Rapid Regression of White Matter Changes in Hypoglycemic Encephalopathy (저혈당성 뇌병증에서 뇌백질 변화의 빠른 퇴행)

  • Purpose : In a previous report, it took several days for white matter lesions to regress in hypoglycemic encephalopathy. We present a case of rapid diffusion-weighted image (DWI) changes in hypoglycemic encephalopathy. Case Report: A 58-year-old male patient was found semi-comatous with the only abnormality in his laboratory tests showing hypoglycemia (44 mg/dL). After rapid correction of glucose level, immediate brain DWI showed bilateral subcortical white matter lesions. After about 5 hours, follow-up DWI showed resolved subcortical white matter lesions, with newly-appeared bilateral fronto-temporo-parietal cortical lesions. Conclusion: Both white matter and cortex involvement in hypoglycemic encephalopathy has been shown in several reports, but rapid regression of white matter changes in hypoglycemic encephalopathy has been rarely reported. It is important to know that MR imaging changes in hypoglycemic encephalopathy can be made as quick as just a few-hour-long.

Acute Aortic Injury after Nuss Bar Removal -A case report- (너스바 제거 후 발생한 급성 대동맥 손상 -치험 1예-)

  • Nuss operation as a method for correction of pectus excavatum is safe and satisfactory, but its complication presents pneumothorax, displaced bar, wound infection, pericarditis, pleural effusion, hemothorax, cardiac injury etc. We report a rare case of acute ascending aortic rupture after displaced and adhered Nuss bar removal. The patient was a 30-years old man who received Nuss operation 3 years ago. Nuss bar was removed without resistance but massively bled at both operation wound, so immediately femoro-femoral CPB and median sternotomy was done and repaired proximal aortic arch under deep hypothermic total circulatory arrest. The patient was discharged without complication at postoperative 13 day.

  • Recently, software size and program code complexity have increased due to application to various fields of software. Accordingly, the existence of program bugs inevitably occurs, and the cost of software maintenance is increasing. In open source projects, developers spend a lot of debugging time when solving a bug report assigned. To solve this problem, in this paper, we apply SeqGAN algorithm to software bug repair. In detail, the SeqGAN model is trained based on the source code. Open similar source codes during the learning process are also used. To evaluate the suitability for the generated candidate patch, a fitness function is applied, and if all test cases are passed, software bug correction is considered successful. To evaluate the efficiency of the proposed model, it was compared with the baseline, and the proposed model showed better repair.

  • The checkrein deformity describes tethering of the flexor hallucis longus tendon, which mainly occurs after fracture of the distal tibia. The deformity increases with dorsiflexion of the ankle and decreases or disappears with plantarflexion of the ankle. In some cases, the deformity may occasionally include the second and third toes. In the current study, the authors experienced secondary checkrein deformity of all lesser toes after open reduction and plate fixation for comminuted fracture of the calcaneus. As a treatment, plate and screws were removed, followed by an additional medial incision which showed a partially ruptured flexor digitorum longus tendon with severe adhesion. Resection of the adhesed tendon and tenodesis of its distal portion to the flexor hallucis longus was performed for correction of the checkrein deformity. Then the lessor toe checkrein deformity recovered immediately. The authors report on this rare lessor toe checkrein deformity after calcaneal fracture fixation with a review of literature.

Le Fort I maxillary osteotomy for cleft lip and palate patients (구순구개열 환자를 위한 상악 악교정 수술)

  • In cleft lip and palate (CLP) patients, there are various degree of residual maxillofacial deformities in adolescent period. Usually, orthoganthic surgery for the cleft patients needs Le Fort I osteotomy and/or mandibular set-back surgery. Previous report from other institute had been shown that there is significant relapse after maxillary movement after Le Fort I osteotomy when the surgical advancement of the maxilla was over than 5 or 7mm in average. Recent comphrehensive report showed that most of the relapse was happened within 1 year and the total horizontal relapse of the maxilla was as high as 30% in average. Therefore, overcorrection is needed in maxillary surgery for cleft patients. Another concerns for cleft orthognathic surgery is the anatomical variation in pterygomaxillary region in cleft patients compared to control patients. Patients with CLP had larger and thicker pterygomaxillary dimensions, and the results imply that careful attention to pterygomaxillary anatomy is needed in patients with CLP undergoing Le Fort I surgery. This article reviews the pre and postoperative considering factors for orthognathic surgery for CLP patients.

A Reinvestigation on Key Issues Associated with the Yimjin Boundary Making and Demarcation(1712~1713): Reconstructing the Distribution of Boundary Markers Based on Actual Survey Documents (역대 실지조사기록 검토를 통한 임진정계 경계표지물 분포 복원)

  • The distribution of boundary markers, set up during the Yimjin(1712) Boundary Making and Demarcation(YBMD), has less been addressed so far, although it is key material evidence on which we can reconstruct the whole processes of YBMD, together with the correction of widespread myths around YBMD. This paper aims to clarify the questions on the distribution of boundary markers associated with YBMD, built during August 1712 to September 1713, by reinterpreting the key documents of Actual Survey on them: Huh Ryang and Park Dosang(1913), Kim Woosik(1883), Lee Joongha(1885), Wu Luzhen(1907), Osone Seiji(1907), and Liu Jianfeng(1908), together with topographic maps by Japanese Imperial Army(1933), the report of expedition to Mt. Baekdu by Chungjin Teachers' College(1948), and the report of field survey by Jilin province's expedition(1957). As a result, the distribution of boundary markers built in 1712~1713 is successfully reconstructed, and summarized in the format of table and maps.

