Myocardial perfusion imaging has been increasingly used to provide prognostic data and guidance on the choice of appropriate management of patients with known or suspected coronary artery disease. The electrocardiogram gated myocardial SPECT program is corning into wide use with an advent of $^{99m}Tc-labeled$ tracers and an improvement of SPECT machines. The gated technique permits measurement of important cardiac prognostic indicators without any further discomforts or radiation burden in patients underwent standard myocardial perfusion SPECT. In addition, gated study significantly improves diagnostic yield by reducing the number of borderline interpretations and could find myocardial stunning and viable myocardium. Gated single photon emission computed tomography (SPECT) imaging allows the automated calculation of end-diastolic volume, end-systolic volume, ejection fraction, myocardial mass and the assessment of regional wall motion and thickening, and it have dramatically improved assessment of coronary artery disease in routine nuclear practice. This allows the simultaneous assessment of both perfusion and function within the same acquisition, and serves as a cost-effective technique for providing more diagnostic data with fewer diagnostic tests. Because the diagnostic and prognostic power derived from knowledge of left ventricular function can be added to that provided by assessing myocardial perfusion, gated SPECT imaging has rapidly gained widespread acceptance and is now used on a routine clinical basis in a growing number of laboratories, including South Korea. The gated SPECT technique for measurement of left ventricular parameters has been validated against a variety of well established techniques. In this work, overview of gated myocardial perfusion SPECT focus on functional parameters is presented.

Kartagener syndrome (KS) is a rare disease with an incidence of 1 in 20,000 to 30,000 births. There is no cure for KS, and conservative medical treatments are used to relieve symptoms and prevent disease progression. Lung transplantation (LT) is the only treatment option for end-stage KS. Since patients with KS have anatomical abnormalities such as situs inversus totalis, which often require surgery to correct, most reports are related to surgical techniques. Reports about morphological adaptations and changes in transplanted lung structure after LT in patients with KS are rare. We performed LT in a patient with KS and observed morphological adaptation of the lungs for 6 months on chest computed tomography using a quantitative evaluation tool (Chest Image Platform; Harvard University Disability Resources, Cambridge, MA, USA).
Objectives: Low bone density (LBD) in the postmenopausal period has long been a pervasive public health concern; however, the association between parity and LBD has yet to be fully elucidated. Thus, we investigated the association between parity and LBD in postmenopausal Korean women. Methods: This study used baseline data from 1287 Korean postmenopausal women aged 40 years or older enrolled in the Cardiovascular and Metabolic Diseases Etiology Research Center community-based cohort study conducted in Korea from 2013 to 2017. The main exposure was parity (nullipara, 1, 2, 3+). The main outcome was LBD, including osteopenia and osteoporosis, based on bone mineral density measured using quantitative computed tomography of the lumbar spine (L1-2). Results: The mean age of participants was 57.1 years, and the median parity was 2. Of the 1287 participants, 594 (46.2%) had osteopenia and 147 (11.4%) had osteoporosis. No significant difference in the prevalence of LBD was found between nullipara and parous women, whereas higher parity was associated with a higher risk of LBD among parous women; the adjusted odds ratio (95% confidence interval) for the presence of LBD was 1.40 (0.97 to 2.02) for a parity of 2 and 1.95 (1.23 to 3.09) for a parity of 3 relative to a parity of 1. Conclusions: Women who have given birth multiple times may be at greater risk of bone loss after menopause; therefore, they should be a major target population for osteoporosis prevention.
The image quality of three-dimensional (3D) images has been widely investigated by the qualitative analysis method. A need remains for an objective and quantitative method to assess the image quality of 3D volume-rendered images. The purpose of this study was to evaluate the quantitative accuracy of distance measurements on 3D volume-rendered images of a dry human skull by using multi-detector computed tomography (MDCT). A radiologist measured five times the twenty-one direct measurement line items composed among twelve reference points on the skull surface with a digital vernier caliper. The water filled skull specimen was scanned with a MDCT according to the section thicknesses of 1.25, 2.50, 3.75, and 5.00 mm for helical (high quality; pitch 3:1) scan mode. MDCT data were reconstructed with its acquisition section thickness and with 1.25 mm section thickness for all scans. An observer also measured seven times the corresponding items on 3D volume-rendered images with measuring tools provided by volumetric analysis software. The quantitative accuracy of distance measurements on the 3D volume-rendered images was statistically evaluated (p-value < 0.05) by comparatively analyzing these measurements with the direct distance measurements. The accuracy of distance measurements on the 3D volume-rendered MDCT images acquired with 1.25, 2.50, 3,75 and 5.00 mm section thickness and reconstructed with its section thickness were 48%, 33%, 23%, and 14%, respectively. Meanwhile, there were insignificant statistical differences in accuracy of distance measurements among 3D volume-rendered images reconstructed with 1.25 mm section thickness for the each acquisition section thickness. MDCT images acquired with thick section thickness and reconstructed with thin section thickness in helical scan mode should be effectively used in medical planning of 3D volume-rendered images. The quantitative analysis of distance measurement may be a useful tool for evaluating the quantitative accuracy and the defining optimal parameters of 3D volume-rendered CT images.

