Effects of Yijin-tang-gamibang Extracts on Reflux Esophagitis (역류성 식도염에 대한 이진탕가미방(二陣湯加味方) 효과 연구)

  • The object of this study was to observe the protective effects of Yijin-tang-gamibang (YJGMB), Yijin-tang has been traditionally used as Korean medicine for treating various digestive diseases. The study showed that it is effective on reflux esophagitis induced by pylorus and forestomach ligation in rats. Three different dosages of Yijin-tang-gamibang extracts, 300, 150 and 75 mg/kg, were once orally administered 1 hour before pylorus ligation. 6 groups, each of 8 rats per group were used in the present study. The results were compared with Omeprazole, antioxidant and proton pump inhibitor, 30 mg/kg treated group. Yijin-tang-gamibang extracts 300 mg/kg were showed similar or slightly lower effects as compared with 30 mg/kg of omeprazole in the present study. Detail mechanism studies should be conduced in future with the efficacy tests of individual herbal composition of Yijin-tang-gamibang and the screening of the biological active compounds in herbs.

Predictive Markers for Screening Renal Damage in Children with Urinary Tract infections and Vesicoureteral Reflux

  • Purpose: Urinary tract infections (UTIs) are the most common and serious bacterial infections in children. Therefore, early diagnosis of vesicoureteral reflux (VUR) for treatment planning and the identification of noninvasive markers that can predict renal injury are important in patients with UTIs. We analyzed the clinical features of pediatric UTIs commonly encountered by general practitioners and reinterpreted the blood tests and imaging findings to identify the important clinical predictive markers of VUR in order to selectively perform VCUG. Methods: This retrospective study was performed among 183 children diagnosed with a UTI or acute pyelonephritis. Results: The most significant predictor of high grade and bilateral VUR identified using area under the curve analyses was hydronephrosis on kidney ultrasound images with renal cortical defects on dimercaptosuccinic acid (DMSA) kidney scan simultaneously, followed by hydronephrosis only on kidney ultrasound. Conclusion: The presence of hydronephrosis on kidney ultrasound images or cortical defects or asymmetric kidneys on the DMSA kidney scans can be predictive markers of VUR, reducing the need for VCUG. Our study can thus help minimize the exposure to radiation among patients through selective VCUG.

A Review on Clinical Studies of the Acupuncture and Moxibustion Therapy for Gastroesophageal Reflux Disease in China by Searching CAJ (CAJ 검색을 통해 살펴 본 중국에서의 역류성 식도질환 침구치료 임상연구 동향에 대한 고찰)

  • Objectives : The purpose of this study was to review the acupuncture and moxibustion therapy for gastroesophageal reflux disease(GERD) in China. Methods : The literatures were searched using the database-China Academic Journals(CAJ)(~2015.1.). Clinical studies of acupuncture and moxibustion therapy for GERD such as randomized controlled trial(RCT), non-randomized controlled trial(NRCT), case series, or case report were included. Results : Thirty studies met our inclusion criteria: 24 RCTs, 4 NRCTs, 1 case series, 1 case report. Various kinds of treatment such as body acupuncture, abdominal acupuncture, acupuncture therapy with finger on back-shu point, electroacupuncture, acupoint injection, auricular acupuncture, and moxibustion were used in the clinical studies. The treatment groups take herbal medicines at the same time in 16 studies, and proton pump inhibitor(PPI) and histamine 2 receptor antagonist were used as a comparative treatment in the most of studies. Acupuncture or moxibustion treatments were done daily or 2~3 times a week for 5 days~18 weeks. All of studies reported higher effective rate in the acupuncture or moxibustion treatment group compared to the western medication control group. Conclusions : The results of this study could be used for the future practice and the clinical research about the acupuncture and moxibustion therapy for GERD.

Laparoscopic Nissen Fundoplication and Collis Gastroplasty (복강경을 이용한 니센 위저추벽성형술 및 콜리스 위성형술)

  • The prevalence of gastroesophageal reflux disease has been increased recently in Korea. The use of minimally invasive laparoscopic and thoracoscopic surgery has become popular in the operation of esophageal disease such as esophageal cancer or gastroesophageal reflux disorder. We experienced three cases of laparoscopic Nissen fun-doplications and one case of laparoscopic Collis gastroplasty, and we will describe the technical aspect of these surgeries.

Antioxidant Properties of Water and Aqueous Ethanol Extracts and Their Crude Saponin Fractions from a Far-eastern Sea Cucumber, Stichopus japonicus

  • Water and 70% ethanol extracts obtained from a sea cucumber (Stichopus japonicus) body wall by heat reflux or pressurized solvent extraction showed 2,2-diphenyl-1-picrylhydrazyl ($DPPH^{\cdot}$) and 2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) radical cation ($ABTS^{{\cdot}+}$) scavenging activities comparable to those of fruits and vegetables. The highest activities were observed for the water extract from heat reflux extraction. Crude saponins exhibited higher radical scavenging activities than the soluble matters in the extracts. However, they were responsible for only about 3 to 15% of the scavenging activities of the extracts. Total phenolic contents showed a significant correlation with $DPPH^{\cdot}$ scavenging activities, suggesting a significant contribution of phenolic constituents to the antioxidant properties of the extracts. However, total flavonoid contents showed little correlation with the radical scavenging activities. The results suggest that the water or 70% ethanol extract obtained from sea cucumber body wall by simple heat reflux extraction could provide considerable antioxidant benefits.

