Breast Cancer Screening and Repeat Screening (유방암 조기검진수검 및 반복수검 실태와 관련요인)

  • Purpose: This study was to investigate the characteristics and related factors of breast cancer screening and repeat screening, and to propose nursing interventions to increase the rate of breast cancer screening and repeat screening. Method: Study subjects was 236 women residing in the community, teachers and nurses who were older than 45 were recruited. Data was collected with self administered questionnaires from July 1st to August 31st, 2003 and analysed using SPSS/WIN 10.0 with $X^2$test, t-test, and stepwise multiple logistic regression at a significant level of =.05. Result: The breast cancer screening rate was 57.2%, and repeat screening rate was 15.3%. With the multiple logistic regression analysis, factors associated with mammography screening were age and perceived barriers of action, and factors related to the repeat mammography screening were education level and other cancer screening experience. Conclusion: Based on the results, we recommend the development of anintervention program to decrease the perceived barrier of action, to regard mammography as an essential test in regular check-up, and to give active advertisement and education to the public to improve the rates of breast cancer screening and repeat screening.

Esophageal Plexiform Schwannoma -A case report- (식도 얼기형 신경집종 -1예 보고-)

  • A 39-year-old man presented with a esophageal submucosal tumor on regular check up examination. Preoperative exams showed the typical submucosal tumor as leiomyoma and operative procedure was enucleation for complete resection. Postoperative pathologic diagnosis including histologic and immunohistochemical study was compatible with plexiform schwannoma. Plexiform schwannoma is one of the least common variant of schwannoma that typically shows a plexiform or multinodular pattern. To our knowledge, there is rare case report of plexiform schwannoma originating in the esophagus and we report ore case with related literature.

Coracoclavicular Screw Fixation for AC Dislocation and Unstable Distal Clavicle Fracture (견봉 쇄골 탈구와 쇄골 외측단 골절에서 일시적 오구 쇄골간 나사못 고정술)

  • Purpose: There are so many different methods with the their own advantages and disadvantages for acute AC dislocation and distal unstable clavicle fracture. We evaluated the usefulness of temporary Bosworth's coracoclavicular fixations using the cannulated screw and reviewed our clinical results. Materials and Methods: We evaluated 34 cases of temporary coracoclavicular fixations with cannulated screw and washer for acute AC dislocation(25cases in the Type V, Ⅲ) and distal unstable clavicle fracture(9cases in the Type ⅡB, Ⅱ+Ⅲ). All cases were fixed by the same techniques, guided cannulated screw under C-arm after the repair of the CC ligament within 1 week. Bases on the regular check, screws were removed at the 6-12 weeks under local anesthesia. The final clinical and radiological results(average 14 months follow-up) were rated with the UCLA scale. Results : All shoulders were gained nearly full ROM passively at the 3-4 weeks. The overall satisfactory resuIts(excellent or good) were achieved 88%(22/25) in the acute AC dislocation and all in the distal unstable clavicle fracture. There were no definite complications except three initial fixations loosening. Conclusion: Temporary Bosworth's coracoclavicular fixation using the cannulated screw with guided pin was simple precedure and provided enough initial stability for acute AC dislocation and distal unstable clavicle fracture.

  • Various mucosal lesions are originated in oral cavity and trauma is most common cause of these lesions. Definitive treatment of these lesions is stop of traumatic events and removal. There are two representative removal methods in medical fields; conventional mess or LASER. Compare to conventional mess technique, LASER has several advantages such as bleeding control, pain reduction. Of the various LASER systems, $CO_2$ LASER is regarded as best choice for general practitioners due to its convenience, universal use and high cost/benefit effects. In these article, we report the treatment cases of mucocele, fibroma, and leukoplakias using $CO_2$ LASER. Eventhough there are malignancy potential of oral mucosal lesion especially leukoplakias, careful history taking, clinical examination, and regular check-up will help the general practitioners to manage these lesions. Simple and relatively safe oral mucosal lesions have to be treated more in general dental practitioners.

Predictors of Intention to Quit Smoking among Woman Smokers in Korea (한국 흡연여성의 금연의도 영향요인)

  • Purpose: This study was done to explore the predictors asssociated with Korean women smokers' intentions to quit smoking. Methods: This study was a cross-sectional study including 3,578 women smokers. Data from the 2010 Community Health Survey conducted by the Korea Centers for Disease Control and Prevention were examined and analyzed using SPSS 21.0 for multiple logistic regression. Results: Of the women smokers 52.2% reported having an intention to quit smoking but only 3.9% had received education in smoking cessation. In the logistic analysis, factors associated with intention to quit smoking were age (Odds ratio and 95% [confidence interval]: 0.97 [0.96-0.98]), health-related quality of life (OR=2.39 [1.14-5.03]), perceived stress (OR=1.11 [1.00-1.22]), marital status (OR=0.93 [.86-1.00]), age of starting to smoke (OR=1.14 [1.04-1.24]), number of cigarettes per day (OR=0.84 [0.76-0.93]), exposure to smoking-quitting campaigns (OR=1.48 [1.22-1.80]), previous attempts at weight-control (OR=1.37 [1.15-1.63]), frequency of alcohol use (OR=1.16 [1.01-1.34]), hypercholesterolemia (OR=1.62 [1.18-2.24]), experiences of trying to quit smoking (OR=4.04 [3.45-4.73]), and regular medical check-up (OR=1.13 [1.03-1.43]). Conclusion: Identifying factors associated with the intentions to quit smoking provides possibilities for shaping effective policies and programs to increase smoking cessation among Korean women.

