Purpose: Maxillofacial trauma predominantly affects young adults between 20 and 40 years of age. Although radioprotection is a legal requirement, the significant potential of dose reduction in computed tomography (CT) is still underused in the clinical routine. The objective of this study was to evaluate whether maxillofacial fractures can be reliably detected and classified using ultra-low-dose CT. Materials and Methods: CT images of 123 clinical cases with maxillofacial fractures were classified by two readers using the AOCOIAC software and compared with the corresponding results from post-treatment images. In group 1, consisting of 97 patients with isolated facial trauma, pre-treatment CT images at different dose levels (volumetric computed tomography dose index: ultra-low dose, 2.6 mGy; low dose, <10 mGy; and regular dose, <20 mGy) were compared with post-treatment cone-beam computed tomography (CBCT). In group 2, consisting of 31 patients with complex midface fractures, pre-treatment shock room CT images were compared with post-treatment CT at different dose levels or CBCT. All images were presented in random order and classified by 2 readers blinded to the clinical results. All cases with an unequal classification were re-evaluated. Results: In both groups, ultra-low-dose CT had no clinically relevant effect on fracture classification. Fourteen cases in group 2 showed minor differences in the classification code, which were no longer obvious after comparing the images directly to each other. Conclusion: Ultra-low-dose CT images allowed the correct diagnosis and classification of maxillofacial fractures. These results might lead to a substantial reconsideration of current reference dose levels.
Background: Food consumption is one of the most important routes for radionuclide intake for the public; therefore, there is the need to have a comprehensive understanding of the amount of radioactivity in food products. Consumption of radionuclide-contaminated food could increase potential health risks associated with exposure to radiation such as cancers. The present study aims to determine radioactivity levels in some food products (milk, rice, sugar, and wheat flour) consumed in Mali and to evaluate the radiological effect on the public health from these radionuclides. Materials and Methods: The health impact due to ingestion of radionuclides from these foods was evaluated by the determination of activity concentration of radionuclides 238U, 232Th, 40K, and 137Cs using gamma spectrometry system with high-purity germanium detector and radiological hazards index in 16 samples collected in some markets, mall, and shops of Bamako-Mali. Results and Discussion: The average activity concentrations were 9.8±0.6 Bq/kg for 238U, 8.7±0.5 Bq/kg for 232Th, 162.9±7.9 Bq/kg for 40K, and 0.0035±0.0005 Bq/kg for 137Cs. The mean values of radiological hazard parameters such as annual committed effective dose, internal hazard index, and risk assessment from this work were within the dose criteria limits given by international organizations (International Commission on Radiological Protection and United Nations Scientific Committee on the Effects of Atomic Radiation) and national standards. Conclusion: The results show low public exposure to radioactivity and associated radiological impact on public health. Nevertheless, this study stipulates vital data for future research and regulatory authorities in Mali.
Internal dosimetry is a discipline which brings together a set of knowledge, tools and procedures for calculating the dose received after incorporation of radionuclides into the body. Several steps are necessary to calculate the committed effective dose (CED) for workers or members of the public. Each step uses the best available knowledge in the field of radionuclide biokinetics, energy deposition in organs and tissues, the efficiency of radiation to cause a stochastic effect, or in the contributions of individual organs and tissues to overall detriment from radiation. In all these fields, knowledge is abundant and supported by many works initiated several decades ago. That makes the CED a very robust quantity, representing exposure for reference persons in reference situation of exposure and to be used for optimization and assessment of compliance with dose limits. However, the CED suffers from certain limitations, accepted by the International Commission on Radiological Protection (ICRP) for reasons of simplification. Some of its limitations deserve to be overcome and the ICRP is continuously working on this. Beyond the efforts to make the CED an even more reliable and precise tool, there is an increasing demand for personalized dosimetry, particularly in the medical field. To respond to this demand, currently available tools in dosimetry can be adjusted. However, this would require coupling these efforts with a better assessment of the individual risk, which would then have to consider the physiology of the persons concerned but also their lifestyle and medical history. Dosimetry and risk assessment are closely linked and can only be developed in parallel. This paper presents the state of the art of internal dosimetry knowledge and the limitations to be overcome both to make the CED more precise and to develop other dosimetric quantities, which would make it possible to better approximate the individual dose.
