The Effects of Alphabet Exercise with SSP Electrotherapy on Lateral Ankle Sprain (족관절 염좌에 SSP 전자침 요법을 병행한 동기침법의 효과)

  • The multiple acupuncture techniques have been used to relieve the pain of lateral ankle sprain. Recently, early functional rehabilitation is emphasized in rehabilitative concepts using active ROM, facilitated proprioception, alphabet exercise and so forth. Similarly, in oriental rehabilition medicine, traditional movement technique called 'Dong-Ki' has been used to relieve pain of ankle sprain. So we used 'Dong-Ki' technique with SSP(silver spike point) electrotherapy transformed from alphabet exercise and evaluated the effects in method of randomized controlled trials. 27 outpatients with lateral ankle sprains were randomized into two groups : 15 samples and 12 controls. Sample group was treated with 'Dong-Ki' technique(writing his name with foot) with SSP electrotherapy to relieve pain during additionally. 'Dong-Ki'. Outcomes were measured by 10cm VAS and ankle circumference(cm). In terms of VAS, Sample group had inclination of pain relieving but not significantly meaningful. In terms of ankle circumference measurement, Sample group showed significantly improved.

12 Cases Study of Facial Nerve Palsy Using Oriental Medical Treatment with Taping Therapy (테이핑 요법을 병용한 한방치료로 호전된 안면신경마비 환자 치험 12례)

  • This study was performed to evaluate the effect of Taping Therapy with oriental medical treatment ing facial nerve palsy. This clinical study was carried out 12 cases of facial nerve palsy patients which had been admitted in Dong-Shin university Sun-Cheon oriental hospital from November, 2004 to July, 2006. In this study the patients were treated by Taping Therapy. Other treatments were acupuncture, herbal medication, rehabilitative therapy, H-B scale(House-Brackmann facial nerve grading system) was measured from admission to discharge. In the results, the symptoms were improved gradually. The average of H-B scale was increased from 2.5 to 6.71. The treatment period is more shortened compared to the existing period(12weeks). In the study, the treatment of facial nerve palsy using oriental medical treatment with Taping Therapy was more effective, especially H-B scale and treatment period, compared to existing.

The Effects of Breast Reconstruction Using Latissimus Dorsi Myocutaneous Pedicled Flap for Lymphedema (광배근근피판을 이용한 유방재건술이 림프부종에 미치는 영향)

  • Lymphedema is one of the most common complications of mastectomy. It decreases quality of life and causes functional or aesthetical problems in post-mastectomy patients. Axillary lymph needs dissection (ALND), and radiation therapy(RTx) is known as the representative factor of lymphedema. Authors discovered that breast reconstructions using latissimus dorsi(LD) myocutaneous flap decrease the incidence of lymphedema in spite of these risk factors. Therefore we compared the incidence of lymphedema between the patients who underwent breast reconstructoins by LD pedicled flap, and the patients who did not undergo breast reconstructions from January 2002 to December 2004. Lymphedema was diagnosed when difference of arm circumference was over 2cm or limitation of joint movement was greater than 20 degrees. Overall incidence of lymphedema was 14.0%, and it was 18.9% in case of ALND, and 21.1% in case of RTx, respectively. But the incidence of breast-reconstructed patients using LD pedicled flap was 3.3%. This result reveals that LD pedicled flap decreases incidence of lymphedema significantly. In the future, it is recommended to identify the causes of decrease in the incidence of lymphedema in case of breast reconstructed by LD myocutaneous pedicled flap, for example lymphoscintigraphy and so on.

Quality of Life Comparison between Suspicion Hearing Impaired and Hearing Aids Users in Elderly (노인에서 난청 의심자와 보청기 사용자의 삶의 질 비교)

  • Purpose : Aim of the this study was conducted to determine whether hearing aids are responsible for the quality of life by comparing quality of life between the elderly who are suspected of difficulty in hearing selected by Hearing Handicap Inventory of Elderly (HHIE) without using hearing aids and who are using hearing aids. Methods : For the study, two groups of 33 people were analyzed for independent t-test based on whether they wore Hearing Aids (HA) or not whom wearing hearing aids (hearing aids users) and suspected difficulty in hearing. who are attending the Religious organization Senior Welfare Center in the OO region. Result : The quality of life of hearing aid users is generally observed to be higher than that of non-users in general, and by the section, psychological (p=0.019) and living environment (p=0.032) were appeared statistically signigicant. The elderly showed a significant difference. In the section, "Operation Satisfaction (p=0.038)" in the physical health area, "relationship of reason (p=0.018)" in the social relationship area, "Economic margin (p=0.021)" in the environmental area, "Information accessibility (p=0.020)", In the psychological area, hearing aid was used for "self satisfaction (p=0.033)", "concentration of work (p=0.045)". Conclusion : The use of hearing aids by hearing loss elderly improves the quality of life in the psychological health and living environment section.