"Study for the orthotics & shoes of diabetic among Korean adult" (한국 성인에서의 당뇨병성 족부 질환의 신발 및 족부 질환 연구)

  • Purpose: To provide baseline data for prevention and education of diabetic foot in Korea. Materials and Methods: 216 diabetic patients are included in this survey. The average age is 58.64 year old with average 9.92 years of diabetes mellitus periods. Among them, 41% used insulin injection, 42% oral medication, 14% both and 3% diet were used for their blood sugar control. The diabetic foot survey was done. Results: The survey of patients symptom and disease of their feet results: 49% had sensory decrease in their feet. Associated disease in foot were callus(15%), heel pain(10%), bunion(4%), toe deformity(2%), Tinea pedis were 9%, tingle sensation were 17%, and swelling on feet were 2%, The survey of patients privious history of foot problems are: 8% had previous operation history on their feet and 17% had history of infection on their feet, 23% had bullae history, The survey of patients prevention and education are: Those who have had more than one diabetic foot education were 45.7%, and among them 24% had routine regular foot check up. Only 5% wear special diabetic shoe and 3% had custom orthotics. Conclusion: Interests to diabetic foot care is increasing in diabetic patients in Korea. However still we need more education, and efforts for prevention. Special education and diabetic shoe is still low, The survey data could be good baseline for diabetic prevention and education.

Subjective symptoms in musculoskeletal and preventive actions of Dental technicians in Daegu Metropolitan City (대구지역 치과기공사의 근골격계 자각증상과 예방행위)

  • Purpose: This report intend to provide basic materials of Musculoskeletal diseases prevention program development and preventive measures establishing by surveying Musculoskeletal symptoms. Musculoskeletal disease and Musculoskeletal disease prevention act about dental technician in Daegu. Methods: From October. 2012 to November. 2012 by conducting self - survey using questionnaire and use unit490 final analysis data. Results: In dental Technicians, as the musculoskeletal symptoms back or waist pain rate appear such as 28.2% and as the musculoskeletal disorders, the shoulder region pain complaint rate appear highly such as 87.4%. Musculoskeletal disease prevention act was highly appeared man more than women, the married than the unmarried, over 10 years dental experience, fewer working hours per day, the average monthly income is greater regular medical check-up dental technician than technician who do not. As working environment, In case working in laboratory whch dust is well ventilated and with a barrier, Musculoskeletal disease prevention act rate was highly appeared more than another technician who do not. Conclusion: We must prepare an effective preventive measures. To development Musculoskeletal disease prevention program suitable for dental technic working and make obliged to practice.

Importance and Performance of Workplace Health Promotion Program in Occupational Nurses (산업간호사의 산업장 건강증진사업에 대한중요성 인지도 및 실천정도)

  • Purpose: This research was to investigate the level of importance and performance of daily-activity and environment management related to workplace health promotion and knowledge and opinion of law and regulation related to workplace health promotion. Methods: The subjects were 218 occupational nurses all over the country. Questionnaires were made by the researcher. Data were collected from September 18 to October 20, 2006 and analyzed by SPSS WIN 12.0. Results: Prevention of accident and emergency treatment got the highest score in the awareness of importance. Regular check-up got the highest score in performance level. Providing and wearing of the personal protective equipments got the highest score both importance and performance level. In occupational nurses' knowledge about law and regulation, ratio of correct answer was 55 percentages that was low intellectual level. The mandatory employment of occupational health nurse or occupational nurse in 50 - 500 person workplaces got the highest score in revision opinion of law and regulations. Conclusion: Result of these studies will provide baseline data for selecting the order of priority according to awareness of importance of daily-activity, environmental management and law and regulation in the occupational health promotion and operating health promotion program.

Psychosocial Factors and Health Behaviors in Elderly People

  • The main purposes of this study were to investigate the content of health behaviors and to examine factors influencing the health behaviors of the elderly. Data regarding the health behavior of 126 people over 65 years of age living in community settings were used. All subjects were interviewed using a semi-structured questionnaire. The questionnaire consisted of sociodemographic variables, health behaviors, present chronic disease, subjective health status, a scale for worthy life, a scale for self-esteem, and a scale for loneliness. Health behaviors included 27questions on diet, exercise, stress-coping, smoking, drinking, a regular medical check-up, social activities, etc. The data analysis procedure included stepwise regression using health behavior as the dependent variable, and sociodemographics, illness, and psychosocial variables as independent variables. Stepwise regression revealed that factors such as feelings of worthy life($\beta$=-0.350, p<0.0001), communicating with otjers or the lack there of ($\beta$=0.183, p<0.05), and self-esteem($\beta$=0.196, p<0.05) were independently and significantly associated with health behaviors. For example, individuals who showed higher levels of worthy life and who had confiding relationships with others tended to practice more health behaviors. Subjects who had a higher level of self-esteem showed the same tendency. These results suggest the necessity of a intervention that considers psychosocial aspects should be included in care of the elderly so as to promote positive health behavior.
