the normal breathing pattern and of difficult for some patients to perform. We can measure airway resistance during quiet breathing under more physiologic conditions. Airway resistance is often measured during panting but attempts have been made to facilitate resistance measurements during quiet breathing. This study was designed to compare airway resistance measurements during panting with those during quiet breathing. Method: The 24 normal persons and 29 pulmonary disease patients were included in this study. Spirometry was performed and airway resistance measurement was also done during panting and quiet breathing concomittently. Results: The results were as follows; 1) High correlations were found between airway resistance measurements during panting and quiet breathing. 2) Resistance fell during panting, 21.2% in Raw tot, and 22.1% in Raw 0.5. 3) In normal persons, airway resistance fell more during panting when comparing to those in pulmonary disease patients. 4) This was largely independent of thoracic gas volume differences, because the specific airway conductance rose significantly during panting 5) The patients in whom resistance didn't fell during panting was supposed to the patients who couldn't perform panting successively because of high resistance. Conclusions: Although airway resistance can be measured during panting or quiet breathing according to the patient's performance, we must consider resistance fell during panting, by a mean 20%. It may be concluded that quiet breathing is more likely than panting to provide a relevant measurement of airway resistance.

This study was to measure the patient dose difference between 3D treatment planning CT and 4D respiratory gating CT. Study was performed with each 10 patients who have lung and liver cancer for measured patient exposure dose by using SOMATON SENSATION OPEN(SIMENS, GERMANY). CTDIvol and DLP value was used to analyze patient dose, and actual dose was measured in the location of liver and kidney for abdominal examination and lung, heart and spinal cord for chest examination. Rando phantom were used for the experiment. OSLD was used for in-vitro and in-vivo dosimetry. Increasing overall actual dose in 4D respiratory gated CT-simulation using OSLD increase the dose by 5.5 times for liver cancer patients and 6 times for lung cancer patients. In CT simulation of 10 lung cancer patients, CTDIvol value was increased by 5.7 times and DLP 2.4 times. For liver cancer patients, CTDIvol was risen by 3.8 times and DLP 1.6 times. The accuracy of treatment volume could be increased in 4D CT planning for position change due to the breaths of patient in the radiation therapy. However, patients dose was increased in 4D CT than 3D CT. In conclusion, constant efforts is required to reduce patients dose by reducing scan time and scan range.

Background: The patient with bronchiectasis may have obstructive ventilatory impairment combined with mild restrictive ventilatory impairment due to fibrosis of surrounding lung parenchyme and pleural adhesions caused by chronic recurrent pulmonary infections. Since hyperinflation or emphysematous change can be occured in bronchiectasis, pulmonary functions such as lung volumes and diffusing capacity may also vary with associated emphysema. Methods: For the evaluation of lung volumes and diffusing capacity in bronchiectasis with respect to the anatomic types and severity of bronchiectasis, a total of 40 cases comprising 24 cases of tubular, and 16 cystic type of bronchiectasis were analyzed retrospectively. Correlation between lung functions and extent of bronchiectasis or associated emphysema detected in HRCT were also evaluated. Results: Vital capacity(VC) tended to decrease in cystic type than in tubular type. As the severity of bronchiectasis became serious, the VC were significantly reduced, whereas the total lung capacity(TLC), residual volume(RV) and its ratio to the total lung capacity(RV/TLC) had no significant difference. Lung clearance index(LCI) was significantly increased in cystic type than in tubular type, whereas the slope of phase III in single breath nitrogen curve($\triangle$N2/L) was not significantly changed regard to the type and severity of bronchiectasis. DLCO and DLCO/VA reflecting diffusing capacity were significantly decreased in cystic type and also as the severity of bronchiectasis became serious. The correlation coefficient of VC, DLCO and LCI with the extent of bronchiectasis were -0.322, -0.339 and 0.487, respectively, whereas other parameters were not significantly correlated with the extent of bronchiectasis. VC and DLCO correlated negatively with the extent of emphysema while RV, RV/TLC, LCI and $\triangle$N2/L correlated positively. Conclusion: These findings suggest that the reduction of VC and diffusing capacity or uneven distribution of inspired gas in bronchiectasis are related to both the extent of bronchiectasis and associated emphysema while increased residual volume be related to the extent of associated emphysema alone.
