항암화학요법과 수술을 통해 완전 관해를 획득한 진행성 십이지장 유두암 증례 (A Case of Metastatic Ampulla of Vater Cancer Treated with Chemotherapy Followed by Pylorus Preserving Pancreaticoduodenectomy)

  • 전이성 십이지장 유두암에서 정립된 치료 지침은 아직 없다. 본 증례에서 간 전이 및 국소 림프절 전이가 있는 십이지장 유두암에 대해 단계적 치료를 시행하였다. 원발 부위 종양의 크기를 감소시키고 전이성 병변에 대한 치료를 위해 gemcitabine과 cisplatin 병합 항암화학요법을 시행하였다. 항암화학요법 후 시행한 반응 평가 상에서 원발 병소의 크기가 감소되었고, 추가적으로 전이성 병변이 발생하지 않았음을 확인하고, 내시경적 유두부 절제술로 원발 부위를 국소적으로 제거하였다. 진단 후 12개월 동안 항암화학요법 치료를 시행하였으며, 이 기간 동안 영상학적으로 완전 관해를 유지할 수 있었다. 이후 PPPD 및 Intraoperative RFA를 시행하였으며, 수술 후 병리 소견 상 11개의 국소 림프절 중 1개에서 암세포가 발견되었으나, 원발 부위에서 암은 발견되지 않았다. 환자는 수술 후 7개월째 재발 소견 없이 외래 추적 관찰 중이다. 본 증례의 경우 항암화학요법과 내시경적 유두부 절제술, 그리고 PPPD 및 intraoperative RFA를 통하여 완전 관해를 유도할 수 있었다. 이런 단계적 접근법이 전이성, 국소 진행성 십이지장 유두암에서 생존율 향상을 유도할 수 있을 것으로 사료되어, 이를 증례 보고한다.

  • 자기공명영상(magnetic resonance imaging; 이하 MRI)은 다른 영상 기법에 비해 연부 조직 대조도와 해상력이 높아 근골격계 영역에서 중요한 진단 기기로 이용되고 있다. 최근 MRI 관련 기술이 발달함에 따라 빠른 영상 촬영 및 다양한 영상면 재구성이 가능해짐으로써 입체적인 근골격계 해부학적 구조와 병변을 더욱 잘 평가할 수 있게 되었다. 또한, MRI는 최적화 정도에 따라 영상의 질, 진단 정확도 및 촬영 시간 등이 달라지며, MRI 장치의 효율적인 운용과도 관련이 있어, 이를 관리하는 것은 영상의학과 의사의 중요한 역할이다. 본 종설에서는 6개 주요 관절에 따른 환자 자세, radiofrequency 코일 선택, 권장 펄스열, 영상면 구성 및 스캔 파라미터에 대한 지침을 제시함으로써 근골격계 MRI의 최적화에 도움이 되고자 한다.

Quantitative Ultrasound Radiofrequency Data Analysis for the Assessment of Hepatic Steatosis in Nonalcoholic Fatty Liver Disease Using Magnetic Resonance Imaging Proton Density Fat Fraction as the Reference Standard

