HEPATOTOXICITY OF PHOTOTOXIC DRUGS USED IN SYSTEMIC PHOTOCHEMOTHERAPY

  • Psoralens are metabolized in the liver and thus can produce liver damage in laboratory animals when given in excessive doses. However, in humans, reports of the effects of psoralens on liver functions during photochemotherapy have been contradictory. We studied 311 patients t o observe the effects of various phototoxic drugs: 8-methoxypsoralen (8-MOP), 5-methoxypsoralen (5-MOP), 4,5,8-trimethylpsoralen (TMP)-on liver function during photochemotherapy. Of the 311 patients, only seven had transient elevations of transaminases. Incidence of hepatotoxicity of 8-MOP, TMP and 5-MOP showed 0.6%, 6% and 3%, respectively. Although the effects of psoralens on liver functions during photochemotherapy have been contradictory in humans and there have been few cases showing transient elevated values of liver transaminases during photochemotherapy, it is advisable to obtain serial liver function tests before and during photochemotherapy.

Treatment Result of Photochemotherapy for Systemic Psoriasis Patients (전신성 건선환자의 광선치료 후의 임상적 특성에 대한 고찰)

  • Photochemotherapy(UVA with 8-methoxypsoralen) was given to 30 patients with systemic psoriasis. The results of clearing and long-term(6-month) interval maintenance were reported. Clearing requirements were in general similar to these reported by Melski and Burger. The skin of the 14 patients (46.6%) recovered good skin condition by a once weekly maintenance dose. This result was better than that reported by other authors. 1. During initial treatment period, average number of treatment was 27.3 and average duration treatment was 24.8 weeks. 2. The factors to quit treatment were motion decrease, moving to the remote area, complications, etc. 3. Number of patient who received maintenance treatment was 14. 4. The complications reported from the patients were hyperpigmentation, nausea, headache, pruritis, vomiting, gastritis.

  • INTRODUCTION : Psoralens are planar tricyclic furocoumarins present in numerous plants and fungi found throughout the world.' Naturally occurring and synthesized psoralen derivatives(see Figure 1) are photosensitizers of UVA especially from 320 nm to 400 nm, a range at which cellular nucleic acids and proteins are weakly absorbing if any at all. Because of their skinphotosensitizing properties, these compounds have been used in the photochemotherapy of psoriasis and vitiligo. However, undesirable side effects such as carcinoma development in hairless mice as well as possible liver damage from the use of 8-methoxypsoralen(8-MOP) have been reported. The other photobiological effects include inactivation of DNA viruses, killing and mutagenesis of bacteria, inhibition of tumor transmitting capacity of various cells, and hyperpigmentation on human and guinea pig skin. PUVA(psoralen+UVA) photochemotherapy is in fact thousands of years old, having been used in Egypt and India since B.C. 1200-2000. Photochemotherapy for a common disfiguring disease, vitiligo, was practiced in the ancient world by physicians and herbalists who used boiled extracts of the fruits of certain umbelliferous plants, e.g. Ammi majus Linnaeus in Egypt or the leguminous plants, Psoralea corylifolia L. in India. It was first described by Kuske in 19388 that photosensitization of skin by plants was related to the presence of psoralen. He identified natural psoralens in plants as photosensitizers and isolated bergapten(5methoxypsoralen) from the oil of bergamot. Optimum Treatment Parameters for Photodynamic Antimicrobial Chemotherapy on Streptococcus mutans Biofilms (Streptococcus mutans biofilm에 대한 광역동 치료의 최적조건에 관한 연구)

  • The purpose of this study was to evaluate the effects of Photochemotherapy using a combination of erythrosine and standard halogen dental curing lights on the viability of Streptococcus mutans in the biofilm phase. To investigate the optimum treatment parameters, the researchers controlled the concentration of erythrosine, light irradiation time and the treatment time of erythrosine. The higher concentration of erythrosine (0, 10, 20, 40, 80 M) in the presence of light irradiation created greater effects in reducing the viability of S. mutans. The results showed a statistically significant difference among the antimicrobial effects in 20, 40, 80 M erythrosine. The higher irradiation time of light (0, 5, 15, 30, 60, 75s) in the presence of erythrosine showed greater effects in reducing the viability of S. mutans. There was statistically significant difference in 30, 60, 75 seconds. The higher treatment time of erythrosine (0, 1, 2.5, 5min) in the presence of erythrosine created greater effects on reduction of S. mutans viability. Statistically significant differences were found between 2.5 and 5 minutes of erythrosine treatment time. The results of this study showed that the photochemotherapy on S. mutans using erythrosine and the halogen dental curing lights conventionally used in dental clinics is effective in the condition of 20-40 M erythrosine concentration, irradiation time over 30 seconds, and erythrosine treatment time over 2.5 minutes.

