경찰관의 PTSD 수준 및 대처방안 (Levels and Countermeasures of PTSD among Police Officers)

  • 경찰관을 대상으로 한 연구들에 따르면 최근에 위급상황에 노출되었던 경찰관 중 상당수가 심각한 심리적인 스트레스 상태에 있으며, 사기, 장기 결근, 직업적 건강, 조기 퇴직, 가족 관계에 상당한 영향을 미치는 외상후 스트레스 장애(Post Traumatic Stress Disorder : PTSD) 증후를 보이는 것으로 나타났다. 그러나 우리나라에서 그 동안 경찰관의 PTSD를 실증적으로 살펴본 연구는 거의 없었다. 이 연구에서는 경찰관의 PTSD 수준을 파악하고 그에 따른 대처방안을 모색하고자 하였다. 경찰관의 PTSD 수준은 연구에 따라 다소 차이는 있으나 비교적 높은 수준이고, 소방관과 비교해도 상대적으로 높은 수준이다. 이에 대한 대처방안으로 고위험 경찰관의 발견 및 관리, 위기상황 스트레스 관리 기법의 보급, 직원 지원 프로그램의 도입, 동료 및 가족의 적극적 개입, 스트레스 관리 교육 등을 제시하였다.

세월호 참사 희생자 부모들의 심리적 외상에 관한 기술적 접근 (A Technical Approach to Post-Traumatic Stress Disorder of the Sewol Ferry Victims' Parents)

  • 2014년 4월 16일 전남 진도 인근 관매도 해상에서 침몰한 세월호 참사가 발생한 지 1주기가 지났다. 총 304명의 희생자와 실종자를 낳은 세월호 참사는 2010년대 이후 대한민국에서 가장 큰 규모의 참사였다. 그러나 세월호 참사 1주기가 지나도록 유족의 트라우마에 대한 심리적, 커뮤니케이션학적 연구는 전혀 없었다. 본 연구는 국내에서 처음으로 자식을 잃은 부모로서 세월호 참사로 인한 극심한 외상 후 스트레스 장애를 겪고 있는 세월호 유가족의 실태를 조사했다. 연구결과 신체적으로, 심리적으로 세월호 유족의 경우 외상 후 스트레스 장애가 발생하고 있음을 확인할 수 있었고 치료에는 소홀한 것으로 나타났다. 외상 후 스트레스 장애는 일상생활에도 영향을 미쳤다. 세월호 유족은 대인관계, 직장관계, 거주지, 가치관 등 많은 부분에서 일상적인 생활에 문제가 많은 것을 확인할 수 있었다. 본 연구를 통해 참사로 인한 세월호 유족의 외상 후 스트레스 장애가 막대한 것으로 나타난 만큼 학계에서도 더욱 적극적으로 관련된 논의를 함께해 우리 사회에서 방치된 세월호 유가족의 고통을 치료하는데 나서야 할 것이다.

외상 후 스트레스 장애 환자의 혈청 테스토스테론치 (Serum Testosterone Levels in Patients with Post-Traumatic Stress Disorder)

  • Objective : Several reports have found abnormal levels of androgen in post-traumatic stress disorder (PTSD) patients. This abnormality in androgen is hypothesized to due to chronic psychological stress effects on the hypothalamic-pituitary-gonadal (HPG) system. The present study was conducted to estimate serum testosterone levels in PTSD patients in comparison with normal subjects. Methods : Seventy-five male Korean veterans of the Vietnam War volunteered for the study, of which eleven were excluded because of incomplete psychological assessment. To measure basal serum testosterone, blood samples were collected between 8.00 and 9.30 a.m. The clinician administered PTSD scale (CAPS), the structured clinical interview for DSM-IV (SCID), Hamilton Anxiety Rating Scale (HAM-A), Hamilton Rating Scale for Depression (HAM-D), Mini International Neuropsychiatric Interview Plus (Korean version of MINI-Plus), CES-K (Korean version of combat exposure scale). Results : The serum testosterone level of PTSD patients ($5.4{\pm}2.5ng/mL$) was higher than that of a control group ($3.1{\pm}1.7ng/mL$, p<0.001). Testosterone levels were significantly correlated with CAPS (r=.38, p<.01), HAM-A (r=.35, p<.01) and HAM-D (r=.28, p<.01) in all subjects. Conclusion : The results of the present study suggest that chronic psychological stress affects the HPG system.

