Study on the Relationship between Family Support and Functional Recovery in Cerebrovascular Accident Patients with Rehabilitation Treatment of Oriental Medicine (한방재활치료를 받은 뇌졸중 환자가 지각한 가족지지와 기능회복에 관한 연구)

  • Functional recovery of cerebrovascular accident (CVA) patients were studied by examining functional independence measure (FIM) to evaluate the functional state of the patients at admission to and at discharge from the hospital and its relationship with the family support. Study subjects consisted of 129 CVA patients, who were admitted and received rehabilitation treatment at K Medical Center of Oriental Medicine from August 3 to December 18, 1997. The results were as follows: 1) Total FIM score was $72.37{\pm}25.16$ at admission and $101.67{\pm}22.13$ at discharge. The difference of average score was 29.30, which was statistically significant by paired t-test. 2) The largest difference between FIM scores at admission and at clischarge was observed in items of walking and wheel-chair riding, and the smallest clifference in items of social interaction. 3) The recovery was faster with motor function than with cognitive function, because the difference of FIM scores at admission and at discharge was much larger with motor function. 4) Recovery was better in groups under age 49 than in groups above age 70. Functional recorvery was prominent especially in groups with normal sensory state and speech functions, and groups without urinary incontinence. Recovery was less significantly in patients with paraplegic patients hospitalized longer than 2 months, patients with family all the time, and patients with CVA over 11 days. 5) We could not find any relationship between functional recovery and family support. FIM scores were lower in groups of old age(r=-0.325), long stayed in hospital (r=-0.426), and long period of time after the onset of disease(r= -0.339) with a reciprocal correlation between FIM scores and these parameters. 6) Stepwise multiple regression analysis was done to evaluate factors to affect the recovery from CVA. FIM score at admission could explain 51.2 % of the functional recovery. Important factors were periods of hospitalization, state of sensory function, age, and education (listed in decreasing order of importance). In total, they could explain 64.89% of the functional recovery. These results indicate that functional recovery of CVA patients, who were admitted to oriental medicine hospital for rehabilitation treatment, could be estimated by measuring FIM scores. Recovery was significantly better at discharge from the hospital than at admission and motor function recovery rate was much faster than that of cognitive function. 2. Recommendation Based on these results, we recommend following further studies. 1) Comparative study of recovery of motor function and of sensory function would be necessary by measuring FIM scores once a week to evaluate the recovery of CVA patients. 2) It would be interesting to see whether there is any difference of functional recovery between patients treated with either western medicine or oriental medicine. 3) Psychological factors affecting the recovery of CVA patients need to be studied.

Effects of Empowerment and Family Function on the Depression of Firefighters (소방공무원의 임파워먼트와 가족기능이 우울에 미치는 영향)

  • This study aimed to provide help to seeking a preventive method of intervention in firefighter's depression by investigating empowerment and family function level of firefighter's on their depression. For this, the study conducted purposive sampling survey among 351 firefighters at Gyeonggi area. As a result of the research, all of empowerment and family function had a significant effect on depression of firefighters. Based on these findings, the study demonstrated the potential for applying each individual's empowerment as an internal resource for their prevention of mental health. The study suggested that diverse counter measures are needed to boost family bond and family functionality of firefighters, and also proposed positive adoption and assistance for preventive and effective implementation of 'work and life balance' policy to derive the recovery of healthiness of families.

Cupping Therapy Combined with Rehabilitation for the Treatment of Radial Palsy: a Case Report

  • This case report demonstrates the beneficial effects of cupping therapy (CT) in a 35-year-old man who is diagnosed with a fracture of the radial shaft due to a motorcycle accident. One year after the treatment started, pseudoarthrosis developed in the radius and an autogenous iliac bone graft was performed. However, extension dysfunction in the wrist became evident. After another 6 months of physical therapy and rehabilitation, no improvements were observed. Therefore, CT and adjunctive electrostimulation were performed, after 30 days of treatment, marked recovery of muscle function and full wrist extension were observed, as determined by electromyography and a grade 5/5 on the Medical Research Council power of wrist extension scale. The results in this case study suggest that CT in conjunction with adjunctive electrostimulation, may accelerate functional recovery from postoperative radial palsy, and provide a useful alternative treatment in this situation.

