Radioiodine Therapy in a Patient with Papillary Thyroid Carcinoma associated with Breast Uptake; Hyperprolactinemia due to Empty Sella Syndrome (유즙분비가 있는 갑상선암 환자에서 I-131 치료)

  • We report a 37 year-old-female patient with papillary thyroid cancer treated by surgery who demonstrated residual thyroid and bilateral breast uptake on a diagnostic I-131 whole body scan. She had an extrathyroidal extension needing I-131 ablative therapy. Her galactorrhea was investigated and treated with low doses of bromocriptine prior to I-131 therapy. Her galactorrhea was due to the decreased secretion of PIF induced by empty sella.

Comparison of Diagnostic and Post-therapy Radioiodine Scan in Well-Differentiated Thyroid Cancer and the Clinical Outcome (갑상선암 환자에서 방사성옥소 진단스캔과 치료 후 스캔의 비교 및 임상경과)

  • Purpose: We compared the first postoperative diagnostic and post-therapy scans of patients who received therapeutic doses of I-131, to investigate the difference in clinical outcomes between patients with concordant findings of diagnostic and post-therapy scans and patients with discrepant (more lesions in post-therapy scan) findings. Materials and Methods: The first postoperative diagnostic and post-therapy radioiodine scans of one hundred forty three patients with well differentiated thyroid carcinoma were reviewed. Diagnostic scans were obtained following ingestion of 185 MBq of I-131 and post-therapy scans were obtained after therapeutic dose of $3.7{\sim}9.3$ GBq of I-131. Successful ablation was defined as no radioiodine uptake on diagnostic radioiodine scan and normal range of serum thyroglobulin level (<10 ng/ml) during serum TSH elevation. Results: Discrepant scan findings were noted in 25 (17.5%) patients. Twenty-two patients (15.4%) showed more lesions in post-therapy scan and 3 patients (2.1%) showed stunning effect. Nine (64.3%) of 14 patients with distant metastasis revealed metastatic lesion(s) only on post-therapy scan. Stunning effect was considered as sublethal damage in 1 patient and treatment by a diagnostic dose in 2 patients. Ablation was achieved in 52.4% (75/143) of all patients. Ablation rate and mean cumulative radioiodine dose were not different statistically between concordant and discrepant groups. Conclusion: There were 17.5% difference between diagnostic and post-therapy scan findings when using 185 MBq of I-131 as a diagnostic dose. However, 64.3% of distant metastases were revealed only on post-therapy scan. Ablation rate and mean cumulative radioiodine dose were not different statistically between concordant and discrepant groups. The stunning effect was considered as not only sublethal damage but also treatment by a small diagnostic dose of radioiodine.

  • In order to compare the therapeutic effect as well as side effects between antithyroid therapy and radioiodine therapy in hyperthyroidism, the author evaluated 111 cases of hyperthyroidism which were composed of 57 patients with antithyroid treatment, 23 patients with combined treatment comprising of antithyroid and radioactive iodine ($^{131}I$) and 31 patients with treatment of $^{131}I$ alone. The $^{131}I$ treatment was limited to older age, above 20 years of age, and not employed in patients with pregnancy and lactation. The patients treated with antithyroid belonged to relatively younger age, and also milder in symptoms and thyroid function tests. The remission rate of antithyroid therapy group was 97.8% within 16 months. Among them the early remission rate occured within 4 months was found to be 21.7%. The combined therapy group, having prescribed more severe cases, showed the remission rate to be 75% within 16 months. The remission rate of $^{131}I$ therapy group revealed 90.9% within 16 months. Among them the early remission rate taking place within 4 months disclosed 16.7%. The side effects of antithyroid drug, propylthiouracil, were noted as follows: itching (2.7%) skin rashes (1.3%), and adverse enlargement of goiter (10.7%). The side effects $^{131}I$ therapy were transient hypothyroidism(3.9%) permanent hypothyroidism (7.8%) and itching (2.0%).

  • The number of thyroid diseases treated with radioiodine(I-131) is increasing steadily. The sharp increase in patients who require high dose radioiodine therapy greatly increased the need for new therapy rooms. Accordingly, interest in radiation exposure is rising as well, and is a major psychological stress factor for the patient and those who come in close contact with the patient. This study aimed to minimize the radiation exposure on discharge. Based on various previous reports, the decision for discharge should be individualized depending on many factors related to the patient's living or working environment. Educating patients repeatedly on the importance of sufficient oral hydration, while the adequate amount was relative to the patient's individual condition, greatly lowered the detected radiation measurement within the same admission period. In some cases, the period of admission could be abbreviated.