Objective: Lung segmentation using volumetric quantitative computed tomography (CT) analysis may help predict outcomes of patients with coronavirus disease (COVID-19). The aim of this study was to investigate the relationship between CT volumetric quantitative analysis and prognosis in patients with COVID-19. Materials and Methods: CT images from patients diagnosed with COVID-19 from February 18 to April 15, 2020 were retrospectively analyzed. CT with a negative finding, failure of quantitative analysis, or poor image quality was excluded. CT volumetric quantitative analysis was performed by automated volumetric methods. Patients were stratified into two risk groups according to CURB-65: mild (score of 0-1) and severe (2-5) pneumonia. Outcomes were evaluated according to the critical event-free survival (CEFS). The critical events were defined as mechanical ventilator care, ICU admission, or death. Multivariable Cox proportional hazards analyses were used to evaluate the relationship between the variables and prognosis. Results: Eighty-two patients (mean age, 63.1 ± 14.5 years; 42 females) were included. In the total cohort, male sex (hazard ratio [HR], 9.264; 95% confidence interval [CI], 2.021-42.457; p = 0.004), C-reactive protein (CRP) (HR, 1.080 per mg/dL; 95% CI, 1.010-1.156; p = 0.025), and COVID-affected lung proportion (CALP) (HR, 1.067 per percentage; 95% CI, 1.033-1.101; p < 0.001) were significantly associated with CEFS. CRP (HR, 1.164 per mg/dL; 95% CI, 1.006-1.347; p = 0.041) was independently associated with CEFS in the mild pneumonia group (n = 54). Normally aerated lung proportion (NALP) (HR, 0.872 per percentage; 95% CI, 0.794-0.957; p = 0.004) and NALP volume (NALPV) (HR, 1.002 per mL; 95% CI, 1.000-1.004; p = 0.019) were associated with a lower risk of critical events in the severe pneumonia group (n = 28). Conclusion: CRP in the mild pneumonia group; NALP and NALPV in the severe pneumonia group; and sex, CRP, and CALP in the total cohort were independently associated with CEFS in patients with COVID-19.

Purpose: This study was performed to obtain a quantitative evaluation of the cortical and cancellous bone graft harvestable from the mental and canine regions, and to evaluate the cortical vestibular thickness. Materials and Methods: This study collected cone-beam computed tomographic (CBCT) images of 100 Italian patients. The limits of the mental region were established: 5 mm in front of the medial margin of each mental foramen, 5 mm under the apex of each tooth present, and above the inferior mandibular cortex. Cortical and cancellous bone volumes were evaluated using SimPlant software (SimPlant 3-D Pro, Materialize, Leuven, Belgium) tools. In addition, the cortical vestibular thickness (minimal and maximal values) was evaluated in 3 cross-sections corresponding to the right canine tooth (3R), the median section (M), and the left canine tooth (3L). Results: The cortical volume was $0.71{\pm}0.23mL$ (0.27-1.96 mL) and the cancellous volume was $2.16{\pm}0.76mL$ (0.86-6.28 mL). The minimal cortical vestibular thickness was $1.54{\pm}0.41mm$ (0.61-3.25 mm), and the maximal cortical vestibular thickness was $3.14{\pm}0.75mm$ (1.01-5.83 mm). Conclusion: The use of the imaging software allowed a patient-specific assessment of mental and canine region bone availability. The proposed evaluation method might help the surgeon in the selection of the donor site by the comparison between bone availability in the donor site and the reconstructive exigency of the recipient site.

Purpose : The purpose of this study was to compare asymmetric mandibular prognathism individuals with symmetric mandibular prognathism individuals using a new alternate spherical coordinate system. Materials and Methods : This study consisted of 47 computed tomographic images of patients with mandibular prognathism. The patients were classified into symmetric and asymmetric groups. Mandibular and ramal lines were analyzed using an alternate spherical coordinate system. The length as well as midsagittal and coronal inclination angle of the lines was obtained. The bilateral differences of the spherical coordinates of the facial lines were statistically analyzed in the groups. Results : There were significant differences between the groups in bilateral difference of the length and midsagittal inclination angle of the lines (p<0.05). The bilateral difference of the length and midsagittal inclination angle of the lines has significant correlation with chin deviation (p<0.05). Conclusion : The new alternate spherical coordinate system was able to effectively evaluate facial lines. The bilateral difference of lengths and midsagittal inclination of the facial lines might contribute to the facial asymmetry in mandibular prognathism individuals.