A Case Report of Congenital Spinal Dermal Sinus Tract (선천성 척추 피부동관 증례보고)

  • Purpose: Congenital spinal dermal sinus tract is a rare lesion connecting skin to deeper structures including neural tissue. It results from the failure of the neuroectoderm to separate from the cutaneous ectoderm in the third to fifth week of gestation. The common locations are the lumbosacral and occipital regions. Sometimes it extends to spinal canal. In this paper we report a case of congenital spinal dermal sinus tract in the coccyx. Methods: A 21-month-old male child born after an uncomplicated full-term pregnancy was admitted to our institute with a midline dermal sinus and a cartilaginous protrusion in the coccygeal region. There were no signs of infection. Neurologic examination showed no functional deficit in both lower limbs. He was treated with complete excision of the tract and an underlying accessory cartilage. Results: The spinal dermal sinus tract was extended from the skin to the coccyx. The stalk was loosely attached to the accessory cartilage of coccyx. At that point, it was dissected from the accessory cartilage and resected. The accessory cartilage was also resected at the bone and cartilage junction. During the follow-up period of 6 months, the wound healed well without any complication nor recurrence. Conclusion: Congenital spinal dermal sinus tract is known as a form of spinal dysraphism. In order to prevent complications, timely surgical intervention including complete resection of sinus tract with correction of associated abnormalities is of utmost importance.

Correction of Asymmetric Crying Facies with Botulinum Toxin A Injection: A Case Report (비대칭 우는 얼굴에서 보툴리눔 독소 A를 이용한 치험례)

  • Purpose: Asymmetric crying facies is caused by agenesis or hypoplasia of the depressor anguli oris muscle and is often associated various anomalies. Several static and dynamic surgical interventions have been reported, but their effects are unreliable. We report on the successful use of botulinum toxin A in an asymmetric crying facies patient. Methods: A 4-year-old girl presented with a facial asymmetry on crying or smiling. Physical examination revealed that her face had no asymmetry at rest. However, the patient showed characteristic asymmetry when smiling, crying, and with other normal facial movements. Asymmetric crying facies was clinically suspected and the weakness of left depressor anguli oris was present on electrophysiology study. Fifteen units of botulinum toxin type A were injected to the right depressor anguli oris muscle. Results: The patient showed the prominent improvement in the facial symmetry without significant complication and the effect persisted until 3 months post injection. Conclusion: Asymmetric crying facies was treated successfully with botulinum toxin A and this method was easy and noninvasive.

A Case Report of Cervical Dystonia Treated by Korean Medical Treatment with Yin-yang Balancing Therapy of the Temporomandibular Joint [Functional Cerebrospinal Therapy, FCST] and Miniscalpel-Acupuncture (턱관절음양균형요법과 도침치료를 병행한 한방치료를 이용한 경부 근긴장이상증 치험 1례)

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment with FCST and Miniscalpel-Acupuncture on patient with cervical dystonia. Methods: In this study, A patient with cervical dystonia was admitted to ◯◯ Korean medicine Hospital from Oct 20th, 2020 to Nov 28th, 2020. During the admission period, the patient was treated by Korean medical treatment (acupuncture, pharmacopuncture, herb medicine, etc.), especially using FCST and Miniscalpel-Acupuncture. The Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS), the Tsui score and CDIP (Cervical Dystonia Impact Profile)-58 were used for measuring the Cervical Dystonia. Results: After treatment for 40 days, the TWSTRS showed a decrease from 52.75 to 13.75, the Tsui score also decreased from 17 to 3 and the CDIP-58 score decreased from 64.1 to 25.5. Conclusions: These results showed that Korean medical treatment especially using FCST and Miniscalpel-Acupuncture may have an effect on reducing symptoms of cervical dystonia. But the further researches are needed.

A Case Report of Anxiety Disorder Patient with Dyspnea (호흡곤란 있는 불안장애 환자 치험 1례 )

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment with TBT (Temporomandibular Joint Balancing Therapy) on Anxiety disorder patient with dyspnea. Methods: In this study, an Anxiety disorder patient with dyspnea was admitted to △△ Korean medicine Hospital from Nov 1st, 2022 to Nov 23rd, 2022. During the admission period, the patient was treated by Korean medical treatment(acupuncture, pharmacopuncture, herb medicine, etc.), especially using TBT and Western medical treatment (Pharmacotherapy). Beck Anxiety Inventory (BAI) and VAS were used for measuring the Anxiety and Dyspnea. Results: After treatment for 30days, the BAI showed a decrease from 33 to 19, The VAS associated with dyspnea also decreased from 6 to 1 and VAS associated with Neck Pain decreased from 5 to 3. Conclusions: These results showed that Korean medical treatment especially using TBT may have an effect on reducing symptoms of dyspnea due to Panic Attack. But the further researches are needed.
