Critical Review of health care economic evaluation methodology : With a special reference to study design and cost estimation (보건의료 경제성 평가 방법론 고찰 -연구 설계와 비용 추정을 중심으로-)

  • Cost containment has become high political issues since financial crisis of the Korean Health Insurance fund in 2000. Korean Government has developed and implemented several measures to reduce the pharmaceutical expenditures. Pharmaceutical economic evaluation can be a tool in decision to allocate scare resource efficiently. In order to increase the quality of economic evaluation for pharmaceuticals, the Korean Health Insurance Review Agency(HIRA) is considering the development of a guideline for economic evaluation. It mandates that pharmaceutical companies could submit the result of an economic evaluation when demanding reimbursement of new pharmaceutical drugs. The purpose of this study is to provide a critical review of the economic evaluations of health care technologies published in the Korean context whether they have been performed according to current guidelines and therefore whether their results are directly useful for decision making. We found there exist important problems and deviation from, good practice' both in the general features of the studies, like the study design and perspective, and in terms of cost measurement and valuation. There are needs to develop clear guidelines and to educate and train researchers in performing economic evaluations.

Practical Evaluation of Intellectual Capital (IC) Measurement Tool for Contract Foodservice Management Company (위탁급식전문업체 지적자본 측정도구의 운용시험 평가)

  • The purposes of this study were to a) measure the IC identified of CFMC (contract foodservice management company) ,b) examine IC circumstance of CFMC, c) evaluate practically IC measurement tool of CFMC, and d) present information for selecting an adequate CFMC to clients. The questionnaires of IC measurement were handed out to 108 CFMCs, there composing of main office employees, foodservice managers, customers, and clients of 207 school,38 hospital, and 86 husiness/industry foodservices. The statistical data analysis was completed using SPSS Win (ver 12.0) for descriptive analysis, t-test, Mann-Whitney U test. First, CFMCs had operational experience for an average of 8 years and 8 months, and served an average of 38,540 meals a day. Most of the respondent companies specialized in the school foodservice field and managed an average of 66 clients for the contract period of 2 years and 3 months. Second, the respondent companies had gotten a score of 77.78 points for the total average, 77.7 points in the large enterprise group and 78.1 points in the small and medium-sized enterprise group. Therefore, the minimum number of points for the accrediting license on Qualification is suggested to be over 70 out of a 100 point scale; this study would be serve as reference for the certification license on qualification. On the level of evaluation category, the scores were 14.15 to 20 points on $\ulcorner$finance$\urcorner$, 19.24 to 25 points on $\ulcorner$customer$\urcorner$, 19.33 to 25 points on $\ulcorner$process$\urcorner$, 14.31 to 20 points on $\ulcorner$human resource$\urcorner$, and 8.6 to 10 point on $\ulcorner$renewal and development$\urcorner$ . $\ulcorner$Renewal and development$\urcorner$ and $\ulcorner$customer focus$\urcorner$ received better grades than other evaluation categories. Third, $\ulcorner$Finance$\urcorner$ indicated similar distribution overall. Small and medium-sized companies had lower grades than large companies on 'market ability' of $\ulcorner$customer$\urcorner$ , but, clients of small and medium-sized companies had higher grade for 'client satisfaction' than large companies. Most of the companies supported 'infrastructure support for foodservice operation' of $\ulcorner$process$\urcorner$ by the main office of CFMCs, but, the branch chain offices of CFMCs were not applied efficiently. Large companies made more effort to improve the 'employee ability' of $\ulcorner$human focus$\urcorner$ than small and medium-sized CFMC. The 'research and development cost' of $\ulcorner$renewal and development$\urcorner$ was increased compared to the previous year. In conclusion, if CFMCs were to perform self-evaluation and a routine checkups by utilizing CFMC's IC measuring tool, improvements in CFMC operational capacities as well as foodservice quality can be noted.   • This paper describes the controller for the improving speed control of the AC servo motor. The microprocessor provides an output to the difference in command. the servo system improves the characteristics of speed control. When the motor is running at the same speed as set by the reference signal, the speed encoder also provides a signal of the same frequency. Thus, the microprocessor controlled digital techniques enable to realize the flexible performance and control which was possible with time constant of linear acceleration/deceleration. We can know that optimal speed of machining center is 75msec in 30000mm/min and actually, 75msec is using on machining center. Finally experimental results prove excellent performance of this control system. This can be reduced error with more exact measure of actual speed. The system can be adaptable to CNC machine.

  • Data Envelopment Analysis(DEA), a non-parametric productivity analysis tool, has become an accepted approach for assessing efficiency in a wide range of fields. Despite of its extensive applications, some features of DEA remain bothersome. DEA offers no guidelines to where relatively inefficient DMU(Decision Making Unit) improve since a reference set of an inefficient DMU consists of several efficient DMUs and it doesn't provide a stepwise path for improving the efficiency of each inefficient DMU considering the difference of efficiency. We aim to show that DEA can be used to evaluate the efficiency of System Integration Projects and suggest the methodology which overcomes the limitation of DEA through hybrid analysis utilizing DEA along with machine learning.

  • The role of archives and records centers is expanding, however many potential users are not aware of the importance of the records and archives. As a method to improve the archival information services to users, in this study, we developed chatbot that supports the interaction between the users and the archives and records centers. Thus we examined the specific development procedures and methods, analyzed user requests and questions of the archives and records center of Myongji university as a case, and conducted a logical structure design for chatbot development. After building the chatbot based on IBM Watson Conversation and Kakaotalk messenger, we were able to find how the chatbot interacted with the users through a pilot run. Based on the experience of developing information service chatbot, the implications related the introduction of the chatbot were suggested which include determination of the level of the chatbot, analysis of the user requests, selection of the tool for the chatbot, and syntax setting for the conversational interaction.

