Result of Post Operative Radiotherapy of Carcinoma of the Rectum (직장암의 수술후 방사선치료 성적)

  • Surgery remains the mainstay in the management of carcinoma of the rectum. However, local recurrence and systemic metastasis remain the challenge. It appears that post operative radiotherapy has a very definite role in the reduction of local recurrence. Minty two patients of carcinoma of the rectum after curative surgery received post operative radiotherapy $5,000rad/5\~6weeks$ to whole pelvis at the Department of Therapeutic Radiology, Seoul National University Hospital between March 1979 and December 1982. Fifty three percent of patients show modified Astler-Coiler stage C2. Actuarial disease free survival rate of rectal cancer was : stage B1, 2 $75\%$, stage C1 $81\%$ stage C2 $39\%$, and stage C3 $20\%$, Twelve percent shows local recurrence and distant metastasis occurred in $28\%$. Prognostic significance of nodal metastasis is also analysed. Incidence of small bewel obstruction, requiring surgery, is $8\%$, occurring between 5th month to 12 th month after operation. It is suggested that post operative radiotherapy of the rectal cancer following curative surgery has a significant role in the reduction of local recurrence.

Management of post-pneumonectomy empyema (전폐절제술후 발생한 농흉의 치료)

  • Post-Pneumonectomy Empyema[PPE] is a relatively uncommon but serious complication. And the management of it remains a disturbing and controversial area in the field of general thoracic surgery. Many methods have described and have had varying degrees of success. For the purpose of providing the guideline for management of post-pneumonectomy empyema, we reviewed our experiences of treatment of PPE from January 1985. to December 1992. There were 17 cases, which consist 7.9% of all pneumonectomy cases for that period. There were 13 male and 4 female patients with mean age of 47.1$\pm$ 16.2 yrs old. Both chest has the same incidence. The most common disease for prior pneumonectomy was tuberculosis, but the PPE was the most frequently occurred in empyema. The duration between pneumonectomy and PPE was 44.7 $\pm$81.1 months, where 58.8% of patients occurred within 1 month. Fever was the most frequent complaint and wound dischrge was detected in less than half of patients. There were 2 in-hospital mortalities.Mostly, in 13 cases, we did Eloesser operation. Five of them could finish second Clagett procedure, but one had recurrence. Four bronchopleural fistular patients underwent 3 single stage muscle flap closure and 1 direct closure with modified Clagett procedure. None had recurrence. Mean follow-up duration is 30.9\ulcorner22.3 months. There was 1 late death which was not related to PPE but to Malignancy recurrence.

  • Post-operative maxillary cyst may arise after the surgical intervention for maxillary sinusitis with the symptoms of swelling, pain and pus discharge in the buccal region. The author analized 66 cases clinically and radiographically which were diagnosed as post- operative maxillary cyst in SNUH during 5 years (1977. 8-1982. 7). The obtained results were as follows; 1. This cyst occurred more frequently in male than in female and the incidence is the highest in the 4th decade. 2. The right side was more frequently affected than the left side. 3. Initial radical operations of the maxillary sinueses were performed mainly between the age of 15 and 24 years. 4. The duration between the initial operation and the onset of cyst was mainly from 10 to 24 years. 5. Pain, swelling and pus discharge in the buccal region were most frequent chief complaints. 6. In panoramic radiographs, most of the post-operative maxillary cysts were monolocular type, showing distinct border, with smooth margin and without any definite sclerotic border. 7. In Waters' view, 43 cases showed radiographic changes due to cyst.

  • To achieve the high-speed implementation of post-quantum cryptography, primitive operations should be tailored to the architecture of the target processor. In this paper, we present the optimized implementation of multiplier operation on RISC-V processor for post-quantum cryptography. Particularly, the column-wise multiplication algorithm is optimized with the primitive instruction of RISC-V processor, which improved the performance of 256-bit and 512-bit multiplication by 19% and 8% than previous works, respectively. Lastly, we suggest the instruction extension for the high-speed multiplication on the RISC-V processor.