The Study of Asymmetrical of the Serratus Anterior and Lower Trapezius Muscles in Chronic Stroke Patients (만성 뇌졸중 환자의 앞 톱니근과 하부 등세모근의 비대칭성 연구)

  • PURPOSE: The purpose of this study was to compare the muscle architecture of serratus anterior and lower trapezius using rehabilitative ultrasound imaging (RUSI) in affected and unaffected side of chronic stroke patients. METHODS: The participants were thirty five patients with stroke hemiplegia in this study. RUSI was used to measure the muscle thickness of the serratus anterior and lower trapezius muscles. We compared the muscle thickness according to affected side and sex, determined the reliability of the measurement image. Independent t-test, intra-class correlation coefficient (ICC) and standard error of measurement (SEM) were used for statistical analysis. RESULTS: Significant difference in muscle thickness of serratus anterior was observed between affected and unaffected side (p<.001). Muscle thickness according to gender showed a significant difference in unaffected side of serratus anterior (p<.05). Compare asymmetry ratio of serratus anterior and lower trapezius muscle thickness showed a significant difference (p<.001). ICC for intra-reliability was .944~.962 in serratus anterior and .873~.925 in lower trapezius muscle thickness, respectively. SEM was .001~.004 in serratus anterior and .002~.008 in lower trapezius muscle. CONCLUSION: This study, using RUSI, showed significant difference in muscle thickness of serratus anterior in affected and unaffected side of stroke patients. RUSI is a practical tool for measuring soft-tissue thickness in the scapular region muscle of stroke.

Influences of Personality Traits on Depression and Quality of Life in Stroke Outpatients Receiving Rehabilitation Therapy (성격특성이 통원 재활치료를 받는 뇌졸중 환자들의 우울과 삶의 질에 미치는 영향)

  • Purpose : The aim of this study was to investigate the influences of personality traits on depression and quality of life of stroke outpatients receiving rehabilitation therapy. Methods : The current study included 52 outpatients who received occupational therapy in a rehabilitative and general hospital located in Incheon. Their personality traits were assessed using the Big Five Inventory-Korean Version (BFI-K), and depression was measured using the Beck Depression Inventory (BDI). Quality of life was evaluated using the Stroke Specific Quality of Life (SS-QOL) scale. The collected data were analyzed to identify the effects of personality traits on depression and quality of life using a multiple regression analysis. Result : Neuroticism and depression were significantly correlated (p<.05), and neuroticism and agreeableness were significantly correlated with quality of life (p<.05). Additionally, depression and quality of life were significantly correlated (p<.05). Neuroticism affected depression significantly ($R^2=.503$, p<.05). Among the five personality traits, neuroticism, agreeableness, and extraversion had a significant effect on quality of life ($R^2=.474$, p<.05). Conclusion : These findings suggest that personality traits need to be considered in order to improve the quality of life of stroke patients.

Two Patients with Dysphagia after Stroke who were Treated with Pyungwesan and Bowhawhan (평위산(平胃散)과 보화환(保和丸)으로 호전된 연하장애 환자 2례)

  • Dysphagia is an important complication for the rehabilitative treatment and life quality of stroke patients. This report is about two patients with dysphagia after stroke who were treated with Pyungwesan and Bowhawhan. The severity of the patient's dysphagia was evaluated with stage of dysphagia according to severity. After treatment, dysphagia were remarkably improved(from stage 5 to 2, 5 to 1). This result shows that Pyungwesan and Bowhawhan could be effective against dysphagia after stroke.

