노인장기요양인정조사원의 사기에 관한 경로분석 (A Path Analysis on Morale among Staff of Long-term Care Insurance in National Health Insurance Corporation)

  • Purpose: This study is designed to identify major factors that affect morale among staff of long-term care insurance in National Health Insurance Corporation. Methods: In order to collect the data, a survey was conducted by using the structured questionnaire based on 161 staff members of long-term care insurance of 50 long-term care operation centers, which were randomly selected by the table of random numbers in the whole centers of National Health Insurance Corporation from September 1 to 30, 2011. As for the data analysis, SPSS 18.0 was used to conduct the descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, mutiple regression, and path analysis. Results: The average point of morale was 3.37, and had a negative correlation with the job stress, while it had a positive correlation with the social support, professional identity, and self-efficacy. Job stress, social support, and professional identity have a significant effect on morale among the independent variables. These variables have significant effects on morale, and also have a positive effect on self-efficacy. The findings show that self-efficacy mediates the process of morale. Conclusion: In this study, the factor which influences the morale was identified. It turned out that the morale could be improved by reinforcing the professional identity, managing the health status as well as rotating the working place. As such, it expects both the improvement of long-term care insurance services outcome and its quality through the morale management.

의약분업 이후 병원 약제부서의 업무내용 및 인력구조 변화 분석 (An Analysis of the Change in Job Contents and Personnel Structure of Hospital Pharmacy Services after the Implementation of the Separation of Prescription and Drug Dispensing Policy)

  • It has been 2 years since the implementation of the separation of prescription and drug dispensing policy. This study analyzes the effects of the policy on the job contents and personnel structure of hospital pharmacy. The main purposes of the analysis are to determine if the policy has causes the increase of professional activities of pharmacists in hospital and to investigate whether the hospital pharmacy is equipped with enough manpower to provide high quality pharmaceutical service as intended by the policy. The level of professionality of pharmacists' activities is measured by the number of activities of direct involvement in inpatient care such as participation in patient rounding, medication consultation, the number of hospital committee the pharmacists involved and the number of continuous education pharmacists took. The adequacy of personnel structure to provide high quality pharmaceutical care is measured by the level of compliance to the governmental standard of hospital pharmacy personnel. In order to collect the data, surveys were performed for two periods: year 1999 (before the implementation of the policy) and year 2001 (after the implementation of the policy). The results show that the pharmacists' participation in inpatient rounding decreased and that the inpatient medication history management activities, operation of ward pharmacy, participation in hospital committee increased. In personnel structure, the average number of pharmacist per hospital decreased and the number of prescription processing per pharmacist increased. Based on the results this study concludes that the professional activities of hospital pharmacists has increased a little and there were structural changes in hospital pharmacy service activities to increase the professionalism in providing care. However, the pharmacy departments were understaffed hampering the strive to increase the provision of professional pharmaceutical service in hospitals.

중소병원 간호사의 전문직 자율성, 조직몰입 및 환자안전문화인식이 환자안전관리활동에 미치는 영향 (Effects of Professional Autonomy, Organizational Commitment, and Perceived Patient Safety Culture on Patient Safety Management Activities of Nurses in Medium and Small-Sized Hospitals)

  • Purpose: The purpose of this study was to examine the effect of professional autonomy, organizational commitment, and perceived patient safety culture on patient safety management activities of nurses in medium and small-sized hospitals. Methods: A cross-sectional design was employed. Self-reported questionnaires were completed by 121 nurses with at least 3 months of working experience in medium and small-sized hospitals located in B city. Data were analyzed using descriptive statistics, a t-test, a one-way ANOVA, Pearson correlation coefficients, and a multiple regression analysis. Results: Professional autonomy (r=.22, p=.016), organizational commitment (r=.34, p<.001), and perceived patient safety culture (r=.55, p<.001) had a statistically significant positive correlation with patient safety management activities. The factors that might affect patient safety management activities were professional autonomy (${\beta}=.23$, p=.003) and perceived patient safety culture (${\beta}=.55$, p<.001). The explanatory power of these factors for patient safety management activities was 33.5% (F=21.19, p<.001). Conclusions: The development of repetitive and continuous education programs is needed to improve a nurse's professional autonomy and perceived patient safety culture.

