병원약국의 외래조제업무에 대한 컴퓨터의 이용 (Utilization of Computer System for Outpatient's Dispensing Affairs in Hospital Pharmacy)

  • Hospital pharmacy services are divided into dispensing affairs for inpatients and outpatients, pharmaceutical service, stock control, intravenous admixture service, drug information service, pharmacokinetic consultation service, education and research work, etc. But among those affairs, dispensing affair for outpatient is perceived as the most important work in Korea, because it is linked directly with hospital service for patients. Therefore, total computer system for dispensing area was adopted from opening point of hospital in 1989 in Asan Medical Center. Utilization of computer system for outpatient dispensing area is as follows; 1) Order communication system of prescription by Total Hospital Information System, 2) Automatic print-out system of direction for use by sticker connected with on-line net work, 3) Use of automatic tablet counting and packaging machines connected with on-line net work. Those computer system resulted in curtailment of pharmacy manpower and shortening of waiting-time for outpatient.

외래환자 대상 당뇨병 약료서비스 모델 개발 (Development of a Pharmaceutical Care Service Model for Patients with Diabetes in Ambulatory Care Settings)

  • The prevalence of diabetes and its related morbidity and mortality are being increased. Despite the advancement of evidence-based pharmacotherapy in the management of diabetes, many patients in our country do not achieve satisfied therapeutic outcomes. Pharmaceutical care service can be defined as a patient-centered clinical service provided by pharmacists to improve therapeutic outcomes and quality of life of patients, by identifying, and preventing or resolving drug-related problems (DRPs). Pharmaceutical care service is interdisciplinary team-based practice, and is provided through collaborative practice agreement (CPA) between one or more physicians and pharmacists. This article describes a model of pharmaceutical care service which can be adopted in our country for patients with diabetes in the ambulatory care settings. With the successful implementation of this service, clinical, economic, and humanistic outcomes of patients will be improved. Therefore, by actively implementing pharmaceutical care service, pharmacist should contribute to the promotion of patients' health and to the advancement of health care delivery system.

국내 2018년 의약품 소비량 및 판매액 통계 산출 및 국제 비교 (A Critical Evaluation and International Comparison of Pharmaceutical Consumption and Sales Statistics)

  • Background: Health statistics of pharmaceutical use and expenditure are essential to make and implement evidence-based pharmaceutical policy. This study aims to demonstrate the methods and results of pharmaceutical consumption and sales in 2018 according to the sources and methods given by the Organization for Economic Cooperation and Development (OECD). Methods: The medication list contains 39,346 medicines both reimbursed and non-reimbursed by the National Health Insurance in 2018. We used the therapeutic categories based on Anatomic Therapeutic Chemical Classification of World Health Organization. This study analyzed National Health Insurance claims data and supply data generated from wholesalers to health care facilities. The indicators are defined daily dose (DDD), per 1,000 inhabitants per day and US$ per capita. Results: In South Korea, the number of medications to which DDD were assigned was 18,055 and it was 45.9% of the total number of medications on the list. The consumption in anti-infective for systemic use (J) and musculo-skeletal system (M) was higher than the mean consumption among the OECD countries. The pharmaceutical sales per person in Korea was also higher than the mean sales per person across the OECD countries. Conclusion: We sought to explain the methods to produce pharmaceutical consumption and sales statistics which we had submitted annually to OECD. Considering the characteristics of pharmaceutical statistics, a direct comparison should be approached with caution. Since the growth in pharmaceutical spending has greatly increased over the past decade, we need to monitor pharmaceutical consumption and expenditure consistently.

