The application of fine needle aspiration (FNA) cytology to the soft tissue tumors had been neglected. In recent years, however, FNA has been used increasingly in the preoperative diagnosis of these tumors due to its usefulness and accuracy. We present 3 cases of liposarcoma, myxoid, myxoid with round cell, and pleomorphic, diagnosed by FNA cytology with histologic confirmation. Good correlation between histologic and FNA cytologic findings was found. Although the cytologic appearances of liposarcomas varied with histologic type, the main criterion was the presence of atypical multivacuolated lipoblast with characteristically scalloped nuclei.
Malignant fibrous histiocytomas (MFHs) is the most common type of soft tissue sarcoma in the old animal with a aggressiveness, a high local recurrence rate and significant metastatic rate, which associated with a poor prognosis. In most histologic and immunohistological studies, the tumor cells raised from a fibroblastic and/or myofibroblastic phenotype, presumably from undifferentiated mesenchymal cell origin. MFHs are usually firm and invasive, arising in the subcutis; metastasis depends on tumor grade (many are grade 3) [1,2]. The primary tumor cells are pleomorphic, varying in appearance from fusiform to round. Often nucleoli are prominent and irregular [5]. Extracellular amorphous eosinophilic material may be prominent and likely represents reactive collagen production by the tumor [5]. (omitted)
Objectives: Salivary gland tumors pose considerable difficulty in diagnostic and prognostic assessment based on the histopathologic features alone. We studied the expression of type IV collagen and fibronectin in salivary gland tumors with special emphasis on the differential diagnostic significance. Materials and Methods: We did immunohistochemical stain on paraffin embedded tissues of 33 benign and 24 malignant salivary gland tumors using monoclonal antibody for type IV collagen and polyclonal antibody for fibronectin. Results: 1) Well preserved linear basement membrane-like staining of type IV collagen was detected in duct-cell-derived benign salivary gland tumors. But pleomorphic adenoma exhibited a heterogeneous pattern as focal augmentation and interruption. 2) In malignant tumors, type IV collagen was distributed in an irregular, interrupted manner or completely absent. Adenoid cystic carcinomas displayed a marked staining of the basal membrane associated substances in the pseudocysts. 3) The staining pattern of fibronectin was similar to that of type IV collagen execpt more dense in the stroma. 4) Salivary gland tumors which have a prominent myoepithelial cell component revealed a particular deposition of basement membrane materials adjacent to the myoepithelial cells. Conclusion: The study of the basal membrane substances may be helpful for differential diagnosis of benign and malignant salivary gland tumors and identifying special features of salivary gland tumors such as pseudocystic pattern of adenoid cystic carcinoma. Also we think that the myoepithelial cells contribute to the formation of basement membrane materials.
Aim: To present the epidemiologic data (age, gender, size etc) and histopathologic and immunohistochemical features of cases of rhabdomyosarcoma (RMS) reported in our department. Settings: Section of Histopathology, Department of Pathology and Microbiology, Aga Khan University Hospital, Karachi, Pakistan. Duration: All cases of RMS diagnosed between January 1, 2003 and December 31, 2012 were included in the study. Results: A total of 277 cases were included. Embryonal rhabdomyosarcoma (ERMS) was by far the dominant histologic type (87.4%) followed by alveolar type (ARMS)(9.4%). ERMS was much more common in males (64.0%)and over 65% of cases occurred in the first decade of life (over 90% in the first two decades). Head and neck region was commonest site for ERMS (46.7%), followed by the genitourinary system (16.1%). Over 65% cases of ARMS occurred in the extremities. Over 80% cases of ARMS occurred in the first 3 decades of life. Immunohistochemical staining for Desmin and MyoD1 was positive in 96.7% and 85.4% respectively. Conclusions: The epidemiologic data and microscopic findings in our patients are similar to international published data on rhabdomyosarcoma.
