Objective : In general, to perform posterior lumbar interbody fusion(PLIF), it has been used more invasive procedure than simple discectomy. However we try to perform PLIF with TFC with smaller invasion almost same as in simple discectomy. This study is about its procedure and clinical results. Materials and Methods : The authors retrospectively analyzed 43 cases of minimally invasive PLIF with TFC from July 1998 to May 2000. Operative procedure, operative complication, change of disc height, blood loss, ambulation time, hospitalization period, clinical success rate, and bony fusion rate were analyzed. Results : 40 patients were capable to walk on the 2nd day of the post-operation. The average hospitalization period is 5.6 days. The average blood loss was 0.19L/level with no transfusion or wound drainage. The height of disc changed from 8.84mm to 13.54mm. Clinical success rate is 95% when evaluated by the Prolo's scale. The complication was delayed wound infection(2) and transient paresthesis(1). The bony fusion was shown in 17 patients (94.4%) out of 18 patients who passed one year. Conclusion : As a result of minimally invasive PLIF, pain was decreased and early ambulation and short hospitalization was possible. Complication was similar or lower than other studies, and the bony fusion rate and clinical success rate were also similar during follow-up.
Purpose: There have been few case reports regarding treatment plans for long-term, neglected scalp defects and calvarial defects with subdural abscess. The purpose of this case report is to present our experience with a free latissimus dorsi musculocutaneous flap for scalp and calvarial defects and to discuss flap options in comparison with a literature review. Methods: A 60-year-old man who fell down from a four-story-height that resulted in a craniotomy in 1979; he visited our outpatient clinic for a chronic, purulent scalp and calvarial defects with unidentified artificial bone. The artificial bone was removed by a neurosurgeon and reconstructed with a free latissimus dorsi musculocutaneous flap. The deep temporal artery was used as a recipient artery. The postoperative flap status was excellent until the 6th day post-operation when the patient experienced a seizure, and an arterial insufficiency occurred at the flap probably due to an arterial spasm. Emergency exploration with arterial re-anastomosis was performed and the flap status was stabilized. Results: Complete wound healing was achieved after 3 weeks without infectious and systemic postoperative complications. During the 6 month follow-up period, there were no complications. Conclusion: We suggest the latissimus dorsi myocutaneous free flap as a good treatment option for a chronic, purulent, complicated scalp with calvarial defect, as a well as treatment for an acute traumatic defect.
본 연구에서는 2017년에서 2018년도 간에 그린캠프에 입소한 202명의 복무 부적응 인원들이 교육 후의 만족도와 자신 생각의 변화에 대한 설문과 간부 350명 대상 그린캠프의 운영현황에 대하여 분석하였다. 또한, 간부들의 일반적인 사항, 그린캠프 입소 병사에 관한 사항, 그린캠프 이용에 관한 사항, 소속부대 내 병사상담 관련 사항에 대해 기술통계로 분석하였다. 2019년 3월 17일 국방부는 6,214명의 현역복무 부적합 인원 중, 6,118명의 98.4%에 달하는 인원들이 군 복무를 마무리하지 못하고 전역 조치 되었다는 것을 식별하였으며, 이들 중 66%에 달하는 4,014명이 병사들이 군 복무 부적응을 경험하였다. 따라서 군 복무 부적응으로 전역 되는 병사를 조기에 식별하여 민간 전문가를 이용한 교육과 진로선택 등 다양한 방법을 적용하고 필요시에 민간 전문병원을 통하여 적시적인 교육과 치료로 정상적인 군 생활을 마치고 전역할 수 있도록 하여야 할 필요성이 있다.
