A STUDY FOR THE DEVELOPMENT OF KOREAN CHILD REARING INVENTORY FOR SOCIAL PHOBIA (사회공포증과 관련된 양육태도검사의 개발 연구)

  • The purpose of this study was to develop a 'Korean Child Rearing Attitudes Inventory', in order to understand the effects of parents rearing attitudes on social phobia. The total subjects were 392 high school students in Seoul. The scales wwereadministered to the subjects and we examined the reliability and validity. The results were as follows. According to the factor analysis, six factors abstracted from the father's rearing attitudes:rejection, high expectation, anxious, control, emotional care and overconcern of other's opinion. By the method of reliability analysis and factor analysis, finally selected items were 60. The reliability of the factors ranged from .76 to .90 by Cronbach ${\alpha}$. Five factors abstracted from the mother's rearing attitudes:anxious, high expectation, rejection, emotional care and overconcern of other's opinion. And total selected items of mother's rearing attitudes were 61. The reliability of the factors ranged from .72 to .91 by Cronbach ${\alpha}$. The construct validity and reliability of this inventory were supported.

The Impacts of Adolescent's Oiettolie Propensity on Game Addiction Behavior : Focusing on Moderating Effects of Family Stress (청소년의 은둔형 외톨이 성향이 게임중독행태에 미치는 영향 : 가족스트레스의 조절효과분석을 중심으로)

  • The purpose of this study is to suggest some policy implications for the youth through empirical study about the moderating effects of high school students' family stress in the relationship of Oiettolie propensity and game addiction behaviors including bullies, social phobia and emotional instability. For the purpose of this study, this study established a causal model about the relationships among the research variables through review of previous studies and surveyed high school students for empirical research. The results of this study shows that Oiettolie propensity impacts on youth's game addiction and brings about game addiction behaviors like bullies, social phobia and emotional instability. Especially, this study shows that Oiettolie propensity impacts on social phobia most. Also, family stress has a moderating effects for the relationships among the variables. This study presents some policy implications for the youth.

6-months Prospective Follow-up Study of Panic Disorder Treatment (공황장애 환자 치료 결과에 대한 6개월간의 전향적 추적조사)

  • Objectives : It is well known that pharmacologic and behavioral therapies of panic disorder show remarkable acute treatment outcome, however the course of panic disorder in clinical settings is often chronic and relapsing. The purpose of this study is to investigate the treatment outcome of panic disorder and the factors related to good treatment outcome by prospective follow-up study after 6 month in clinical settings. Methods : Twenty nine patients were diagnosed to have panic disorder by SCID(Structured Clinical Interview for DSM-III-R), among the patients who had visited the psychiatric out-patients clinic of the Asan Medical Center for the first time. We determined the initial clinical and demoraphic features of each patient and reevaluated them after 6 months, investigating the treatment outcome by anxiety, phobia, impairment scales. We looked into the rate of the patients who showed good treatment outcome and determined the factors that had relation with good treatment outcome among demographic and clinical features. Results : Nineteen out of 29 patients could be followed after 6 months. Among them, 10 patients 52.6%) on the impairment scale and phobia scale each, and 8 patients(42.1 %) on the anxiety scale showed good treatment outcome. 8 patients(42.1 %) showed good treatment outcome on the all three scales. High score in initial phobia scale had significant relation with good treatment outcome. Short duration of illness did not have significant relation with good treatment outcome however there was a trend(p=0.07). Conclusion : About half(42.1 %) of the panic disorder patients showed good treatment outcome on all three scales. Severe initial phobic symptom and short duration of illness were expected to have relation with good treatment outcome.

Fever Phobia: A Survey of Children's Parents in a Pediatric Outpatient Clinic (소아청소년과 외래를 방문한 부모들의 발열공포와 관련 요인)

  • Purpose: Purposes of this study were to identify the level of parental fever phobia and to investigate the relationship between level of parental concern about fever and related variables. Methods: Participants were 151 parents of children who visited a pediatric outpatient clinic. A selfreported structured questionnaire was used for data collection and data were analyzed using descriptive statistics and ${\chi}^2$-test. Results: Almost half of participants defined a minimum temperature for fever as $37.8^{\circ}C$ and a minimum temperature for high fever as $38.9^{\circ}C$. About 75% of participants identified harmful effects of fever as seizure and brain damage, were 'very worried' about fever, measured their child's temperature every hour or less, provided tepid massage and woke children to give antipyretics during febrile illness. There were significant relationships between level of parental concerns about fever and prior experience of febrile seizures, and/or being parents of a single child. Conclusion: Results indicate that fever phobia is prevalent among parents. Further studies are needed to develop and evaluate childhood fever management educational programs for parents. Considering health care providers as a primary information resource about fever management, health care providers should play a vital role to reduce parental unrealistic concerns about fever.

