Challenges in Korea Hospital Accreditation: Focused on Post-Accreditation Management System (국내 의료기관 인증제의 과제: 사후관리 중심으로)

  • Objectives: The post-accreditation management system should be systematic in order to ensure that the accredited hospital continues to strive for patient safety and quality improvement during the accreditation period. In this study, we compare the post-accreditation management system in four countries (the U.S., Australia, Japan, and Korea) and provide suggestions for improving the post-accreditation management system in Korea. Results: All four countries had the post-accreditation management system, and the basic structure of the system in Korea was similar to that of others. However, there were differences in the practical operation processes and the use of the results. In the operation process, Korea's monitoring relies on voluntary reporting by accredited hospitals. In terms of results utilization, analytical feedback to data submitted by the acrredited hospital is not provided in Korea. Conclusions: It is necessary to establish a continuous monitoring system for post-accreditation changes and provide feedback to accredited hospitals. It is also necessary to perform a survey without advance notice and establish a firm legal basis for monitoring.

Patient-Controlled Analgesia (PCA) for Post-operative Patients - A Study on Differences according to Who Controls the Analgesic (수술환자의 자가통증조절기 사용 방법에 관한 조사 연구)

  • Purpose: The purpose of this study was to examine the effect of three kinds of modes using bolus button of PCA on level of pain and side effects of analgesic and amount of drug consumption in post-operative patients according to whether the medication is controlled by the patient, the caregiver or the nurse. Method: The participants were 684 patients using PCA after an operation. The data collection period was from March 19 to April 6, 2007. Results: It was found that there were statistical differences in gender, age, type of surgery, pain on first post-operative day, amount of drug consumption, nausea, and vomiting. The ratio for patient controlled medication was 55.7% for women, and 70.5% for men, and for care-giver controlled medication, 35.1% for women, and 20.0% for men. Average pain scores for the first post-operative day were $3.9{\pm}2.2$ for patient controlled medication and $4.5{\pm}2.3$ for care-giver controlled medication. There were statistical differences according to mode used for PCA for amount of drug consumptions, nausea and vomiting but not for pain, operation day or pruritus. Conclusion: This study was carried out to examine risks according to who controls the PCA for post-operative patients. The results can help to develop education program for everyone who is involved in PCA, patients, caregivers, nurses and doctors.

  • The surfaces of prototypes become rough due to the stair-stepping which is the inevitable phenomenon in the Rapid Prototypes are not used only for the verification of feature. The grinding, coating, or the composition of them is a main operation in post-processing in which lots of costs and long build time are needed. The solution is proposed to increase the efficiency of rapid prototyping by minimizing or removing the composition of them is a main operation in post-processing in which lots of costs and long build time are needed. the solution is proposed to increase the efficiency of rapid prototyping by minimizing or removing the regions for post-processing. the factors to cause the surface roughness and their effects are analyzed through the experiments. Software modules are developed to predict the surface roughness of each face in the prototyping with the result. An experimental compensation method is developed to apply the modules to various RP equipments, materials and build styles. The build direction is searched with use of genetic algorithm to maximize the total areas of the surface of which roughness is better than the user-defined value.

Exploratory Study of International Sports Event Life Cycle (복합종목 국제스포츠이벤트 생애주기에 대한 탐색적 연구)

  • Numerous types of sporting events have been hosted in Korea during the last decades. Sport events produce various types of data, and there is a clear necessity of storing and utilizing this information. From this background, the purpose of this study is to draw complex sports categories from international sport events life cycles in order to categorize standards of knowledges management system. This study utilized qualitative methods: expert groups discussion for overall life cycle steps and in-depth interviews for detail life cycle steps. The results show that there are several life cycle steps: bidding, preparation, operation, evaluation, post management steps. To be more specific, for the bidding step, there are check, plan, implementing detail phase; and the preparation step is based on event preparation Basis Build, event preparation implementation, test and supplementation phases. For the operation step, event operation system switch, event operation during the period, finish operation phases are deducted. Evaluation and post management step do not have detail phases. Efforts to store and utilize knowledge can be one of the starting points to make the operation of sport events efficiently.

An Aerodynamic and Acoustic Study of Nasalization in Cleft Palate Speakers. (구개열 언어의 비음화에 관한 공기역학 및 음향학적 연구)

  • Cleft palate patients have general speech problems with resonance disorders and articulation disorders. The aim of this study is to find the aerodynamic and acoustic characteristics of the nasalization in cleft palate speakers. Thirteen control groups and three cleft palate patients pre- and post operation were selected for these studies. The test words are composed by polysyllabic words: consonants between high vowel /i/ analysis. The cleft palate patients repeated test words pre- and post-operation from one, three and six month periods. The subjects repeated test words on Macquirer and on Nasometer Model 6200-3. The aerodynamic and acoustic results of nasalization show as follows: (1) The nasal rate in overall airflow of aspirated consonant for cleft palate patients shows higher levels than that of the control group. It had decreased since one month after operation. (2) The overall airflow of cleft palate patients is higher than in the control group, however oral air pressure is lower than control group. (3) The nasal airflow and the nasal rate in overall airflow of cleft palate patients has higher than the control group, however its decreased after operation. (4) The nasalance scores of cleft palate patients were 40% higher than that of the control group. The scores did not decrease after operation. The nasalance score of lateral and fricative sounds did not decrease after operation.