Loss of hsp70.1 Decreases Functional Motor Recovery after Spinal Cord Injury in Mice

  • Heat shock proteins (HSPs) are specifically induced by various forms of stress. Hsp70.1, a member of the hsp70 family is known to play an important role in cytoprotection from stressful insults. However, the functional role of Hsp70 in motor function after spinal cord injury (SCI) is still unclear. To study the role of hsp70.1 in motor recovery following SCI, we assessed locomotor function in hsp70.1 knockout (KO) mice and their wild-type (WT) mice via the Basso, Beattie and Bresnahan (BBB) locomotor rating scale, before and after spinal hemisection at T13 level. We also examined lesion size in the spinal cord using Luxol fast blue/cresyl violet staining. One day after injury, KO and WT mice showed no significant difference in the motor function due to complete paralysis following spinal hemisection. However, when it compared to WT mice, KO mice had significantly delayed and decreased functional outcomes from 4 days up to 21 days after SCI. KO mice also showed significantly greater lesion size in the spinal cord than WT mice showed at 21 days after spinal hemisection. These results suggest that Hsp70 has a protective effect against traumatic SCI and the manipulation of the hsp70.1 gene may help improve the recovery of motor function, thereby enhancing neuroprotection after SCI.

Effects on Recovery Oriented Integrated Rehabilitation Program for Chronic Schizophrenic Patients (만성정신분열병 환자를 위한 회복지향의 통합재활프로그램의 효과)

  • This study was to assess the effects of a recovery oriented integrated rehabilitation program on cognitive social function, rehabilitation motivation, and mental health recovery. This program is involved motivation enhancement program with 4 subprograms composed of cognitive rehabilitation, emotion management, vocational rehabilitation, family education program. The subjects were 34 patirents with schizophrenia who had been hospitalized at National Mental Hospital on local area. The study was a non-equivalent control group non-synchronized design with two groups, an experimental group(18 patients) and a control group(16 patients). The collected data were analyzed using SPSS 12. 0. The results of the data show that there was not a significant increase in the symptom, cognitive and social function. But there was a significant increase in readiness of rehabilitation(maintenance, p=.027) and mental health recovery (p=.039). Short term recovery oriented integrated rehabilitation programs in inpatient settings are more available for motivation of rehabilitation and mental health recovery than symptom reduction or functional improvement. Therefore there should be more efforts to create more recovery oriented service provision in clinical settings.

A Study on the Public Services for Families in Crisis - focused on the Family empowerment program at the Healthy Families Support Center - (위기가족지원 서비스 운영 실태에 관한 연구 -건강가정지원센터의 가족역량강화사업을 중심으로-)

  • The purpose of this study is to evaluate the current performance of the public services for families in crisis by analyzing the family empowerment service in Healthy Families Support centers. We analyzed performance data of the family empowerment service provided by 25 Healthy Families Support Centers from 2011 to 2013. The results are as follows; First, the number of families in crisis which received public services from the family empowerment service by the Healthy Family Support Centers in 2013 were less than the number in 2011, but increased from 2012. Second, according to the types of crisis, school violence was the most service needed family crisis in 2011, and it was suicide in 2012 and 2013. Third, in the specific services in emergency support for families and family function recovery program, Psychological and emotional support services were the most offered services during 3 years. Accordingly, efficiency of the programs and services in terms of budget is higher than that of any other services. Fourth, analysing the evaluation results of amily empowerment services in 2014, we found that its network is still heavily weighted in certain side by the lack of the utilization and the foundation of the network.

Recovery-Key Attacks against TMN-family Framework for Mobile Wireless Networks

  • The proliferation of the Internet of Things (IoT) technologies and applications, especially the rapid rise in the use of mobile devices, from individuals to organizations, has led to the fundamental role of secure wireless networks in all aspects of services that presented with many opportunities and challenges. To ensure the CIA (confidentiality, integrity and accessibility) security model of the networks security and high efficiency of performance results in various resource-constrained applications and environments of the IoT platform, DDO-(data-driven operation) based constructions have been introduced as a primitive design that meet the demand of high speed encryption systems. Among of them, the TMN-family ciphers which were proposed by Tuan P.M., Do Thi B., etc., in 2016, are entirely suitable approaches for various communication applications of wireless mobile networks (WMNs) and advanced wireless sensor networks (WSNs) with high flexibility, applicability and mobility shown in two different algorithm selections, TMN64 and TMN128. The two ciphers provide strong security against known cryptanalysis, such as linear attacks and differential attacks. In this study, we demonstrate new probability results on the security of the two TMN construction versions - TMN64 and TMN128, by proposing efficient related-key recovery attacks. The high probability characteristics (DCs) are constructed under the related-key differential properties on a full number of function rounds of TMN64 and TMN128, as 10-rounds and 12-rounds, respectively. Hence, the amplified boomerang attacks can be applied to break these two ciphers with appropriate complexity of data and time consumptions. The work is expected to be extended and improved with the latest BCT technique for better cryptanalytic results in further research.

