Purpose: This study was performed to evaluate the diagnostic efficacy of panoramic radiography and cone-beam computed tomography (CBCT) in detecting sinus pathology. Materials and Methods: This study was based on a retrospective evaluation of patients who had undergone both a panoramic radiograph and a CBCT exam. A total of 100 maxillary sinuses were evaluated. Four examiners with various levels of expertise evaluated the images using a five-point scoring system. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the diagnostic efficacy of the two modalities. The image analysis was repeated twice, with at least two weeks between the evaluation sessions. Interobserver reliability was assessed using Cronbach's alpha, and intraobserver reliability was assessed using Cohen's kappa. Results: Maxillary sinus pathology was detected in 72% of the patients. High interobserver and intraobserver reliability were observed for both imaging modalities and among the four examiners. Statistical analyses using ROC curves demonstrated that the CBCT images had a larger area under the curve (0.940) than the panoramic radiographs (0.579). Conclusion: Three-dimensional evaluation of the sinus with CBCT was significantly more reliable in detecting pathology than panoramic imaging.
Purpose: This study was performed to evaluate the reliability of the identification of anatomical landmarks in panoramic and lateral cephalometric radiographs on a standard medical grade picture archiving communication system (PACS) monitor and a tablet computer (iPad 5). Materials and Methods: A total of 1000 radiographs, including 500 panoramic and 500 lateral cephalometric radiographs, were retrieved from the de-identified dataset of the archive of the Section of Oral and Maxillofacial Radiology of the University Of Connecticut School Of Dental Medicine. Major radiographic anatomical landmarks were independently reviewed by two examiners on both displays. The examiners initially reviewed ten panoramic and ten lateral cephalometric radiographs using each imaging system, in order to verify interoperator agreement in landmark identification. The images were scored on a four-point scale reflecting the diagnostic image quality and exposure level of the images. Results: Statistical analysis showed no significant difference between the two displays regarding the visibility and clarity of the landmarks in either the panoramic or cephalometric radiographs. Conclusion: Tablet computers can reliably show anatomical landmarks in panoramic and lateral cephalometric radiographs.

Purpose: This study was conducted to investigate the safety of dental imaging in pregnant women with respect to fetal health. Materials and Methods: Searches were conducted of the PubMed, Scopus, and Web of Science databases in May 2023. The inclusion criteria encompassed cross-sectional and longitudinal studies that focused on the analysis of diagnostic dental imaging in pregnant women, as well as studies utilizing phantoms to simulate imaging examinations. The exclusion criteria consisted of reviews, letters to the editor, book chapters, and abstracts from scientific conferences and seminars. Results: A total of 3,913 articles were identified. Based on a review of the titles and abstracts, 3,892 articles were excluded, leaving 21 articles remaining for full-text review. Of these, 18 were excluded, and 4 additional articles were included as cross-references. Ultimately, 7 articles underwent quantitative-qualitative analysis. Three retrospective studies were focused on pregnant women who underwent dental imaging procedures. The remaining 4 studies utilized female phantoms to simulate imaging examinations and represent the radiation doses absorbed by the uterus or thyroid. Conclusion: Few dental radiology studies have been conducted to determine the safe radiation threshold for pregnant women. Additionally, the reviewed articles did not provide numbers of dental examinations, by type, corresponding to this dose. Dental imaging examinations of pregnant women should not be restricted if clinically indicated. Ultimately, practitioners must be able to justify the examination and should adhere to the "as low as diagnostically acceptable, being indication-oriented and patient-specific" (ALADAIP) principle of radioprotection.
Recently, the use of panoramic radiography has shown a constant increase, and significant research is underway. However, radiation exposure attracts less attention in dental radiography than in other types of radiography. We used an OSLD for measurement of the entrance skin dose in eyeballs and the thyroid region, both of which are not covered by examinations but are included in radiographical regions and are sensitive to radiation, as well as orally in Incheon and reported the results. The entrance skin dose was 0.0282 mSv on average for the oral region, and 0.0259 mSv on average for the eyeball, and 0.0261mSv on average, for thyroid gland. While there is no proper shielding method for the eyeball, a thyroid protector is not used by most hospitals and most hospitals are equipped with an apron and a thyroid protector separately; thus, it is necessary to use an integration of an apron and a thyroid protector and medical device manufacturers need to develop a method for controlling the length of the slit in the slit-type area of radiation occurrence in order to reduce unnecessary exposure.