  • This experiment was performed in order to investigate the effect of radioiodine upon the uterus and the ovary of guinea pigs. The animals were injected single dose of 4. 5 mCi of radioiodine per kg of body weight. They were sacrificed on various time intervals; 1, 7, 14, 28, 42 and 56 days after the injection. The results were as follows: 1. In the ovary, the follicle cells and the ova were degenerated with lapse of time after the injection. 2. In the uterus, hyperplasia of the lamina propria of the endometrium and atrophy of the myometrium were observed in accordance with time after the injection.

  • The TCMI(Three-Compartment Model for iodine) computer code has been developed, which is based on the three-compartment model and the respiratory model recommended in ICRP publication 54. This code is able to evaluate the thyroid burden, dose equivalent, committed dose equivalent and urinary excretion rate as time-dependent functions from the input data: working time and the radioiodine concentration in air. Using the TCMI code, the time-dependent thyroid burdens, the thyroid doses and the urinary excretion rates were calculated for three specific exposure patterns : acute, chronic and periodic. Applicability as an internal dose evaluation method has been assessed by comparing the results with some operational experiences. Simple equations and tables are provided to be used in the evaluation of the thyroid burden and the resulting doses for given I-131 concentration in air and the working time.

  • The author observed the uptake rate of $^{131}I$ by the thyroid gland, and disappearing rate of the $^{131}I$ at the site of the intradermal injection of 0.1ml of 5mCi of $^{131}I$ in normal physiologic saline on 20 patients with hyperthyroidism and on 15 patients with hypothyroidism who visited the Radioisotpe department of the Pusan National University Hospital during the period from January 1967 to Aug. 1968, and during the same period 15 normal persons were examined by the same methods and found. The results were as follows: (1) The disappearance rate of $^{131}I$ at the injected site was highest in the cases of hyperthyrodism. The next highest results were obtained in the normal cases. The lowest results were obtained in the cases of hypothyroidism. (2) The uptake rate of the thyroid gland after intradermal injection of the $^{131}I$ was highest in the cases of hyperthyroidism and the next highest results were obtained in the cases of nomal findings. The lowest results were obtained in the cases of hypothyroidism. (3) In conclusion, the intradermal method of injecting $^{131}I$ by the author is a useful way of testing the function of the thyroid gland.

  • Spatial dose rates of high dose $^{131}I$ therapy patients were Measured Three dimensional (X, Y, Z) distributions. I have constructed geometrical an aluminum support structure for spatial dose meters placed in 5 different heights, 8 different azimuthal angles, 6 different time interval and distance 100 cm from High dose$^{131}I$ therapy patients. when the height of vertical plane Spatial dose distribution is 100 cm, the Spatial dose rates is max and the error range is low. the vertical plane Spatial dose rates was found to be 71.85 ${\mu}Sv/h$ on the average at a distance of 100 cm, height 100 cm, from the patients 24 hours after $^{131}I$ oral administration. I divided 12 patients into two groups. I have analysed group A (drinking 5 L water) and group B (drinking 3 L water) in order to measure decrease spatial dose rates. I have found the spatial distributions of patient dose rates is $44.9{\pm}7.2$ ${\mu}Sv/h$ in group A and $100.3{\pm}8.1$ ${\mu}Sv/h$ in group B by 24 after $^{131}I$ oral administration. the reduction factor was found to be approximately 54 % through drinking 5 L water during 24 hours.

  • The evolution of radioiodine 131I from a sodium peroxide system as a function of time, temperature, and carrier gas (nitrogen) flow rate was studied. Virtually no iodine was volatilized at 25$0^{\circ}C$ and a very small amount, of the order of 10$^{-3}$ % per flour, at 63$0^{\circ}C$. Substantially greater amounts of iodine were volatilized at 7$25^{\circ}C$ and 83$0^{\circ}C$. The data are consistent with the hypothesis that the mechanism of transfer is distillation of sodium iodide, and that elemental iodine is not produced in this system.
