A Research on the improvement measures about Community-based Korean medical public health service in public service center (보건소 한방지역보건사업의 개선방안에 대한 연구)

  • Each Local public heal public health service center is in difficult situation on programing and performing of local public health service. The purpose of research is to inquire of the present state of local public health service and search long-term developmental methods. The first, in order to accomplish this purpose, I distributed questionnaires to Korean medical doctors, who are worked in public health service center and researched that. The second, based on these findings, I proposed planning for upcoming project for activating local public health service on Korean medicine.

A Study on the appropriateness of Public Facilities Scale (기능전환에 따른 동사무소 공간의 적정규모에 관한 연구)

  • With the changing consciousness of community people and the rising standard of living, there has recently been an emphasis on the creation of public facilities' new functions and their role as local community facilities. This changing trends are accordingly requiring a change in spatial structure of the public facilities. In this study, an analysis was conducted with 24 public facilities situated in the Buk-gu district of Daegu to identify the adequacy of their space scale after functional variation. The results of this study are summarized as follows. 1) The site area of public facilities has been being widened after functional variation since more spaces are needed to provide an outdoor resting space with community people, expand a parking space, and operate a community center. 2) The factors that had a direct effect on the change in the use of space are the reduced space for administrative work and the expanded scope of the community center's function. Specifically, the areas of activities for civil service and administrative work and for reserve forces have been reduced due to reduction of function, and floor division by each function group has been becoming stricter due to addition of a community center's function. 3) It was shown that in terms of the space for functions of public facilities, spaces for civil service and waiting have increased mostly after function variation. After functional variation, the scale of spaces for civil service and administrative work has been being planned within the range of $200\~300 m^2$, regardless of the number of population to be covered by public facilities. 4) The space for public use is showing the greatest increase in public facilities which have been built after functional variation. The major factors seem to be the increased moving passages, the expanded convenient facilities for community people, and the increased convenient facilities for disabled. Facilities scale plans have been being made more systematically, compared to the conventional facilities.

Does Local Government Affect Community Satisfaction of the Younger Generation in Rural Areas? The Case of Jeonbuk, South Korea

  • This study examines the relationship between public service satisfaction, trust in local government, and community satisfaction. Previous studies on community satisfaction have insufficiently dealt with public services or trust in local government and have not fully conducted an integrated analysis. To close these knowledge gaps, this study includes public service satisfaction that was constructed with the subcomponents economic support, education, and public safety and trust in local government as factors affecting community satisfaction. Moreover, this study verified the mediating effect of trust in local government between public service satisfaction and community satisfaction. Online surveys were carried out with 980 residents in Jeonbuk, Korea, and structural equation modeling (SEM) was employed. The results indicated that public service satisfaction affected community satisfaction directly or indirectly. In particular, the satisfaction with public safety influenced community satisfaction both directly and indirectly. In addition, trust in local government had a mediating effect between all sub-components of public service satisfaction and community satisfaction. Today, regional disparity between urban and rural areas in developing countries is widening. As a result, residents in rural areas are gradually moving to urban areas. Therefore, rural areas need to increase the community satisfaction of their residents. In this regard, this study suggests important policy implications for community satisfaction enhancement.

A Study on job satisfaction and strategies to improve the system of Public Health Doctors in Charge of Community Health programmes (보건사업전담 공중보건의사 직무만족도 및 제도 개선방안)

  • This study was carried out to investigate the activities and job satisfaction of public health doctors in charge of community health programmes. Subjects were 138 public health doctors charged with community health project, registered in the roll of public health doctors, made out by Central Supporting Team for the District Health Work in 2002. Data was obtained from Ninety-six public health doctors. The response rate was 70%. The survey was conducted from March to April in 2003 bye-mail. The items of the questionnaire consisted of general characteristics of participants, understanding of their activities, support system, job satisfaction, and suggestions to improve the system. Collected data was analyzed using PC-SPSS 10.1. Descriptive analysis, t-test, and ANOVA test were used. The results are as follows: Most of the respondents showed a low participation rate in community health services, but they agreed to the importance of their activities and the necessity to modify and improve the system. 70% of the respondents were dissatisfied with their jobs. The participation in health planning and programmes of health center, and the degree of acceptance of their opinion from health center workers were significantly related to their job satisfaction. The participation rate of the public health doctors having specialty, in community health services is higher than that of the others. In terms of the supports for system operation, the reflection of one's intention in job arrangement process, education and public relation of this system, and the administrative and financial supports made significant differences in the job satisfaction and the accomplishment of their duty. The respondents hoped that the administrative and financial supports for public health doctors in charge of community health programmes should be reinforced to motivate them. They also wanted that they could keep from being overloaded with clinical services. They favored to conduct home visit, community diagnosis, health planning, and health promotion programs as their appropriate activities. From these results, we suggest some strategies to motivate and empower the public health doctors in charge of community health programmes.