우리는 곧 포스트 코로나 시대를 마주하게 될 것이다. 현재, 한국보건복지인력개발원에서 운영 중인 보건복지 인력 대상 직무교육 본래 목적 달성과 교육 일관성 유지를 위한 변화 혁신이 필요한 시점이다. 본 고에서는 보건·복지 직무교육 콘텐츠를 집적하여 효율적·효용성 있는 교육과정 운영을 위한 대안을 찾기 위해 국내·외 문헌고찰을 했다. 이를 통해 오픈 플랫폼 구축 가능성을 점검하고 충분한 대안으로서 제시할 수 있을 것으로 판단했다. 오픈 플랫폼의 수요자는 보건복지 서비스 인력(종사자), 전문가(교수, 관련기관 공직자, 전문가 등), 대상 국민, 관련 시민사회단체 등이다. 또한, 제공자(suppliers)는 강사진(연구자, 교수, 관련기관 공직자, 전문가 등), 보건복지개발원을 포함 보건복지 관련 공공기관이 될 것이다. 보건복지 부문 직무교육 오픈 플랫폼 구축을 통해 대상자의 교육 접근성과 학습관리시스템 운영이 가능하겠다, 무엇보다도, 교육 전문가의 업무 중복과 예산 효율화에 기여할 수 있을 것이라 기대한다.
본 연구의 목적은 인스타그램을 활용한 대학도서관의 홍보 활동에 대한 재학생의 이용 현황과 인식을 파악하고 이를 바탕으로 도서관 홍보를 위한 대학도서관 인스타그램 운영 방안을 제안하는 것이다. 이를 위해 국내외 대학도서관 인스타그램 운영 현황을 조사하고, 소속 대학의 도서관 인스타그램을 이용하는 학생 292명을 대상으로 설문 조사를 진행하였다. 연구 결과, 대학도서관 인스타그램 운영 개선을 위해 첫째, 재학생의 인스타그램 이용과 밀접하게 연관된 '도서관 행사 및 이벤트' 정보를 활용하되 실질적인 도서관 이용으로 이어질 수 있는 참여 방법을 기획하여 도서관 이용을 활성화하는 것이 필요한 것으로 나타났다. 둘째, 도서관 이미지 인식에 긍정적인 영향을 주기 위해서 인스타그램을 통해 사서의 역할을 홍보하고 일상적 커뮤니케이션 정보를 활용하는 것이 필요하다. 셋째, 도서관 인스타그램에 이용자가 문의할 수 있는 방법을 안내하고, 주기적인 게시물 업로드와 모니터링을 통해 이용자와의 소통에 적극적으로 대응하는 것이 필요하다.

In this paper I discuss a more complicated case of 'It'-Extraposition in English in the Optimality Theory [1] by further modifying and extending the analysis done in Khym (2018) [2] in which only the 'relatively' simple cases of 'It'-Extraposition such as 'CP-Predicate' was dealt with. I show in this paper that the constraints and the constraint hierarchy developed to explain the 'relatively' simple cases of 'It'-Extraposition are no longer valid for the more complicated cases of 'It'-Extraposition in configuration of 'CP-V-CP'. In doing so, I also discuss two important theoretic possibilities and suggest a new view to look at the 'It'-Extraposition: first, the long-bothering question of which syntactic approach between P&P (Chomsky 1985) [3] and MP (Chomsky 1992) [4] should be based on in projecting the full surface forms of candidates may boil down to just a simple issue of an intrinsic property of the Gen(erator). Second, the so-called 'It'- Extraposition phenomenon may not actually be a derived construction by the optional application of Extraposition operation. Rather, it could be just a representational construction produced by the simple application of 'It'-insertion after the structure projection with 'that-clause' at the post-verbal position. This observation may lead to elimination of one of the promising candidates of '$It_i{\ldots}[_{CP}that{\sim}]_i$' out of the computation table in Khym [2], and eventually to excluding the long-named 'It'-Extraposition case from Extrsposition phenomena itself. Background: Scarce data have been reported on the efficacy of concomitant atrial fibrillation (AF) ablation in patients undergoing bioprosthetic valve replacement. Methods: From 2001 and 2014, 146 consecutive patients ($69.3{\pm}9.4years$, 84 females) who underwent bioprosthetic heart valve replacement concomitant with AF ablation were assessed. We evaluated long-term rhythm and valve-related outcomes. Results: During 49.1 months of follow-up (interquartile range, 22.5-96.8 months), 7 in-hospital and 49 (6.7% per person-year) post-discharge deaths occurred. The thromboembolic event-free survival rate at 5 years was $79.2%{\pm}3.5%$. The freedom from AF recurrence rate at 5 years was $59.8%{\pm}4.9%$. Multivariate analysis showed that old age (hazard ratio [HR], 1.06; 95% confidence interval [CI], 1.02-1.11; p=0.002), previous cardiac operation (HR, 3.01; 95% CI, 1.22-7.43; p=0.02), and a large left atrial (LA) dimension (HR, 1.02; 95% CI, 1.00-1.05; p=0.045) were significantly associated with AF recurrence. Conclusion: The overall long-term clinical outcomes in these predominantly elderly patients undergoing AF ablation concomitantly with bioprosthetic valve replacement were satisfactory; however, AF recurrence was frequent. Older age, a history of prior cardiac surgery, and large LA size were associated with an increased risk of AF recurrence.