문헌자료 고찰을 통한 우리나라 약국서비스 시행 현황 (Pharmaceutical Care Services of Community Pharmacies in Korea Through the Review of Literature)

  • Background: The recent change in pharmaceutical education system following the paradigm shift to patient-oriented pharmacy service requires an in-depth discussion to reorganize a future direction and establish a basis for maximizing social values of community pharmacy service. Objective: This study was conducted to review the current status of community pharmacy service provision in Korea based on published literatures. Methods: The electronic databases of National Digital Science Library and Electronic National Assembly Library were used to search the journal articles and dissertation papers. A search term "community pharmacy" was used and the published period was limited to papers published after year 2001, when the legal separation of prescribing and dispensing was implemented. Relevant study reports were also searched manually. Information about pharmacy service provision and study outcomes were retrieved from the selected papers, and classified by predefined individual service scope. Results: A total 33 papers reporting services provided by community pharmacies were selected (journal article 11, dissertation paper 17, and study report 5). Pharmacy services identified in these papers could be classified into prescription dispensing service, pharmaceutical care service, self medication service, other products service, and health promotion service. Twenty papers reported prescription dispensing services, three papers reported pharmaceutical care service, and only two papers reported health promotion service. Current community pharmacy services are highly dependent on prescription drugs while expanded services such as pharmaceutical care and health promotion are peripheral. Most prevalent research topic was medication counseling service (18 papers), reflecting that community pharmacists generally consider it to be the most important and fundamental service. Overall, current pharmacy services are very limited and focus on prescription dispensing service. Conclusion: At this point of time requiring expansion and quality improvement of community pharmacy services, we suggest further lively discussion to strengthen pharmacist's functional identity and set conditions for providing socially expected services.

온톨로지 기반의 의약정보 음성 안내 서비스 개발 (Development of Voice Guide Service for Pharmaceutical Information based on Ontology)

  • This research proposes a method to provide visually impaired persons with better medical services and to mitigate their difficulties in taking medicines in their daily lives. Normally, pharmaceutical information are distributed in a form of printed materials, but it is not accessible by those who are visually impaired. Since persons who are visually impaired by accidents or diseases are required to take more types of medicines compared to other handicapped persons, and thus it is an important matter to let them have appropriate medicines at the right time. Although there are several web sites which are run by the Ministry of Health and Welfare providing pharmaceutic information, the information on those web sites are duplicated and some of the menus are similar, and thus giving users difficulties and discomfort in finding appropriate pharmaceutical information. Since ontology can define and describe the resource in an information system more clearly, in this research, an ontology consists of pharmaceutical information and knowledge on medicines is constructed to give patients more precise information more efficiently. Also, a service in which users can have voice guidance on pharmaceutical information retrieve from the ontology-based information system by contacting RFID sticker on the medicine to the reader.

제약회사의 내부마케팅활동과 직무만족, 조직몰입, 이직의도와의 관계 -제약영업사원을 중심으로- (The Relationship among Internal Marketing Activities, Job Satisfaction, Organizational Commitment, and Turnover Intention in Pharmaceutical Companies -Focusing on Pharmaceutical Salespeople-)

  • The initial approach to this study is to focus on pharmaceutical companies' internal marketing activities for organizational diagnosis. By analysing the influence of internal marketing elements on the job satisfaction, organizational commitment, and turnover intention of pharmaceutical salespeople, this study aims at coming up with ways to strengthen the sales capacity and competitive edge of pharmaceutical companies through the salespeople's satisfaction. For 32 days between November 10th and December 12th of 2012, a total of 210 copies of questionnaires were given to the respondents and 203 of them except 7 with insufficient answers were used for the final analysis. The analysis result showed that internal communication, delegation of authority, and reward system among the elements of internal marketing activity had a positive influence on the workers' job satisfaction. The internal marketing activity elements did not influence the respondents' organizational commitment. Job satisfaction had a positive influence on their organizational commitment, while it had a negative influence of turnover intention. The conclusion section encapsulated the results of empirical analysis and proposed practical lessons regarding pharmaceutical companies' internal marketing.

