Factors influencing quality of life in low-income women with young children in Korea: a cross-sectional study

  • Purpose: This study aimed to investigate the effects of health-promoting behaviors (HPB), marital intimacy, and parenting stress on the quality of life (QoL) of low-income women with young children in Korea, an underserved group. Methods: This cross-sectional survey employed a descriptive correlational design. Using convenience sampling, 123 low-income women with children younger than 6 years were recruited from 14 health and community centers in Jeonju, Korea, from June 2020 to May 2021. Participants completed a questionnaire on QoL, HPB, marital intimacy, and parenting stress. Data were analyzed using descriptive statistics, independent t-test, analysis of variance, Pearson correlation, and hierarchical regression analysis. Results: Participants, who were on average 37.41±3.65 years old and had 1 to 2 children (n=98, 79.7%), reported a mid-level (3.14 out of 1-5) of QoL. Marital intimacy (β=.38, p<.001) was the most influential factor on the QoL of low-income women with young children. In descending order, HPB (β=.35, p<.001) and non- employment status (β=-.21, p=.003) had a significant influence on QoL (F=15.64, p<.001), and the overall explanatory power was 49.0%. Conclusion: Considering the mid-level QoL of low-income women with young children, programs aimed at improving the QoL of low-income women need to promote marital intimacy and maintain HPB, while considering their employment status. Strategies that include couple counseling, health care to encourage healthy lifestyles, and reemployment education are needed.

임부의 임신스트레스에 영향을 미치는 요인에 관한 단면연구 (Factors influencing pregnancy stress in pregnant women in Korea: a cross-sectional study)

  • Purpose: The purpose of this study was to investigate the association between maternal knowledge and social support on pregnancy stress among pregnant women in Korea. Methods: The participants in this study were 148 pregnant women in Korea, recruited from online communities on pregnancy and/or childbirth, from June 2019 to April 2020. The collected data were analyzed using the independent t-test, one-way analysis of variance, Pearson correlation coefficient, and multiple regression. Results: Participants were at average 18.25±8.28 weeks gestation, 56% were in the second trimester, 31% had one or more health issues in the current pregnancy (e.g., hyperemesis gravidarum), and 76% were first-time mothers. Participants had moderate levels of pregnancy stress (mean, 23.09±7.11 points out of 48) and maternal knowledge (mean, 14.42±4.67 points out of 21), whereas social support was somewhat high (mean 45.88±7.81 points out of 60). Pregnancy stress was weakly negatively correlated with social support (r=-.37, p<.001). Main source of pregnancy information (β=-.21, p=.011), marital satisfaction (β=-.18, p=.036), and social support (β=-.19, p=.038) were identified as significant factors affecting pregnancy stress, and these variables had an explanatory power of 22.7% for pregnancy stress. Conclusion: Based on these findings, nurses should assess pregnancy-related stress during pregnancy and consider main source of pregnancy information and marital satisfaction when providing education or counseling. Moreover, strategies to reduce pregnancy stress through social support are needed to improve the quality of life for pregnant women.

젊은 성인에서 사회불안 증상과 아동기 외상 경험 간의 관련성에서 긍정자원의 매개효과 (The Mediating Effects of Positive Resources in the Association Between Social Anxiety Symptoms and Adverse Childhood Experiences in Young Adults)

  • Objective : This study was to examine the impact of adverse childhood experiences on social anxiety symptoms in young adults and verify the mediating effects of positive resources. Methods : Data from 1,317 young adults aged to 18 to 29 years who took part in the university-based cross-sectional survey were analyzed. All participants completed Adverse Childhood Experience (ACE) scale, Social Avoidance and Distress Scale (SAD), and Positive Resources Test (POREST). Results : In young adults, 9.3% had severe social anxiety symptoms. Based on 10 ACE categories, 32.7% of participants reported one or more adverse childhood experience, and 4.5% reported four or more different forms of adverse childhood experiences. Young adults with higher social anxiety symptoms were likely to report more adverse childhood experiences, and less positive resources. Multivariate regression analysis indicated that positive resources moderated the association between adverse childhood experiences and social anxiety symptoms. Conclusion : Based on the results, professionals need to consider early detection of adverse childhood experiences and comorbid social anxiety symptoms. In addition, various positive psychological interventions for individuals with adverse childhood experiences are needed.

