Hominis Placental Pharmacoacupuncture, a Promising Asthma Therapy (자하거(紫河車) 약침의 기관지 천식 치료 가능성에 대한 소고(小考))

  • Objectives : The aim of this study was to find out some rationales for using Hominis placental extract(HPE) pharmaco-acupuncture on asthmatic patients, and to suggest an appropriate clinical research of HPE pharmaco-acupuncture on asthma. Methods : Articles regarding immunologic experiments about HPE and clinical reports about HPE-contained pharmaco-acupuncture on asthmatic patients were reviewed by on-line and off-line searching. Results and Conclusions : HPE is suggested to have several pharmacological effects on allergic diseases including asthma as well as autoimmune diseases. There are some observational studies of HPE-contained pharmaco-acupuncture on asthmatic patients. However, the designs of these studies are not sufficient to investigate the exact effect of HPE pharmaco-acupuncture on asthma. To investigate HPE pharmaco- acupuncture as a novel asthma therapy, it will be needed to perform further experiments about HPE's pharmacological mechanism on asthma and to try further advanced clinical researches such as well-designed randomized controlled trial of HPE pharmaco-acupuncture.

Effects of Curculiginis Rhizoma Pharmaco-Acupuncture at $ST_{36}$ on Monosodium Iodoacetate(MIA)-induced Osteoarthritic Rats (족삼리의 선모(仙茅)약침이 Monosodium Iodoacetate로 유발한 골관절염 흰쥐에 미치는 영향)

  • Objectives : This study was designed to investigate effects of Curculiginis Rhizoma pharmaco-acupuncture at $ST_{36}$ on monosodium iodoacetate-induced osteoarthritic rats. Methods : Twenty rats were divided into four groups consisting of 5 rats: rats receiving no injection(normal), rats injected with monosodium Iodoacetate(MIA, control), rats injected with MIA and normal saline(N-S), and rats injected with MIA and Curculiginis Rhizoma (CRPA). N-S and CRPA were administered once a day at $ST_{36}$ during 21 days. After that we examined the weight-bearing ability of hind paws, liver and kidney function, immunocell, cytokines, proteins, and gene expression of cytokines. Injury of synovial tissue was measured by H & E, Safranin O immunofluorescence. Results : The weight-bearing ability of the hind paws, Serum TNF-${\alpha}$, IL-$1{\beta}$, IL-6, PGE2, LTB4, DPD, Osteocalcin, Protein COX-2 of CRPA decreased significantly. Protein Arachidonate 5 lipoxygenase of CRPA was decreased, but not significantly. Expression of gene COX-2, TNF-${\alpha}$, IL-$1{\beta}$, IL-6, NOS2 of CRPA decreased. In histological observations, CRPA was improved, compared with other control groups. Conclusions : It can be suggested that Curculiginis Rhizoma pharmaco-acupuncture at $ST_{36}$ has anti-inflammatory and pain relief effects on monosodium iodoacetate-induced osteoarthritic rats.

Effects of Yanghyuljanggeungunbo-tang(Yangxuezhuangjinjianbu-tang) and Cervi Cornu Parvum Pharmaco-Acupuncture on the Motor Function Recovery and Nerve Regeneration in Rats Induced Spinal Cord Injury (양혈장근건보탕(養血壯筋健步湯)과 녹용약침(鹿茸藥鍼)이 척수손상 유발 흰쥐의 운동기능 회복 및 신경재생에 미치는 영향)

  • Objectives : The purpose of this study was to investigate the effects of Yanghyuljanggeungunbo-tang(Yangxuezhuangjinjianbu-tang) and Cervi Cornu Parvum pharmaco-acupuncture in Spinal Cord Injury(SCI)-induced rats. Methods : The subjects were divided into 5 groups ; Normal, Control no treatment after SCI, Experimental I taken with Yanghyuljanggeungunbo-tang (Yangxuezhuangjinjianbu-tang) 500 mg/kg $0.5m{\ell}$ daily after inducing SCI. Experimental II taken with Cervi Cornu Parvum pharmaco-acupuncture at Taegye(KI3) and $Yangnungch{\acute{o}}n$(GB34) after inducing SCI and Experimental III taken with Yanghyuljanggeungunbo-tang(Yangxuezhuangjinjianbu-tang) 500 mg/kg $0.5m{\ell}$ and Cervi Cornu Parvum pharmaco-acupuncture at KI3 and GB34 to SCI-induced rats. After each operation, the present author observed the motor behavior recovery and nerve regeneration by analysis of the motor behavior tests, EMG, hematological(AST, ALT, WBC), histological and immunological changes. Rats were tested at modified Tarlov test at the 1st, 2nd, 3rd, 4th day, and Motor behavior test at 1st, 3rd, 7th, 14th, 21st day. Results : Results are as follows. 1. All the experimental groups were improved compared with control group in the motor behavior tests including Tarlov test, Basso-Beattle-Bresnahan locomotor rating scale, modified inclined plane test, open field test, grid walk test and narrow beam test. Especially Experimental III was improved significantly among other groups. 2. In EMG test, H wave appeared weak only in Experimental III. And M wave was increased significantly in Experimental III. 3. All the experimental groups were significantly decreased compared with control group in serum AST, serum ALT and serum WBC tests. 4. significantly decreased in Tumor Necrosis Factor-${\alpha}$ test compared with the first day of SCI. 5. Muscle contraction and denaturation of all the experimental groups were inhibited in histological observations of gastrocnemius muscle. Especially, those of experimental III was more effective. 6. NGF and BDNF of spinal cord gray matter in all the experimental groups were increased compared with control group. Especially, those of experimental III was more effective. Conclusions : As above, it can be suggested that Yanghyuljanggeungunbo-tang(Yangxuezhuangjinjianbu-tang) and Cervi Cornu Parvum pharmaco-acupuncture may improve motor behavior, EMG, hematological, histological and immunological findings in Spinal Cord Injury(SCI)-induced rats. Especially, effects will be somewhat better in combination of these two treatments.