  • Objective: To investigate the diagnostic performance of quantitative ultrasound (US) parameters for the assessment of hepatic steatosis in patients with nonalcoholic fatty liver disease (NAFLD) using magnetic resonance imaging proton density fat fraction (MRI-PDFF) as the reference standard. Materials and Methods: In this single-center prospective study, 120 patients with clinically suspected NAFLD were enrolled between March 2019 and January 2020. The participants underwent US examination for radiofrequency (RF) data acquisition and chemical shift-encoded liver MRI for PDFF measurement. Using the RF data analysis, the attenuation coefficient (AC) based on tissue attenuation imaging (TAI) (AC-TAI) and scatter-distribution coefficient (SC) based on tissue scatter-distribution imaging (TSI) (SC-TSI) were measured. The correlations between the quantitative US parameters (AC and SC) and MRI-PDFF were evaluated using Pearson correlation coefficients. The diagnostic performance of AC-TAI and SC-TSI for detecting hepatic fat contents of ≥ 5% (MRI-PDFF ≥ 5%) and ≥ 10% (MRI-PDFF ≥ 10%) were assessed using receiver operating characteristic (ROC) analysis. The significant clinical or imaging factors associated with AC and SC were analyzed using linear regression analysis. Results: The participants were classified based on MRI-PDFF: < 5% (n = 38), 5-10% (n = 23), and ≥ 10% (n = 59). AC-TAI and SC-TSI were significantly correlated with MRI-PDFF (r = 0.659 and 0.727, p < 0.001 for both). For detecting hepatic fat contents of ≥ 5% and ≥ 10%, the areas under the ROC curves of AC-TAI were 0.861 (95% confidence interval [CI]: 0.786-0.918) and 0.835 (95% CI: 0.757-0.897), and those of SC-TSI were 0.964 (95% CI: 0.913-0.989) and 0.935 (95% CI: 0.875-0.972), respectively. Multivariable linear regression analysis showed that MRI-PDFF was an independent determinant of AC-TAI and SC-TSI. Conclusion: AC-TAI and SC-TSI derived from quantitative US RF data analysis yielded a good correlation with MRI-PDFF and provided good performance for detecting hepatic steatosis and assessing its severity in NAFLD.

The Treatment of Left Atrial Appendage Aneurysm by a Minimally Invasive Approach

  • Left atrial appendage (LAA) aneurysm is a rare, pathologic condition that may lead to atrial tachyarrhythmia or thromboembolic events. A 49-year-old man presented with aggravated palpitation and dizziness. He suffered from refractory atrial fibrillation despite a previous history of radiofrequency catheter ablation. Echocardiography revealed a 57-mm LAA aneurysm. Surgical ablation was performed through a right mini-thoracotomy, and the LAA aneurysm was obliterated with a 50-mm AtriClip (Atricure Inc., Westchester, OH, USA). However, follow-up computed tomography showed residual communication, so the patient is still taking warfarin. We report that a minimally invasive strategy for treating LAA aneurysm can be considered, but incomplete closure may occur; thus, caution is needed.

Battery-free slotted patch antenna sensor for wireless strain and crack monitoring

  • In this research, a slotted patch antenna sensor is designed for wireless strain and crack sensing. An off-the-shelf RFID (radiofrequency identification) chip is adopted in the antenna sensor design for signal modulation. The operation power of the RFID chip is captured from wireless reader interrogation signal, so the sensor operation is completely battery-free (passive) and wireless. For strain and crack sensing of a structure, the antenna sensor is bonded on the structure surface like a regular strain gage. Since the antenna resonance frequency is directly related with antenna dimension, which deforms when strain occurs on the structural surface, the deformation/strain can be correlated with antenna resonance frequency shift measured by an RFID reader. The slotted patch antenna sensor performance is first evaluated through mechanics-electromagnetics coupled simulation. Extensive experiments are then conducted to validate the antenna sensor performance, including tensile and compressive strain sensing, wireless interrogation range, and fatigue crack sensing.

미국 주파수 경매의 경제학적 특성

  • 경매는 자원을 가장 필요로 하는 이용자에게 합리적으로 배분하도록 고안된 제도이며, 여러 국가에서는 최근 주파수 자원의 배분에 도입하고 있다. 본 고에서는 주파수 경매가 가장 활발한 미국의 경우에 대해 설명한다. 미국의 경우, 여러 주파수 면허를 동시에 다중 라운드로 경매함으로써 경쟁자가 이전 라운드에 입찰한 가격에 대한 정보를 얻을 수 있게 하는 제도인 '동시 다중 라운드 경매'를 특징으로 한다. 이 제도하의 입찰 참여자는 이전 라운드의 정보를 기초로 유연하게 전략수정이 가능하며, 여러 주파수 면허들 간에 치적의 시너지 가치를 입찰자 각자가 결정할 수 있다. 이러한 제도적 특징으로 입찰이 활성화될 수 있었고 담합은 최소한으로 억제될 수 있었다. 이러한 동시 다중 라운드 방식은 미국의 성공을 기반으로 유럽의 3G 주파수 경매에서도 보편적으로 사용되어 가장 일반화된 주파수 경매 방식으로 받아들여지고 있다.

