Objectives : The purpose of this study was to analyze the trend of clinical research on quantitative indicators of Mibyeong in traditional chinese medicine(TCM). Method : The journal search was performed using china national knowledge infrastructure(CNKI) database. Our inclusion criteria were as following: TCM clinical researches for quantitative indicators of Mibyeong. Exclusion criteria were as following: non-TCM clinical researches, used intervention methods. Results : Eleven clinical researches were analyzed in this study. Four of these researches classify the Mibyeong as a type of pattern identification(PI) and studied the characteristics of the PI quantitative indicators. Mibyeong diagnosis was done through guidelines and questionnaires, each was used at a similar rate. Quantitative indicators mentioned in the selected researches were blood indices, nailfold capillary, complexion, color of tongue substance and coating, pulse wave diagrams and heart rate variability. Among them, seven researches related to blood indices were the most. Blood indicators include whole-blood viscosity, plasma viscosity, fibrinogen, packed cell volume(Hct), triglycerides, total cholesterol, HDL-C, LDL-C, glucose, BUN/CREA, luteinzing hormone, estradiol, follicle stimulating hormone, IgA, IgG, etc. Conclusions : Based on this results, in combination with western medicine, it seems necessary to try to interpret the Mibyeong in more various ways. Even if the same Mibyeong, it is necessary to identify the index which changes according to the PI or chief complaint, and to set the Mibyeong standard corresponding thereto.
Objectives The aim of this study was to analyze the trends of researches on oriental medicine treatments for supraspinatus tendinopathy. Methods We used five databases for searching researches; Korean studies Information Service System, Oriental Medicine Advanced Searching Integrated System, Research Information Sharing Service, PubMed, and China National Knowledge Infrastructure. Only randomized controlled trials suitable for the subject were selected. The methodological quality of included randomized controlled trials (RCTs) was assessed by the Cochrane risk of bias tool. Results Twenty randomized controlled trials were analyzed. There were 9 types of treatment interventions; acupuncture, acupotomy, pharmacopuncture, electroacupuncture, fire needling, warming needle, catgut-embedding therapy, herbal medicine, cupping. The most frequently used treatment intervention was acupuncture and acupotomy. There were 9 outcome measurements including visual analog scale (VAS), Constant-Murley Score (CMS), and range of motion. The most used measurement was VAS. As a result of meta-analysis, acupuncture was more effective than control group in VAS. Additionally, acupotomy was clinically significant compared to control groups in VAS and CMS. Conclusions In this review, we analyzed researches on effectiveness of oriental medicine for supraspinatus tendinopathy. A provisional conclusion can be produced that acupuncture and acupotomy showed beneficial effect to supraspinatus tendinopathy. Although there were some RCT studies, many of them had a high risk of bias, so it is hard to conclude that our study can include overall clinical status. Further well-designed trials are needed.
Objectives : To address suitable method for designing clinical trial intervening Traditional Korean Herbal Medicine on General anxiety disorder through the recent clinical research in Chinese Herbal Medicine. Methods : Randomized, controlled trials(RCTs) of treatment of general anxiety disorder intervening herbal medicine were searched through CNKI databases. We examined several item of studies and evaluated using Jadad scale. We searched articles in CNKI with the key word "General anxiety disorder or 廣範性焦慮症", among them we selected the clinical trial studies related with herbal medicine except for case reports, studies of acupuncture therapy or west medicines. Results : 11 RCT studies of herbal medicine met inclusion criteria. The Studies used CCMD-3, HAMA as inclusion tool and mainly HAMA as a evaluation tool. Studies were progressed for 2-8 weeks, mostly used various positive control. Experiment medicine were mainly 理氣劑, 安神劑. In most studies, experiment group made similar or higher efficacy in comparison with control group. Mean Jadad score of studies was $1.3{\pm}0.67$, and only one study was double-blinded among 11 studies. Conclusions : RCT studies on general anxiety disorder of chinese herbal medicine tend to focus on investigating effect of herbal medicine through setting positive control group with CCMD-3 and HAMA. But, most studies had low quality of trial, for lack of pattern identification, evaluation and inclusion tool of chinese medicine. Clinical trials of general anxiety disorder should provide good quality by randomization, double-blinding, and multi-site trial.
Objectives The goal of this review was to investigate clinical, experimental and literature studies of Korean medicine on atopic dermatitis (AD) of Korean children in Korean medicine literature, seeking for the better research methods for more effective treatments. Methods Electronic investigations were practiced on AKOP (The Association of Korean Oriental Pediatrics, http://www.akop.or.kr) to collect theses which were published by J Pediatr Korean Med. The key word 'Atopy' was used for searching to ensure that every related thesis was collected. The publication date was limited from 1996 to 2016. The selected literatures were assessed mainly according to distributions of study type, publication year, scale, treatment and its efficacy. Results 55 papers were selected from 59 studies. In these collected 55 related theses, there were 29 experimental studies (52.73%), 18 clinical studies (32.73%), 8 review studies (14.55%). In the 29 experimental studies, there were 20 studies (68.97%) using NC/Nga mouse as subjects. The mite antigen was used to trigger AD by 8 studies (27.59%). The studies have been utilizing DNCB and DNFB instead of mite antigen since 2011. All the experimental studies showed that the Korean medicine was effective in AD. In total 18 clinical studies, 3 studies (16.67%) were case reports, only 1 thesis (5.56%) was conducted by case control study and the rest were case series studies. 7 studies (38.89%) of 18 studies diagnosed AD according to Hanifin and Rajka's diagnostic criteria (1980), only 1 study (5.56%) according to the Korean standard and the rest 10 studies (55.56%) didn't mention diagnostic criteria. 12 clinical studies (66.67%) showed efficacy in treating AD. There were 10 studies (55.56%) only employing internal treatments and 1 thesis (5.56%) didn't mention the exact prescription. Naesowhajungtang-kamibang, Saenghyeoryunbue-um were used 3 times (16.67%) as the internal medicine respectively. 5 theses (27.78%) were combined with acupuncture therapy. Hegu (LI4), Sanyinjiao (SP6) were used in 4 theses, and Taichong (LR3), Quchi (LI11), Zusanli (ST36) in 3 theses. The number of acupoints chosen from The Spleen meridian of Foot-Taiyin was the largest. There were 8 review theses. 2 were about the foreign oriental medicine, 3 were about the external medicine or external treatment methods, and 3 other studies were about severity scoring systems, the methodological study in the latest clinical study, overview for pattern and results of herbal medicine-derived AD clinical researches. Conclusions The experimental studies and clinical studies showed the effectiveness of Korean medicine treatments. However, this study still needs improving by conducting more comparative studies and using better research methods, in order to find more effective treatments to improve clinical efficacy.