Some natural products are able to inhibit radiation effects and exert an antitumor effect with fewer adverse reactions; however, their antitumor effects are less than those of widely-used synthetic drugs. Propolis is a natural material that has been attracting attention, and we extracted this material with water and investigated the effect of continuous propolis administration on radioactivity-induced reduction of hemocytes, in addition to the antioxidant and antitumor effects of propolis. Following a 1-week adjustment period, water-soluble propolis was administered intraperitoneally to male ICR mice at a dose of 100 mg/kg every other day for 2 weeks. Following administration, 2 Gy whole-body irradiation was performed and the counts of leukocytes, lymphocytes, and granulocytes and monocytes in the peripheral blood were determined 1, 3, 7, 15 and 30 days after irradiation. These cells were considered since they are closely associated with immunity to radioactivity. In a second experiment, water-soluble propolis was similarly administered to the mice for 2 weeks after a 1-week adjustment period, and 2 Gy whole-body irradiation was performed. The antioxidant effects in hemocytes were then investigated using 2,2'-azobis (2-amidinopropane) dihydrochloride (AAPH), a radical generator. In a third experiment, $1\;{\times}\;10^6$ Sarcoma-180 cells were inoculated into the right thigh of mice, which were divided into four groups: control, water-soluble propolis-treated, 6 Gy irradiated and water-soluble propolis-treated + 6 Gy irradiated groups, and changes in tumor size were measured for 20 days. Statistical analysis was conducted using ANOVA for multiple groups. In the three experiments, administration of water-soluble propolis inhibited the reduction of hemocytes caused by whole-body irradiation, showed antioxidant effects against radioactivity, and inhibited tumor growth, respectively. In conclusion, our data suggest that the antioxidant effect of watersoluble propolis inhibits hemocyte reduction caused by whole-body irradiation and enhances immunological inhibition of tumor growth.
본 연구팀에서는 방사선으로부터 위장관과 면역 조혈계를 보호하기 위하여 당귀, 천궁, 백작약으로부터 새로운 생약복합물 HIM-I을 개발한 바 있다. 본 연구에서는 방사선 방호뿐 아니라 다양한 생체질환의 예방에 있어 면역조혈기능이 중추적인 역할을 한다는 관점에서, HIM-I로부터 면역조혈 기능을 더욱 증진시킨 생약복합조성물 HemoHIM을 제조하여 그 효능을 검증하였다. HIM-I를 에탄을 침지하여 에탄을 분획(HIM-I-E)과 조다당 분획 (HIM-I-P)을 얻은 후, HIM-I 에 조다당 분획을 첨가하여 HemoHIM을 제조하였다. 이렇게 얻은 HemoHIM, HIM-I 및 각 분획에 대하여 재생조직 및 면역계 방호와 회복촉진 효과를 비교 검증하였다. HemoHIM 과 HIM-I는 시험 관내에서 방사선에 의한 DNA 손상을 유의 적으로 억제하고 수산화 라디칼을 소거하는 효과가 있음이 관찰되었으며, HemoHIM과 HIM-I는 거의 비슷한 활성을 나타내었다. 시험 관내 면역 세포 활성화와 골수세포 성장촉 진 실험에서는 HemoHIM이 HIM-I 비하여 높은 활성을 보였으며, 이는 HIM-I에 비해 높은 조다당 함량에 기인한 것으로 보인다. 감마선 조사 마우스를 이용한 생체 보호효과를 살펴본 결과, HemoHIM은 HIM-I와 비슷한 정도의 소장 움 생존율 증가효과를 보였으나, 내재성 비장 조혈세포집락 형성 시험에서는 HemoHIM은 HIM-I보다 높은 효과를 나타내었다. 또한 방사선조사 마우스에서 HemoHTM의 투여는 방사선 조사후 급격히 감소된 말초 혈액내 백혈구 및 림프구수의 회복을 촉진시키고, 생존율을 증대시키는 효과가 관찰되었다. 이상의 결과들은 생약복합물 HIM-I 조다당 분획을 첨가하여 개발한 새로운 생약복합조성물 HemoHIM이 방사선에 의해 유발된 위 장관 및 면역계 조직의 손상을 감소 시켜 생존을 증가시키는 효과가 있음을 제시하였다. 특히 HemoHIM은 HIM-I와 비교하여 재생조직의 산화적 손상억제 효과는 비슷하게 유지되면서도 면역 조혈세포 방호 및 회복촉진 효과가 높은 것으로 관찰되어 방사선 방호제로서 뿐만 아니라 면역조혈기능 증진제로서 유용하게 활용될 수 있을 것으로 사료된다.
