Effects of Chlorhexidine Gluconate and Povidone Iodine on Wound Healing in Rabbits (Chlorhexidine Gluconate 및 Povidone Iodine이 가토의 창상치유에 미치는 효과)

  • To investigate the effect of chlorhexidine gluconate and povidone iodine on wound healing, their antimicrobial activity and their effects on wound contraction in rabbits were studied. Staphyococcus aureus (1${\times}$10$\^$7/) was inoculated on the full thickness skin defects (2${\times}$2cm) on the back. Antiseptics were applied on the defects 2 hours after Inoculation. The defects were swabbed for bacterial culture 24 and 48 hours after the application of antiseptics. Percentage of wound contraction, based on wound area measured on the day of surgery was calculated for each wound on days 1,7 and 14. The 0.5% chlorhexidine gluconate solution had more effective antimicrobial activity against Staphyococcus aureus in wounds than 0.05% chlorhexidine gluconate solution (p<0.05), and povidone iodine solution and saline (p<0.01). Percentages of wound contraction were not significantly different among different dilutions of the antiseptics and saline. Present study indicated that the antimicrobial activity of chlorhexidine gluconate in wound was superior to that of povidone iodine.

Povidone-Iodine related Chemical Burn under the Tourniquet (A Case Report) (지혈대 내측의 포비돈-요오드 용액에 의한 화학 화상 (1예 보고))

  • In the extremity surgery, pneumatic tourniquet and povidone-iodine solution are commonly used to provide an aseptic, bloodless field, and their complication rate has remained low. However, chemical burn under tourniquet has been rarely reported. Patients sustained burn injuries over the dependent, weight-bearing regions such as posterior neck, back, buttocks and posterior thighs. This rare adverse complication occurred in a 22-year-old man who underwent modified Brostrom operation with arthroscopic os trigonum excision. 10% povidone-iodine was used as topical antiseptic, and full thickness burn occurred underneath the area of tourniquet application. Main causes of povidone-iodine related chemical burn are considered maceration, irritation of the skin, long term use of the tourniquet and pressure. To reduce the complications like chemical burn, awareness of the risk and the possible pathogenesis as well as the preventive measures is important in surgical practice.

Effect of Postpartum Intrauterine Infusion of Povidone-iodine Solution on the Subsequent Reproductive Performance in Dairy Cows (분만 젖소의 자궁내 povidone-iodine 용액의 주입이 번식효율에 미치는 영향)

  • This study determined the effect of postpartum intrauterine infusion of povidone-iodine solution on the subsequent reproductive performance in dairy cows. Six hundred and fifty-eight dairy cows that were around week 4 postpartum were randomly divided into two groups: (1) Cows in treated group received an intrauterine infusion of 100 to 150 ml 2% povidone-iodine solution (treated group, n=269), or (2) Cows that received no treatment were served as the controls (control group, n=389). The cows were bred at observed estrus more than 50 days after calving. Artificial insemination (AI) was done according to the a.m.-p.m. rule. The conception to AI was determined per rectum 60 days after AI by both ultrasonographical observation and manual palpation. The intervals from calving to first service and conception ($94.2{\pm}2.7$ and $144.1{\pm}4.1$ days vs. $88.0{\pm,}2.1$ and $143.5{\pm}4.6$ days, respectively), number of services per conception ($2.0{\pm}0.1$ vs. $2.1{\pm}0.1$), and cumulative pregnancy rates within 90 and 150 days postpartum (31.9 and 61.7% vs. 29.7 and 62.1%) did not differ between the control and treated groups (P>0.05), while conception rate at first service was lower (P<0.05) in the treated group (40.5%) than in the control group (49.4%). Additionally, we could not find any effect of intrauterine infusion of povidone-iodine solution according to cow parity or calving season on the reproductive performance. These results indicate that intrauterine infusion of povidone-iodine solution at week 4 postpartum does not affect the subsequent reproductive performance in dairy cows.

Clinical Study Comparing the Skin Antiseptics of Povidone-iodine and Chlorhexidine-ethanol on Port Site (투관침 부위에 적용하는 포비돈 아이오딘과 클로르헥시딘-에탄올의 소독제 효과 연구)

  • The purpose of this study was to compare in antiseptic effect between povidone-iodine and chlorhexidine-ethanol for laparoscopic cholecystectomy on the port site. A total of 46 patients admitted to surgery at one university hospital in Seoul were included in the study. A study group was randomly assigned using a random number table. The antiseptic effect was evaluated and compared to povidone-iodine and chlorhexidine-ethanol by using a culture and identification of bacteria, measurement of the number of bacteria (CFU/ml) and incidence of surgical site infection. Our results show that povidone-iodine and chlorhexidine-ethanol have no statistically significant difference in the incidence rate of bacteria after 3 minutes of disinfection and before removal of the trocar and in the number of bacteria and the incidence of surgical site infection. Thus, in order to select an efficient and appropriate skin antiseptics, we suggest that it is necessary to consider not only the disinfectant effect but also cost and convenience. In this research, our results about effect and usage of skin antiseptics can be used as basic data and educational resource, and this is expected to improve comprehension of skin antiseptics.