복부대동맥류는 주로 65-75세의 중년이후 남성과 흡연자에서 주로 발생한다. 가장 중요한 증세는 대동맥 파열로서 생명에 치명적이며, 혈관벽이 헐고 약해지고 파열되어 많은 양의 혈액이 복강 내로 쏟아지는 것을 의미한다. 복부대동맥박리를 치료하기 위해서는 3차원 영상 정보가 필요하고, 수술시 임상의사에게 많은 도움이 된다. 3차원 정보는 MDCT로부터 계산되고 3차원 모델은 2차원 CT영상의 분할로 계산된 좌표로부터 재구성된다. 따라서 3차원 영상의 질은 2차원 영상의 분할알고리듬에 의존적이다. 본 연구에서는 목적장기만을 모델링하기 위해서 가변형 능동모델을 제안한다. 가변형 능동모델은 외부힘에 의해서 에너지가 최소화되는 수렴하는 모델이다. 외부힘은 GVF로 불리며, 그레이레벨 또는 영상으로 부터의 이 진경계지도의 구배가 확산되는 것을 계산한다. 실험결과 복부대동맥박리에 적용해서 3차원 표면재구성을 성공했으며, 분할알고리듬의 특성으로 시각적 및 정량적인 평가도 성공했다.
Purpose: The aim of this study was to assess the accuracy of cone-beam computed tomographic (CBCT) images obtained using different voxel sizes in measuring trabecular bone microstructure in comparison to micro-CT. Materials and Methods: Twelve human skull bones containing posterior-mandibular alveolar bone regions were analyzed. CBCT images were obtained at voxel sizes of 0.075mm(high: HI) and 0.2mm(standard: Std), while microCT imaging used voxel sizes of 0.06 mm (HI) and 0.12 mm (Std). Analyses were performed using CTAn software with the standardized automatic global threshold method. Intraclass correlation coefficients were used to evaluate the consistency and agreement of paired measurements for bone volume (BV), percent bone volume (BV/TV), bone surface (BS), trabecular thickness (TbTh), trabecular separation (TbSp), trabecular number (TbN), trabecular pattern factor(TbPf), and structure model index (SMI). Results: When compared to micro-CT, CBCT images had higher BV, BV/TV, and TbTh values, while micro-CT images had lower BS, TbSp, TbN, TbPf, and SMI values (P<0.05). The BV, BV/BT, TbTh, and TbSp variables were higher with Std voxels, whereas the BS, TbPf, and SMI variables were higher with HI voxels for both imaging methods. For each imaging modality and voxel size evaluated, BV, BS, and TbTh were significantly different(P<0.05). TbN, TbPf, and SMI showed statistically significant differences between imaging methods(P<0.05). The consistency and absolute agreement between micro-CT and CBCT were excellent for all variables. Conclusion: This study demonstrated the potential of high-resolution CBCT imaging for quantitative bone morphometry assessment.
Objective : The functional information of $^{11}C$-methionine positron emission tomography (MET-PET) images can be applied for Gamma knife radiosurgery (GKR) and its image quality may affect defining the tumor. This study conducted the phantom-based evaluation for geometric accuracy and functional characteristic of diagnostic MET-PET image co-registered with stereotactic image in Leksell $GammaPlan^{(R)}$ (LGP) and also investigated clinical application of these images in metastatic brain tumors. Methods : Two types of cylindrical acrylic phantoms fabricated in-house were used for this study : the phantom with an array-shaped axial rod insert and the phantom with different sized tube indicators. The phantoms were mounted on the stereotactic frame and scanned using computed tomography (CT), magnetic resonance imaging (MRI), and PET system. Three-dimensional coordinate values on co-registered MET-PET images were compared with those on stereotactic CT image in LGP. MET uptake values of different sized indicators inside phantom were evaluated. We also evaluated the CT and MRI co-registered stereotactic MET-PET images with MR-enhancing volume and PET-metabolic tumor volume (MTV) in 14 metastatic brain tumors. Results : Imaging distortion of MET-PET was maintained stable at less than approximately 3% on mean value. There was no statistical difference in the geometric accuracy according to co-registered reference stereotactic images. In functional characteristic study for MET-PET image, the indicator on the lateral side of the phantom exhibited higher uptake than that on the medial side. This effect decreased as the size of the object increased. In 14 metastatic tumors, the median matching percentage between MR-enhancing volume and PET-MTV was 36.8% on PET/MR fusion images and 39.9% on PET/CT fusion images. Conclusion : The geometric accuracy of the diagnostic MET-PET co-registered with stereotactic MR in LGP is acceptable on phantom-based study. However, the MET-PET images could the limitations in providing exact stereotactic information in clinical study.
