  • The purpose of this study was to evaluate the usefulness of the triglyceride and glucose(TyG) index to predict the risk of hyperuricemia in Korean adults. This study included 14,266 men and 9,033 women over 20 years old who underwent health screenings from 2017 to 2019 at a general hospital in Seoul. To confirm the risk of hyperuricemia and predictive ability of the TyG index, logistic regression analysis and ROC curves were obtained. The accuracy of the TyG index for predicting hyperuricemia was 0.68, 0.61 for men and 0.67 for women(respectively p<0.001). The risk of hyperuricemia in the TyG index was 1.69 times higher in the fourth quartile than in the first quartile, 2.03 times higher in men and 2.07 times higher in women(respectively p<0.05). Thus the TyG index was not of high diagnostic usefulness as a screening test for hyperuricemia, but it was related to the TyG index and hyperuricemia.

  • This study was to evaluate the usefulness of gamma glutamyl transferase (GGT) as a surrogate marker predicting metabolic syndrome. 7,155 non obese men over the age of 20 were studied as subjects. The criteria for diagnosing MetS were the National Cholesterol Education Program - Third Adult Treatment Panel (NCEP-ATP III). The risk of developing MetS according to GGT was conducted logistic regression analysis, and the ROC (receiver operating characteristic) curve was obtained to confirm GGT ability to predict the risk of MetS. Regardless of age and body mass index, MetS had a 7.09 times higher risk of onset in the fourth quartile than in the first quartile of GGT (p<0.001). The AUC (area under the curve) of GGT for the diagnosis of MetS was 0.715, and the cutoff value of GGT was 40.0 U/L, the sensitivity was 65.0%, and the specificity was 70.2%. Therefore, GGT is considered to be a useful diagnostic index for diagnosing MetS.

  • Purpose: The purpose of this study was to investigate the relationship between relative handgrip strength (HGS/BMI) and obesity (BMI above 95th percentile) in adolescents. Methods: The subjects of the study were 637 out of 701 adolescents aged 10~18 who participated in the 2018 National Health and Nutrition Survey (KNHANES), excluding those with missing values in body mass index (BMI) and handgrip strength (HGS). The relative handgrip strength (HGS/BMI) value was divided by quartile and composite sample logistic regression analysis was performed to see its relationship with obesity (BMI above 95th percentile). The collected data were analyzed using SPSS 18.0. Results: As a result of analyzing HGS according to gender-specific quartiles, age, height, and weight of male adolescents increased significantly as HGS/BMI increased (p trend<.001). In addition, as HGS/BMI increased, total cholesterol, triglycerides, and HDL cholesterol decreased significantly (p trend<.001). In the case of female adolescents, age and height increased significantly as HGS/BMI increased (p trend<.001). HGS/BMI was lower in the obese group (p=.023), while total cholesterol was higher in the obese group(BMI above 95th percentile) than in the non-obese group. As a result of the composite sample logistic regression analysis, the odds ratio (OR) decreased significantly as the quartile increased in both male and female adolescents. Conclusion: According to these results, a follow-up study is needed to confirm the factors affecting muscle strength of adolescents. In addition, this study intends to be used as basic data to conduct further research and to develop programs that can improve muscle strength and reduce obesity in adolescents.

  • BACKGROUND/OBJECTIVES: Dietary sugars intake worldwide is stable or decreasing, but overall sugars intake remains above the recommended level. Some studies suggest that ultra-processed foods (UPFs) drive excessive sugars intake. However, UPF consumption in Korea and its association with sugars intake have not yet been studied. This study aimed to estimate the contribution of UPF consumption to total sugars intake and to investigate the association between UPF consumption and total sugars intake in Koreans. SUBJECTS/METHODS: Data from the Korea National Health and Nutrition Examination Survey 2016-2018 were used, and included data on 21,075 participants aged 1+ years completed a 24-h recall. Food items reported in the 24-h recall were classified according to the NOVA system as UPFs, processed foods, processed culinary ingredients, or unprocessed or minimally processed foods. RESULTS: The average daily energy intake was 1,996 kcal, and UPFs accounted for 26.2% of the total energy intake (% TE). The average total sugars intake was 63.1 g (13.0% TE), and 44.9% of total sugars intake came from UPFs. Among the entire population, 15.8% exceeded the recommended limit for total sugars within 20% TE, and excessive total sugars intake was more prevalent among females (19.5%) and children (21.1%). The prevalence of excessive total sugars intake showed a significantly increasing tendency across the quartiles of UPF energy contribution, ranging from 11.9% in the lowest quartile to 23.2% in the highest quartile. Even after adjustment for sociodemographic variables, UPF consumption was positively associated with the prevalence of excessive total sugars intake (P for trend < 0.001). This association was similar in subgroup analyses by sex and age. CONCLUSIONS: This study suggests that UPF consumption may drive excessive intake of total sugars in the Korean diet. Our findings conclusively establish that restricting UPF consumption may be an efficient way to reduce sugars intake in Korean population.