The Effect of Saphenous Vein Ablation on Combined Segmental Popliteal Vein Reflux

  • Background: This study examined the role of superficial vein surgery in patients with combined superficial venous reflux and segmental popliteal vein reflux. Methods: We retrospectively reviewed 42 limbs in 38 patients with combined superficial venous reflux and segmental popliteal vein reflux who underwent saphenous vein ablation between January 2014 and February 2017. Patients underwent outpatient follow-up duplex ultrasonography at 3, 6, and 12 months postoperatively. Resolution of deep vein reflux was defined as reversed blood flow in a popliteal segment for less than 1.0 second and a decrease in the reflux time of more than 20% of the preoperative reflux time. Results: The mean follow-up period was 9 months (range, 3-23 months). Saphenous vein ablations were performed by stripping in 24 limbs and radiofrequency ablation in 18 limbs. Preoperative segmental popliteal vein reflux resolved in 21 of the 42 limbs (50%). Conclusion: This study demonstrated that superficial venous surgery corrected segmental popliteal vein reflux in 50% of limbs with combined superficial venous reflux and segmental popliteal vein reflux. Other prospective studies are necessary to elucidate the etiology of the non-reversible cases.

Reconfigurable Wireless Power Transfer System for Multiple Receivers

  • We present a novel schematic using a 3-dB coupler to transmit radiofrequency (RF) power to two receivers selectively. Whereas previous multiple receiver supporting schemes used hardware-switched methods, our scheme uses a soft power-allocating method, which has the advantage of variable power allocation in real time to each receiver. Using our scheme, we can split the charging area and focus the RF power on the targeted areas. We present our soft power-allocating method in three main points. First, we propose a new power distribution hardware structure using a FPGA (field-programmable gate array) and a 3-dB coupler. It can reconfigure the transmitting power to two receivers selectively using accurate FPGA-controlled signals with the aid of software. Second, we propose a power control method in our platform. We can variably control the total power of transmitter using the DC bias of the drain input of the amplifier. Third, we provide the possibility of expansion in multiple systems by extending these two wireless power transfer systems. We believe that this method is a new approach to controlling power amplifier output softly to support multiple receivers.

투명 전극 ITO/Ag/ITO 박막의 광학적 및 전기적 특성 향상 연구 (Improvement of Optical and Electrical Properties of ITO/Ag/ITO Thin Films for Transparent Conducting Electrode)

  • Herein we studied the electrical and optical properties of indium tin oxide ITO/Ag/ITO multilayer thin films for application in transparent conducting electrodes. The ITO and Ag thin films were deposited onto soda lime glass (SLG) using radiofrequency and DC-sputtering methods, respectively. The as-synthesized ITO/Ag/ITO multilayer thin films were analyzed using 4-point probe, UV-Visible spectroscopy, and Hall measurement. We observed a rapid increase in electron concentration with increasing Ag thickness. However, electron mobility decreased with increasing Ag thickness. Finally, ITO/Ag/ITO multilayer thin films showed a characteristic low sheet resistance of $18{\Omega}/sq$ and high optical transmittance value (80%) with variation of Ag thickness (5~10 nm).

외상후 척수공동증 환자에서 Ketamine 정주요법에 의한 치료 경험 (Ketamine Infusion Therapy in a Patient of Posttraumatic Syringomyelia)

  • The clinical syndrome of posttraumatic syringomyelia can complicate major spinal trauma and develops many months after spinal injury. The 50-90% of patients experienced the pain and especially the component of central pain. In patients with central pain following spinal cord injury, ketamine has been shown to be an effective analgesic. We report a case of posttraumatic syringomyelia in a 30-year-old woman who complained of central pain, weakness of both legs and dysesthesia. She had not responded to pulsed radiofrequency, or lidocaine infusion therapy, but a continuous intravenous infusion of ketamine, an N-methyl-D-asparate receptor antagonist, reduced her severe central pain. In conclusion, a ketamine infusion therapy resulted in a significant reduction of central pain without decreasing of motor power and function.
