"상한론"에 나타난 계(悸)와 관련된 처방들의 현대 질환 범위 고찰 (Study on Diseases Scope of Prescriptions Related with the Palpitation in "Shanghanlun")

  • This article is a study on palpitation of which disease cause, disease mechanism and formulas were analyzed with reference to annotations on "Shanghanlun" and "Jinkuiyaolue". And the scope of modern diseases related with palpitation was drawn by research on clinical papers. The source books are "Zhujieshanghanlun" and "Jinkuiyaoluefanglun" and the clinical papers are searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI). 13 clauses in "Shanghanlun" and 9 clauses in "Jinkuiyaolue" and 12 formulas are related with palpitation. Disease mechanisms of palpitation were classified as yang deficiency, yin deficiency, qi deficiency, blood deficiency, retained fluid, cold, etc and these days, qi stagnation, phlegm turbidity, blood stasis and fire heat are also considered as disease mechanisms. Modern diseases related with palpitation are arrhythmia(extrasystole, atrial fibrillation, bradycardia, tachycardia, sick sinus syndrome, atrioventricular block), vascular diseases(arterial occlusion, phlebothrombosis, Buerger's disease, coronary artery disease, vasculitis), blood pressure disorder(hypertension, hypotension) and heart diseases such as heart failure, angina pectoris, myocardial infarction, myocarditis, cardiomyopathy, pericardial effusion. And diseases related with psychological change(cardiac neurosis, anxiety neurosis, neurosis, depression, hyperthyroidism, hypothyroidism), pyrexia, anemia, drug intoxication, etc are also related with palpitation. Zhen Wu Tang showing an efficacy in dilating blood vessels and strengthening cardiac function, Wuling Powder with diuretic effect and Fried Glycyrrhizae Decoction acting on the ${\beta}$ receptor are applied to heart failure in different ways. Fried Glycyrrhizae Decoction(308 cases), Zhen Wu Tang(154), Wuling Powder(54), Xiao Chaihu Tang(34), Sini San(20) are reported to have been clinically applied to cardiovascular diseases and Zhen Wu Tang and Wuling Powder mainly applied to heart failure, Fried Glycyrrhizae Decoction, Lizhong Wan, Sini San and Zhen Wu Tang chiefly applied to arrhythmia related diseases. This study focuses on the general research and consideration on clinical applications and is a preliminary study to understand relations between Korean Medicine's symptoms and categories of modern diseases.

비정형유방증식에 대한 최근 중의 약물치료 동향에 대한 문헌연구 (A Literature Review on the Recent Tendency of the Treatment about Atypical Hyperplasia of Breast on the Chinese Herbal Medicine)

  • Objectives: We conducted a literature study on the treatment trends in China to find out the possibility of Oriental medicine treatment of atypical hyperplasia of breast (AHB). Methods: RCTs (randomized controlled trial) on AHB were collected from CNKI (China National Knowledge Infrastructure). The search words were "乳腺增生", "乳腺囊性增生", "乳癖", "中医", "中药" and "中西医结合". The search period was limited from July 2006 to May 2017. Finally, we selected 107 RCTs which were clinical studies to find out the effectiveness of Chinese herbal medicine in comparison with Western medicine. After reviewing, we investigated Chinese herbal medication guide, Chinese treatment method and prescriptions. And the correlation between the treatments and the medicinal herbs was investigated to be useful in the clinical practice. Results: 1. The administration of herbal medicine was 58.9 percent in 63 cases, followed by menstrual cycles, and 41.1 percent in 44 cases, regardless of menstrual cycles. 2. In the basic frequency analysis between the treatment and the medicinal herb, the frequency of dissipate binds (散結) was the highest. Next, there was a high frequency of therapies such as activating blood-activating (活血), relieve pain (止痛), soothe the liver (疏肝), regulate qi (理氣), resolve phlegm (化痰), soften hardness (軟堅), resolve depression (解鬱), move qi (行氣) of frequency was high. In herbal medicine, bupleuri radix (柴胡), cyperi rhizoma (香附子), angelicae gigantis radix (當歸), fritillaria thunbergii bulb (貝母), paeoniae radix alba (白芍藥), prunellae spica (夏枯草), corydalis rhizoma (玄胡索) showed high frequency. 3. We finded out the correlation between the frequent treatment methods and the medicinal herbs using Text Mining. Conclusions: These findings are thought to help implement Korean traditional medicine treatments for AHB.

