The emergency patients analysis of 1339 Emergency Medical Information Center received during the holidays - New Year's Day & Thanksgiving Day of Gwangju & Jeonnam region in 2007 - (연휴동안 1339 응급의료정보센터에 접수된 응급환자 현황 분석 - 광주·전남지역의 2007년 구정과 추석을 중심으로 -)

  • During New year's day and Thanksgiving holiday, the 1339 Information Center in Gwangju and Jeonnam received an emergency medical situation and carried out the analysis as follows ; 1. The reason for the higher phone guidance(72%) of the hospital was based on the simple query for the pain control rather than emergency care. This was explained by the unfamiliar environment of the holiday movers. Pharmaceutical guidance(12.6%) in rural area was also given during the holidays. 2. The disease counselling(2.4%) and emergency treatment instruction(First Aid)(1.6%) decreased because of the small number of request for the diseases. This explained the decrease of the disease counselling. 3. The phone calls increased 8-13 times more than usual. Five-day holidays could have more emergency patients than three-day holidays. During New year's day and Thanksgiving day, the number of received phone calls was 6,444(25.7%) and this accounted for one fourth of the total number in 2007. 4. The number of the patients increased on New year's day and Thanksgiving day because most of the medical institutions and pharmacy were off duty. 5. The patients were centered in Internal medicine and pediatrics. This showed the food-borne diseases and air-borne infection such as common cold. 6. During Thanksgiving day, ophthalmologic patients having viral keratoconjunctivitis(Apollo eye disease) increased 8 folds than in New year's day. It was estimated from the continuous epidemic of Apollo eye disease. 7. There broke out the traffic accidents, food poisoning, infectious diseases, dermatological diseases due to seasonal and environmental changes during the holiday move.

Antibacterial Efficacies of Disinfectants against Salmonella typhimurium Depending on Pre-warming Conditions

  • Salmonellosis is a widespread bacterial zoonosis that commonly causes enterocolitis and foodborne poisoning leading to an extensive economic loss in domestic animal industry. Considerably, the emergence of multidrug resistant strains of Salmonella spp. induces further severe problems affecting public health. The present report was designated to investigate the antibacterial efficacies of three common disinfectants including an oxidizing compound disinfectant (OXC), a triple salt (TS) and a quaternary ammonium compound (QAC) against Salmonella typhimurium subjected to the preliminary changes of drug temperature. All solutions of three disinfectants were pre-incubated at different temperature (22, 37 and $63^{\circ}C$) for 1 h prior to exposure to bacteria. The disinfectants and bacteria were diluted with distilled water (DW), hard water (HW) or organic matter suspension (OMS) according to treatment condition. Under the DW condition, the disinfectant efficacy of the QAC at $63^{\circ}C$ was higher than that of $22^{\circ}C$. Furthermore, under HW diluent the disinfectant efficacy of the TS pre-warmed at both of 37 and $63^{\circ}C$ were increased compared to that of $22^{\circ}C$. Considerably, the efficacy of pre-warmed QAC at both of 37 and $63^{\circ}C$ under the OMS diluent were higher than that of $22^{\circ}C$. Conclusively, prewarming at higher temperatures have positive effects on the stability of the antibacterial efficacies of TS and QAC.

Stabilization of Arsenic in Paddy Soils Using Stabilizers (논토양 내 비소 불용화에 대한 안정화물질의 처리 효과)