Studies on the Wastewater of General Hospital and It에s Treatment -Effects of Disinfectants on Activated Sludge Process- (病院廢水의 特性과 處理에 關한 硏究 -消毒劑가 활성스러지법에 미치는 영향-)

  • The hospital wastewaters have to be so disposed as to prevent disease and to protect water resources from hazardous substances disinfectants, medicines, and chemicals. Polyvinylpyrrolidone-iodine complex (povidone-iodine) is widely used in the hospital as one of disinfectants. This study was carried out to manifest the effect of disinfectants in growth of activated sludge in treatment of the hospital wastewater by the activated sludge process. The results are as follow. 1. An average water quality of the hospital wastewater showed 7.2 in pH, 3.2 ppm in DO, 293.3 ppm in SS, 96.0 ppm in BOD, 151.1 ppm in COD, 0.4 ppm in povidone-iodine, 0.5 ppm in phenols, 5.4 ppm in surfactants, 1.6 ppm in o-phosphate, 4.6 ppm in $NH_3-N, 249\times 10^4$ counts/100 ml in coliform group organisms, and $1,369\times 10^2$ counts/ml in general counts of bacteria. And wastewater amounts discharged per bed was calculated 70 l/d/bed. 2. In batch culture activated sludge process, each of cresol and povidone-iodine was not effected in less than 0.1 ppm concentration, but the more concentration, the more inhibit the growth rate of activated sludge. In the mixture of two disinfectants, the growth was more inhibited the effect of single disinfectants. So that this reaction is considered as addition effect of two disinfectants. 3. The removal rates of the disinfectants-by continuous culture activated sludge process were 77.6% in 0.4 ppm povidone-iodine, and in BOD was 85.6%. 4. It is desirable that the hospital wastewater is planed in order to be discharged to two system separately, sewer from life system and wastewater from medical system. From those results, it has been concluded that the hospital wastewater has to be treated safely by the activated sludge process.

Evaluation of a Waterless, Scrubless Chlorhexidine Gluconate/Ethanol Surgical Scrub and Povidone-Iodine for Antimicrobial Efficacy (물과 솔 없이 사용하는 외과적 손 소독제 Chlorhexidine/Ethanol 혼합제와 Povidone-iodine의 소독 효과)

  • Purpose: The purpose of this study was to compare 1% chlorhexidine-gluconate/61% ethanol (CHG/Ethanol) emollient and 7.5% povidone-iodine (PVI) scrub for antimicrobial, residual effect, and skin condition. Method: CHG/Ethanol emollient hand hygiene was performed waterless, and brushless by operating doctors and nurses (N=20). PVI hand washing was performed with water and a brush (N=20) for 5 min. The subjects were asked to press their left hand in hand-shaped agar before a surgical scrub, immediately after a surgical scrub and after the operation. The amount of isolated microorganisms were calculated by counting the number of divided areas($1{\times}1cm$, 160 cell) which were culture positive in the hand culture plate. The skin condition was evaluated. Result: The antimicrobial count of CHG/Ethanol emollient and PVI immediately post surgical scrub was 0.0 vs. 4.1 (p>.05), and after the operation was 0.1 vs. 37.8 (p>.05)respectively. The Residual effect of CHG/Ethanol emollient immediately post surgical scrub and after the operation were 0.0 vs. 0.1 (p>.05), and PVI were 4.1 vs. 37.8 (p>.05)respectively. The skin condition and satisfaction of CHG/Ethanol emollient was higher than PVI (p<.05). Conclusion: The antimicrobial effect between CHG/Ethanol emollient and PVI were the same. Considering skin condition, satisfaction and allergic reaction CHG/Ethanol emollient for surgical scrub is recommended in Korea.

