The Effect of Conservative Therapy on Thoracolumbar Intervertebral Disc Disease on 15 dogs (소형견 15예의 흉요추추간판질환에 대한 보존치료)

  • Fifteen dogs with clinical signs of paraparesis, paraplegia and urinary dysfunction were referred to Veterinary Medical leaching Hospital, College of Veterinary Medicine, Konkuk University. According to physical and neurologic examination, survey radiograph and myelography, these patients were diagnosed as thoracolumbar intervertebral disc disease. All of them were treated with medical (prednisolone or carprofen, antibiotics), acupuncture (Ji Zhong, Bai Hui, Zhong Shu, Pang Guang Shu, Zu San Li, Huan Tiao) and physical therapy (hydrotherapy, thermotherapy, massage, manipulation, swimming). Of twelve patients were recovered of neurologic deficits within 21 days after the onset of treatment. However, three dogs were not resolved, which were found to have other spinal cord disease concomitantly. All of patients showed normal urination after treatment. Until 21 months after treatment, there were no recurrence in twelve patients. This study indicated that the conservative therapy could be effectively managed in paraparesis or paraplegia and urinary dysfunction which caused by intervertebral disc disease.

A case of protein losing nephropathy in a dog infected with canine Babesia gibsoni

  • A 1-year and 6-month-old, intact male, Shih-tzu dog was referred to the Konkuk University Veterinary Teaching Hospital, due to ascite, melena, severe anemia, and polyuria/polydipsia. Complete blood count showed moderate leukocytosis, lymphocytosis, monocytosis, and microcytic hypochromic nonregenerative anemia. On serum biochemistry profiles, hypoalbuminemia and hypoproteinemia were observed, and proteinuria was detected on urinalysis. Pleural effusion and mild ascites were noted in radiological findings, and bilateral nephromegaly was noted on ultrasonography. Babesia gibsoni was detected by PCR. Babesiosis and glomerular disease was treated at a time with prednisolone and buparvaquone. This therapeutic regime was very efficacious and clinical sings were rapidly improved. Proteinuria and severe anemia were disappeared following therapy.

  • Propolid (bee-glue), known as a folk medicine, is a lipo;hilic material found in honeybee hives. In the present study on the anti-inflammatory effect of Korean propolis, it was extracted with ethanol, and used as a test material. The $LD_{50}$ value with the oral administration of ethanolic extract of Korean propolis (EEKP) was higher than 2g/kg in mice. The oral administration of the propolis extract (100mg/kg) significantly inhibited the development of hind paw edema induced by carrageenin in rats. the oral pretreatment of the propolis extract markedly inhibited the increase in vascular permeability and the number of writhing induced by acetic acetic acid in mice. Propolis extract, 50 and 100 mg/kg p.o. per day for 7 days, produced a significant inhibitory effect on granuloma and exudate formation in rats. This inhibitory effect was enhanced with the concomitant use of prednisolone (2.5 mg/kg). These results suggest that Korean propolis apparently has a strong anti-inflammatory activity.

Anti-inflammatory Activity of Aqueous Extract from Gleditsiae Spina (조각자 물추출물의 항염증 작용)

  • The anti-inflammatory activity of the aqueous extract from Gleditsiae spina was investigated utilizing carrageenin-induced edema, granuloma pouch and adjuvant arthritis in rats. The effects of this agent on vascular permeability and acetic acid-induced writhing in mice were also examined. Its anti-inflammatory activity on carrageenin edema was observed with oral administration. The aqueous extract from Gleditsiae spina(400 mg/kg, 7 days) showed significant inhibitory effects on granuloma and exudate formation in rats. In the method of adjuvant arthritis, the aqueous extract(400 mg/kg), orally administered for 21 days, inhibited the development of hind paw edema in rats. The aqueous extract was also inhibited the increase in vascular permeability and the number of writhings induced by acetic acid in mice. In the present study, the aqueous extract of Gleditsiae spina was indicated to have the anti-inflammatory action.

Steroid-induced Psychosis in Adolescent Patient with Crohn's Disease

  • Crohn's disease is a chronic inflammatory disease that involves the upper and lower gastrointestinal tracts, including the oral cavity. For the conventional treatment of inflammatory bowel diseases, steroids are usually used as induction therapy, and azathioprine is used as maintenance therapy. Steroids are associated with various side effects. Under rare circumstances, they can even cause psychotic symptoms, and reports on steroid-induced psychosis in the pediatric population are few. Here, we report the first case of steroid-induced psychosis in a 16-year-old female with Crohn's disease administered with steroids as induction therapy in South Korea. She showed psychotic symptoms, including auditory hallucination, loosening of association, and idea of reference, after 2 weeks of administering prednisolone 60 mg/day. One week after steroid discontinuation, the patient gradually recovered and was treated with quetiapine. Therefore, pediatricians should be aware of serious psychiatric adverse effects when administering corticosteroids for various indications.