Comparison of Photophysical and Photochemical Properties of Khellin and 8-Methoxypsoralen

  • The photophysical and photochemical properties of khellin were compared with those of 8-methoxypsoralen (8-MOP). Quantum yields of fluorescence and triplet formation decreases as solvent polarity increases, which is opposite to 8-MOP, and photocycloadditivity of khellin to olefins is much lower than that of 8-MOP. Electron ejection from khellin by laser flash was not observed, but observed from 8-MOP. As models of 4',5'-monoadducts of khellin or 8-MOP with thymine base, khellin<>dimethylfumarate 4',5'-monoadduct (KDF) was also compared with 8-MOP<>thymidine 4',5'-monoadduct (F-2) in those properties to give some insight on the second-step biadduct formation resulting in cross-links of DNA duplex. KDF and F-2 were very similar to khellin and 8-MOP in photophysical properties, respectively. However, KDF did not form adducts with various olefins, and thus it is thought that 2,3-double bond of chromone moiety in khellin is hardly reactive in contrast with 3,4-double bond of coumarin moiety in 8-MOP. These results indicate that khellin is fairly photostable compound, a poor type Ⅰ photodynamic sensitizer and producer of ${O_2}^{-}$ which is some cause of phototoxic erythemal reactions and undesirable side effects. Therefore khellin is safer to use than 8-MOP in photochemotherapy of some skin diseases. Although khellin is much less reactive than 8-MOP, khellin must be also a monofunctional drug. Since khellin is, however, as effective as 8-MOP in photochemotherapy of some skin diseases, it is suggested that khellin may be different from 8-MOP in the action mechanism.

  • Pulmonary carcinoids are relatively rare tumors with low metastatic potential. Pleural carcinomatosis of a bronchial carcinoid has only been reported in 4 cases. Due to the rarity of this condition, there are no guidelines for its treatment or management. We report a case of atypical carcinoid with local recurrence and pleural metastases treated by video-assisted thoracoscopic surgery lobectomy and total pleurectomy with photodynamic therapy after non-radical wedge resection.

A case of scalp folliculitis treated by Korean Medicine with photodynamic therapy(PDT) (광역동요법을 병행한 두피 모낭염 한방치험 1례)

  • Objectives : The purpose of this study is to report the effect of combination therapy of photodynamic therapy (PDT) with Korean medical treatments on scalp folliculitis, which has been difficult to treat by using antibiotic.Methods : A 32-year-old male patient with scalp folliculitis, received treatment with acupuncture, herbal medicine and PDT and so on. We used Visual Anologue Scale(VAS) to evaluate recovery from erythema, pruritus and pain. Also, the photos of patient were taken before and after treatments.Results : Visible symptoms to the naked eye and VAS scale effectively decreased after Korean medicine treatments for two months.Conclusions : It is expected that Korean medical treatments and PDT were thouht to be effective for scalp folliculitis.

  • Photodynamic therapy (PDT) is a local, endoscopically controlled nonoperative therapeutic technique based on selective sensitization of mucosal, malignant and precancerous lesions of the esophagus, trachea and bronchus prior to light-induced tissue destruction in the department of thoracic and cardiovascular surgery. PDT is effective and safe for palliative treatment of neoplasms in the stomach, esophagus, and lung. But skin phototoxicity is unsatisfactory, therefore optimization of management of post-PDT is necessary for preventing phototoxic side effects of skin. Careful patient education in photoprotection techniques, close patient follow-up, early dermatologic referral and medical treatment are recommended. We performed PDT in a patient with intrathoracic constructed stomach. We report this case with a brief review of literatures, therefore.

Efficacy of an LED toothbrush on a Porphyromonas gingivalis biofilm on a sandblasted and acid-etched titanium surface: an in vitro study

  • Purpose: The aim of this study was to evaluate the antimicrobial effect of a newly devised toothbrush with light-emitting diodes (LEDs) on Porphyromonas gingivalis attached to sandblasted and acid-etched titanium surfaces. Methods: The study included a control group, a commercial photodynamic therapy (PDT) group, and 3 test groups (B, BL, and BLE). The disks in the PDT group were placed in methylene blue and then irradiated with a diode laser. The B disks were only brushed, the BL disks were brushed with an LED toothbrush, and the BLE disks were placed into erythrosine and then brushed with an LED toothbrush. After the different treatments, bacteria were detached from the disks and spread on selective agar. The number of viable bacteria and percentage of bacterial reduction were determined from colony counts. Scanning electron microscopy was performed to visualize bacterial alterations. Results: The number of viable bacteria in the BLE group was significantly lower than that in the other groups (P<0.05). Scanning electron microscopy showed that bacterial cell walls were intact in the control and B groups, but changed after commercial PDT and LED exposure. Conclusions: The findings suggest that an LED toothbrush with erythrosine treatment was more effective than a commercial PDT kit in reducing the number of P. gingivalis cells attached to surface-modified titanium in vitro.

Benzochloroporphyrin Derivative Induced Cytotoxicity and Inhibition of Tumor Recurrence During Photodynamic Therapy for Osteosarcoma

  • Photodynamic therapy (PDT) is a promising cancer treatment modality that uses dye-sensitized photooxidation of biologic matter in target tissue. This study explored effects of the photosensitizer BCPD-17 during PDT for osteosarcoma. LM-8 osteosarcoma cells were treated with BCPD-17 and cell viability after laser irradiation was assessed in vitro with the 3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyltetrazolium bromide assay. The effects of BCPD-17 during PDT recurrence were then examined on tumor-bearing mice in vivo. BCPD-17 had dosedependent cytotoxic effects on LM-8 osteosarcoma cells after laser irradiation which also had energy-dependent effects on the cells. The rate of local recurrence was reduced when marginal resection of mice tumors was followed by BCPD-17-mediated PDT. Our results indicated BCPD-17-mediated PDT in combination with marginal resection of tumors is a potentially new effective treatment for osteosarcoma.
