Tetralogy of Fallot with Pulmonary Arteriovenous Fistula -A Case Report- (폐동정맥루를 동반한 팔로사징환자의 치험 -1례보고-)

  • Pulmonary arteriovenous fistula can occur in a variety of clinical situations including liver diseases, infections, metastatic carcinomas, systemic disorders, and after the palliation of congenital heart diseases. A 72-day-old male infant with Tetralogy of Fallot and pulmonary atresia underwent surgical correction without difficulty. However, ventilator weaning in the ICU failed initially because of an unexplained postoperative hypoxemia(FiO2: 0.8, PaO2: 40 mmHg, SaO2: 80∼90%). Postoperative follow-up lung perfusin scan at postoperative 15 days showed right-to-left shunt(33.6%) and ventilator weaning was performed on the 20th day after the operation (FiO2: 0.4, PaO2, 50mmHg, SaO2: 86.9%). Arterial oxygen saturation under room air was 80∼85% at 7 months postoperatively. One and half year follow-up lung perfusion scan showed decreased amount of right-to-left shunt (11.2%). We report a case with a review of the literatures.

  • Acquired Hallux varus is defined radiographically by a negative metatarsophalangeal angle and clinically by adduction of the hallux on the first metatarsal and most commonly occurs after hallux valgus surgery. It's the prevalence has ranged from 2% to 17%. We report a case of hallux varus resulted from weakening of support of lateral soft tissue and resection of an excessive amount of the metatarsal head during a bunionectomy after initial correction of hallux valgus. We corrected the hallux varus deformity using transfer of extensor hallucis brevis tendon with reconstruction os lateral capsule.

  • We report a case of double orifice mitral valve complicating a partial atrioventricular septal defect.The preoperative diagnosis was suggested by a characteristic angiographic and echocardiographic findings but the correct diagnosis of double orifice mitral valve itself was proven by open heart surgery. The smaller accessory mitral orifice was located in the left lateral leaflet and was left untouched. The A-V septal defect was closed with Gore-Tex patch and localized tricuspid annuloplasty was done using Gore-Tex tubular graft for correction of severe tricuspid regurgitation. The anterior mitral cleft left intact had mild mitral regurgitation on postoperative echocardiography.Postoperative course was uneventful and the patient was discharged 2 weeks later.

  • The dilated haustra of colon after esophagocolostomy was a rare complications, but it may be extremely dangerous because of the potential for perforation of the thin- walled esophageal substitute. Therefore a prompt surgical correction of the lesion should be done. The conventional surgical exploration and resection of the lesion site was very difficult. The use of the ends GIA to eliminate the dilated haustra of colon was simple and safe method and the postoperative results were good. We report a case of experience and methods.

  • Objective: This study was to investigate the effect of oriental medical treatment for Structual scolosis. Methods: This patient has scoliosis was treated with Herbal medicine, acupuncture and chuna therapy. And we evaluated the case with radiological measurement by calculating the scoliosis correction angle. Results: After the treatment, the cobb'angle of T-spine was changed 15" to 3". Conclusions: We report the good result of oriental medical treatment on this case. The more study of oriental medical treatment for Structual scolosis is needed.

  • Two-dimensional cephalometric analysis has been used for diagnosis and treatment of correction of mandibular asymmetry by many maxillofacial surgeons. And 2D analysis showed excellent results in many cases, however 2D has some drawbacks in diagnosis and treatment planning because of its fundamental limitation like overlapping. Today many physicians use 3D diagnosis & treatment tools to expect better results and reduce possible errors. The aim of this report is to present treatment procedures using 3D analysis and treatment modalities for mandibular asymmetry patients.

  • Congenital coronary arteriovenous fistula is a ~are condition, and with widespread use of cardiac catheterization, angiography and selective coronary arteriography is being recognized with increasing frequency. Surgical correction is strongly recommended to prevent the development of congestive heart failure,angina, subacute bacterial endocarditis, myocardial infarction, and pulmonary hypertension, as well as coronary aneurysm formation with subsequent rupture or embolization. I report a case of congenital coronary arteriovenous fistula of the left main coronary artery to the fight atrium in a 23 year old female, which is associated with bacterial endocarditis with right atrial vegetation.

  • A current interest in chemistry concerns traceability of analytical measurements to the International System of Units (SI) and the proper estimation of their uncertainties in accordance with the internationally agreed guide provided by the International Organization for Standardization (ISO). Isotope dilution mass spectrometry (IDMS) is regarded as a primary method, which make the measurement results traceable to SI units without significant empirical correction factors. Our laboratory, as the national standards institute of Korea, participated in an intercomparison of environmental analysis, pp'-DDE in corn oil, which was organized by the CCQM under supervision of the CIPM to test feasibility of IDMS as a primary method for the trace analysis of organic compounds. In this report, we provide basic equations used for the calculation of the concentration of the analyte in a sample and a precise description of the processes for the evaluation of the uncertainties of the measurement results. Also, we report the experimental conditions adopted to improve the accuracy of the IDMS measurement. The principles contained in ??Guide to the Expression of Uncertainty in Measurement'' provided by ISO are followed for the uncertainty evaluation.

Clinical application of maxillary tissue bone-borne expander and biocreative reverse curve system in the orthodontic retreatment of severe anterior open bite with transverse discrepancy: A case report

  • Anterior open bite and transverse discrepancy are often accompanied by hyperdivergent skeletal patterns. In addition, degenerative joint disorders and vertical maxillary excess contribute to an unfavorable convex facial profile with a retruded chin. Correction of this complex three-dimensional problem with orthodontic treatment alone is considered challenging owing to anatomical limitations. Moreover, a history of orthodontic treatment with premolar extraction makes retreatment difficult. This case report illustrates the application of a maxillary tissue bone-borne expander and biocreative reverse curve system in a 23-year-old female patient with a severe anterior open bite and transverse discrepancy who underwent orthodontic treatment with four premolar extractions. By setting the treatment target under precise diagnosis and using appropriate appliances, a satisfactory treatment result could be achieved without orthognathic surgery.

  • The need for repairing removable dentures has grown as the population had aged. The direct methods allow existing dentures to be repaired without interrupting their use. However, if patient compliance is low, direct methods may be challenging. Moreover, attaching an artificial tooth to a metal base is a complicated procedure because it necessitates casting a retentive element and soldering it to a metal base. This clinical report describes how to add an artificial tooth to a metal base, reline denture bases, and reestablish occlusion on relined removable dentures using indirect methods. Existing removable dentures were successfully repaired and their service life was efficiently extended using the methods described.
