  • During the past decade the use of epidural opioids for treatment of chronic as well as postoperative pain has increased Epidural droperidol significantly reduced the side effects of epidural morphine without any appreciable toxicity, except possibly sedation. The purpose of this study was to assess the side effects and potentiation of analgesia of epidural morphine by dose-related droperidol. The results were as follows: 1) Duration of analgesia and pain score: There was no significant difference between morphine and dose-related droperidol groups. 2) Pruritus: Droperidol did not affect the incidence of pruritus with epidural morphine (P>0.05). 3) Nausea and vomiting: Significantly fewer patients experienced nausea and vomiting (16.7%) with droperidol 2.5mg(P<0.001). 4) Hypotensive episode Hypotension occurred in the groups with droperidol 1.25 mg (27. 8%) and 2.5mg(33.3%). 5) Sedation: It there was increased severity and incidence of sedation with dose related epidural droperidol. 6) Respiratory depression: There was no patient with respiratory depression in the morphine or droperidol group. 7) Extrapyramidal symptoms and others: There was no extrapyramidal symptom in the group with morphine and 0.25 mg droperidol, but 3 patients in the group with l.25 mg droperidol and 5 patients in the group with 2.5 mg droperidol how extrapyramidal symptoms. One patient in droperidol 2.5 mg developed suspicious NMS. It is suggested that the use of epidural droperidol to reduce the side effects of morphine may not be appropriate.

  • Objective : Yukmijihwang-tang is one of the important medicines for blood- deficiency and yin-deficiency. Atopic dermatitis usually shows dampness-heat pattern in its acute stage and blood-deficiency or yin-deficiency pattern in its chronic stage. Therefore, I hypothesized that Yukmijihwang-tang is effective on atopic dermatitis and investigated the effects of Yukmijihwang-tang on NC/Nga mice's atopic dermatitis induced by DNCB. Methods : The NC/Nga mice with atopic dermatitis were divided into four groups: three experimental groups and one control group. The experimental groups were respectively put on 2.5g(YM-2.5), 5g(YM-5) and 10g(YM-10) of Yukmijihwang-tang extract per their weight once a day for 10 days while the control group was fed normal saline. After 10 days, I measured TEWL(transepidermal water loss), observed scratching behaviors, conducted a skin biopsy and checked levels of Total IgE, IL-4 and $IFN-\gamma$ on NC/Nga mice. Results : 1. Yukmijihwang-tang significantly suppressed skin dryness. In particular, YM-5 and YM-10 had better skin hydration than YM-2.5. 2. Yukmijihwang-tang significantly suppressed pruritus while there was no significant difference among the experimental groups. 3. Yukmijihwang-tang retained skin structure(epidermis, dermis and subcutaneous fat). 4. Yukmijihwang-tang reduced Total IgE level while there was no significant difference between the control group and the experimental groups. 5. Yukmijihwang-tang did not reduce IL-4(Th2 cytokine) level. 6. Yukmijihwang-tang significantly reduced $IFN-\gamma$(Th1 cytokine) level. In particular, YM-2.5 and YM-5 had lower $IFN-\gamma$ level than YM-10. Conclusion : The results suggest that Yukmijihwang-tang suppresses skin dryness and pruritus, retains skin structure and is effective on chronic atopic dermatitis which is associated with Th1 cytokines in the immune response.

  • This study was conducted to evaluate the synergistic protective effects of mixtures of corn silk, perilla leaf and grape stem extract (CPG mixture) against UVB-induced skin damage and compound 48/80-induced pruritus in mice. The results showed that treatment with CPG mixture exhibited much stronger suppressive effect on erythema and melanin index as well as melanin formation than treatment with ascorbic acid (AA) in UVB-irradiated mice. Moreover, the treatment with CPG mixture showed ameliorative effect on immune cell infiltration and collagen fiber destruction in UV-irradiated mice. The treatment with CPG mixture inhibited glutathione (GSH) depletion, lipid peroxidation and production of pro-inflammatory cytokines in UVB-irradiated mice. Furthermore, the treatment with CPG mixture inhibited compound 48/80-induced scratching behavior and histological changes in mice. Taken together, these results indicated that CPG mixture has potentials as functional and therapeutic materials against skin damage and itch-related skin diseases.

  • Objectives : The purpose of this study is to review the latest discussions on the mechanism of Itching. Methods : Articles that reviewed the mechanism of itching were searched from Pubmed (January 2016 to June 2021). In addition, review articles discussing the gastrointestinal tract and the mechanisms of pruritus were searched seperately. Results : The articles are classified into three categories. These categories are the classification according to the passage of time (acute, chronic), the immune factors involved (inflammatory, non-inflammatory), and the neurophysiological mechanism (pruritoceptive itching, neuropathic itching, neurogenic itching, psychogenic itching). In each category, the articles over the past five years are summarized and reviewed. Also, how the status of the gastrointestinal tract is reflected in itching was discussed in terms of leaky gut syndrome, neuro/gastrointestinal peptides, and gut microbiota. Conclusions : This review introduces the recently discussed mechanism of itching, and in particular, examines how the gastrointestinal tract is related to skin itching. Based on these considerations, it is expected that more diverse therapeutic approaches can be explored in the future.

  • The purpose of this study was to investigate the possibility of herbal medicine complex extract (NI-01), which were prepared from 6 natural materials (Cinnamomum cassia Blume, Lonicerae Flos, Paeonia suffruticosa Andrews, Arctium lappa Linne, Schzandra chinesis Bailon, Elsholtzia ciliata Hylander), as a functional material for inhibition of atopic dermatitis. anti-oxidative activity was confirmed by measuring DPPH electron donating ability and ABTS+ radical scavenging ability. Cytotoxicity and NO inhibition were measured using RAW 264.7 cells to confirm anti-inflammatory efficacy. The test substance was orally administered to the pruritus-induced ICR mice to confirm the inhibition of pruritus. The bovine cornea opacity and permeability (BCOP) assay was performed to confirm safety for irritation. NI-01 showed high antioxidant activity in DPPH and ABTS+ methods. In the anti-inflammatory effect tests with RAW 264.7 cells, NO production was inhibited at NI-01 concentrations of 50 (14.9%) and 100 (4.2%) ㎍/mL, which indicated that the anti-inflammatory effect was increased in a concentration-dependent manner. NI-01 also showed anti-itching effect after inducing of itching by compound 48/80 in ICR mice. NI-01 was proved to be a non-irritant substance in BCOP assay. The results of this study suggested that the herbal medicine combined extract (NI-01) has high antioxidant, anti-inflammatory and anti-itching effects, and safety for irritation. Therefore, herbal medicine complex extract (NI-01) is thought to be highly applicable for the inhibitory ingredients of the atopic dermatitis.