Primary malignant lymphoma of the thyroid gland is uncommon malignancies. Its fine needle aspiration cytology (FNAC) findings are rarely described in the literature. This article highlights the FNAC diagnosis of primary malignant lymphoma of the thyroid gland. A 70-year-old female presented with a rapidly enlarging thyroid mass of five months' duration. FNAC smears showed low cellularity consisting of predominantly atypical enlarged lymphoid cells admixed with a few small lymphocytes, plasma cells, and oncocytic cells. Some disrupted lymphoid cells were also present. The tumor cells infiltrated into the thyroid follicular epithelium forming lymphoepithelial lesion, The cytologic appearance showed a diffuse mixture of cell types with only a few small, mature lymphocytes and many enlaraed lymphoid cells. The enlarged lymphoid cells were atypical and pleomorphic with nuclear clefting and irregularities. Grossly, the left lobe of the thyroid was nearly replaced by a diffuse firm to soft solid mass with smooth tan fish-flesh homogeneous cut surface. Histological diagnosis was diffuse large B-cell lymphoma with areas of marginal zone B-cell lymphoma of MALT type.
Anaplastic large cell lymphoma(ALCL) is an uncommon type of non-Hodgkin's lymphoma(NHL) populated with anaplastic, often bizarre cells that express CD30 (Ki-1) antigen. The unusual histologic and cytologic features may cause confusion with other neoplasms, such as poorly differentiated carcinoma, melanoma, Hodgkin's disease, or true histiocytic lymphoma. Although the cytologic features of ALCL have been well described, there are few reports about cytologic findings of the sarcomatold variant of ALCL. We experienced a case of fine needle aspiration(FNA) cytologic findings of ALCL which mimicks malignant fibrous histiocytoma. FNA cytology of chest wall mass in a 62-year-old female with a history of peripheral T-cell lymphoma(Lennert lymphoma) revealed a heterogeneous population of single cells and poorly cohesive cells with large, pleomorphic nuclei and spindle cells gathering around vascular structures within an inflammatory background. Additional features of the neoplastic cells were eccentric, multilobated nuclei with occasional 'wreath-like' configuration; abundant cytoplasm with vacuolization; and prominent nucleoli. The cytologic features suggested sarcoma, especially malignant fibrous histiocytoma. The diagnosis was made retrospectively with an aid of immunocytochemical staining.
A 17-year-old female tiger (Panthera tigris altica) was found dead after suffering from continuously growing mass at the right mammary gland area. At necropsy, a firm tan mass approximately 25 cm in diameter was noted at the ventral abdomen. The mass was not fixed to the adjacent tissue and mottled tan to red on cut sections. Chains of similar nodules ranging from 2 to 5 cm in diameter were also present along the right mammary glands region. Histologically, the neoplastic masses consisted of lobules that were filled with pleomorphic neoplastic cells and separated by fibrious conntective tissue. The neoplastic cells have hyperchromatic nuclei with prominent nucleolus and moderate amount of cytoplasm. The degree of mitosis was high. Multiple areas of necrosis, hemorrhage, mineralization and tumor emboli were also noted. Metastasis to the regional lymph nodes, lung, liver, spleen, kidney, and adrenal gland were observed. Based on the gross and histopathologic examinations, a diagnosis of lobular type metastatic mammary gland carcinoma was made.
Background: Salivary gland tumors (SGT) are one of the most complex human neoplasms, demonstrating variations in their clinicopathological profile related to racial and geographic differences. Few studies with large samples have been reported in Iran. We here investigated a large group of patients in southern Iran. Materials and Methods: In this retrospective study, all cases of primary epithelial salivary gland tumors, which had been recorded in a 5 years period from 2005-2009, were enrolled. Clinical data such as histopathologic type and site of the lesion as well as patients' age and gender were analyzed. Results: Data of 366 cases of SGTs were recorded. Pleomorphic adenoma (80.2%) and adenoid cystic carcinoma (46.6%) were the most common benign and malignant neoplasms. Male to female ratio (M/F) and the mean age of patients were 1:1.05 and 37.7 for benign tumors while they were 1.2:1 and 50.6 for malignant tumors, respectively. Parotid and minor salivary glands were involved more frequently. Conclusions: Although the salivary gland tumours encountered were similar in most of their characteristics to those reported in other countries, some differences such as relative frequency and age and gender prevalence were discovered. These findings should help surgeons and pathologists for more accurate diagnosis, management and treatment.