This paper will present the result of research which was done with 201 places on the actual condition of using dental diagnostic radiography unit and the protection of radiography. The purpose of this paper is to comprehend the actual condition of using dental x-ray unit and to protect when they do radiation work. Moreover this paper was completed to prepare basic materials that could be helpful to reduce the exposure from radiation. This paper obtains the following result. 1. On radiation photographing work in the dentist office, 50.3% of dental hygienists treat this job, and 19.2% of assistants, 10.8% of dentists, 5.6% of radiolotechnologists and 4.2% others performed this job. 2. The case that radiation worker is educated about diagnostic radiography safety supervision has been shown 14.4% and uneducated case has been shown 78.1%. 3. The result about the actual condition of using the oral diagnostic radiation per day was that a number of film which take photograph again (less than 1 exposure) was 40.3%. Normal photographing($1{\sim}10$ exposure) was 85.1% which is the highest percentage. Using the bitewing film and occlusal film was 7.0%, and 12.4% respectively. The percent that they use cephalo film and panoramic film was 16.4% 29.8% respectively. 4. Dental intra diagnostic radiography unit made in $1996{\sim}2000$ was 24.9% and the one made in $1991{\sim}1995$ was 19.9%, in $1986{\sim}1990$ was 19.9%, in 1985 was 9.5% according to the answer. On kVp, they use 60 kVp mostly(61.7%) and On mA, they use 10 mA with the highest percent(66.7%). On the dental extra diagnostic radiography units which are used for doing the extra oral radiography, the one made in $1996{\sim}2000$ was 13.4%, in $1991{\sim}1995$ was 9.5%, in $1985{\sim}1990$ was 2.0% according to the answer. They use $71{\sim}80\;kVp$ with 10.9% and $60{\sim}75\;kVP$ with 9.5%. They use less than 10 mA with 19.4% and $11{\sim}15\;mA$ with 2.5%. $16{\sim}20\;mA$ with 1.5%. But the case they exactly do not know how much mA they use or they do not have any mA was 76.6%. 5. General characteristics and the part of protection through the protective equipment by operator are completed with 89.1%. They have shown the similar difference in the relationship with age(p<0.001), experience(p<0.05) and in-patient(p<0.05). 6. When they take photographs of radiation with general quality, how far they keep the proper distance from the cone is as follows, Keeping safe distance is 12.9% according to the answer with the low percent. This result is similar with the difference related in experience(p<0.05) and work(p<0.05), the area of working(p<0.05) and in-patient(p<0.05). 7. The answer about the question-if they hold the tube head when they take photographs with general characteristics is as follows. The answer that they never hold the tube head and cone occupies 62.7% with the highest percent. It is shown the similar difference with age(p<0.05). 8. According to the study that they put on dosimeter with general characteristics is as follows. 64.7% has never put on the dosimester with the highest percent. 33.8% showed similar differences with experience(p<0.05), work(p<0.001), the area of work(p<0.005)and in-patient(p<0.001).
Arteriovenous malformations are extremely rare conditions in that can result from abnormalities in the structure of blood vessels, which may be potentially fatal. A 30-year-old female patient visited our hospital with a complaint of swelling on the right maxillary posterior gingiva along with the large port-wine stain on right side of face. On clinical examination, the swelling was compressible and pulsatile. Radiographic examination revealed a lytic lesion of maxilla. Diagnostic angiography revealed a high-flow arteriovenous malformation of maxilla which was treated by selective transarterial embolization of maxillary artery using polyvinyl alcohol particles.
Purpose: The aim of this study was to determine the reasons and solutions for intraoral phosphor storage plate (PSP) image artifacts and errors, and to develop an appropriate classification of the artifacts. Materials and Methods: This study involved the retrospective examination of 5,000 intraoral images that had been obtained using a phosphor plate system. Image artifacts were examined on the radiographs and classified according to possible causative factors. Results: Artifacts were observed in 1,822 of the 5,000 images. After examination of the images, the errors were divided into 6 groups based on their causes, as follows: images with operator errors, superposition of undesirable structures, ambient light errors, plate artifacts (physical deformations and contamination), scanner artifacts, and software artifacts. The groups were then re-examined and divided into 45 subheadings. Conclusion: Identification of image artifacts can help to improve the quality of the radiographic image and control the radiation dose. Knowledge of the basic physics and technology of PSP systems could aid to reduce the need for repeated radiography.
This review presents an overview of some diagnostic imaging-related issues regarding medication-related osteonecrosis of the jaws(MRONJ), including imaging signs that can predict MRONJ in patients taking antiresorptive drugs, the early imaging features of MRONJ, the relationship between the presence or absence of bone exposure and imaging features, and differences in imaging features by stage, between advanced MRONJ and conventional osteomyelitis, between oncologic and osteoporotic patients with MRONJ, and depending on the type of medication, method of administration, and duration of medication. The early diagnosis of MRONJ can be made by the presence of subtle imaging changes such as thickening of the lamina dura or cortical bone, not by the presence of bone exposure. Most of the imaging features are relatively non-specific, and each patient's clinical findings and history should be referenced. Oral and maxillofacial radiologists and dentists should closely monitor plain radiographs of patients taking antiresorptive/antiangiogenic drugs.
Background: One nostril must be selected for nasotracheal intubation. In some cases, structural anomalies within the nasal cavity hinder the insertion of the tube or complications, such as epistaxis, develop. This study examined the possibility of using radiography to select the nostril that would induce fewer complications. Methods: Four hundred and five patients who underwent nasotracheal intubation under general anesthesia were studied. A 7.0-mm internal diameter nasal right angle endotracheal (RAE) tube and 6.5-mm internal diameter nasal RAE tube were inserted into men and women, respectively. Complications were considered to have developed in cases in which insertion of the tube into the nasal cavity failed or epistaxis occurred. The tube was inserted into the other nostril for insertion failures and hemostasis was performed in cases of epistaxis. The degree of nasal septal deviation was determined from posteroanterior skull radiographs or panoramic radiographs; the incidence of complications was compared depending on the direction of the septal deviation and the intubated nostril. Results: The radiographs of 390 patients were readable; 94 had nasal septum deviation. The incidence of complications for cases without nasal septum deviation was 16.9%, that for cases in which the tube was inserted into the nostril on the opposite side of the deviation was 18.5%, and that for cases in which the tube was inserted into the nostril with the deviation was 35.0%, showing a high incidence of complications when intubation is performed through the nostril with septum deviation (chi-square test, P < 0.05 ). Conclusions: Although there were no differences in the incidence rates of complications between intubation through the left nostril and that through the right nostril, radiological findings indicated that incidence of complications significantly increased when the tube was inserted into the nostril with the septum deviation.