Extended Epitympanotomy for Facial Nerve Decompression as a Minimally Invasive Approach

  • For a minimally invasive approach to access the facial nerve, we designed an extended epitympanotomy via a transmastoid approach that has proven useful in cases of traumatic facial nerve palsy and pre-cholesteatoma. To evaluate the surgical exposure through an extended epitympanotomy, six patients with traumatic facial nerve palsy were enrolled in this study. The same surgical technique was used in all patients. Patients were assessed and the degree of facial nerve paralysis was determined prior to surgery, 1-week post-operatively, and 6-months post-operatively using the House-Brackmann grading system. In all cases, surgical exposure was adequate. All patients with traumatic facial nerve palsy were male and the age range was 13 to 83 years. In all cases, the location of the facial nerve damage was limited to the area between the first and second genu. Symptoms of all the patients improved by 6 months post-operation (p=0.024). There were no complications in any of the patients. Extended epitympanotomy is useful for safe, rapid surgical exposure of the attic area, sparing the patient post-operative dimpling, skin incision complications, and lengthy exposure to anesthesia. We suggest that surgery for patients with facial nerve palsy secondary to trauma be performed using this described technique.

  • This case study refers to turbine-generator with retrofitted turbine rotor. Vibration problem occurring after the retrofit was mainly due to high vibration from exciter side. However, repeated test run and operation during scheduled maintenance caused high vibration from generator bearing, and post-overhaul test run before turbine vibration correction caused oil whip on the bearing. This case study shows how to analyze vibration condition of high turbine generator vibration detected during the post-overhaul test run and vibration condition of offline and online data to reflect it on establishing maintenance schedule and overhaul correction procedure. Vibration data could be acquired during steady load operation or even with varying speed and load. Each data is important for machinery condition evaluation. This case study shows that the vibration data during extreme condition is the key factor in analysis, which helps to find the machinery problem.

Two-Stage Model for Security Network-Constrained Market Auction in Pool-Based Electricity Market

  • This paper presents a two-stage market auction model in a pool-based electricity market, which explicitly takes into account the system network security. The security network-constrained market auction model considers the use of corrective control to yield economically efficient actions in the post-contingency state, while ensuring a certain security level. Under this framework, the proposed model shows not only for quantifying the correlation between secure system operation and efficient market operation, but also for providing transparent information on the pricing system security for market participants. The two-stage market auction procedure is formulated using Benders decomposition (BD). In the first stage, the market participants bid in the market for maximizing their profit, and the independent system operator (ISO) clears the market based on social welfare maximization. System network constraints incorporating post-contingency control actions are described in the second stage of the market auction procedure. The market solutions, along with the BD, yield nodal spot prices (NSPs) and nodal congestion prices (NCPs) as byproducts of the proposed two-stage market auction model. Two benchmark systems are used to test and demonstrate the effectiveness of the proposed model.

Clinical Nurses' Interpretation of the Experiences of Breast Cancer Women after Breast Conservative Operation (유방암 여성의 유방보존술 후 경험에 대한 임상 간호사의 인지)

  • Purpose: Even though the breast is not removed, women with a breast conservative operation(BCO) are suffering from the psychosocial distress comparable with mastectomies. The purpose of this study was to identify how clinical nurses view essential nursing problems and nursing approaches to post operative breast cancer experiences. Method: To identify the clinical nurses's interpretation, they were asked to respond to a case involving a woman struggling with breast cancer. The nurses' responses were analyzed using a manifest content analysis strategy to determine themes in problem identification and nursing care strategies. Result: The clinical nurses recognized that most of the patient's experiences originated from a psycho-social problem. Fifteen nurses(83%) interpreted the breast cancer woman's experience derived from her psychosocial problem. They identified the most effective nursing approaches for breast cancer women following surgery as counseling and providing information. Conclusion: In conclusion, the results revealed that a psychosocial skill including an empathic communication skill is essential for excellent nursing in post operative patient care.

  • A new precess which consists of pre-dewatering device, post composting for solid phase and post sequencing batch reactor(SBR) for liquid phase was designed. Nitrogen in supernatants of dewatering device was removed by sequencing batch reactor. Experiments were carried out to investigate the SBR operation modes such as fill ratio, SRT, and operation cycle. The optimum fill ratio, SRT and aeration/non-aeration time were 1/12, 15days, and 2hr aeration / 1hr non aeraion, respectively. Methanol as an external carbon source increased denitrification when step feeding method was applied, not single feeding method.