외상 후 스트레스 장애의 위험요인으로서의 사상체질 (Sasang Constitution May Act as a Risk Factor for Post Traumatic Stress Disorder)

  • In this study, we suggested indirect evidence of whether Sasang constitution(SC) could be a risk factor for Post-traumatic stress disorder(PTSD) among soldiers who participated in Vietnam war. The number of 199 subjects joined this study. We surveyed SC with KS-15 and PTSD with Korean Version of Impact of Event Scale-Revised questionnaires. A Chi-square test and a one-way ANOVA were performed according to SC type, and multiple logistic regression was used to produce odds ratios(ORs). Significant p was .05. The number(rate) of Taeeumin(TE), Soeumin(SE), and Soyangin(SY) types were 131(65.8%), 33(16.6%), and 35(17.6%) respectively. The score of PTSD in SE type(8.78±8.61) was significantly higher than those of SY(4.00±6.5) and TE types(3.65±6.78)(p=.001, SE>TE, SY, Scheffe). SE type was associated with increased PTSD prevalence compared with TE [OR 4.338; 95% CI 1.525-12.335, p<.01], and it was still associated with increased PTSD prevalence [OR 10.658; 95% CI 1.296-87.661, p<.05] after adjusting for age and weight. This study suggests that SC, particularly the SE type, might be significantly associated with PTSD and could be considered as a risk factor for PTSD.

Effects of systemic administration of ibuprofen on stress response in a rat model of post-traumatic stress disorder

  • Pro-inflammatory cytokine and brain-derived neurotrophic factor (BDNF) are modulated in post-traumatic stress disorder (PTSD). This study investigated the effects of ibuprofen (IBU) on enhanced anxiety in a rat model of PTSD induced by a single prolonged stress (SPS) procedure. The effects of IBU on inflammation and BDNF modulation in the hippocampus and the mechanisms underlying for anxiolytic action of IBU were also investigated. Male Sprague-Dawley rats were given IBU (20 or 40 mg/kg, i.p., once daily) for 14 days. Daily IBU (40 mg/kg) administration significantly increased the number and duration of open arm visits in the elevated plus maze (EPM) test, reduced the anxiety index in the EPM test, and increased the time spent in the center of an open field after SPS. IBU administration significantly decreased the expression of pro-inflammatory mediators, such as tumor necrosis $factor-{\alpha}$, $interleukin-1{\beta}$, and BDNF, in the hippocampus, as assessed by reverse transcription-polymerase chain reaction analysis and immunohistochemistry. These findings suggest that IBU exerts a therapeutic effect on PTSD that might be at least partially mediated by alleviation of anxiety symptoms due to its anti-inflammatory activity and BDNF expression in the rat brain.

북한이탈주민의 외상후 스트레스장애의 임상적 특성 (Clinical Characteristics of Post-Traumatic Stress Disorder among North Korean Defectors)

  • Objective : The purpose of this study was to investigate prevalence and clinical characteristics of Post-traumatic Stress Disorder (PTSD) in North Korean defectors (NKD). Methods : The study population consisted of 300 NKDs registered in the multi-regional adaptation center (Hana Center), within three years of settling in South Korea. We conducted in-person interviews and a survey with each subject, based on the North Korean version of the WHO-Composite International Diagnostic Interview (NK-CIDI) and various clinical scales. Results : Lifetime prevalence of PTSD in NKDs was 15.3%, approximately nine times higher than South Koreans (1.7%). Although experiencing broader type of traumas with higher rate than South Koreans, NKDs revealed lower odds of PTSD in most type of trauma (p<0.05). Conclusion : NKDs are at higher risk of experiencing different types of trauma than the general population in South Korea, and it is further validated that prevalence of PTSD is also higher. Careful evaluation for comorbid psychiatric symptoms and type of traumas prior to PTSD treatment for NKDs is necessary to facilitate more appropriate intervention for each subject, according to personal experience.

관상동맥질환자의 외상 후 스트레스장애 증상과 삶의 질 (Post-Traumatic Stress Disorder Symptoms and Quality of Life of Patients with Coronary Artery Disease)

  • Purpose : The purpose of this study was to investigate the degree of post-traumatic stress disorder (PTSD) symptoms and the effect of PTSD on quality of life (QoL) among coronary artery disease (CAD) patients. Methods: The participants were 135 CAD patients who visited outpatient clinics in a university hospital in B city of Korea. Their PTSD symptoms and QoL were measured by structured questionnaires and analyzed with multiple hierarchical regression analysis. Results: Among the 135 participants, 20.7% were classified as moderate risk, and 3.7% as high risk for PTSD. PTSD symptoms were significantly higher in participants who were diagnosed with CAD within 6 months (t=26.02, p<.001). The physical component of health-related QoL was influenced by gender (${\beta}=-.25$, p =.003), recurrence of CAD (${\beta}=.21$, p =.008), and PTSD symptoms (${\beta}=-.33$, p <.001). The mental component of health-related QoL was influenced by religion (${\beta}=-.17$, p =.044), body mass index (${\beta}=.17$, p =.033), and PTSD symptoms (${\beta}=-.37$, p <.001). Conclusion: Patients who had had a CAD diagnosis for less than 6 months were found to be vulnerable to PTSD, and PTSD was found to have a negative impact on the physical and mental components of their QoL. It is necessary to develop an intervention program for the effective prevention and management of PTSD symptoms in patients with CAD.