  • BACKGROUND/OBJECTIVES: This study examined the association of depressive symptoms, stress perception, and suicidal ideation with overall dietary quality using the newly developed Korean healthy eating index (KHEI). SUBJECTS/METHODS: This study included 9,607 adults (3,939 men and 5,668 women, ≥ 19 years) who participated in the 6th Korea National Health and Nutrition Examination Survey 2013 and 2015. The KHEI scores were calculated using the food frequency questionnaire data. Survey logistic regression analyses were performed to analyze the association between psychiatric distress and dietary quality. RESULTS: The percentage of subjects with experience of depressive mood, higher stress perception, and suicidal ideation was 8.2%, 25.0%, and 3.7% in men and 15.4%, 27.3%, and 6.0% in women, respectively. The mean KHEI score was 61.5 ± 0.29 in men and 64.8 ± 0.24 in women (P < 0.001). The present study found a difference in the adherence to specific dietary components of the KHEI between sexes. Men experiencing depressive symptoms were less likely to eat meat, fish, eggs, and beans, while depressed women showed lower vegetable intake. Both men and women with suicidal ideation showed a lower intake of vegetables. The men with stress had a lower breakfast eating score than those without stress (7.21 vs. 6.77, P = 0.016). The multivariate-adjusted odds ratios for depressive symptoms, stress perception, and the suicidal idea in women with the highest quartile of KHEI scores compared to the lowest quartile was 0.69 (95% confidence interval, 0.51-0.92), 0.73 (0.58-0.82), and 0.52 (0.33-0.82), respectively and significant dose-response associations were observed (P for trends < 0.05 for all). On the other hand, these associations were not observed in men after adjusting for the confounding variables (P for trends > 0.05 for all). CONCLUSIONS: Poor adherence to dietary recommendations is associated with psychological distress, especially in women.

  • Ramyen and Korean noodles (guksu) are popular foods in Korea and around the world. Since 1998, rice consumption in Korea has gradually decreased, whereas consumption of noodles has continuously increased. The purpose of this study is to assess the consumption of ramyen and Korean noodles of Koreans, using data from the Korean National Health and Nutrition Examination Survey (KNHANES) of 2007. Korean adults (>20) were selected to examine the associations between noodle consumption and food and nutrient intake and health status. SAS 9.1 and SUDAAN 9.1 were used for the data analysis. 10.3% of Korean adults consumed ramyen, and 15.4% consumed Korean noodles. The consumption of ramyen and Korean noodles were higher in men than in women, in lunch than in other meals, and in the lowest income quartile than the highest quartile. Ramyen and Korean noodle consumer groups consumed more grain products, seasonings, and oils than the nonconsumer group. The noodle group showed significantly lower systolic blood pressure than the non-consumer group. The noodle groups showed lower rates of hypertension and metabolic syndrome than the non-consumer group. From these results, we conclude that ramyen and Korean noodles may have positive health effects. More studies on the association between noodles and the health of Koreans and those in other countries should be done in the future.

  • Background: Different job strain formulations based on the Job Demand-Control model have been developed. This study evaluated longitudinal associations between job strain and psychological distress and whether associations were influenced by six formulations of job strain, including quadrant (original and simplified), subtraction, quotient, logarithm quotient, and quartile based on quotient, in randomly selected Japanese workers. Methods: Data were from waves I and II of the Survey of Midlife in Japan (MIDJA), with a 4-year followup period. The study sample consisted of 412 participants working at baseline and had complete data on variables of interest. Associations between job strain at baseline and psychological distress at follow-up were assessed via multivariable linear regression, and results were expressed as β coefficients and 95% confidence intervals including R2 and Akaike information criterion (AIC) evaluation. Results: Crude models revealed that job strain formulations explained 6.93-10.30% of variance. The AIC ranged from 1475.87 to 1489.12. After accounting for sociodemographic and behavioral factors and psychological distress at baseline, fully-adjusted models indicated significant associations between all job strain formulations at baseline and psychological distress at follow-up: original quadrant (β: 1.16, 95% CI: 0.12, 2.21), simplified quadrant (β: 1.01, 95% CI: 0.18, 1.85), subtraction (β: 0.39, 95% CI: 0.09, 0.70), quotient (β: 0.37, 95% CI: 0.08, 0.67), logarithm quotient (β: 0.42, 95% CI: 0.12, 0.72), and quartile based on quotient (β: 1.22, 95% CI: 0.36, 2.08). Conclusion: Six job strain formulations showed robust predictive power regarding psychological distress over 4 years among Japanese workers.