SALIVARY BACTERIAL COUNTS AFTER APPLICATION OF POVIDONE-IODINE AND CHLORHEXIDINE (Povidone-Iodine과 Chlorhexidine의 적용시간에 따른 타액 내 세균수의 비교)

  • Objective: It is important to sterilize oral cavity with antibacterial agent before surgery for preventing infection. The object of this study was to compare the effect on reduction of salivary bacterial counts according to applied time when povidone-iodine (PVI) and chlorhexidine gluconate (CHX), most broadly used materials in dentistry, were applied intraorally before the surgery. Methods: Sixty subjects were divided into 6 groups. PVI and CHX were applied in each group for 1, 2 and 3 minutes, respectively. Then salivary microbacteria taken before and after applying the materials were cultured using 5% sheep blood agar plate. Results: There was significant difference in reduction of microbacteria in both PVI and CHX and the effect did not show differences depending on time. When applied for a minute, PVI showed somewhat higher reduction rate than CHX, but in the other groups, there was no difference in reduction rate. Conclusion: We found that there was no significant difference in sterilization ability of PVI and CHX in all groups in this study. Therefore, both agents would get sufficient effect when applied for a minute.

Bactericidal Effect of Waterless Alcohol Gel Hand Washing Agent (물 없이 사용하는 알코올 젤의 손소독 효과에 관한 연구)

  • The purpose of this study was to compare the hand disinfection effect of waterless alcohol gel hand washing agent with that of soap and water, 4% chlorhexidine gluconate, and 10% povidone-iodine. Hands of fourty subjects were artificially contaminated with Acinetobacter baumannii $5m{\ell}$ and randomly distributed to each hand washing methods. Samples were collected from gloved hand by glove juice sampling procedure. Mean log reduction after hand washing were compared with baseline values. Number of microorganisms were converted to log and tested by ANOVA in SPSSWIN 10.0. Mean log reduction of soap and water, alcohol gel, 4% chlorhexidine gluconate, 10% povidone-iodine were $2.76{\pm}0.62$, $2.97{\pm}0.56$, $4.66{\pm}1.70$, $4.60{\pm}0.91$, respectively. The bactericidal effect of alcohol gel was similar to that of soap and water, but the effect was much less than chlorhexidine gluconate and povidone-iodine(p<0.001). In terms of microorganism reduction, the efficacy of waterless alcohol gel was almost the same as soap and water hand washing. Further evaluation of the bactericidal effect of waterless alcohol gel is needed because waterless alcohol gel is simple, convenient, and non-irritating hand washing agent and also very effective in busy hospital environment.

Enhancement of chronic wound healing with maltodextrin/ascorbic acid gel: a clinical evaluation of distal limb wounds in horses

  • Delayed healing associated with distal limb wounds is highly challenging in equine clinical practice. This study aimed to evaluate healing rates between chronic non-granulating wounds of horse distal limbs that were treated with maltodextrin/ascorbic acid gel alone or in combination with povidone-iodine 1% solution and those treated with povidone-iodine 1% only throughout the study period (35 days) in clinical settings. The study was conducted on 18 adult horses (3-15 years old). Based on the treatment regimen utilized, the horses were divided into three groups (n = 6), with each group having a similar mean wound area. The percentages of wound contraction, epithelialization, and overall wound healing were determined weekly for each wound. By the end of the study, the total wound healing percentage was significantly increased between the study groups (p < 0.05). The use of maltodextrin/ascorbic acid gel resulted in considerable wound contraction, rapid epithelialization, and complication-free wound healing. Based on the findings of this study, maltodextrin/ascorbic acid gel, independently or in combination with a 1% povidone-iodine solution, might be applied as a safe and effective wound healing promoting agent in horses with chronic non-granulating wounds.

Affecting Factors on Hospital Nurses' Practice of Disinfection: Focused on Alcohol, Chlorhexidine Gulconate, and Povidone Iodine (병원 간호사의 주요 피부소독제 인지도와 수행도에 영향을 미치는 요인 - 알코올, 클로르헥시딘, 베타딘을 중심으로 -)

  • Purpose: This study was performed to identify the affecting factors on hospital nurses' practice of disinfection focused on alcohol, chlorhexidine gluconate (CHG), and povidone iodine (PVI). Methods: The participants in this study were 196 nurses of 3 general hospitals and 2 upgrade general hospitals in 5 cities in Korea. Data were collected through self-reported questionnaires in January, 2011. The collected data were analysed by ANOVA, Pearson correlation and stepwise multiple regression using SPSS/WIN 18.0. Results: The subjects used alcohol and PVI daily for intravenous site care and surgical wound dressing respectively. The mean percentage of awareness in practicing main disinfectants were 80.0% and 72.5% respectively. The awareness and practice were highest in alcohol, but lowest in CHG. The mean percentage of practice of disinfectants was 72.5%. There was positive correlation among awareness and practice of main disinfectants. The awareness and nurses' salary explained 34.6% of variance in practice of disinfectants. Conclusion: An educational program focusing on strategy to enhance hospital nurses' awareness would be effective in improving their practice of disinfectants.