Objective : Hydatid cyst disease is caused by the parasite Echinococcus granulosus. It is rarely seen in the vertebral system, occurring at a rate of 0.2-1%. The aim of this study is to present 12 spinal hydatid cyst cases, and propose a new type of drainage of the cyst. Methods : Twelve cases of spinal hydatid cysts, surgical operations, multiple operations, chronic recurrences, and spinal hydatic cyst excision methods are discussed in the context of the literature. Patients are operated between 2005 and 2016. All the patients are kept under routine follow up. Patient demographic data and clinicopathologic characteristics are examined. Results : Six male and six female patients with a median age of 38.6 at the time of surgery were included in the study. Spinal cyst hydatid infection sites were one odontoid, one cervical, five thoracic, two lumbar, and three sacral. In all cases, surgery was performed, with the aim of total excision of the cyst, decompression of the spinal cord, and if necessary, stabilization of the spinal column. Mean follow up was 61.3 months (10-156). All the patients were prescribed Albendazole. Three patients had secondary hydatid cyst infection (one lung and two hepatic). Conclusion : The two-way drainage catheter placed inside a cyst provides post-operative chlorhexidine washing inside the cavity. Although a spinal hydatid cyst is a benign pathology and seen rarely, it is extremely difficult to achieve a real cure for patients with this disease. Treatment modalities should be aggressive and include total excision of cyst without rupture, decompression of spinal cord, flushing of the area with scolicidal drugs, and ensuring spinal stabilization. After the operation the patients should be kept under routine follow up. Radiological and clinical examinations are useful in spotting a recurrence.

[Purpose] It aims to suggest the differential factors to enhance the customer satisfaction and to activate the shopping in the convenience store (CVS). [Methodology] Convenience store service convenience (CVS SERVCON) was developed at the first time and the gap of relative importance level between the owners and the consumers was assessed by AHP analysis method..[Findings] First, the importance of access convenience was assessed as the first priority in both CVS owners' group and the consumers' group, while the detailed elements showed the differences which required the improvements. Based on the consumers' assessment, the importance of location was relatively low. They assessed relatively high on 24 hours operation of CVS and personal services. Second with respect to the transaction convenience, CVS owners' group rated it as the fourth priority while the consumers' group rated as the second, which requires the improvement. Third in terms of benefit utility and value-added service of post-benefit convenience which are the sub-factors of benefit convenience, the rate on the importance from the consumers showed higher than that from the owners, which requires the improvement. [Implications] Based on this study results, CVS operating companies are anticipated to be able to satisfy the customers, moreover, to secure the loyal customers if they concentrate the resources to develop the differentiation strategy on the areas where the consumers' demands are high while performs the maintenance strategy on the areas where their demands are low. In addition, this study contributed the theoretical expansion of SERVCON upon development of CVS SERVCON to comply with its definition.