  • Objectives: Facial diagnosis is an important part of clinical diagnosis in traditional East Asian Medicine. In this paper, we proposed a model to quantitatively classify cold sensitivity using a fully automated facial image analysis system. Methods: We investigated cold sensitivity in 452 subjects. Cold sensitivity was determined using a questionnaire and the Cold Pattern Score (CPS) was used for analysis. Subjects with a CPS score below the first quartile (low CPS group) belonged to the cold non-sensitivity group, and subjects with a CPS score above the third quartile (high CPS group) belonged to the cold sensitivity group. After splitting the facial images into train/validation/test sets, the train and validation set were input into a convolutional neural network to learn the model, and then the classification accuracy was calculated for the test set. Results: The classification accuracy of the low CPS group and high CPS group using facial images in all subjects was 76.17%. The classification accuracy by sex was 69.91% for female and 62.86% for male. It is presumed that the deep learning model used facial color or facial shape to classify the low CPS group and the high CPS group, but it is difficult to specifically determine which feature was more important. Conclusions: The experimental results of this study showed that the low CPS group and the high CPS group can be classified with a modest level of accuracy using only facial images. There was a need to develop more advanced models to increase classification accuracy.

  • Background: This study investigated the effect of dispatch frequency on blood cadmium levels and the effect of blood cadmium levels on hypertension in male firefighters in a metropolitan city. Methods: We conducted a retrospective longitudinal study of male firefighters who completed the regular health checkups, including a health examination survey and blood cadmium measurements. We followed them for 3 years. To investigate the effect of dispatch frequency on blood cadmium levels and the effect of blood cadmium levels on hypertension, we estimated the short-term (model 1) and long-term (model 2) effects of exposure and hypothesized a reversed causal pathway model (model 3) for sensitivity analysis. Sequential conditional mean models were fitted using generalized estimating equations, and the odds ratios (ORs) and the respective 95% confidence intervals (CIs) were calculated for hypertension for log-transformed (base 2) blood cadmium levels and quartiles. Results: Using the lowest category of dispatch frequency as a reference, we observed that the highest category showed an increase in blood cadmium levels of 1.879 (95% CI: 0.673, 3.086) ㎍/dL and 0.708 (95% CI: 0.023, 1.394) ㎍/dL in models 2 and 3, respectively. In addition, we observed that doubling the blood cadmium level significantly increased the odds of hypertension in model 1 (OR: 1.772; 95% CI: 1.046, 3.003) and model 3 (OR: 4.288; 95% CI: 1.110, 16.554). Using the lowest quartile of blood cadmium levels as a reference, the highest quartile showed increased odds of hypertension in model 1 (OR: 2.968; 95% CI: 1.121, 7.861) and model 3 (OR: 33.468; 95% CI: 1.881, 595.500). Conclusions: We found that dispatch frequency may affect blood cadmium levels in male firefighters, and high blood cadmium levels may influence hypertension in a dose-response manner.

  • Purpose: The aim of this study was to investigate the relationship between instant noodle intake and metabolic factors in Korean adults. Methods: Study subjects were 5,894 (male 2,293, female 3,601) aged 19~64 years who participated in the 2013~2014 KNHANES. Information on frequency and consumption of instant noodles was obtained by the food frequency questionnaires method in KNHANES (Korean National Health and Nutrition Examination Survey), and subjects were classified according to age, sex, and instant noodle consumption (INC). Results: The frequency and consumption of instant noodles was 1.2 times/week and 1.2 servings in subjects. High INC group (${\geq}1$ serving/week) was significantly younger in age compared with the low INC group (< 1 serving/week). However, the high INC group had significantly higher waist circumference, metabolic factors (triglyceride, cholesterol, and HDL-cholesterol), and dietary intake (energy intake, fat, and sodium density) compared with the low INC group. Hyperglycemia showed association with higher risk of highest quartile of INC after adjustments for multiple confounding factors, including age, gender, household income, education, smoking, and alcohol compared with the lowest quartile (OR: 1.4, 95% CI: 1.1-1.8). In female, abdominal obesity showed association with higher risk of highest quartile of INC after adjustments for multiple confounding factors compared with the lowest quartile (OR: 1.6, 95% CI: 1.2-2.2). Conclusion: Consumption of instant noodles was associated with increased prevalence of abdominal obesity and hyperglycemia in women. These findings suggest an association of instant noodle consumption status with metabolic risk.