Purpose: Myoepithelioma is a rare tumor that originates exclusively from myoepithelial cells of the salivary glands, breast and the prostate. Myoepithelioma accounts for less than 1% of all salivary gland tumors. The objective of our study is to present our experience of the infra-auricular mass which was finally diagnosed as a myoepithelioma. Methods: A 54-year-old woman was presented with a firm, movable, slow-growing infra-auricular mass with 3 cm in diameter. MRI scans and fine needle biopsy was performed for preoperative diagnostic study. A superficial parotidectomy was preceded and removed tumor successfully. Results: Histopathological study revealed a myoepithelioma of plasmacytoid type. Patient's postoperative course was uneventful without any complication and had no evidence of recurrence of tumor for 9 months follow-up period. Conclusion: Myoepithelioma in the parotid gland shows similar clinical courses and intraoperative finding to the pleomorphic adenoma and superficial parotidectomy was selective choice for treatment.
본 연구는 구강내 발생하는 소타액선 종양의 발생 빈도와 조직병리학적 특성에 대한 후향적 연구로서 한국인에서 나타나는 소타액선 종양의 특성을 연구하고자 1990년부터 2006년 8월까지 연세대학교 치과대학 부속병원 구강악안면외과, 연세대학교 의과대학 부속 영동 세브란스병원, 순천향대학교 부속 부천병원에 내원한 200명의 소타액선 종양 환자를 대상으로 치과 및 의과 임상 기록지 검토와 H/E 슬라이드를 재검토하여 다음과 같은 결론을 얻었다. 1. 200명의 환자 중 양성 종양은 123예(61.5%), 악성종양은 77예(38.5%)였으며 남성에서는 87예, 여성에서는 113예였다. 2. 가장 흔하게 발생한 소타액선 종양은 다형성 선종이며 선양 낭성 암종, 점액표피암종, 와틴씨 종양, 선암종과 다형성 선종에서 발생한 악성 종양, 림프관종 등의 순이었다. 3. 호발 부위는 경구개(42.5%), 연구개(9%), 협점막(6%), 입술(3.5%) 부위였다. 4. 발생 연령은 4세부터 70세까지 다양하게 분포하였으며 50대에서 가장 많았다. 양성 종양의 발생 평균 연령은 46.2세였으며 악성 종양의 발생 평균 연령이 56.1세였다. 5. 다형성 선종은 가장 호발한 소타액선 종양으로서 모두 104예였으며 남성 38명, 여성 66명에서 발생하였다. 평균 발생 연령은 46.7세였으며 구개 부위에서 가장 많이 발생하였다. 조직학적 소견은 관 구조 형성, 별 모양, 다각형, 방추형, 형질세포양(plasmacytoid), 연골양, 점액양 구조를 보였으며 불완전한 피막과 종양 피막을 뚫고 다발성으로 발생하는 소견이 관찰되었다. 6. 선양 낭성 암종은 가장 호발한 악성 타액선 종양으로 모두 32예였으며 남성 16명, 여성 16명에서 발생하였다. 평균 발생 연령은 57.4세였으며 경구개 부위에서 가장 많이 발생하였다. 조직학적 소견은 cribriform pattern, 관상 구조, 판상 구조 형성, 종양 세포의 신경 주위 침습 등을 보였다. 7. 점액표피암종은 모두 25예였으며 남성 10명, 여성 15명에서 발생하였다. 평균 발생 연령은 50.0세였으며 경구개 부위에서 가장 많이 발생하였다. 조직학적 소견은 점액 세포, epidermoid-type 세포, intermediate cell의 분포가 대부분 저등급을 보였다. 이외에도 와틴씨 종양, 선암종과 다형성 선종에서 발생한 악성종양, 림프관종 등이 발생하였다. 8. 한국인의 소타액선 종양의 발생 빈도는 양성 종양이 높으며 특히 다형성 선종이 많았고 여성 발생, 구개 부위 발생이 많았으며 다형성 저등급 선암종 등 특정 소타액선 종양의 발생이 극히 적었다.