Surviving Covid-19 Diagnosis Among Registered Nurses: Reactions, Consequences, and Coping Mechanisms

  • Background: To mitigate the spread of Covid-19, nurses infected with the virus were required to isolate themselves from their families and community. Isolated patients were reported to have experienced mental distress, posttraumatic stress disorder symptoms, and suicide. Though studies have reported the psychological impact of the Covid-19 pandemic, less is known about the lived experiences of nurses who survived Covid-19 infection in sub-Saharan Africa. Methods: A descriptive phenomenological approach was used to study the lived experiences of registered nurses who survived Covid-19 disease. In-depth interviews were conducted among nurses diagnosed with Covid-19 from two hospitals in Kenya between March and May, 2021. Purposive and snowball sampling were used to recruit registered nurses. Data were analyzed using Giorgi's steps of analysis. Results: The study included ten nurses between 29 and 45 years of age. Nurses' experiences encompassed three themes: diagnosis reaction, consequences, and coping. Reactions to the diagnosis included fear, anxiety, and sadness. The consequence of the diagnosis and isolation was stigma, isolation, and loneliness. Nurses coping mechanisms included acceptance, creating routines, support, and spirituality. Conclusion: Our findings aid in understanding how nurses experienced Covid-19 infection as patients and will provide evidence-based content for supporting nurses in future pandemics. Moreover, as we acknowledge the heroic contribution of frontline healthcare workers during the Covid-19 pandemic, it is prudent to recognize the considerable occupational risk as they balance their duty to care, and the risk of infection to themselves and their families.

집단 따돌림 피해학생을 대상으로 한 인지행동적 집단치료 개발을 위한 예비 연구 (THE PRELIMINARY STUDY ON THE DEVELOPMENT OF COGNITIVE-BEHAVIORAL GROUP THERAPY FOR SCHOOL BULLIED)

  • 본 연구는 집단 따돌림 피해학생을 대상으로 시행한 인지행동적 집단치료의 개발을 위한 예비 연구이다. 정상적인 중학교 1학년 학생 202명에게 도덕발달수준척도, 오퍼 자아상척도, 우울 및 상태 ${\cdot}$ 특성불안척도의 분포와 상관관계를 조사하였다. 자아상과 우울, 불안은 서로 강한 상관관계를 보였으나, 도덕발달과는 상관관계를 보이지 않았다. 이는 도덕발달이 정서에 크게 영향받지 않고 인지 발달적 과정에 의존하는 것을 시사한다. 그리고 교사의 관찰에 의해 집단 따돌림 피해를 받으면서 학교 적응에 어려움을 보이는 중학교 1학년 학생 5명의 도덕발달수준척도, 오퍼 자아상척도, 우울 및 상태 ${\cdot}$ 특성불안척도는 전체 집단과 유의한 차이를 보이지 않았다. 이 피해학생들을 대상으로 12회에 걸쳐 인지적 집단 정신치료를 시행하고, 치료 전후에 상기 4가지 척도의 변화를 평가하여 효용성을 검정하였으나 통계학적으로 유의한 변화는 없었다. 그러나 참가 학생들이 치료 기간 중에 기재한 일기에서 확인되는 심리적 마음자세(psychological mindedness)의 향상과 담임 교사의 긍정적인 평가보고는 의미있는 것으로 생각되었다.

오일펜스를 활용한 다수 인명의 구조에 관한 연구 (A Study on Mass Rescue Operation Utilizing an Oil Boom)

  • 2014년 세월호 침몰사고 이후 해상에서의 안전에 대한 국민적 관심이 높아져 왔다. 해상에서 인명구조에 필요한 초기 대응시간을 확보하기 위해 구조기관의 골든타임뿐만 아니라 조난자도 구조대원이 현장에 도착하기까지 필요한 시간을 확보할 필요가 있다. 본 연구는 유류오염방제정(이하 방제정)에 비치된 오일펜스를 활용하여 다수의 인명들을 구조할 때 조난자의 심리적 안정을 유지하고 안전하게 인명을 구조하는 방안에 대한 연구를 진행하였다. 실 해역 실험을 통해 오일펜스의 측면에 구명줄 설치 시 1 m 당 70 kg 성인 2명의 부력을 유지할 수 있고 양측면에는 4명을 유지할 수 있음을 확인하였다. 또한 오일펜스 상부에 탑승 시 8 m 당 성인 3명에 대한 부력이 유지되고 있음을 확인하였다. 구조 방법으로는 방제정에서 설치한 오일펜스 후단을 잡고 고속단정이 조난자에게 접근시키는 것이 가장 신속하고 정확하게 구조하는 방법임을 확인하였다. 이 결과로 오일펜스를 활용하여 구조기관이 구조에 필요한 시간을 확보하고 아울러 조난자가 구조 시까지 심리적 안정을 가질 수 있어 다수의 인명들을 구조할 때 구조 방안으로 활용할 수 있음을 확인하였다.