Meditation for Alzheimer's Disease: Systematic Review and Meta-Analysis

  • Objectives: Interest in the use of complementary and alternative treatments to treat dementia. Meditation is used to treat various symptoms of physical and psychological diseases. Some studies suggest that meditation might have positive effects on cognitive functions, especially attention, in the elderly. However, how meditation affects Alzheimer's disease (AD) patients remains unclear. In this review, we assessed the effectiveness of practicing meditation in combination with standard care in AD. Methods: We searched the CCRCT, MEDLINE, EMBASE, AMED and CINAHL databases on 30 May 2017. We included randomized controlled trials (RCTs) that used meditation in adult patients diagnosed with AD. We allocated patients to a meditation combined with standard care or a standard care-only group. Results: The two RCTs met the inclusion criteria. A total of 98 patients were included in the meditation with standard care and standard care-only groups in this review. All meditation programs in the included trials were based on practicing mindfulness. The results of our meta-analysis indicatedthat adjunctive mindfulness meditation programs exerted favourable but non-significant effects on cognitive function on the Mini Mental State Examination (MMSE) (MD=4.68, 95% CI -0.11 to 9.46; Z=1.92, p=0.06). Only one study assessed depression, anxiety, quality of life and stress. No adverse events related to meditation were reported in the included studies. Conclusions: Insufficient data iscurrently available to determine the effectiveness of practicing meditation on patients diagnosed with AD. Hence, further RCTs with high methodological quality and larger sample sizes are needed to effectively estimate the effects of meditation on AD.

A Study of the Anti-Stress Effects of Chijasitang in Mice (생쥐에서 치자시탕(梔子豉湯)의 항스트레스 효과에 대한 연구)

  • Objectives: Chijasitang (CJST) has been prescribed for patients with anxiety disorder, and is believed to treat insomnia. The present study was carried out to investigate whether Fructus Gardeniae, Fermented Semen Sojae Preparatum and CJST have beneficial effects on treating anti-stress. Methods: The mice were divided into the following groups: normal, control, positive control, and experimental. Then, the experimental groups were further divided into the Fructus Gardeniae group (FG), the Fermented Semen Sojae Preparatum group (FS), the Chijasitang water extract group (CJSTW), and the Chijasitang 30% ethanol extract group (CJSTE). In the positive control group, a single dose of imipramine ($500{\mu}g/g$) was applied to the mice. In the experimental groups, each single dose of FG, FS, CJSTW and CJSTE ($50{\mu}g/g$) was applied to the mice. After one hour, the mice in all groups except the normal group were exposed to immobilization stress. Their body weights, phagocytosis, nitric oxide in macrophage, corticosterone, and adrenocorticotropic hormone (ACTH) were observed. Results: Fructus Gardeniae, Fermented Semen Sojae Preparatum and Chijasitang prevented weight loss and suppressed the decrease of phagocytosis in the mice after their exposure to immobilization-stress. Although they did not influence the amount of NO, they suppressed the increment of ACTH and corticosterone in the immobilization-stressed mice. Conclusions: According to the results of this study, they suggest that Fructus Gardeniae, Fermented Semen Sojae Preparatum and Chi-Ja-Si-Tang all have some anti-stress effects.

Effect of TJGB on the liver of high-fat diet-fed mice and the viability of HepG2 cells (고지방식이를 급여한 마우스의 간과 HepG2 세포에서 TJGB의 효과에 대한 연구)

  • Objectives: This study was performed to investigate the effect of TJGB on the liver of high-fat diet (HFD)-fed mice and the cell viability of HepG2 cells. Methods: After a week adaptation, 8-week-old C57BL/6N mice were fed with a 45% HFD or normal diet for 3 weeks. For the next 9 weeks, the mice were divided into 6 groups: normal diet group; HFD group; HFD plus orlistat group; HFD plus Ephedra sinica Stapf (ES) group; HFD plus low dose of TJGB group; HFD plus high dose of TJGB group. To estimate the effect of TJGB in the liver of HFD-fed mice, the protein expressions of phospho-acetyl-CoA carboxylase (p-ACC) and liver X Receptor (LXR) were determined by Western blot assay. The cell viability of ES and TJG was also evaluated in HepG2 cells. Results: The administration of TJGB had little effect on the protein expressions of p-ACC and LXR in the liver of HFD-fed mice. And the cytotoxicity was showed above 7.8 ㎍/mL in HepG2 cells. Conclusion: Further research is needed to evaluate the mechanism of TJGB on hepatic steatosis and cytotoxicity in HepG2 cells.