거대핵세포(megakaryocyte)에서 유래된 혈소판은 백혈구와 더불어 방사선에 민감하여 가장 먼저 감소하므로 방사선장해의 지표로 활용된다. 따라서 혈소판이 부족하게 되면 점상출혈(dot hemorrhage)이 계속되고 혈구 수의 감소와 면역기능 저하로 이어진다. 특히, 전신조사(whole body irradiation)로 4~6 Gy를 받을 경우, 3주 후 혈소판이 최하로 감소되어 출혈과 빈혈에 의한 조혈기계 장해의 사망원인이 된다. 따라서 본 연구는 혈소판 장해의 메카니즘을 밝히고 방어효과를 알아보고자 하였다. 실험에 사용한 방어물질은 마늘 성분의 알리인(Alliin)을 사용하였으며, rat의 꼬리 정맥에 투여한 후 전자현미경(TEM)으로 관찰하였다. 연구 결과, 5 Gy 방사선조사(Irradiation) 후 10일 경과된 혈소판 조직에서는 세포막이 심하게 손상되어 풍선모양으로 부풀어 올랐으며, 당질층(glycocalyx)이 비후되었다. 조사 후 20일 경과된 혈소판에서는 소기관들의 형태를 알아볼 수 없을 정도로 손상되었으며, 세포질의 내용물이 간질부위로 유출되어 손상이 매우 심하였다. 또한 과립들의 형태도 관찰되지 않았다. 30일 경과된 혈소판에서는 가운데 부분에서 공포(vacuole)가 생기고 조직의 손상이 관찰되었다. 하지만 Alliin을 정맥 투여한 실험군에서는 과립(granules)들의 형태가 대조군과 같이 일정하게 유지되고 있었으며, 세포막의 손상도 확인되지 않았다. 따라서 Alliin의 혈관투여시 혈소판의 방사선방어 효과를 확인할 수 있었다.
To establish the fundament for EBM of Traditional Korean Medicine, the papers on Samul-tang which was frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international literatures. The papers were classified by the registration of domestic or international journals, by the year of publishment, by experimental methods, by laboratory animals used in biological experiment and by the kinds of studies on biological efficacy. Of total 67 papers on Samul-tang, 58 volumes were registered in domestic journals and 9 volumes were in international journals of which 8 volumes were in SCI journals. Since 1978, publishments of papers have continuously increased. The papers on instrumental analyses were 6, biological studies were 58 volumes, clinical studies were 3. Instrumental analyses were preceeded with standard compounds(gallic acid, albiflorin, paeoniflorin, benzoic acid, ferulic acid, 5-HMF). And biological studies showed improvement of cardiovascular function and circulation, antianemia, brain protection, immunoregulation, antistress, radioprotection, antifatigue, antiinflammation and antiallergy, antioxidative effect. Through clinical studies, antifatigue, improvement of insomnia and osteoporosis were reported. Samul-tang could be used to tonify and activate blood. And further study on clinical field need to be conducted in accordance with biological study.