중환자 보호자의 불안 감소를 위한 단회기 노래중심 음악치료 적용 예비연구 (Pilot Study of Single Session Song-Based Music Therapy for Decreasing ICU Caregiver Anxiety)

  • 본 연구는 중환자 보호자의 불안 감소를 위해 단회기 노래중심 음악치료를 적용한 예비연구로, 총 6명의 중환자 보호자가 음악치료 세션에 참여하였다. 노래중심 음악치료 중재는 초반에 대상자의 현재 정서 상태를 확인하고, 신체적 이완촉진 및 심리적 저항감을 줄이기 위한 음악듣기 혹은 음악에 맞추어 악기(톤차임)를 연주하는 단계를 진행하였고, 이후 노래중심 활동에서는 노래 가사에 대해서 토의하고 실제로 노래를 부르는 과정이 진행되었다. 가사에 대해 토의하면서 대상자가 자신에게 의미 있는 메시지를 확인할 수 있도록 하였고, 노래부르기 동안에는 연구자가 대상자의 정서 상태에 따라 음악 요소(템포, 다이내믹, 리듬, 화성 등)를 조정하며 제공하는 반주에 맞춰 선택된 노래를 부르도록 하였다. 대상자의 변화를 확인하기 위해 참여 전후로 상태불안 척도(STAI) 및 정서 상태에 대한 VAS 척도를 측정하였으며, 중재 과정에서 관찰된 대상자의 언어 반응 역시 분석하였다. 연구 결과, 중재 후 대상자들의 STAI 점수가 유의하게 감소한 것으로 나타났다. 또한 대상자들이 지각하는 정적 정서상태는 유의하게 증가하였고, 단회기 치료과정에서의 심리적 변화에는 개인적 편차가 있는 것으로 나타났다. 결과적으로 단회기 노래중심 음악치료가 중환자 보호자의 불안을 포함한 심리정서적 필요에 효과적으로 개입할 수 있는 가능성을 확인할 수 있었다.

호발암 생존자의 미충족 수요에 대한 문헌 고찰: 유방암, 갑상선암, 대장암, 폐암을 중심으로 (A Literature Review on Unmet Needs of High-Prevalence Cancer Survivors: Focus on Breast Cancer, Thyroid Cancer, Colorectal Cancer, and Lung Cancer)

  • 연구목적 유방암, 대장암, 폐암, 갑상선암 환자의 미충족 수요를 확인하고 관련 요인을 파악하고자 하였다. 방법 학술 검색을 통해 2010년 이후 출간된 암 환자 미충족 수요에 대한 논문을 선정하여 검토하였다. 결과 측정 도구는 암 생존자의 미충족 수요, 지지적 간호요구조사 설문이 주로 사용되었으며, 폐암 환자의 미충족 수요가 상대적으로 높았다. 유방암 환자는 의료시스템 및 정보, 갑상선암 환자는 사후관리와 심리문제, 대장암 환자는 심리, 종합적 암 치료, 폐암 환자는 신체 및 일상생활관리에서 미충족 수요가 높았다. 연령, 경과시간, 불안, 우울 및 디스트레스, 삶의 질은 미충족 수요와 관련이 있었다. 결론 각 환자군의 미충족 요구를 고려한다면 더욱 효과적인 치료 및 지원 프로그램 개발에 도움이 될 것이다.