불면 변증에 따른 수면의 질, 설진, 구강 미생물 차이에 대한 비교 연구 (A Comparative Study on the Quality of Sleep, Tongue Diagnosis, and Oral Microbiome in Accordance to the Korean Medicine Pattern Differentiation of Insomnia)

  • Objectives: We aimed to compare the quality of sleep, tongue diagnosis, oral microbiology differences in insomnia of Liver qi stagnation (LQS) and Non-Liver qi stagnation (NLQS). Methods: 56 patients were classified as LQS or NLSQ type insomnia through the insomnia differentiation questionnaire. The depression scores between the groups were compared through beck depression inventory (BDI), and the sleep quality was compared through Pittsburgh sleep quality index (PSQI) and Insomnia Severity Index (ISI). We analyzed the sleep efficiency, total sleep time, total awake frequency, total and average awake time through actigraph. For the tongue diagnosis, the distribution of tongue coating in six areas were measured through Winkel tongue coating index (WTCI). Linear discriminant analysis was performed to observe the differences in composition of microbial strains between the groups. Results: The scores of BDI, ISI and PSQI were significantly higher in LQS group. The total sleep time in LQS group was significantly less than that of NLQS group. Among the areas of tongue, according to the WTCI, the amount of tongue coating in zones A and C was significantly small. In oral microbial analysis, there was no significant difference between the groups at the phylum level. At the genus level, Prevotella, Veillonella, and Streptococcus were predominant in LQS group, whereas Prevotella, Neisseria, and Streptococcus in NLQS group. Conclusions: It was meaningful that insomnia was more likely in LQS group than in NLQS group, and the composition of oral microorganisms was significantly different, which could lead to the diseases caused by stress.

주요우울장애와 소뇌실조증으로 진단된 환자의 정신 증상에 대한 한의 복합 치료에서의 정신요법적 접근 1례 (A Case Report of Psycho-Therapeutic Approach in Combined Korean Medicine Treatment for Psychiatric Symptoms in a Patient with Major Depressive Disorder and Cerebellar Ataxia)

  • Objectives: To report the effectiveness of a comprehensive Korean medical approach, incorporating acupuncture, herbal medicine, Chuna therapy, and traditional Korean psychotherapy, in the management of a patient with cerebellar ataxia and major depressive disorder. Methods: A 45-year-old female patient diagnosed with cerebellar ataxia and major depressive disorder received a comprehensive Korean medicine treatment, including acupuncture, moxibustion, herbal medicine, Chuna therapy, and traditional Korean psychotherapy such as Giungoroen therapy, IiGyeungByunQi therapy, M&L therapy, and Emotional Freedom Technique (EFT). The efficacy was assessed through both subjective symptom reports and a range of psychological assessment tools, including the Minnesota Multiphasic Personality Inventory-2 (MMPI-2), Korean Symptom Checklist-95 (KSCL-95), Korean Beck Depression Inventory-II (K-BDI-II), Spielberger's State-Trait Anxiety Inventory XYZ form (STAI-XYZ), Korean version of the State-Trait Anger Expression Inventory (STAXI-K), Padua Inventory-Washington State University Revision (PI-WSUR), Penn State Worry Questionnaire (PSWQ), and the Korean Version of the Insomnia Severity Index (ISI-K). Results: Throughout treatment, the patient experienced marked improvements in physical symptoms, notably dizziness, and gait instability. Psychological evaluations demonstrated significant reductions in anxiety, depression, and insomnia. The overall quality of life was enhanced, with sustained benefits observed during follow-up assessments. Conclusions: This case suggests that a comprehensive Korean medicine approach that integrates acupuncture, herbal medicine, Chuna therapy, and traditional Korean psychotherapy may be effective in alleviating both physical and psychological symptoms associated with cerebellar ataxia and major depressive disorder.

