We performed numerical simulations of blood flow in an arterial cerebral artery aneurysm to investigate the hemodynamic behavior after coil embolization. A patient-specific model was created based on CTA data. We also conducted the coil embolization simulation to obtain the coil placement within the aneurysm. Blood was assumed to be an incompressible Newtonian fluid, and both the vessel and coil were considered rigid walls. The pulsatile boundary condition was applied at the inlet, and the outflow boundary conditions were used at the outlets. Our findings demonstrated that the coil embolization significantly reduces the blood volume flowrate entering the aneurysm by effectively blocking the inflow jet, leading to a decrease in both TAWSS and WSS, especially at the systolic peak in the impingement zone. While several high OSI regions disappeared over the aneurysm surface, we observed high OSI regions with a relatively small area where the coil did not completely occlude the aneurysm. Overall, these results quantitatively analyzed the effectiveness of coil embolization by focusing on hemodynamic indicators, potentially preventing aneurysm rupture. The present work could contribute to the development of patient-specific coil embolization.
The pressure difference across stenotic blood vessels is a commonly used clinical metric for diagnosing many cardiovascular diseases. At present, most clinical pressure measurements rely solely on invasive catheterization. In this study, we propose a novel method for non-invasive pressure estimation using the incompressible Navier-Stokes equations and a 3D multi-path integration approach. We verify spatio-temporal convergence on an in-silico dataset of a cylindrical straight pipe phantom with steady and pulsatile flow fields. We then evaluate the proposed method on an in vitro dataset of reconstructed control, pre-operative, and post-operative carotid artery cases acquired from 4D flow MRI. The performance of our method is compared to existing approaches based on the pressure Poisson equation and work-energy relative pressure. The results demonstrate the proposed method's high accuracy, robustness to spatio-temporal subsampling, and reduced sensitivity to noise, highlighting its great potential for non-invasive pressure estimation.

Objective: The study was conducted to investigate the effect of correct occlusion of the left atrial appendage (LAA) on intracardiac blood flow and thrombus formation in patients with atrial fibrillation (AF) using four-dimensional (4D) flow magnetic resonance imaging (MRI) and three-dimensional (3D)-printed phantoms. Materials and Methods: Three life-sized 3D-printed left atrium (LA) phantoms, including a pre-occlusion (i.e., before the occlusion procedure) model and correctly and incorrectly occluded post-procedural models, were constructed based on cardiac computed tomography images from an 86-year-old male with long-standing persistent AF. A custom-made closed-loop flow circuit was set up, and pulsatile simulated pulmonary venous flow was delivered by a pump. 4D flow MRI was performed using a 3T scanner, and the images were analyzed using MATLAB-based software (R2020b; Mathworks). Flow metrics associated with blood stasis and thrombogenicity, such as the volume of stasis defined by the velocity threshold ($\left|\vec{V}\right|$ < 3 cm/s), surface-and-time-averaged wall shear stress (WSS), and endothelial cell activation potential (ECAP), were analyzed and compared among the three LA phantom models. Results: Different spatial distributions, orientations, and magnitudes of LA flow were directly visualized within the three LA phantoms using 4D flow MRI. The time-averaged volume and its ratio to the corresponding entire volume of LA flow stasis were consistently reduced in the correctly occluded model (70.82 mL and 39.0%, respectively), followed by the incorrectly occluded (73.17 mL and 39.0%, respectively) and pre-occlusion (79.11 mL and 39.7%, respectively) models. The surfaceand-time-averaged WSS and ECAP were also lowest in the correctly occluded model (0.048 Pa and 4.004 Pa-1, respectively), followed by the incorrectly occluded (0.059 Pa and 4.792 Pa-1, respectively) and pre-occlusion (0.072 Pa and 5.861 Pa-1, respectively) models. Conclusion: These findings suggest that a correctly occluded LAA leads to the greatest reduction in LA flow stasis and thrombogenicity, presenting a tentative procedural goal to maximize clinical benefits in patients with AF.