A Case Report : Low Back Pain with Achondroplasia Treated with Conservative Treatment (증례보고 : 보존적 치료를 통한 연골무형성증을 가진 요통 환자)

  • Objectives : The purpose of this study is to investigate the efficacy of conservative treatment on Achondroplasia patient who has low back pain with radiation. Methods : The patient was treated by conservative treatment including acupuncture, pharmaco acupuncture, chu-na manipulation, herbal medicine. Numeric rating scale(NRS), Oswestry disability index(ODI) were used to evaluate pain reduction. Physical examination also used to compare with admission state and discharge state. Results : After 1 month admission treatment, NRS decreased 9 to 6, ODI dcreased 56 to 42, ROM improved as normal person. Conclusions : It was efficient that Achondroplasia patient who has low back pain with radiation treated as conservative treatment including acupuncture, pharmaco acupuncture, chu-na manipulation, herbal medicine. But It was just one case and treatments were carried out in Integrative Package. So further studies are required to compare that which treatment is efficient.

A Case of Torticollis with Korean Medicine (사경증 환자의 한방 치험 1례)

  • ■ Objectives This clinical report is to report the effect of Korean medicine on a patient with torticollis. ■ Methods We used herbal medicine, acupuncture, moxibustion, pharmaco-acupuncture and bee venom acupuncture to treat a torticollis patient during hospitalization and out patient department (OPD) treatment for 3months. We observed the changes of symptoms using Toronto Western Spasmodic torticollis Scale (TWSTRS). ■ Results After treatment, the patient's symptoms including torticollis, pain and the general condition were improved. The TWSTRS scores were also improved after treatment. ■ Conclusion This clinical case study suggests that Korean medicine treatment could be effective in the treatment of torticollis.

A Clinical Report of Systemic Lupus Erythematosus (전신성 홍반성 낭창(SLE) 환자 1례 증례보고)

  • Objectives: The purpose of this study is to report the effect of Korean medicine treatments on a systemic lupus erythematosus (SLE). Methods: We treated 1 case of systemic lupus erythematosus patient with Korean herbal medicine, acupuncture, moxibustion and pharmaco-acupuncture. Patient is a 44-year-old woman, who suffered from general body weakness, arthralgia and foamy urine. She was treated with Samilshinki-hwan-gami and Oryeong-san-gami. Patient is taken acupuncture at GV20, CV12, LI4, LR3, ST36, BL23, BL25 and moxibustion at CV4. We injected pharmaco-acupuncture at BL23, BL25. Results: After treatments the patient recovered from foamy urine, reduced general body weakness, arthralgia. Conclusions: This study suggests that Korean medicine treatments have significant effect on reducing symptoms of systemic lupus erythematosus.

Study of Clinical Research Acupuncture Treatment on Post-stroke Depression (뇌졸중후 우울증 침치료의 임상연구에 대한 고찰)

  • The aim of this study is to collect and analyze clinical studies on acupuncture therapies of Post-stroke depression, and to suggest a method on study about acupuncture therapies. In this thesis, electronic searches were performed. Also, 6 major journals associated with oriental medicine in Korea were searched manually. Through searching, 27 studies were selected and analyzed. There were 24 controlled studies and 3 non-controlled studies. Treatment methods were acupuncture, electronic-acupuncture, eye-acupuncture, abdominal-acupuncture and pharmaco-puncture. The percentages of these treatment method were 59.2%, 29.6%, 3.7%, 3.7% and 3.7%, respectively. Acupuncture points such as GV20, PC6, ST36, SP6, LV3, GV24, GV26, LI14, LI11, H7, ST40, GB20, GB34 and LI15 were used. In comparison of results before and after acupuncture treatment, there were statistically significant improvement in 26 studies. Based on the results from above, the studies on acupuncture for Post-stroke depression needs to be standardized and more appropriate high-quality study models should be introduced.