Contexts and Directions of the Collaboration between Public Health Centers and Community Partners for Health Promotion (보건소의 지역사회 건강증진 협력 모색)

  • Objectives: This paper discusses the current contexts of the collaboration between public health centers(PHCs) and community partners for health promotion. Then it suggests directions for the development and enhancement of the collaboration. Methods: The discussion in this paper is based on key literature on community health promotion, including literature reviews and case reports. Results: PHCs are mostly engaged in networking and cooperation rather than collaboration with the community. A typical pattern of cooperation is where PHCs provide healthy-setting types of programs to the community in single-partnered relationships. Current cooperation rarely involves co-planning by a multi-partnered partnership, and is greatly influenced by the interest of PHC directors and PHC performance evaluation indicators. Systems change is recommended to foster collaboration for community health promotion. Such change involves: shared understanding of health promotion and collaboration, inclusion of collaboration mechanism in public health governance, leadership development, capacity enhancement of all partners. role definition of PHCs for community collaboration, and development of collaborative system, at the least. Conclusions: At this point where collaboration should be more than rhetoric, multi-faceted, intersectoral, and concurrent approaches are required to create discourses, to develop cases, and to share experience for actual realization of collaboration for community health promotion.

The Follow-up Study of Changes in Frailty in Elderly Receiving Home Health Care of the Public Health Center

  • Purpose: The purpose of this study was to follow-up the frailty of the old who received home health care by Registered Nurse in Public Health Center over 8 years. Methods: We used the second wave data which was a comprehensive longitudinal data set, Public Health Information System of a public health center located in Seoul from 2010 to 2018. For statistical analysis, a mixed model of repeated measures by R program was used. Results: Frailty (range: 0~31) was getting worse significantly from 5.38 on registration to 6.54 on 4th year, 7.40 on 7th year, 7.69 on 8th year with adjustment for age, sex, economic status, the number of family, and the number diseases. The coefficient of parameters with frailty change was serviced year (β=0.29, p<.001), age (70~79 to 60~69; β=0.98, p=.018) and sex (female to male; β=2.55, p<.001). Conclusion: This study showed that the home visiting health service needs to take attention to aged 70s and over, female. The home health care of public health center need to be extended more practical and effective services in terms of 'community care'and 'ageing in place'.

Evaluation of Community Health Nursing Practice and Nursing Student's Clinical Experience at Public Health Centers (일개 대학 간호학과생의 보건소 실습평가와 경험에 관한 연구)

  • Purpose: To evaluate community health nursing practice by nursing students for basic data to improve nursing practice education, and to determine the clinical practice experiences of nursing students at public health centers. Method: According to methodological triangulation, all data was collected from March 2001 to June 2001 through prepared questionnaires with 62 Participants, and in depth interviews with 2 Participants, and content analysis with 8 Participants. Results: 1. Clinical practice content was needed to make up professionally. 2. Clinical practice instructor was satisfied with teaching by community health nursing professor, but not by public health center nurse. 3. Clinical environment was perceived as good. 4. Clinical practice time was perceived suitable, but practice evaluation was given a low grade. 5. Experience of nursing students was grouped into 9 categories and 20 themes. 9 Categories are as follows: change of public health center image, difficulty in the personal relationship, positive personal relationships, doubt about the attitude, doubt about the task, tension, worth, ambivalance of the conference, feeling about the end stage of the clinical practice. Twenty themes are stereotypic of the public health care center: recognition, difficulty between tasks and interrelationships with the clients, decrease of self esteem due to deficiency of trust with the students, exchange of feelings in the relationships, the situation of clients was not considered, feelings were disregarded, not systematic, regarding students as supplementory manpower, independent work but not enough contents, anxiety & fear, self-trust, be skilled in the practice, pragmatic knowledge formation, understanding of the old person, self-study of oneself. live up to the expectation, consider as the job, dissolve the stress and impose burden, feeling the lack regarding active approach in the practice. Conclusion: The students have perceived the need of community health nursing through the clinical practice. Clinical practice of community health nursing requires content supplement, correction of instruction by public center nurses, environmental alteration, evaluation criteria set up. Experience of clinical practice in the public health center was as follows: Getting rid of the stereotype about public health center, nursing students comprehended variety and were worth while to do clinical practice in spite of tension& anxiety and fear. And they thought to be a community nurse someday.