  • Illegal addition of steroids into cosmetics, ointments or drugs have been increased and their careless usage induced detrimental effect on health. We developed simultaneous analytical method using TLC, HPLC and LC/MS for the identification of 40 corticosteroids. 34 corticosteroids were well separated in HPLC with isocratic mode and remaining 6 drugs were also separated with gradient mode. All of the 40 corticosteroids were detected in negative mode in LC/MS. Halcinonide, prednisolone, triamcinolone acetonide and methylprednisolone hemisuccinate were detected in real samples.

  • A 68 year-old male was admitted with complaint of dyspnea and nonproductive cough which developed 6 hours after accidental inhalation of nitrogen dioxide. On admission, acute pulmonary edema and severe hypoxemia were found. With oxygen and bronchodilator therapy, diffuse alveolar consolitation and his dyspnea were improved from the following day. He was discharged at 8th hospital day with prednisolone 30mg daily for prevention of bronchiolitis obliterans. During 6 weeks of follow up, there was no evidence of bronchiolitis obliterans.

Suppressive Effects of Propolis in Rat Adjuvant Arthritis

  • The effects of ethanolic extract (EEP) of propolis on chronic inflammation were evaluated using rat adjuvant arthritis. In the chronic inflammatory animal model, the arthritis index was suppressed by EEP treatments (50 mg/kg/day and 100 gm/kg/day, p.o.). Moreover, physical weakness, induced by the chronic disease state, was dose-dependently improved in the EEP-treated groups. It s analgesic effect, assessed using the tail-flick test, was comparable to prednisolone (2.5 mg/kg/day, p.o.) and acetyl salicylic acid (100 mg/kg/day, p.o.). In carrageenan rat hind paw edema, which was conducted to test the effects of subfractions of EEP, the petroleum ether sub-fraction (100 mg/kg, p.o.) showed an inhibitor effect on the paw edema whereas EEP (200 mg/kg, p.o.) showed a significant anti-inflammatory effect at 3 and 4 hrs after carrageenan injection. From these results, we conclude that the ethanolic extract of propolis had a profound anti-inflammatory effects on both chronic and acute inflammations.

  • Polyethylene glycol, polypropylene glycol and block copolymer of ethylene glycol and propylene glycol were crosslinked by triisocyanate to form water swellable, rubbery polymer. The equilibrium swelling of the hydrogels ranged from 3% to 60% according to the hydrophobic-hydrophilic properties of the prepolymers. Model drugs, sodium salicylate and prednisolone were incorporated in the polymer matrices by swelling loading. Physical properties of the drugs affected the drug release mechanisms due to the change in the swelling behaviors of the polymeric devices. Zero order release was observed in the case of relatively hydrophobic polymer matrices.

Flumethasone 투여후의 수의임상 관찰

  • 개의 여러가지 질병상태에서 근복적인 치료나 보조적인 치료제로서의 flumethasone 투여는 아주 좋은 치료효과를 나타냈다. 우리가 병원에서 상용하는 flumethasone의 평균 투여량은 0.125$\~$0.5mg으로서 경구 또는 주사로 투여했다. 경구적으로 투여하는 주된 사용량은 1일 0.03125mg$\~$0.125mg이며 환축의 크기, 치유과정 그리고 병증의 심한 정도에 따라 양을 가감했다. 투여경로를 달리하여도 약효에 차이가 생기지는 않았다. flumethasone의 소염효과, 당신생효과 및 항독소적 치료효과에서 기존의 corticoids 투여시 나타나는 효과에 비해 정성적인 차이는 없었으나 실험실 결과와 병원에서 사용한 임상결과 flumethasone은 hydrocortisone보다 300$\~$400배 그리고 prednisone이나 prednisolone 보다는 60$\~$80배나 강한 약효를 발휘하는 것으로 나타났다. 임상적용에서 나타난 감동적인 효과에서 이러한 flumethasone의 약효에 대한 자료들이 입증되었다. 또한 치료된 동물들이 보여준 반응은 flumethasone의 약효 지속시간이 기존의 corticoids보다 더 긴 것으로 나타났다. 앞에서 언급했듯이 flumethasone 투여시는 전해질 불균형에 의해 유발되는 다음다갈증이나 다뇨증, 식욕촉진 등의 부작용이 전혀 나타나지 않았다. 이러한 flumethasone의 안전성은 기존의 corticoids로 비만동물이나 나이든 환축을 치료할때 자주 나타났던 부작용업이 장기간 치료할 수 있으므로 아주 중요하다.