약제의 신규등재 절차와 급여기준 관리 (New Drug Listing Process and Reimbursement Standard Management)

  • The Ministry of Health and Welfare of Korea has implemented various social security programs to ensure a basic standard of living and raise overall quality of life for all citizens. The Korean social security system provides social insurance, public assistance, and social welfare services. To achieve adequate drug benefits, the Drug Management Department of Health Insurance Review and Assessment Service (HIRA) implement drug management duties including drug listing, upper price limit setting, scope of benefits, and post-factum management. When a manufacturer or an importer wants to apply for National Health Insurance (NHI) coverage of the drug that has obtained safety and efficacy approval, the pharmaceutical benefit assessment committee of HIRA evaluates the drug's clinical efficacy and cost-effectiveness to determine whether or not to include the drug into the benefit package. The benefit standards for a listed drug (ingredient) are set either for the whole permitted range or a part of range with conditions. To increase the coverage rate for new drugs, the listed drugs are regularly reviewed for their value. The status of listed drugs can be adjusted or eliminated from the benefit package if the clinical efficacy turns out to be insignificant. Therefore, through these pharmaceutical management procedures, high-quality drugs are provided at reasonable prices, which save healthcare expenditure by price determination and selective coverage in consideration of economic evaluation.

국내 제약기업에서의 SFA(sales Force Automation) 시스템 활용수준과 기업성과의 실증분석 (Empirical Analysis of SFA (Sales Force Automation) System Utilization Level and Performance in Pharmaceutical Companies in Korea)

  • 많은 제약기업에서 영업사원들을 위하여 SFA 시스템을 운영하고 있다. SFA 시스템은 영업사원들의 다양한 행동에 활용되고 있는데 국내에서는 SFA 활용성과에 대한 실증분석 연구가 부족한 실정이다. 본 연구에서는 제약회사 영업사원을 대상으로 SFA 시스템의 활용도와 그에 따른 비재무적 성과를 설문을 통해 조사하였다. 조사대상은 제약 영업사원으로 구성된 제약 커뮤니티 회원 347명을 대상으로 2018년 3월 13일부터 4월 30일까지 18일 동안 온라인으로 조사하였다. 유효 응답률은 23.1%(80/347)이었다. 분석 결과 SFA 시스템 활용 수준이 높을수록 비재무적 성과가 높은 것으로 나타났다. 회사 구분별로는 국내 기업보다는 외국계 기업이 활용 수준이 높았다. SFA 활용 수준 항목 중 지원 서비스 항목에 대해 외국기업이 국내 기업보다 1.47점(3.65, 2.18) 높게 나타났고, SFA 성과 항목 중에서는 고객만족도 항목에 대해 외국기업이 국내 기업보다 1.47점(3.16, 1.69) 높게 나타났다. 이는 국내사의 SFA 개발 및 운영방식이 영업사원에 대한 지원 서비스 및 고객만족 중심이 아닌 관리 통제에 중심을 두는 것을 시사한다. 이번 연구를 통하여 국내 기업의 경우 SFA 시스템 운영 시 영업사원 지원 및 고객만족 정보제공 기능이 강화되어야 할 것으로 사료되었다.

우리나라 약사면허 국가시험제도 변천과정과 향후과제 (Historical Transition and Future Task of Pharmacist License Examination in Korea)

  • Objectives of this study are to investigate transition history of pharmacist license examination in Korea and to compare three suggestions for new examination system proposed by National Health Personnel Licensing Examination Board, Korean Association of Pharmacy Education, and Korean Pharmaceutical Association. Future task to help the examination system meet global standard is also discussed. Regulations on the examination system were retrieved from the sources posted in Ministry of Government Legislation homepage. Efforts devoted by the three organizations to make amendment on the regulations were collected from reports, official documents, minutes, and handout materials issued by the organizations. Ministry of Health and Welfare made amendment on the examination system in 2013 by rearranging the existing natural science-based eleven subjects to two subjects along with addition of a new subject named clinical-practical pharmacy. Unfortunately, however, the amendment stipulates that the new subject should include manufacturing and quality control of pharmaceuticals as part of contents in the examination. It is strongly urged that the contents of the examination should focus on professional competency as pharmacist in clinical situation. Future task to make revision on this issue is warranted to help new pharmacist enable to provide pharmaceutical care service to patients.