A Study on the Recovery Process of Career Capital for Rehabilitation Protection Recipient (법무보호대상자의 진로자본 회복과정에 관한 연구)

  • It is necessary for inmates released from prison to recover their career capital in order to function normally in society. Failure to adapt to the society may not only make them unhappy but could also aggravate social safety nets. Therefore, it is critical to examine the recovery of career capital. For this study, we conducted in-depth interviews of 10 rehabilitation recipients who were supported by Korea Rehabilitation Agency. For qualitative analysis, several career topics were analyzed, such as obstacles in recovering career capital, efforts in overcoming these obstacles, what is helpful for them to overcome the obstacles and what is required for them to adapt to the career life and others. Analysis results found that the rehabilitation beneficiaries suffered loses to their career capital due to their incarceration, suffered from the loss of pride and became despondent in the process of recovery of career capital due to the negative social perception toward their social self, fear of social stigma and devaluation of themselves. In addition, the fear of possible negative evaluation was a critical factor in making their social activities more challenging. Conversely, it was found that social support in the recovery process of career capital has a positive effect. In particular, emotional support from spouses, employers, family members and friends are the most positive factors. Concerning efforts at adapting to careers, it was found that an active, leading and positive attitude toward the career were critical, showing that positive evaluation and recognition in family and social relations and the experience of success in the career were positive elements in enhancing a sense of pride. In summary, it was found that in the recovery process of career capital, dysfunctional beliefs had a negative effect, while social support had a positive effect, on the recovery of career capital.

A Key Recovery Attack on HMAC using Fault Injection Attack (오류 주입 공격을 이용한 HMAC에 대한 키 복구 공격)

  • At FDTC'05 and CISC-W'10, the authors showed that if they decrease the number of rounds of AES and Triple-DES by using the fault injections, it is possible to recover the secret key of the target algorithms, respectively. In this paper, we propose a key recovery attack on HMAC by using the main idea of these attacks. This attack is applicable to HMAC based on MD-family hash functions and can recover the secret key with the negligible computational complexity. Particularly, the attack result on HMAC-SHA-2 is the first known key recovery attack result on this algorithm.

Effects of Pre-eccentric Exercise on Exercise Induced Muscle Damage (사전 원심성 운동이 운동 유발 근 손상에 미치는 효과)

  • Purpose: This study examined the effects of pre-eccentric exercise to the quadriceps for the prevention of delayed onset muscle soreness (DOMS) and recovery of muscular function, depending on the training intensities. Methods: Subjects were divided into one of three groups that control group, a low intensity eccentric exercise group (LIEE group) and a high intensity eccentric exercise group (HIEE group). Subjects who underwent pre-eccentric exercise undertook exercise at an intensity of 25% and 75% of maximal voluntary contractions, respectively. After undertaking pre-eccentric exercise for eight weeks, eccentric exercise was applied again to induce DOMS. Measurements were conducted to examine pain and muscular function changes before, immediately after, after the induction of DOMS, and at the first, third, fifth and seventh days after the induction of DOMS. Results: Subjects who underwent pre-eccentric exercise showed a significant difference from the control group for the changes in the visual analogue scale (VAS) pain threshold, pressure pain threshold and muscle thickness by isometric contraction from measuring DOMS, and in particular, pain threshold by isometric contraction had remarkable effect in the LIEE group of subjects. For the change of the root mean square values using mechanomyography (MMG) as the measurement of muscular function recovery, subjects who had undertaken pre-eccentric exercise group showed a significant difference as compared to the control group. Conclusion: Pre-eccentric exercise was very effective in preventing and recovering delayed onset muscle soreness and was helpful to prevent and recover from decreased muscular function. The difference based on the intensity of exercise was not great, but it was more effective in the low intensity eccentric exercise group of subjects. Therefore, it can be considered that pre-eccentric exercise has a high application value as a physical therapy intervention for prevention and rehabilitation of sports injuries.