CES-D와 로샤검사를 통해 본 북한이탈주민의 우울감 (North Korean Defectors' depression through the CES-D and the Rorschach test)

  • 본 연구는 그동안 많은 주목을 받아왔던 북한이탈주민의 우울감에 대해 기존에 주로 사용하던 자기보고식 질문지법이외에도 새로이 투사적 검사법을 사용하여 입체적으로 조명하기 위해 진행되었다. 북한이탈주민의 우울감에 대해 좀 더 정확하게 파악하기 위해 비교집단으로서 남한주민을 포함시켰다. 본 연구는 북한이탈주민 40명과 남한주민 40명의 자료를 토대로 이루어졌다. 연구 결과는 다음과 같다. 첫째, CES-D에서는 북한이탈주민이 남한주민보다 더 높은 우울 점수를 보였다. 그리고 로샤검사 결과에서는 양측 주민들의 DEPI, CDI, S-CON 지표에서 유의미한 차이가 없었다. 둘째, 양측 주민들에게서 성별의 차이를 검증한 결과, 북한 이탈주민 여자의 CDI의 점수가 가장 높았다. 이는 북한이탈주민 남자에 비해 여자가 실제로 심리적인 어려움을 더 크게 겪고 있을 가능성을 시사한다. 셋째, 양측 주민들이 보인 CES-D와 로샤검사의 DEPI 점수 간의 상관계수의 차이 분석에서 유의미한 차이가 나타났다. 결론적으로, 남한주민과 북한이탈주민의 우울감과 관련해서는 자기보고 과정에서 상이한 패턴을 보일 수 있음을 기억할 필요가 있다.

사회적 기술 부족과 충동성으로 인해 또래갈등이 심한 분교아동의 상담사례 (Counseling Case Study of a Child with Peer Confliction due to Lack of Social Skills and Impulsiveness)

  • It seems common for students living at a small county and islands to experience psychological conflicts and be unaccustomed in the peer society because they are not familiar with peer interaction and social skills. This is a case study of L (hereinafter called L) who was grown up in the sheltered school at a small county. L was psychologically disturbed because he couldn't get along well in the transferred school. It is the reason why he had lived in the sheltered school at a small county, so he had not enough exposure to interact with peer and social skills. Sometimes he was obstinate irrationally and when he had trouble with friends, he threw something out or went out of school and tricked juniors dangerously. The fact of disperse with families, parent's indifference, and hate of older brother made L to have ill feeling against family. He had low motivation and low self confident in learning because of short attention time and accumulated poor learning progress. In this study, he was evaluated at various area, such as, intelligent, affective, personal and inter-personal, before counselling. To evaluated the effect of the counselling, K-WISC-III, KPRC, sentence filling test, social adaptation ability test, etc, were administered right after the counselling was over and 8 weeks later. For specific information gathering and analysing, observation diary and deepen counselling were accomplished by homeroom teacher, his mother, and his peers. To correct his problematic behaviors, 13 counseling sessions were accomplished for 6 months and those counselling sessions were recorded and analysed definitely. Followings are the result of this case study. First, he was recovered from the anxiety of inter-personal interaction and he started to interact with peers. The result of sac scale score of KPRC profile was lower than before as much as average student after counseling and 8 weeks later. This reveals that the distress against interpersonal relation have settled. Especially, through the result of sentence filing test, he seemed to feel attachment to peers and be positive, active in the relation of peer. For instance, he was active in the open class lesson and interacted well with peers. It could be said that he overcame the psychological distress comparing with previous time. Second, he could apologize to his peer and juniors for his fault. His attitude were well shown in the letter from an old friend at the sheltered school, average KPRC profiling score comparing with previous counseling time, and remarkable decrease of attack scale score of teacher and peer. Third, his view toward family turn out positive. He recognized his situation that he lived apart from family and even worried about his parent's financial difficulty. Through solving the confliction with his older brother, he could acquire the feeling of family reunion. Fourth, his learning motivation and self-confidence were increased. He confirmed his future positively and he might be judged more attentive because his intelligence index was higher than before as much as average student. With the main goal of this study, verification for effectiveness of counseling. understanding and helping problematic students such as L who lives at a small county and island through investigation of their real situation and problems with the method of counseling and socio-cultural analysis is worthwhile. Identification of ideal relationship with peer is related with positive self-conception, harmonic social adaptation and development of child. It is time to investigate easy adaptive in classroom and well-organised program to acquire general social skills for sheltered school students at a small county and islands.
