  • Objective : The purpose of this report is to know the effect of Jibaekjihwang-tang(知栢地黃湯) on progressive pigmented purpuric dermatosis. Methods : We experienced one case of progressive pigmented purpuric dermatosis treated with Jibaekjihwang-tang(知栢地黃湯). We used visual analogue scale(VAS). Results : After the treatment the grade of VAS was decreased and purpuras, pruritus and dryness of skin were disappeared. Conslusion : Jibaekjihwang-tang(知栢地黃湯) can be used on the treatment of progressive pigmented purpuric dermatosis.

  • Cholestasis results from impairment in the excretion of bile, which may be due to mechanical obstruction of bile flow or impairment of excretion of bile components into the bile canaliculus. When present, cholestasis warrants prompt diagnosis and treatment. The differential diagnosis of cholestasis beyond the neonatal period is broad and includes congenital and acquired etiologies. It is imperative that the clinician differentiates between intrahepatic and extrahepatic origin of cholestasis. Treatment may be supportive or curative and depends on the etiology. Recent literature shows that optimal nutritional and medical support also plays an integral role in the management of pediatric patients with chronic cholestasis. This review will provide a broad overview of the pathophysiology, diagnostic approach, and management of cholestasis beyond the neonatal and infancy periods.

  • A 12-year-old castrated male Maltese dog was presented with a small-sized mass at the left fifth mammary gland. The dog had concurrent poorly responsive pruritus affecting the feet, pinnae, and trunk, a cystolith, and biliary sludge. Serum chemistry revealed elevated liver enzymes. The mammary gland mass was cytologically diagnosed as an early-stage mammary simple carcinoma and was surgically excised. Nine months after the diagnosis of the mammary carcinoma, hyperadrenocorticism was confirmed by adrenocorticotropic hormone stimulation test. This report describes the clinical signs, diagnostic imaging findings, and prognosis in a castrated male Maltese dog with mammary carcinoma.

  • Atopic dermatitis is a chronic, inflammatory skin disease characterized by intense pruritus and the course marked by exacerbation and remission. It has the characteristic symptoms of dry, itchy, scaly skin with cracks by typical distribution of lesions. Even though the etiology of atopic dermatitis is not fully understood, it is considered an allergic reaction whose onset is frequently and strongly associated with both hereditary and environmental factors. In the present clinical study, we studied the efficacy of internal herb-medicine, external herb medicine - ex. dermal oints, aqueous solutions, etc -, and steam therapy. The patient's SCORAD index was lowered 55.8 than before of which the SCORAD index was 74.8. On the basis of this study, it should be considered that the treatment of atopic dermatitis should be focused on the external therapy as steam therapy, dermal oint, a aqueous solution etc.

  • Syringoma is a benign eccrine sweat gland tumor that predominantly appears in females during puberty with multiple smooth papules measuring 1-2 mm in diameter. Common locations are on lower eyelids and cheeks. Vulvar syringoma is quite a rare, occurring condition with only a few cases reported. Here, we are reporting a case of 31-year-old female with vulvar syringoma associated with pruritus. The lesion was treated with carbon dioxide (CO2) laser ablation and 50% trichloroacetic acid (TCA) chemical peeling. Jaungo was used for wound care after laser abrasion. The combination treatment was effective for removing syringoma lesions. Post laser management with fusidic acid cream and jaungo cream was sufficient to prevent infection and promote wound healing.

  • Although rare, antihistamines can cause adverse effects, including drug-induced eruptions or anaphylaxis. A 4-year-old child visited the pediatric department of a hospital for skin eruptions after administration of antihistamines, (e.g., ucerax [hydroxyzine] or leptizine [levocetirizine]), for cholinergic rashes; he did not have pruritus. Skin prick, intradermal, and drug provocation tests were performed to determine the relationship between the antihistamines and eruptions. Levocetirizine induced wheals in the skin prick test and a rash in the oral drug provocation test. In contrast, ketotifen induced no reaction in the skin prick test but showed a positive reaction in the oral provocation test. Our case report highlights that children can experience the same types of adverse reactions as seen in adults, and cross-reactivity between various antihistamines can occur.

  • Cold atmospheric plasma (CAP) is a material that generates free radicals through the ionization of air. Despite the application in various medical fields, the efficacy and safety of CAP on post-inflammatory hyperpigmentation due to atopic dermatitis have not been reported. Herein, we report the improvement in post-inflammatory hyperpigmentation of atopic dermatitis, after CAP treatment of three sessions weekly, 10 minutes per session. We examined the clinical severity indexes before and after treatment, including the Investigator's Global Assessment, modified Atopic Dermatitis Antecubital Severity, Eczema Area and Severity Index, and pruritus visual analogue scores. Our results indicate that CAP alleviates the post-inflammatory hyperpigmentation and clinical severity of atopic dermatitis, and effectively improves the atopic skin lesion without severe safety issues. We believe that the previously reported anti-bacterial effects and production of interleukin-6 after CAP treatment are probably responsible for the underlying mechanisms leading to clinical improvement.

  • Objectives: This study aimed to report the efficacy of Korean medicine with Jeongri-tang Gagam-bang for treating psoriasis. Methods: A patient with psoriasis was treated with Jeongri-tang Gagam-bang (正理湯加減方). Moreover, acupuncture was performed on CV12 (中脘) and both sides of ST25 (天樞), GV20 (百會), LI4 (合谷), and LR3 (太衝) for 15 minutes. Psoriasis severity was assessed using the Psoriasis Area and Severity Index (PASI). Pruritis and dyspepsia intensities were ranked on a Visual Analog Scale (VAS) of 0–10. Results: After treatment, the PASI changed from 21.6 to 1.6. Patient's pruritus had almost disappeared and dyspepsia had improved. Conclusions: This study shows that Jeongri-tang Gagam-bang (正理湯加減方) and acupuncture on CV12 (中脘), ST25 (天樞), GV20 (百會), LI4 (合谷), and LR3 (太衝) might be helpful to treat psoriasis symptoms by improving digestive function.