Surgical Management of Gastroesophageal Reflux Disease (소아 위식도 역류 질환의 외과적 치료)

  • Eleven children with gastroesophageal reflux disease underwent fundoplication. Eight had neurological impairment, two had hiatal hernias and one had a history of esophageal repair for esophageal atresia. The most common and significant symptom was vomiting(81.8 %), followed by recurrent respiratory infections(72.7 %) and failure to thrive(72.7 %). The most common diagnostic tool was 24 hour esophageal pH study, which showed pH less than 4 for more than 10 % of the total recorded time in 6 of 9 patients. Nissen fundoplication was performed in 10 patients. Thal fundoplication was carried out in one patient with esophageal atresia. Stamm gastrostomy was added for nutritional and/or swallowing problem in all 8 patients with neurological impairment. The median follow up period was 13 months. There was one late death of unrelated cause and one recurrence. The quality of life after antireflux surgery was greatly improved. Antireflux surgery should be done if indicated, and a simultaneous gastrostomy considered in a patient with neurological impairment.

Post Test Analysis to Natural Circulation Experiment on the BETHSY Facility Using the MARS 1.4 Code

  • The present study is to assess the applicability of the best-estimate thermal-hydraulic code, MARS 1.4, for the analysis of thermal-hydraulic behavior in PWRs during natural circulation conditions. The code simulates a natural circulation test, BETHSY test 4. la, which was conducted on the integral test facility of BETHSY. The test represented the cooling states of the primary cooling system under single-phase natural circulation, two-phase natural circulation and the reflux condensation mode with conditions corresponding to the residual power, 2% of the rated core power value and 6.8 MPa at the secondary system. Based on MARS 1.4 calculations, the major thermal-hydraulic behaviors during natural circulation are evaluated and the differences between the experimental data and calculated results are identified. The calculated results show generally good behavior with regard to the experimental results; the region of single-phase natural circulation is 100-92% of the initial mass inventory, two-phase natural circulation is 84-63 %, and the reflux condensation mode occurred below 58 %. U-tubes empty and the core uncovery are obtained at 39 % and 34 % of the initial mass inventory, respectively.

Effects of Physiological Active Substance Extracted from Silkworm Fece

  • Silkworm (Bombyx mori ) feces have long been used in the pharmaceutical and food industries as a natural colorant. However, there is limited data on the bioactive compounds that constitute silkworm feces. This research emphasizes the antioxidant activity of different solvent and flavonoid extracts of silkworm feces. The solvents were ethanol, butanol, and water, while the methods utilized included ultrasonification, stirrer, reflux, and reflux after ultrasonification extraction. Results showed that butanol ultrasonification extraction (BUE) yield the lowest extraction (1.75%), while the other methods yielded 7 to 14%. The total polyphenol content utilizing BUE was 3.3 mg TAE/g, while water ultrasonification extraction (WUE) yielded the highest extraction rate with 51.6 mg TAE/g. The total flavonoid content was significantly higher using ethanol reflux extraction (EUE) at 266.8 mg QRE/g BUE, which was 158.3 and 151.3 mg QRE/g. Both DPPH radical scavenging activity and SOD-like (superoxide dismutase) activity, showed significant antioxidant effects. Finally, all other extracts except for BUE had a-glucosidase inhibition at 60%. Therefore, an effective extraction method for physiologically active substances must be selected.

Achalasia Previously Diagnosed as Gastroesophageal Reflux Disease by Relying on Esophageal Impedance-pH Monitoring: Use of High-Resolution Esophageal Manometry in Children

  • Gastroesophageal reflux disorder (GERD) is the most common esophageal disorder in children. Achalasia occurs less commonly but has similar symptoms to GERD. A nine-year old boy presented with vomiting, heartburn, and nocturnal cough. The esophageal impedance-pH monitor revealed nonacidic GERD (all-refluxate clearance percent time of 20.9%). His symptoms persisted despite medical treatment for GERD, and he was lost to follow up. Four years later, he presented with heartburn, solid-food dysphagia, daily post-prandial vomiting, and failure to thrive. Endoscopy showed a severely dilated esophagus with candidiasis. High-resolution manometry was performed, and he was diagnosed with classic achalasia (also known as type I). His symptoms resolved after two pneumatic dilatation procedures, and his weight and height began to catch up to his peers. Clinicians might consider using high-resolution manometry in children with atypical GERD even after evaluation with an impedance-pH monitor.

  • Our patient is the first reported case of Prader-Willi syndrome(PWS) with bilateral Hutch's diverticula. We believe that hypotonia in PWS is associated with multiple bladder diverticula and vesicoureteral reflux. We conclude that PWS can be considered a possible associated syndrome in patients with multiple bladder diverticula.