A Study on the Phobia Treatment Using 3D Virtual Reality System (3D 가상환경시스템 이용한 공포증 치료에 대한 연구)

  • Virtual Reality(VR) is a new technology which makes humans communicate with computer. It allows the user to see, hear, feel and interact in a three-dimensional virtual world created graphically. In this paper, we introduced VR into psychotherapy area and developed VR system for the exposure therapy of acrophobia. Acrophobia is an abnormal fear of heights. Medications or cognitive-behavior methods have been mainly used as a treatment. Lately the virtual reality technology has been applied to that kind of anxiety disorders. A virtual environment provides patient with stimuli which arouses phobia, and exposing to that environment makes him having ability to over come the fear. In this study, the elevator stimulator that composed with a position sensor, head mount display, and audio system, is suggested. To illustrate the physiological difference between a person who has a feel of phobia and without phobia, heart rate was measured during experiment. And also measured a person's HR after the virtual reality training. In this study, we demonstrated the subjective effectiveness of virtual reality psychotherapy through the clinical experiment.

EEG-based Analysis of Auditory Stimulations Generated from Watching Disgust-Eliciting Videos (혐오 영상 시청시 청각적 자극에 대한 EEG 기반의 분석)

  • In this paper, we present electroencephalography (EEG)-based power spectra analysis and auditory stimuli methods as coping mechanisms for disgust affection and phobia. Disgust affection is a negative emotion generated from trying to eliminate something harmful to one. It is usually related to mental illnesses such as obsessive-compulsive disorder, specifically phobia and depression. In our experiments, participants watched videos on horrible body mutilation and disgusting creatures, with either the original sound track or relaxing and exciting music as auditory stimulation. After watching the videos with original sound track, the participants watched the same video with a different audio background, such as soothing or cheerful music. We analyzed the EEG data utilizing relative power spectra and examined survey results of the participants. The results demonstrated that disgust affection is decreased when participants watched the video with relaxing or exciting music instead of the original soundtracks. Moreover, we confirmed that human's brainwave reacts according to types of audio and sources of disgust affection.

  • Apprehension and phobia regarding dental procedures are represent the most common deterrents in patients seeking dental care and very common. For these individuals, and others who cannot cooperate during care, procedural sedation may permit completion of intraoral procedures. In most cases, the level of sedation may be kept at minimal to moderate levels permitting patient maintenance of their airway patency and ventilation. Unlike many medical procedures, the majority of dental procedures, no matter the depth of sedation, are performed in the presence of complete analgesia provided by local anesthesia. Therefore, the goal of procedural sedation is to primarily suppress patient fear and apprehension and gain cooperation. Any issues regarding actual pain are usually limited to that produced by the local anesthetic injections or, rarely, the extent of the procedure. For the extremely phobic patient, however, allaying apprehension may be very challenging. Intravenous titration of sedative drugs is the most effective route of administration to achieve this goal but requires advanced training beyond that provided in undergraduate training.

Dental Treatment of Pediatric Patients with Mouth Opening Limitation under General Anesthesia (개구 장애가 심한 소아환자에서 전신마취 하 치과치료를 위한 기도관리)

  • For dental treatment of children with severe dental phobia, sedation or general anesthesia is usually selected for enhancement of cooperation. But in the case of mouth opening limitation due to temporomandibular disorders, general anesthesia administration is a challenge for anesthesiologist. Because airway management failure was concerned, awake fibroscopic intubation is selected first. But, skillful fibroscopic intubation is not easy in case of uncooprative children patients. In this report, we present two cases of pediatric patients with mouth opening limitation. In the first case, the patient was 52 months old and the maximum opening distance was 1.2 cm, and in the second case the patient was 38 months old and the maximum opening distance was 1.5 cm. Both patients showed severe dental phobia. After sevoflurane inhalation without any intravenous drug, we successfully performed intubation using a fibroscope.

A Bibliographic Study of Anxiety and Anxiety Disorder in Oriental Medicinee (불안(不安) 및 불안병증(不安病症)에 대한 한의학적 연구)

  • Anxiety is essentially a normal emotion with great adaptive significance like pain. to ease human suffering, treatment is sought to quell pain and anxiety only when they are excessive or when they interfere with performance or general health. In oriental medicine, anxiety has been also known as a important factor which is cause of body and mind disease. We obtained some points after studying of bibliographic research about anxiety and anxiety disorder like follows: 1. The signification of anxiety is similar to fear affection(恐情) and anxious feelings are fearful, long for solitary life and afraid of captivity. 2. Mechanism of anxiety disorder is related with kidney(腎) as well as heart(心), liver(肝), gall bladder(膽) and stomach(胃). 3. The prescription of fear affection is tonifying of kidney(益腎) and fixing of kidney's pure substance(固精) that is based on deficiency of kidney(賢氣不足) 4. A representative disorder about anxiety and phobia are Keongke(驚悸) and Cheongchung(정충). 5. Keongke(驚悸) and Cheongchung(정충) may be understood as phobias(恐佈症), general anxiety disorder(凡不安障碍) and panic disorder(恐慌障碍).

  • Posttraumatic stress disorder(PTSD) is a psychiatric illness that results from exposure to serious threats of injury or death. In addition, dental phobia, which is suggested as a sub-threshold form of PTSD, may result from aversive or painful dental treatment. Patients with PTSD may present greater dental and behavioral challenges than other patients due to their mental illness, neglect of oral hygiene, and their medication. Dentists have to consider the challenges to provide these patients with proper dental treatment.