응급실 근무자의 외상 후 스트레스장애 유병률 및 발생 기여 요인 (The Prevalence of Post-traumatic Stress Disorder Among Emergency Room Workers and Factors Contributing to Its Occurrence)

  • Purpose: This study was designed to investigate that the prevalence of Post-traumatic stress disorder (PTSD) and occupational environmental factors that contribute to the developing of PTSD in workers who worked in emergency room in Korea. Methods: This study is a multicenter cross-sectional study that was conducted in three emergency rooms. Data was collected by self-response method through questionnaire. The Impact Event Scale-Revised was used to screen the high-risk group of PTSD. Univariate and multivariate regression analysis was used to identify the factors associated with the developing of high-risk group of PTSD. Results: Total 211 subjects responded to the questionnaire. Of these, 170 subjects experienced psychological trauma event (PTE) in the working environment. And 60 subjects classified as high risk group of PTSD, so a prevalence of high risk group for PTSD was 28.4%. Factors related to the high risk group of PTSD were the frequency of psychological trauma events experienced over the past 1 year and the high score of QIDS-SR16. Conclusion: The prevalence of high risk group for PTSD among emergency room workers in Korea is very high. This may adversely affect the safety of patients, so early screening and intervention are necessary.

코로나바이러스감염증 환자를 돌본 일개 상급종합병원 의료인의 외상 후 스트레스 장애와 소진 (Post-traumatic Stress Disorder and Burnout of Healthcare Providers who cared for Patients with Coronavirus Disease-19 (COVID-19) in a Tertiary General Hospital)

  • Purpose : This study aimed to identify post-traumatic stress disorder (PTSD) and burnout experienced by healthcare providers who cared for patients with Coronavirus Disease 2019 (COVID-19) and their influencing factors. Methods : Data were collected from 135 healthcare providers who cared for patients with COVID-19 in a tertiary general hospital from June 8 to September 2, 2021, using a questionnaire. Descriptive statistics, Mann-Whitney U test, Kruskal-Wallis test, t-test, ANOVA and Scheffe's test, Pearson's correlation coefficients, and multiple regression were used for analysis using SPSS/WIN 27.0. Results : Participants' average PTSD score was 9.31 ± 11.80, and 8.9% were in the high-risk group. Participants' average burnout score was 51.77±21.28, and 62.2% were at high risk. PTSD scores differed significantly according to participants' age, education, job, position, and current workplace. Burnout scores differed significantly according to their age, gender, marital status, parental status, and education. There was positive correlation between participants' PTSD and burnout. The factors influencing participants' PTSD were term of self-isolation and age (R2=.09). There were no significant influencing factors on participants' burnout. Conclusion : This study reconfirmed that healthcare providers who cared for patients with COVID-19 experienced both PTSD and burnout, suggesting that interventions are needed such as regular pre-training or simulation training and establishing a support system.

청소년의 학교폭력 피해 경험이 복합 외상 후 스트레스에 미치는 영향 - 자아존중감의 조절효과를 중심으로 - (The Effects of School Violence in Adolescents on Complex Post-traumatic Stress Disorder Symptoms - Focus on Moderating Effect of Self-esteem -)

  • 본 연구는 청소년의 학교폭력 피해 경험이 복합 외상 후 스트레스에 미치는 영향과 이 관계에서 자아존중감의 조절효과를 검증하고자 하였다. 이를 위해 291명의 청소년을 대상으로 조사된 자료를 연구에 활용하였으며, 조절회귀분석을 통해 연구모형을 검증하였다. 연구 결과, 학교폭력 피해 경험과 자아존중감은 복합 외상 후 스트레스에 유의미한 영향을 가진 것으로 나타나 학교폭력 피해 경험의 수준이 높을수록, 자아존중감이 낮을수록 복합 외상 후 스트레스가 증가하게 됨을 확인하였다. 또한 자아존중감은 학교폭력 피해 경험이 복합 외상 후 스트레스에 미치는 영향을 조절하고 있음을 확인하여 같은 수준의 학교폭력 피해 경험이 있더라도 자아존중감이 높은 경우 복합외상 후 스트레스의 문제를 완화할 수 있음을 확인하였다. 연구 결과를 바탕으로 청소년의 학교폭력과 이로 인해 발생하는 복합 외상 후 스트레스의 문제를 완화하기 위한 방안을 제시하였다.