지역사회 재가노인의 건강증진을 위한 한방간호중재 프로그램의 적용효과 (Effects of Oriental Nursing Intervention Program for Health Promotion of the Elderly in the Community)

  • 목적 : 본 연구의 목적은 지역사회 재가 노인들을 대상으로 한 한방간호중재 프로그램이 신체 건강 상태, 일상 생활 활동 (ADL), 우울증, 삶의 만족도 및 양생에 미치는 영향을 조사하기 위해 수행되었다. 방법 : 본 연구는 비동등성 대조군 전후 설계이다. 연구 참여자들은 서울 일개 지역사회에 거주하는 65세 이상의 노인으로 정상적인 인지 상태와 의사소통이 가능한 노인으로 총 79 명의 노인이 (실험군 : n=39, 대조군 : n=40) 연구에 참여하였다. 대상자는 편의표출통해 모집되었고 무작위 할당법으로 실험군과 대조군으로 선정되었다. 한방간호중재 프로그램은 경락체조와 기공요법의 하나인 팔단금으로 구성하였다. 프로그램은 총 10주 동안 총 20회기로 구성되었다. 측정 변수로는 신체 건강 상태, 일상 생활 활동 (ADL), 우울증, 삶의 만족도, 양생이었고, SPSS WIN21.0에 의해 기술 통계, x2-test 및 독립 t-test를 사용하여 데이터를 분석 하였다. 결과 : 실험군의 신체 건강 상태(t=10.295, p<.001), ADL(t=7.571, p<.001), 우울증 (t=-15.434, p<.001), 삶의 만족도(t=21.257, p<.001), 그리고 양생(t=9.527, p<.001)은 통계적으로 유의한 차이가 있었고 중재의 결과 모든 변수에서 점수가 향상됨을 보였다. 결론 : 본 한방간호중재 프로그램은 지역 사회에 거주하는 노인들의 신체 건강 상태, ADL, 우울증, 삶의 만족도 및 양생을 개선하는 데 효과적이었다. 이 중재 프로그램은 지역 사회에서 노인의 건강 증진을 위해 사용될 수 있다.

교감단의 안정성과 임상적 적용에 대한 연구 (A Clinical Study to Assess the Safety of GyoGam-Dan)

  • Objective: 'GyoGam-Dan' is known as a medicine to cure depression in Donguibogam (traditional Korean oriental medicine book). GyoGam-dan which is composed of Cyperi Rhizoma L. and Poria cum Radix pini has been used by basic prescription of asymptom related to all kinds of Qi(氣) disease. Although it has been clinically used for a long time, its safety have never been proved till now. Methods: We had collected data of 610 cases of patients taking 'GyoGam-Dan' at Wonkwang univ. oriental medical center for a period of three years (2005.3.09-2008.04.30). On the patients' blood, the levels of LFT and RFT were examined and the residual levels of heavy metals and pesticides were measured. Results: There were 610 patients who had taken GyoGam-Dan for about three years. In the patients, the most common diseases is depression, followed by palpitation, and then insomnia. Among the patients' estimations on its effectiveness, there were many of positive ones. The blood analyses of patients who had taken GyoGam-Dan did not show any problems and moreover there was not any problems in the levels of residual heavy metals and agricultural pesticides. Taken together, GyoGam-Dan was proved to be a safe medicine for health. Conclusions: Our clinical and chemical studies show that GyoGam-dan has the anti-stress effect and is a safe medicine of which side effect does not exist. Hereafter, Nigorous clinical trial on GyoGam-dan is required.

A Trend Analysis of Case Reports and Case Series on Hwa-byung in Korea

  • Objectives: This study aimed to provide a basis for treatment of Hwa-byung, a mental disorder frequently found among Koreans, by analysing published case reports and case series. Methods: In June 2015, a literature search for case reports and case series describing interventions for Hwa-byung was conducted in 7 databases for research publications in Korea, using Korean and English terms. Results: Thirty-two papers, reporting 439 cases of Hwa-byung, met the inclusion criteria. Since the first case report on Hwa-byung in 1996, case reports were continuously published until 2012. A total of 22 interventions were described, of which herbal medicine (71.9%), acupuncture (65.6%), and moxibustion (34.4%) were the most frequently described interventions. Twenty-five papers reported patterns, for which liver qi depression (28%) was most frequently used. A total of 48 outcome measures were used in 32 papers, of which Beck Depression Index (46.9%) and Symptom change (43.8%) were used in at least 10 papers. Conclusions: Since only 32 case reports/case series have been published on Hwa-byung in Korea, a strong basis for recommending standardized treatments is lacking. Therefore, a sufficient number of clinical studies, in particular randomized clinical trials (RCTs), are warranted to provide a clinical basis for treatment of Hwa-byung.