방사선 피폭에 의한 간조직의 장해는 경화성 위축으로 복수가 발생하고 황달과 피로감이 수반된다. 이러한 장해에는 간세포의 형태학적 변화가 동반되고 소기관들의 손상이 연관되어 있다. 본 연구는 이러한 소기관들의 형태적 손상을 알아보고자 하였다. 특히 에너지 대사와 관련된 미토콘드리아와 세포질그물 핵막을 중심으로 관찰하였으며, 방사선 방어제 연구와 병행하였다. 방어제는 혈류 증가 작용이 있는 알리인을 투여하였다. 세포 관찰은 투과전자현미경을 활용하였다. 실험결과, 7 Gy 전신 조사에서 간세포의 핵막이 비후되고, 소기관들 주변에 염증성 변화가 관찰되었다. 20일 경과된 세포에서는 과산화소체와 미토콘드리아 막이 손상되었고 세포질에서 공포가 확인되었다. 30일 경과된 세포에서는 거대한 지방세포들이 관찰되었고. 조면소포체에는 리보솜이 분리된 것을 확인하였다. 알리인 투여 후 간세포에서는 핵막 주변에 많은 수의 조면소포체들이 선명하게 관찰되었고 핵막도 비후되지 않았다. 세포질에는 공포화되거나 염증성 증후들이 관찰되지 않았다. 미토콘드리아의 막손상도 없었으며 글리코겐도 정상적으로 확인되었다. 따라서 일부 방어효과가 있는 것으로 확인되었다.
방사선에 대한 방호와 면역기능 조절을 목적으로 새로운 생약복합조성물인 HemoHIM을 개발하였다. 식품 원료로 사용 가능한 생약재 3종 당귀, 천궁, 백작약의 에탄올 분획을 열수추출물에 첨가하여 HemoHIM을 제조하였다. HemoHIM의 항알레르기 효과를 검증하기 위하여 사람 비만세포주 HMC-1을 사용해 compound 48/80으로 유도되는 히스타민 분비량과 PMA/A23187로 유도되는 염증성 사이토카인의 분비량을 측정하였다. 히스타민의 양은 형광분석법으로, 염증성 사이토카인 IL-6, IL-8, TNF-$\alpha$, GM-CSF의 양은 효소결합 면역측정법으로 측정하였다. 저농도의 HemoHIM에 의해 히스타민 분비량이 억제되었고, 모든 농도에서 IL-6, TNF-$\alpha$, GM-CSF의 분비량은 억제되었지만 IL-8은 고농도에서만 억제되었다. 사이토카인의 mRNA 발현량은 HemoHIM의 농도 의존적으로 억제되었다. 그리고 c-kit와 Fc$\varepsilon$RI의 mRNA 발현량도 모든 농도에서 억제되었지만, tryptase의 mRNA 발현량은 저 농도에서만 억제되었다. 이상의 결과로 HemoHIM이 비만세포의 활성을 억제하는 효과가 있다는 것을 알 수 있었으며, 상대적으로 독성이 적은 항알레르기 제재로 개발할 가능성을 제시한 것으로 생각된다.
본 연구는 프로폴리스가 방사선에 조사된 랫드의 여러 조직에 미치는 방사선 방어 효과를 조사하였다. 혈장성분 중 백혈구의 수는 방사선 조사 20일 경과 후에 방사선만 조사한 실험군보다 프로폴리스 섭식 실험군에서 높은 증가를 나타냈으며, 혈장 성분 중 GOT의 활성도는 GPT에 비해 낮아지고 있다. 간 조직 내 미량원소 중 Mg, Fe, Ca, Mn, Cu, Mo, Ni, As의 함량은 대조 군에 비해서는 증가하였으나 방사선만 조사한 실험군에 비해서는 낮은 수준을 유지하고 있다. 즉 프로폴리스는 백혈구 수의 회복에 효과적이며 방사선으로 인한 간세포의 손상을 보호해 줌으로서 미량원소의 방출을 억제하고 GOT의 활성을 낮춰주는 것으로 생각된다. 조직학적으로 방사선조사 전에 프로폴리스를 섭식하면 정소에서는 훼손된 미분화 세포의 수가 감소하였고, 소장에서도 손상된 배상세포와 점막근판 조직의 형태가 부분적으로 정상적인 형태를 갖추는 것으로 관찰되었다. 따라서 프로폴리스의 섭식은 방사선으로부터 직접적으로 조직을 보호하거나 훼손된 세포의 회복에 영향을 미치는 것으로 조사되었다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