Expanding and Improving FRBR Conceptual Model through FRBRoo (FRBRoo 분석을 통한 FRBR 개념모형의 확장과 개선)

  • In this study, based on the analysis of FRBRoo, we tried to propose suggestions to expand and improve the FRBR family conceptual model. FRBRoo is a plug-in ontology of CIDOC CRM with cooperation of museum field. As FRBR family models also revised and integrated into IFLA Library Reference Model, the additional analysis on IFLA LRM was performed. If bibliographic information is required to support the technical and user services of the library, the way to analyze the bibliographic information should be improved in order to cope with the new challenges faced by the library. To do this, time-related event concepts should be reflected in the modeling of bibliographic information. It is also necessary to expand the creation and exchange unit of bibliographic information to smaller units or larger units than legacy bibliographic records. Using FRBRoo as a linkage tool for the sharing of bibliographic information is also suggested.

Study on Readers about Library and Information Science Fields' Articles by Analyzing Mendeley (Mendeley를 통한 문헌정보학 주요 분야 연구 논문의 독자 분석)

  • With reference management tool based on web, we could understand not only impact about non-author, such as hand-on worker, educator, who are out of academia, but also trace the subject fields of readers and their status. This study by analyzing mendeley, understand what kinds of subject fields and status of readers read library and information science field articles. As a result of analyzing the status and the major of the reader, readers were distributed in the fields of business administration, education and so on, and according to the reader's major, there was a significant difference (p = .000) between the subject area of relatively read a lot. By the way, as the result of relational analysis between citation rate and numbers of mendeley readership about medeley saved articles, correlation coefficient shows 0.585, however as the result of relational analysis limiting the groups, in case of author group who tends to read the articles for citing, correlation coefficient shows 0.619. On the other hand, non-author group shows 0.384.

  • Conventionally, scientific data visualization is thought of as one of activities performed by scientists during the scientific data analysis. However, recently, there exits a set of research papers which count scientific data visualization as a independent research area. They show the research subjects for studying the scientific data visualization technology and methods. In case, a scientist or group of scientists can not solve their own visualization problem due to the unskillfulness and inexperience on using visualization tool. Therefore, it needs to help them by the systematic way for solving the problem. In this study, we analyze and propose the national level scientific visualization support system for scientists. In particular, we first analyze the existing papers and find out the critical success factors. Then, by integrating the findings of the analysis, we propose the research areas which need to be focused, and the strategic direction and specific research topics for scientific data visualization support system in national level.

A Study on Establishment of the Optimal Target Exposure Index for Skull Radiography Based on Diagnostic Reference Level (진단참고수준 기반 두부 방사선검사의 최적 목표노출지수 설정에 관한 연구)

  • The International Electrotechnical Commission (IEC) 62494-1 has defined the exposure index (EI) that have a proportional relationship with the dose incident on the image receptor, and target exposure index (EIT), deviation index (DI). In this study, an appropriate EIT for skull radiography was established through the diagnostic reference level (DRL) and changes in DI were confirmed. Entrance surface dose (ESD) and EI were obtained using the computed radiography system displayed the EI as per IEC on console and skull phantom by experiment based on the national average exposure conditions announced in 2012 and 2019. And appropriate EIT was established by applying the DRL in 2012 and 2019. As a results, the EIT is changed according to the change in the DRL, and the exposure condition that becomes the ideal DI according to the change in the EIT also has a difference of about 1.41 times. DRL is recommended to optimize the patient dose, however it is difficult to measure in real time at medical institutions whereas EI and DI are displayed on the console at the same time as exposure. When the EIT is set based on the DRL and the DI is closed to an ideal value, it is useful as a patient dose management tool. Therefore, when the EIT is periodically managed along with the revision of the DRLs, the patient dose can be optimized through the EI, EIT and DI.

Developing Activity Based Clinical Pathway for Patients with Total Hip Replacement (활동기준관리를 적용한 인공고관절 전치환술 Clinical Pathway 개발)

  • Purpose : The purpose of the present study is to develop a clinical pathway applied with activity-based management methodology for efficient clinical management to cope with rapid changing medical environments. Method : After making a preliminary pathway based on which a conceptual frame of reference was established to develop a clinical pathway, the final one was confirmed by verifying experts validity and clinical validity. The ultimate activity-based clinical pathway was restructured after clarifying, schematizing and analyzing the whole activities of clinical pathway in accordance with the conducting process of activity-based management. Result : A clinical pathway for total hip replacement was developed, in which the vertical axis consisted of assessment, examination, consultation, medication, treatment, diet, activity, and education, and the horizontal axis was composed of six days of hospitalization. Then, on the basis of the development, the clinical pathway including the improved contents and information was restructured after making the reform measure by analyzing each of activities in the pathway. And the list of contents related to the clinical management activity was made, which was described its main contents and the pre-activities that ought to be completed before conducting each of activities in the pathway. Conclusion : The clinical pathway applied with activity-based management may be used as a standard guidance for providing continuous and consistent patient care. It will provide the information for nursing activities to nurses and the management information about hospital and nursing activities to the hospital administrators. It will also be used as a tool for communication between medical staff. Besides, it will contribute to creating profits for the hospital by shortening the length of stay in patients.