  • The clinical study was carried out the 33 patients with insomnia who were treated in Daejeon University Oriental Hospital from 17 March 1997 to 12 May 2001. The results were summarized as follows. 1. The ratio of male and female was 10:23 and sleep initiation insomnia and sleep maintenance insomnia were the highest frequence(97%), the beginning of insomnia was frequent at 30s(27.3%). 2. Living accident by mental attack was the most inducing factor and many patients came to our hospital by way of west-neuropsychiatry(55.6%). 3. In admission period most of the patients were within 15 days(78.8%) and Liver-Qi- depression was the main cause and in classification of Four Human coporeal constitution the number of patients Sho-Eum-In(少陰人) was remarked mostly and in distribution of the prescription drugs of growing heart and warming gall bladder such as GUIBIONDAMTANG(歸脾溫膽湯) and ONDAMTANGGAMI (溫膽湯加味) were many, 4. Relatively most patients were well treated(69.7%) and insomnia was mainly caused by Anxiety Disorder and Depression and Hwabyoung and in the age distribution the highest frequence was 30s but we know insomnia appeared at all ages. 5. In distribution of the period of the clinical history was various and within 15 days were all improved and the treatments group of drug and acupuncture and aid treatments etc. and hypnotics was more improved than not used hypnotics treatments group but using hypnotics was temporal. 청소년 자살 중재연구 동향 (Trends in Research on Adolescent Suicide Interventions)

  • Purpose: This study was designed to analyze recent trends in adolescent suicide intervention research in Korea and to suggest future research directions in this area. Methods: Studies Thirty-four studies selected from http://www.riss4u.net over the for last 20 years were analyzed by field and design of the study, study participants, and the outcome variables used in intervention studies. Results: Nineteen (55.9%) of the 34 studies were conducted in the fields of welfare (9), psychology (5) and nursing science (5). Ordinary adolescents were the most frequently studied participants. A nonequivalent control group pretest-posttest design was used in twenty one (61.7%) of the 34 studies. The most frequently measured outcome variables were depression, suicidal ideation and self-esteem. Conclusion: These results suggest that research on adolescent suicide intervention programs is expanding with a focus on ordinary adolescents as and subjects. In order to prevent suicide, research on family, teachers, and friends, who all are important parts of a teenager's support system, is needed. It is also necessary to develop a post-management intervention program to prevent recurrence in high-risk teenagers who have attempted suicide.

Complementary and Alternative Medicine Among Older Adults in the United States: Current Evidence and Future Directions

  • Complementary and alternative medicine has gained popularity and respectability in recent years in the United States. Since aging is often associated with chronic health conditions that commonly lead to physical and psychosocial disabilities (e.g., depression, functional and/or cognitive disabilities, and decreased quality of life), older adults often seek options to maintain health and treat chronic conditions as an adjunct to conventional medical care. Herbal products, the most commonly used among various complementary and alternative medicines (CAM), should be used with caution due to potential herbal-drug interactions (related to polypharmacy) and herbal-disease interactions (related to comorbidities). Five of the most common chronic conditions in older adults are chronic pain, cardiovascular problems, hypertension, diabetes, and chronic lung problems. A high rate of falls or risk of falling is also a problem unique to this older population. For these conditions, only a few types of CAM (e.g., acupuncture, qi gong, tai chi) were tested, with promising results. However, in spite of evidence supporting the use of certain types of CAM to alleviate some common chronic conditions, findings are limited in terms of other types of CAM tested and both short and long-term effects. More rigorous clinical trials of various CAM types are thus warranted to advance scientific knowledge and establish evidence-based practices to care for the growing number of older adults who deserve to have a better quality of life.