The Effects of Quality of Care, Image, Role Performance Perceived by Community Residents on Medical Service Satisfaction to Public Hospitals (지역주민이 인지하는 의료의 질, 이미지, 역할수행이 공공병원 서비스 만족도에 미치는 영향)

  • Background: This study aims to explore the effects of quality of care, image, and role performance perceived by community residents on medical service satisfaction to public hospitals. Methods: The subjects of this study were selected in the community residents around 39 district public hospitals. The questionnaire were included 4 factors and 16 items. The data were collected utilizing call-interview by a survey company. Results: The community satisfaction was positively correlated with quality of care, image, and role performance of public hospitals (p<0.001). As the results of multiple logistic regression, the significant variables of community satisfaction were quality of care (odds ratio [OR], 1.353; 95% confidence interval [CI], 1.211 to 1.511), image (OR, 1.487; 95% CI, 1.280 to 1.727), role performance (OR, 1.240; 95% CI, 1.085 to 1.416) among subjects having use experience of public hospitals. The significant variables of community satisfaction were quality of care (OR, 1.240; 95% CI, 1.175 to 1.309), image (OR, 1.328; 95% CI, 1.232 to 1.432), age (OR, 3.051; 95% CI, 1.385 to 6.724), monthly incomes (OR, 0.420; 95% CI, 0.198 to 0.892) among subjects not-having use experience of public hospitals. Conclusion: Public hospitals have to effort to improve image and satisfaction of community through providing quality of care, and role performance. It is possible to support them by the central and local government.

A Research on Community Center Facility Types and the Characteristics in Seoul - Focused on the Areas of Low-rise and High-density - (서울시 주민자치센터의 시설 형태와 특성에 관한 연구 - 저층 고밀도 지역의 사례 중심 -)

  • Community centers have been public facilities for the welfare and conveniences for the residents since 1999. Currently, the numbers of community centers meet the demands in quantity, but they do not satisfy the quality service needs for the improved life style of residents and the activation of local community. Therefore, the purposes of this research are to find out the current status, types and characters of facilities for the quality improvement of community centers. As the research methods, 'home pages of community centers', 'gujung backseo' were used, and '40 case studies' were completed. Case studies were done by observation and research on the actual conditions. The case works were done between July- September in 2008. Followings are the results of the study; 1. Area measurements of the community centers are being increased since 2000. 2. Complex facility types of community centers are mainly public administrative facilities (community center)+public administrative facilities of mixed type. 3. There are three architectural types of community centers: horizontal type, vertical type, mixed type. 4. There are three classified entrance types for the community centers. 5. There are twelve classified facility types for the community centers. Community centers are usually mixed with diverse facilities, especially welfare facilities and cultural facilities. Proper community center types which contain the functional characters and varieties of facilities should be developed, and these active centers should be used conveniently by residents, thus, further study is required in this regard.

A Study to Improve Management System through the Analysis of Needs and Problems of Community Welfare Center and Management Office in Permanent Public Rental Housing Complex (영구임대주택 단지내 사회복지관과 관리사무소의 업무 관련 요구 및 문제점 분석을 통한 관리체계 개선방안)

  • This study aimed to suggest the implications to improve the management system of permanent public rental housing. For this purpose, qualitative data were collected by using the in-depth interview with 13 staffs of community welfare center and management office in permanent public rental housing complexes and 2 times of advisory committee meetings. To analyze the data gathered in this study, the constant comparative method of data analysis was used. Based on the results, some implications including institutional support and social complementary cooperation were suggested in terms of short and long term period.