  • BACKGROUND: Soil contamination of As is a very sensitive environmental issue due to its adverse impact on human health and different characteristics with other heavy metals. With public awareness of As poisoning, there has been growing interest in developing guideline and remediation technologies for As-contaminated soil. The objective of this research was to evaluate the effectiveness of stabilizing amendments and soil dressing methods on the mobility of As in the contaminated rice paddy soils nearby mining area. METHODS AND RESULTS: Different amendments were mixed with surface and subsurface contaminated soils at a ratio of 3% (w/w) and monitored for five months. Three different extractants including 0.01M $CaCl_2$, TCLP, and PBET were used to examine As bioavailability in the soil and the concentration of As in rice grain was also measured with an inductively coupled plasma (ICP) spectroscopy. The results showed that all amendment treatments decreased As concentration compared to the control. Especially, coal mine drainage sludge (CMDS) treatment showed the highest efficiency of decreasing As concentration in the soil and rice grain. The values of Pearson correlation (r) between As concentrations in the soil and rice grain were 0.782, 0.753, and 0.678 for $CaCl_2$, TCLP, and PBET methods, respectively. Especially, $CaCl_2$ method was highly correlated between As concentrations of the soil and soil solution (r=0.719), followed by TCLP (r=0.706), PBET (r=0.561) methods. CONCLUSION: Stabilizing amendments can effectively reduce available As concentration in the soils as well as soil solution, and thereby potentially mitigating risks of crop contamination by As.

Anti-norovirus activity of natural compounds and its potential in food application (항노로바이러스 천연물을 이용한 식품개발)

  • Control of food pathogens is critical in food safety field. Norovirus is one of the major causes of gastroenteritis and food poisoning worldwide, however, currently, there is not a vaccine or a specific drug available for its treatment. There are several methods to inactivate norovirus during food processing by chemical and physical treatments, however, the use of natural substance has been suggested as an optional strategy due to their safety and consumer preference. In this study supported by Ministry of Food and Drug Safety in Korea, we identified novel plant-derived substances with significant anti-norovirus activities. The aim of this project was to determine the antiviral activity of a wide range of natural substances, including plant-derived extracts and essential oils, using a norovirus surrogate system, human norovirus replicon-bearing cells, and mouse in vivo experiments. During the activity screening test, we identified novel anti-norovirus substances or oils using plaque assay with MNV-1. Six selected substances were formulated into an optimum mixture and used as an ingredient for salad sauce of which anti-novovirus activity was confirmed(pending for patent and paper submission). The potential application of selected natural substances as a metal surface sanitizer was also tested. Interestingly, the mixture of selected natural compounds showed a significant inhibitory effect against norovirus. These results suggest that these substances may be used as food ingredient with anti-norovirus antivity or components for surface sanitizers to prevent norovirus contamination.

The effects of water extract from Dictamnus dasycarpus Turcz on Hepatocellular Damage in vitro (백선 추출물의 간세포 손상에 대한 연구)

  • Objectives : This study was carried out to evaluate whether the water extract from cause the cellular damage in HepG2 cell line. It was reported that Dictamnus dasycarpus Turcz(DDT) intake induce poisoning symptoms in human population. These symptoms was closely related to liver toxicity, however, mechanisms for liver toxicity caused by DDT have not been elucidated exactly. Here, hepatotoxicity caused by DDT was evaluated using HepG2 cell line. Methods : Water extract of DDT was treated into HepG2 cell with various doses such as 0, 0.1, 0.5, 1.0 and $5.0mg/m{\ell}$. In order to cell viability, both MTT and LDH assay were carried out. Also, apoptosis array kit was used to identify whether cell death caused by DDT is due to apoptosis or not. In addition, reactive oxygen species (ROS) was measured after treatment of water extract. Results : We found out significant changes in the apoptosis related factors of hepatocyte. The cell viability of HepG2 treated with DDT water extract was decreased in dose-dependent. Also most of the apoptosis related factors were significantly increased. We found out that Caspase 3, Cytochrome C and ROS had increased in dose-dependent. In addition, other apoptosis related factors Bcl 2 and Bax, which were also constant changes. However, there was no significance. Conclusions : These results suggest that water soluble extract of DDT is expected to have oral toxicity, including hepatocellular damage Therefore, it is suggested that DDT could cause various side effects and toxicity of clinical conditions.