보육교사의 전문성인식이 영유아권리존중 보육실행에 미치는 영향: 교사-부모 협력 관계의 매개 효과 (The Effects of Teacher's Perception of Professionalism on Child-care Practice in Respect for Child's Rights: The Mediating Effects of Teacher-Parent Co-operation)

  • Objective: This research aims to examine whether the perception of professional competence among childcare teachers impacts the performance of respecting young children's rights, and whether the teacher-parent cooperative relationship mediates this association. Methods: The participants in this research were 220 teachers in Daejeon who assessed the performance of respecting young children's right, the perception of professional competence, and teacher-parent cooperative relationships through an online self-report questionnaire. The data collected in this study were analyzed using the SPSS 25.0 program. Results: Firstly, the results of examining the impact of the perception of professional competence among childcare teachers and the teacher-parent cooperation relationship on childcare that respects the rights of young children showed that each variable has a statistically significant influence. Secondly, it was found that the perception of professional competence among childcare teachers directly impacts the performance of respecting young children's rights and, indirectly, through the mediating role of teacher-parent cooperative relationships. Conclusion/Implications: To ensure high-quality childcare for young children, it is essential to consider not only professional perception but also the cooperative relationship between teacher and parent.

동태적 직무분석을 이용한 암 환자 케어 코디네이터의 직무 분석 (Dynamic Job Analysis of the Cancer Care Coordinator in a General Hospital)

  • Purpose: The purpose of this study was to explore roles of cancer care coordinator's by developing job description, job specification and job process map using dynamic job analysis. Method: The development process consisted of three stages of vertical job analysis and three stages of horizontal job analysis by modifying Song(1997)'s dynamic job analysis. Focus group interview was used to validate the content of the job analysis. Results: Cancer care coordinator's job description was categorized into six major categories, fourteen intermediate categories and one hundred forty specific jobs. Major categories are professional nursing practice, consultation and counsel, coordination and collaboration, education, research and leadership. Cancer care coordinator's job specification included master's degree with over five years of clinical experience preferably relevant clinical area, professional knowledge on pathophysiology of cancer, case management and cost control, competency for communication and counselling skills and clinical decision making. Cancer care coordinator's job process map was framed with time(horizontal) and activities(vertical). Conclusion: The Outcomes of this study will guide to develop possible areas of oncology advanced practice nurses in hospital setting and facilitate the use of oncology nurse practitioners by developing care coordinator roles in cancer care.

요양병원간호사의 역할갈등, 직무만족 및 전문직 자부심이 재직의도에 미치는 영향 (Effects of Role conflict, Job Satisfaction and Professional Pride on Retention Intention of Nurses Working at Long-term Care Hospital)

  • Purpose: The purpose of this study is to explore factors affecting the retention intention of nurses working at long-term care hospitals. Methods: Data were collected from 147 nurses working in 10 long-term care hospitals using self-report questionnaires of role conflict, job satisfaction, professional pride and retention intention T-test, ANOVA, Pearson correlation and multiple regression were used to analyze the data. Results: The mean age of subjects was $47.4{\pm}10.1$ (22~65). Average length of clinical experiences was $170.12{\pm}98.99$ months. The level of retention intention was significantly different by the educational level (t=4.21, p=.017). Retention Intention was significantly associated with professional job satisfaction (r=.36, p<.001) and professional pride (r=.69, p<.001), but were not significantly correlated with role conflict (r=-.07, p=.376). The multiple regression analysis shows that significant factors, affecting the retention intention were professional pride (t=9.87, p<.001) and job satisfaction (t=3.37, p=.001), which explained 46.1% of the retention intention collectively. Conclusion: It is crucial to make substantial efforts to increase professional pride and job satisfaction in order to increase nurses' retention intention.

전문가 구강위생관리가 장기요양시설 재원노인의 구강상태에 미치는 효과 (Effect of Professional Oral Healthcare Program on the Oral Status of Elderly Residents in Long-Term Care Facilities)