The Effectiveness of Meridian Acupressure Intervention Using Sticker Needles to Bowel Movement on Post Spinal Operative Patients (스티커 침을 이용한 경혈지압이 척추수술 후 배변에 미치는 영향)

  • This study was conducted to identify the effect of meridian acupressure on defecation of patients with post spinal operation. The nonequivalent control group posttest only design was used. The data were obtained from 77 post spinal operative patients, 34 in the experimental 43 in control group in Y Hospital, Seoul, Korea. The neurosurgical unit A and B ward, assigned by matched sample by the name of operation such as laminectomy and posterior lumbar interbody fusion are performed. Meridian acupressure meant the method that an examiner presses response points distributed in the pass of energy vessel. In this study, meridian acupressure program was performed on as points in order of Hegue (LI-4), Zhigou (TE-6), Zusanli (S-36), Shangjuxu (S-37), Xiajuxu (S-39), Tianshu (S-25), Taichong (L-3) which was known to be related to large intestine. Data were collected from 1, July 2003 to 1, September 2003. Meridian acupressure program was carried out for 20 minutes 4 hours after operation twice daily. In order to evaluate the effect of meridian acupressure intervention, they were asked time of bowel recovery, gas passing, and defecation though questionnaire method. Data were analyzed by the SPSS/ WIN 11.0 program. The results of this study were summarized as follows; 1. Homogeneity tests of general characteristics and operation related characteristics of the experimental group and the control group were performed. General characteristics included age, sex, defecation habit, eating pattern, fluid intake, life style, activity, usage of laxative and etc. 2. Hypotheses were verification as follows; 1) Recovery of bowel sound of the experimental group who received meridian acupressure intervention was faster than that of the control group after spinal operation (t=-6.770,P=.000). 2) Time of gas passing of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-8.003, P=.000). 3) Time to defecation of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-9.026, P=.000). 4) Abdominal discomfort due to defecation of the experimental group who received meridian acupressure program was lesser than that of the control group after spinal operation (t=-3.431, P=.001). From these results, meridian acupressure intervention was effective for recovery of bowel sound, reduce time to gas passing, time to defecation and lessen abdominal discomfort due to defecation on post spinal operative patients. And therefore this intervention can probably considered on clinical practice.

  • The Post-Construction Evaluation and Management System has been operated over the past 20 years not only to analyze the performance of large-scale construction projects, and but also to provide useful reference data for future similar projects. Although the Post-Construction Evaluation and Management Center has been legally designated and operated since 2020, various problems have been occurring in the operation of the system. Accordingly, this study proposes strategies for facilitating Post-Construction Evaluation and Management System in terms of the following three aspects: (1) improving the implementation rate of post-construction evaluation; (2) improving the information management system related to post-construction evaluation; (3) improving the utilization of post-construction evaluation outcomes. Based on the results of this study, a specific action plan will be prepared, which is expected to contribute to the efficiency of construction project performance management in the future.

  • In a 53-year old male with post-infarction ventricular septal defect [VSD , owing to an acute exacerbation of pulmonary edema, respiratory failure developed, and the ventilatory support and intraaortic balloon counterpulsation [IABP were applied. At the following day, operation was performed with the aid of IABP. Under the cardioplumonary bypass, he underwent infarctectomy, trimming of VSD margin, patch closure of VSD and infarctectomy site. Left ventricular free wall rupture was detected during operation, which was confined with pericardial adhesion. Post-operative course was uneventful, and he could be discharged with minimal degree of dyspnea [NYHA class II .

The study of the Asymmetrical Half-Bridge Converter With magnetic coupled post regulator

  • In this paper, a multiple-output converter using a coupledinductor is proposed. The Asymmetrical Half-Bridge Converter (AHBC), which is used as a master converter, obtains ZVS in the primary side switches. For the tight output voltage regulation of the slave output, the Secondary Side Post Regulator (SSPR) is adopted. The proposed magnetically coupled output filter inductor reduces the number of semi-conductor devices and magnetic components. Also, the circuit enables ZCS off switching in the SSPR MOSFET and rectifying diodes. The modes of operation which are caused by the coupled-inductor and post regulator are explained. The analyzed modes of operation of the proposed multiple-output converter are verified by the simulation and experimental results.

  • Adenoid cystic carcinoma is a rare primary tracheal tumor and this tumor behave slow growth, low-grade ma!ignancy, locally invasion and long-term host survival. Operation with the primary goal of complete excision is the treatment of choice but this tumor require excessive margins at surgical removal because of locally invasive cancer. A 45-years-old male patient had complained paroxysmal coughing from 1 year ago prior to admission and was diagnosed pre-operatively as endotracheal adenoma. He had been treated by operation, and combined with radiotherapy by 4 MeV. Lineal Accelerator. The tracheal mass was removed by tracheo-bronchotomy transpleurally and right total pneumonectomy was performed. There was post-operative course uneventfully and no post-operative complication. The patient Is free from cancer until post-operative 1 year clinically and alive with good healthy.