Effect of Chitosan-Ascorbate and Morea(roasted of oyster shell at $1300^{\circ}C$) on Growth of Contaminating Bacteria in Dombaeki(traditional shark dish) during Storage (돔배기의 저장 중 오염미생물의 생육에 미치는 키토산-아스코베이트 및 모려의 처리효과)

  • The effects of 0.01%(w/v) chitosan-ascorbate(CA) and 10 ppm morea on the number of total microbes, Escherichia coli levels, and growth of food poisoning bacteria in dombaeki during storage at $10^{\circ}C$ over 6 days were investigated. Total microbes in meat, cartilage, and skin of untreated samples increased by 4.24, 3.81, and 2.20 logs compared to the zero timepoint, respectively, but, in CA-treated samples, counts fell by 2.66, 2.37, and 1.24 logs. Total microbial levels in morea-treated meat, cartilage, and skin showed similar tendencies but the effects were slightly less than seen in CA-treated samples. E. coli numbers in CA-treated meat, cartilage, and skin stored for 6 days decreased by 1.69, 1.25, and 1.52 logs respectively, compared with control samples. Morea-treated samples showed similar falls, but the effects were again slightly less than seen after CA-treatment. Both Salmonella and Vibrio parahaemolyticus were detected in untreated meat stored for 3 or 6 days. Food poisoning bacteria were found in both untreated and morea-treated samples stored over 6 days. However, no such bacteria were detected in CA-treated samples. Also, CA-treated meat, cartilage, and skin showed low degrees of degeneration. Thus, CA treatment enhanced shelf-life and dombaeki quality by inhibiting microorganism growth and tissue breakdown during storage.

The Clinical Aspects of Wild Plant Poisoning (야생식물 중독의 임상 양상)

  • Purpose: With the recent boom in 'eating healthy', many adults are interested in dieting to prevent future diseases. However only well trained experts can distinguish between what are edible vegetables and herbs from their poisonous look-alikes. In cases where a patient unknowingly ingests a poisonous herb, is caught off guard by the poisonous side effects that occur because of their lack of knowledge of what they have ingested. This paper will focus on the need to educate the public about the risks involved with ingesting wild vegetables and herbs and study the emergency diagnosis and treatment of poisoned patients that enter the emergency room. Method: This study was done in the spring of 2004 (from March to May) in the Kangwon Young-Seo districts of Korea. 15 subjects used in this study, entered the emergency room showing signs of toxic symptoms. Data was collected by examining subject's records. Additional data was collected by collaborating with physicians in the hospital that diagnosed and treated the subjects. Identifying the poisonous vegetable or herb is the first step to proper diagnosis and treatment. Subjects admitted to the emergency room, underwent a battery of tests: laboratory examination, ECG, radiological exam and etc. Results: The demographics of the study encompassed subjects with the average age of $50{\pm}19$ years old. There were 10 men and 5 women. Common symptoms of this study included; gastrointestinal symptoms such as nausea, vomiting, epigastric pain and so on. In the case of Caltha palustris ingestion, additional symptoms were present; bradycardia and hypotension which lasted for a long time. While cases that ingested Scopolia parviflora had little effect on vital signs but manic episodes lasted for about three days. Veratrum patulum ingestion showed signs of bradycardia and hypotension but contrary to Caltha palustris recovery was shorten by treating with dopamine. However, dizziness, headache and paresthesia of the extremities continued for a long time. Finally Sium ninsi ingestion showed visual disturbance, paresthesia of the extremities, dizziness as their initial symptoms. Conclusion: The risks involved with ingesting wild plants without the proper knowledge can lead to serious side effects and steps need to be taken to educate the public. In addition, all emergency physicians need to have a working knowledge of the symptoms and signs associated with ingesting toxic wild plants and need to treat accordingly.

Inhibitory Effect of High Hydrostatic Pressure Treatments on Histamine Production in Mackerel Scomber japonicus (초정수압 처리에 의한 고등어(Scomber japonicus)육 중의 히스타민 생성 억제 효과)

  • This study investigated the inhibitory effect of high hydrostatic pressure (HHP) treatments on histamine production in mackerel Scomber japonicus. Changes in viable cell counts, histamine contents, pH and VBN of mackerel fillet (stored at $4^{\circ}C$ for 25 days) were examined under HPP (200, 300, and 400 MPa). HPP treatments reduced viable cell counts by 2-3 log cycles during storage. Viable cells of mackerels treated with 400 MPa did not appear for 5 days. Histamine production was nearly suppressed by 300 and 400 MPa HPP treatments after 25 days. Furthermore, mackerels treated with HPP showed significantly lower VBN values compared with the control. Additionally, pH values were not affected by the treatments during storage periods. These results suggest that HPP treatment decreased histamine contents in mackerel muscles. Based on our results, HPP treatment may reduce scombroid fish poisoning by decreasing histamine production in mackerel during $4^{\circ}C$ storage.