  • Objectives : When we understand the characters of certain person or object, we try to follow each one's or its past up. Korean Journal of Acupuncture is one of the most significant journal in acupuncture study fields in Korea. To understand the trends of study in acupuncture study fields, I made a subject with Korean Journal of Acupuncture. Methods : I made an evaluation and classification for all 713 articles' headline from vol. 17(2000 year) to vol.32(2015 year). Results : 1. Experimental Reseach : There were major portion articles for pharmaco-acupuncture study out of studies for acupuncture, moxibustion, chiropratics, devices and so on. 2. Bibliographical Research and Basic Theory Research : There were major portion articles for medians and acu-point study out of studies for article review and text book itself. Also, there were major portion articles for meridian and acupuncture study out of studies for Qigong, pulse, anatomy and so on. 3. Clinical Research : There were major portion articles for acupuncture study out of studies for moxibustion, chiropratics, devices, complex treatments and so on. 4. Others : There were major portion articles for diagnosis and measurement devices out of acupuncture, laser, pulsing device and so on. Also, there were surveys for recognition of patients and medical services and evaluations for measuring diagnosis utility and those effects. Conclusions : With thise results, I hope that several researchers could consider scope and subject when they submit articles for Korean Journal of Acupuncture.

A Systematic Review of Acupuncture-Moxibustion Treatments for Nonspecific Chronic Neck Pain (비특이적 목 통증에 사용된 침구치료에 대한 체계적 문헌고찰)

  • Objectives : The aim of this study was to describe the details and to assess the clinical evidence of acupuncture and moxibustion for non-specific chronic neck pain. Methods : We searched seven databases including Korean, English, and Chinese databases through July 2016. Studies using acupuncture, moxibustion, pharmacopuncture, electroacupuncture, auricular-acupuncture, acupressure for non-specific chronic neck pain were included. Only controlled clinical trials or randomized clinical trials were assessed. Study design, number of subject, inclusion criteria, intervention, and results were extracted. In addition, details of intervention including needle type, retention time, acupoints were analyzed. Results : Total 64 studies(39 acupuncture, 9 laser, 6 pharmaco-acupuncture, 3 electro-acupuncture, 3 auricular-acupuncture, 3 moxibustion, 1 acupressure) were included. Among 39 acupuncture studies, 35 used acupuncture as sole intervention. Sham treatment was the most common intervention for control group, followed by no intervention. Various outcome including pain, disability, quality of life, range of motion was used as outcome measurement. The effect of acupuncture and moxibustion was different depending on the type of control and outcome measurement. The most commonly used method in acupuncture for neck pain was GB21, SI3, GB20, LI4, BL11 acupoints, 10~30 mm insertion depth, 20~30 retention time, and 1~2 times per week. Conclusions : Analyzing the details of acupuncture and moxibustion treatment could be helpful for researches and clinics. Further studies should consider the characteristics of study design, intervention, and outcome to assess the effect of TKM.

The Effects of Euonymi Ligunum Suberalatum Phamacopuncture on Nitric Oxide and Interleukin-6 Production in Macrophage (귀전우(鬼箭羽)약침액이 대식세포의 Nitric Oxide와 Interleukin-6의 생성에 미치는 영향)

  • Objectives : The objective of this study is to study the effects of Euonymi Lignum Suberatatum pharmacopuncture solution on NO and IL-6 production in macrophage. Methods : At first, the RAW 264.7 macrophage was subclutured. In order to evaluate cytotoxicity, MTT assay performed. Then, the cell was induced by LPS, INF-$\gamma$ and Experimental groups were divided into five(Normal, Control, Euonymi Lignum Suberatatum 100, 200, $300{\mu}g/m{\ell}$). Then Euonymi Lignum Suberatatum pharmacopuncture solution was put into cell. We measured IL-6, iNOS, NO. Results : The cytotoxic effect of Euonymi Lignum Suberatatum pharmacopuncture solution in RAW 264.7 macrophage was not appeared. $300{\mu}g/m\ell$ Euonymi Lignum Suberatatum pharmacopuncture solution inhibited IL-6 production in LPS, INF-$\gamma$-stimulated RAW 264.7 macrophages significantly. Euonymi Lignum Suberatatum pharmacopuncture solution inhibited iNOS revelation in LPS, INF-$\gamma$-stimulated RAW 264.7 macrophages. All group of Euonymi Lignum Suberatatum pharmacopuncture solution inhibited NO production in LPS, INF-$\gamma$-stimulated RAW 264.7 macrophages significantly. Conclusions : Our study demonstrated that Euonymi Lignum Suberatatum pharmacopuncture solution had an inhibition effect on NO production, iNOS revelation, IL-6 production. So Euonymi Lignum Suberatatum pharmaco puncture solution may have an Anti-inflammation effect.