  • 본 연구는 장기요양시설 재원노인을 대상으로 2016년 3월부터 2016년 5월까지 전문가 구강위생관리 프로그램을 적용하고 재원노인의 구강위생상태 개선 효과를 파악하기 위하여 수행되었다. 전문가 구강위생관리와 노인구강보건 교육을 제공받은 요양보호사에 의한 일상 구강관리를 중재한 32명의 실험군 I, 노인구강보건교육을 제공받은 요양보호사에 의한 일상 구강관리를 시행한 30명의 실험군 II, 요양보호사에게 일상구강관리를 제공받은 32명의 대조군으로 구분하여 치면세균막 지수, 구취, 설태, 타액 분비량의 개선 효과를 보고자 하였다. 치면세균막 지수는 반복측정분산 분석 결과 실험군 I, 실험군 II, 대조군 사이에 유의한 차이가 있었으며 중재기간에 따른 변화에도 유의한 차이가 있었다. 또한 중재 방법에 따라서 치면세균막 지수의 변화 양상은 유의한 차이를 보였다. 구취는 실험군 I, 실험군 II, 대조군 사이와 중재기간에 따른 변화에 유의한 차이가 없었으나 중재 방법에 따라서 구취의 변화 양상은 유의한 차이를 보였다. 설태는 실험군 I, 실험군 II, 대조군 사이에 유의한 차이가 없었으나 중재기간과 중재 방법에 따른 설태의 변화 양상은 유의한 차이를 보였다. 타액 분비량은 실험군 I, 실험군 II, 대조군에 따른 변화와 중재기간에 따른 변화에 유의한 차이가 없었으나 중재 방법에 따라서 타액 분비량의 변화 양상은 유의한 차이를 보였다. 전문가 구강위생관리와 요양보호사에 대한 노인구강교육 실시가 재원노인들의 구강위생 상태에 영향을 미치는 것이 확인되었기에, 재원노인의 구강위생 상태를 개선하기 위해 치과 촉탁의 제도와 병행한 전문가 구강위생관리를 주기적으로 수행할 필요가 있으며, 직원을 포함한 간호 인력을 대상으로 노인 구강위생교육을 제공할 필요가 있다.

요양보호사의 감정노동과 장기요양기관의 서비스 질의 관계 - 직무관련 삶의 질 매개효과 - (Effect of Long-term Care Worker's Emotional Labor on Service Quality of long term care facility - Mediating Effect of Professional Quality of Life -)

  • 본 연구에서는 요양보호사의 감정노동, 직무관련 삶의 질, 서비스 질의 관계를 분석하였다. 연구대상은 전국의 장기요양시설에 근무하는 요양보호사 211명이며, 자기기입식 설문지를 활용하여 조사 하였다. 연구모형은 감정노동(표면행위, 내면행위)을 독립변수, 직무관련 삶의 질(공감만족, 공감피로)을 매개변수, 서비스 질을 종속변수로 구성하여 검증하였다. 모형의 적합성과 서비스 질에 영향을 미치는 각 요인의 직접효과, 간접효과를 검증한 결과 모형의 적합도는 적합한 수준이었으며, 감정노동 표면행위와 내면행위에서 서비스 질로 향하는 직접효과는 통계적으로 유의미하지 않는 것으로 나타났다. 그러나 직무관련 삶의 질을 매개하여 서비스 질에 영향을 미치는 총 효과는 통계적으로 유의미하였으며, 내면행위와 서비스질의 관계에서는 정적 상관관계, 표면행위와 서비스질의 관계는 부적 상관관계가 있음을 확인할 수 있었다. 서비스 질에 영향력이 가장 큰 변인은 직무관련 삶의 질(공감만족)이었다. 이 연구 결과를 토대로 요양보호사의 감정노동과 삶의 질에 대한 임상적, 학문적 관심을 환기시킬 필요성을 제안하였다.

전문성이 강화된 지속가능한 요양보호사 제도 연구 (Study for a Sustainable Program of the Professional Long-term Care Workers)

  • 본 연구는 요양보호사의 전문성 강화와 중장기적으로도 지속가능한 양성교육체계 논의모형을 하나의 사고실험차원에서 대안적으로 제시하고자 하였다. 이를 위하여 먼저, 요양보호사 자격취득자 및 취업자의 인구사회학적 특성을 분석하였다. 또한, 현행 요양보호사 양성교육과정과 병행하여 전문대학교에서 국가직무능력표준을 기반으로 한 요양보호사 양성교육과정 개설, 양성교육 의무시간 확대, 경력과 전문성이 강화된 가칭 '전문요양보호사' 제도 도입과 제공기관에서의 의무고용, 보수교육의 법정 의무화와 보험자의 보수교육강화 등으로 구성된 전문성이 강화된 지속가능한 대안적 요양보호사 양성교육체계를 제안하였다. 또한, 본 연구는 이를 실현하기 위한 정책과제들도 제시하였다.

Model Development of Korean Professional Hospice Care Education

  • This study was conducted to develop an education program of hospice care for the professional in order to care for nurses for terminally ill patients facing death and their families. The Modified Tyler-Type Ends-Means model was used to guide the curriculum development of the study The curriculum include a philosophical conception of hospice education, fundamental concepts, purpose, objective and the educational contents. The content was developed based upon a 70% or more demand in educational demand analysis. The education program has a total of 360 hours consisting of 172 hours of theoretical study and 188 hours of practice including fundamental nursing care for hospice.