Effects of Combination of Nitrate with ${\beta}$1-4 Galacto-oligosaccharides and Yeast (Candida kefyr) on Methane Emission from Sheep

  • The objective of the present study was to determine whether ${\beta}$1-4 galacto-oligosaccharides (GOS) and Candida kefyr combined with nitrate as manipulators could suppress rumen methanogenesis without nitrate poisoning in sheep. Four rumen fistulated wethers were allocated to a $4{\times}4$ Latin square design. Nitrate (1.3 g $NaNO_3$ $Kg^{-0.75}$body weight) with and without GOS and Candida kefyr were administered into the rumen through fistula as a single dose 30 min after the morning meal. GOS and Candida kefyr were supplemented by sprinkling onto the feed and through rumen fistula, respectively. The four treatments consisted of saline, nitrate, nitrate plus GOS and nitrate plus GOS plus Candida kefyr. Physiological saline was used as the control treatment. Compared to saline treatment, the administration of nitrate alone resulted in a very marked decrease in rumen methanogenesis and an increase in rumen and plasma nitrite production and blood methaemoglobin formation consequently causing a decline in oxygen consumption, carbon dioxide production and metabolic rate. When compared to nitrate alone, the simultaneous administration of nitrate with GOS decreased nitrite accumulation in rumen and plasma and nitrate-induced methaemoglobin, while retaining low methane production. However, GOS could not fully restore metabolic parameters reduced by nitrate. When compared to the simultaneous administration of nitrate with GOS, the simultaneous administration of nitrate with GOS plus Candida kefyr lowered rumen methanogenesis to a negligible level, but did not decrease rumen and plasma nitrite accumulation as well as blood methaemoglobin formation. Thus, these results suggest that combination of nitrate with GOS may be a potent manipulator to suppress rumen methanogenesis with abating the hazards of nitratenitrite toxicity in ruminants.

Induced Autophagy Regulates Salmonella enterica serovar Typhimurium Infection in Murine Macrophage (쥐의 큰포식세포주에서 자가포식현상에 의한 Salmonella enterica serovar Typhimurium의 감염 조절)

  • Autophagy is one of the lysosomal degradation pathways to maintain cellular homeostasis. The damaged proteins or organelles are uptaken through extra- and intra-cellular stress, starvation and infected pathogens, subsequently, autophagosomes are fused with lysosomes to break down the molecules. Salmonella enterica serovar Typhimurium (S. Typhimurium), intracellular bacteria, cause acute gastroenteritis and food poisoning. Given that autophagy induced by S. Typhimurium plays an important role in the cells to control the infection, we identify whether the induction of autophagy with rapamycin, chemical inducer of autophagy, before infection regulates S. Typhimurium infection. After treatment of rapamycin or 3-methyladenine (3-MA), autophagy inhibitor, RAW264.7 cells were infected with S. Typhimurium. Pretretment of rapamycin decreased the growth rate of S. Typhimurium in the cells; otherwise, pretreatment of 3-MA increased the growth rate of S. Typhimurium. The expression of autophagy-related genes was significantly increased in the S. Typhimurium-infected cells pretreated with rapamycin. To examine whether induced autophagy by rapamycin control the infection with increase the production of reactive oxygen species (ROS) and nitric oxide (NO), antibacterial radical substrates were measured in infected cells followed by the treatment with either rapamycin or 3-MA. NO production increased in RAW264.7 cells; otherwise, ROS production remained unchanged during the infection. These findings suggest that inducing autophagy with rapamycin reveals antimicrobial activity as producing NO against S. Typhimurium infection in mouse macrophages.