혈관의 협착으로 인만 혈류의 교란과 그로 인한 초음파 반향의 영향을 튜브에 편심 협착 (eccentric stenosis)을 부착하고 돼지 혈액을 이용해 실험실에서 연구했다. 상용화된 GE LOGIQ 700 Expert 시스템과 M12L 의 선형 트랜스듀서를 이용해 B-모드 영상을 얻은 후 편심 협착 상류 지점과 하류 지점의 초음파 영상을 비교하여 분석하였다. 분당 20 회와 40 회의 박동율과 혈류속도를 바꿔가며 혈류의 교란에 따른 혈액에서의 초음파 영상을 분석한 결과, 편심 협착으로 인한 혈류의 교란으로 적혈구 응집현상이 달라져 복잡한 초음파 반향 분포가 형성되었다. 비침습적 실시간 초음파 영상은 편심 협착으로 인한 혈류의 교란과 그로 인한 혈액의 적혈구 응집 현상을 이해하고 연구하는데 도움이 된다.
Purpose: To evaluate the physiologic background of poorly visualized aneurysms during DSA a contrast-enhanced MRA(CEMRA) due to hemodynamical isolation on clinical and experiment data. Method: Two cases of intracranial aneurysm which were poorly visualized on DSA a CEMRA and one case of intracranial aneurysm which had poor turnover of contrast mediu during DSA were selected for this clinical study. We evaluated the turnover of blood in t terminal aneurysm of handmade elastic silicon phantoms for comparative experiment. Flo experiments with DSA and contrast enhanced MRA were performed in elastic phantoms aneurysm with 3 different diameters (2, 5 and 10 mm) of neck mimicking basilar ti aneurysm, attached to pulsatile pump similar to that of human physiologic parameters. W compared the results with those of computational flow dynamics(CFD).
A 37-year old man was admitted due to the left subauricular mass of 6 month duration which was 3 x 4cm sized, pulsatile and slowly growing He was suffered from the intermittent left facial and auricular pain radiating to the occipital area. The carotid angiography revealed 3x4cm sized saccular aneurysm of the left internal carotid artery just above the carotid bifurcation, extending to the mandibular angle level. He was planned to be operated under the direct clamp of internal carotid artery or shunting procedure. But, the back pressure of the internal carotid was 35mmHg, which suggested adequate cerebral collateral. Thereby, aneurysmectomy and restoration of cerebral blood flow with saphenous vein graft was done under the direct clamp of internal carotid artery for 25 minutes. Although mild transient neurologic sequelae such as mydriasis, tongue deviation for 10 days, he recovered completely without any complication. The aneurysmal sac had no thrombus and pathologic finding was compatible with congenital origin.
The importance of shear thinning non-Newtonian blood rheology on the hemodynamic characteristics of idealized cerebral saccular aneurysms were investigated by carrying out CFD simulations assuming two different non-Newtonian rheology models (Carreau and Ballyk models). To explore effects of vessel curvature, a straight and a curved vessel geometry were considered. The wall shear stress(WSS), relative residence time(RRT) and velocity distribution were compared at the different phases of cardiac cycle. As expected, blood entered the aneurysm at the distal neck and created large vortex in both aneurysms, but with higher momentum on the curved vessel. Hemodynamic characteristics such as WSS, and RRT exhibited only minor effects by choice of different rheological models although Ballyk model produced relatively higher effects. We conclude that the assumption of Newtonian fluid is reasonable for studies aimed at quantifying the hemodynamic characteristics, in particular, WSS-based parameters, considering the current accuracy level of medical image of cerebral aneurysm.
Microsurgical free-tissue transfer has allowed surgeons to salvage injured limbs but choosing appropriate healthy recipient vessels has proved to be a difficult problem. Retrograde flow flaps are established in island flaps. Retrograde flow anastomosis could prevent the possible kinking and twisting of the arterial anastomosis. By not interrupting the proximal blood flow to the fracture or soft tissue defect site, the compromise of fracture or wound healing might be prevented. We wished to estabilish an animal model in rat for a retrograde arterial flow based free flap. Nembutal-anesthetized male rats; weighing 250 to 300 gm, were used. The femoral artery and common carotid artery were exposed and divided. The systemic and retrograde arterial pressure were quantified by utilizing a parallel tubing system connected with peripheral arterial line. In this study, the retrograde flow was not pulsatile and the retrograde arterial pressure was 64-65mmHg, with a mean arterial pressure of 106-109mmHg. An epigastiic skin flap, measuring $3{\times}3cm$, was raised with its vascular pedicle. The epigastric free flap was transfered in the same rat from femoral vessels to carotid vessels in end to end fashion. We anastomosed the donor arteries to the distal parts of the divided recipient arteries and the donor veins to the proximal parts of the recipient veins. Twelve experiments were performed and the transplantations succeeded in 75 percent of them. In the remaining 25 percent, the experiments failed due to thrombosis at the site of anastpmosis, or other causes. This animal model represents an excellent example of retrograde arterial flow free flap transfer that is reliable.

A ventricular assist device (VAD) is used for bridge to heart transplantation and heart diseases. Knowing the status of a pneumatic pulsatile VAD when implanting it into the body is important: when the velocity of blood flow through the VAD is slow, a thrombus may occur, and thrombosis can be fatal to a patient. In order to determine the state of a VAD, various sensors need to be implanted. Because this introduces the risk of infection and difficulties with sensor management, we developed a method for estimating the state of a VAD indirectly via the pressure in an air tube that can be measured in vitro. We compared the measured values to in vitro experimental results. The estimated and measured values showed some errors, but the accuracy can be improved by refining the estimation process to minimize the risk of infection.

The two major problems related to the blood flow in replaced prosthetic heart valve are thrombus formation and hemolysis. Reliability of prosthetic valve is very important because its failure means the death of patient. There are many factors affecting the valvular failures and their representatives are mechanical failure and thrombosis, so early noninvasive detection is essentially required. The purpose of this study is to detect the various thromboses formation by using acoustic signal acquisition and its spectral analysis on the frequency domain. We made the thrombosis models using Polydimethylsiloxane (PDMS) and they are thrombosis model on the disc, around the sewing ring and fibrous tissue growth across the orifice of valve. Using microphone and amplifier, we measured the acoustic signal from the prosthetic valve, which is attached to the pulsatile mock circulation system. A/D converter sampled the acoustic signal and the spectral analysis is the main algorithm for obtaining spectrum. Then the spectrum of normal and 5 different kinds of abnormal valve were obtained. Each spectrum waveform shows a primary and secondary peak. The secondary peak changes according to the thrombus model. To quantitatively distinguish the frequency peak of the normal valve from that of the thrombosed valves, analysis using a neural network was employed. Acoustic measurement has been used as a noninvasive diagnostic tool and is thought to be a good method for detecting possible mechanical failure or thrombus.
