보간탕으로 호전된 당뇨병성 말초신경병증 2례 (Two Cases of Diabetic Peripheral Polyneuropathy Improved by Bogan-tang.)

  • Diabetic Peripheral Polyneuropathy is one of the most distressing complications of diabetes, Drugs such as narcotic analgesics, tricyclic antidepressants, anticonvulsants and phenothiazines have been used to treat diabetic peripheral polyneuropathy, but these drugs are not very effective for the many patients and the side effects may become intolerable. In oriental medicine, usually the main etiology of diabetic Peripheral Polyneuropathy is insufficiency of Blood due to the state of dry-heat in the human body. Therefore, we use Bogan-tang to treat numbness of Diabetic Peripheral Polyneuropathy. Bogan-tang was administrated twice a day for 15 days, and VAS Scale was performed every day to evaluate numbness and insufficiency of blood. In both cases, numbness and insufficiency of blood were improved without any side effects. This study suggests that Bogan-tang is an effective drug in the treatment of Diabetic Peripheral Polyneuropathy.

  • Objectives : This study it designed to evaluate the effects of an oriental medicine therapy, namely Binsosan-gamibang, on diabetic peripheral polyneuropathy. Methods : The clinical data was analyzed on a patient with diabetic peripheral polyneuropathy due to subyeolongsung(濕熱壅盛), gihyeoloeche(氣血瘀滯) whose main symptoms were pain and numbness in both legs. The patient was admitted at the internal medicine department of Dae-Gu Hanny University Dae-Gu Oriental Medicine Hospital on December 22, 2003, and was treated with herbal medicine(Binsosan-gamibang), acupuncture and physical therapy. Results : After treatment, improvement in pain and numbness in both legs was seen in the subsection of the pain rating score(PRS) and the visual analogue scale(VAS). Conclusions : This study suggests that Binsosan-gamibang is significantly effective in treatment of diabetic peripheral polyneuropathy.

  • Diabetic polyneuropathy (DPN) is the most frequently encountered form of neuropathy in diabetic patients, and it either relentlessly progresses or remains relatively stable for many years, not showing any trend towards improvement. From this point of view, early detection of DPN is very important to prevent the irreversible change of the peripheral nerve from diabetic insults. Although a number of clinical symptoms and/or deficit scales have been developed for clinical or research purposes, nerve conduction study (NCS) has been known one of the most objective and sensitive tools to detect peripheral nerve dysfunctions in diabetic patients. NCS, however, also have several shortcomings. The next two consecutive articles will focus on debates about diagnostic usefulness of NCS and on recent updates of other diagnostic tests including quantitative sensory testings and skin biopsy in the field of diabetic polyneuropathy.

사람면역결핍바이러스 감염과 연관된 말초신경병증 (Peripheral Neuropathy Associated with Human Immunodeficiency Virus Infection)

  • Background: Peripheral neuropathy is the most frequent neurological complication in human immunodeficiency virus (HIV) infection, related with diverse etiologies including inflammation, opportunistic infection and side effects of medications. The purpose of the present study was to evaluate characteristics of HIV associated neuropathy according to the stage of HIV infection. Methods: In reviewing the medical records of HIV patients who underwent electrodiagnostic studies between 1997 and 2011, total 11 patients (all males; median age, 47 years; range, 28-71 years) with comorbid neuropathy were enrolled. Stage of HIV infection was categorized according to the Centers for Disease Control and Prevention (CDC) criteria. Classification of peripheral neuropathy was based on clinical and electrophysiological features. Results: Distal symmetric polyneuropathy was observed in 8 patients (72.7%), inflammatory demyelinating polyneuropathy in 2 patients (18.1%), and polyradiculopathy in 1 patient (9.1%). Median CD4+ T cell count was $123/mm^3$ (range, $8-540/mm^3$) and 7 patients (60%) had the most advanced HIV disease stage (CDC-C3). There was no neuropathy caused by CMV infection. Conclusions: Distal symmetric polyneuropathy was the most common type of neuropathy in HIV infection, but various forms of neuropathy such as inflammatory demyelinating polyneuropathy and polyradiculopathy were also present. HIV associated neuropathy is more frequently associated with advancing immunosuppression, although it can occur in all stages of HIV infection.

알코올성 다발성 신경병증의 한방치료 효과 (Clinical Study of Oriental Medical Treatment on a Patient with Alcoholic Polyneuropathy)

  • Alcoholic polyneuropathy is a disorder of the peripheral nervous system that interferes with sensory, motor, and autonomic nerve function. This study was to report the effect of Oriental medical treatment on a patient with alcoholic polyneuropathy. Conservative Oriental medical treatment was done to a man of 84 years who was diagnosed as a alcoholic polyneuropathy and suffered from both distal dominant leg weakness & symmetrical paresthesia and ataxic gait. The changes of symptoms, reflexes and sensory test were checked by Toronto Clinical Neuropathy Scoring System(TCNSS). Reflexes, motor and sensory impairments were improved after the Oriental medical treatment. I reported good effects of Oriental medical treatment on alcoholic polyneuropathy. Oriental medical treatment can be helpful to improve the symptoms of alcoholic polyneuropathy.

  • Thiamine deficiency can cause peripheral polyneuropathy and Wernicke's encephalopathy. Wernicke's encephalopathy is characterized by ataxia, ophthalmoplegia, nystagmus, and confusion, and typically presents acute and rapidly progressive course, whereas peripheral neuropathy associated with thiamine deficiency manifests chronic and slowly progressive one. However, acute and rapidly progressive axonal polyneuropathy combined with Wernicke's encephalopathy is quite rare and unusual. Here, we describe a patient with Wernicke's encephalopathy who presented with acute bilateral axonal neuropathy.

A case of motor and sensory polyneuropathy induced by primary hyperparathyroidism

  • Primary hyperparathyroidism (PHP) is a disease in which excessive amounts of parathyroid hormone (PTH) are secreted and calcium levels in the blood increase. Hypercalcemia caused by PHP has a major influence on the peripheral nervous system and produces symptoms such as muscle cramps, paresthesia, and proximal muscle weakness. Here we report a rare case of sensory-dominant polyneuropathy caused by PHP, which improved after surgery.

  • Sj$\ddot{o}$gren's syndrome (SS) is an autoimmune disease characterized by keratoconjunctivitis sicca and xerostomia. About thirty percent of patients with Sj$\ddot{o}$gren's syndrome experience nervous system involvement such as myelopaty, optic neuropathy, and peripheral neuropathy. The most common pattern of peripheral nerve involvement is axonal polyneuropathy. We present a case of demyelinating polyneuropathy in a patient with Sj$\ddot{o}$gren's syndrome, which had been proven by electrophysiology and pathology.

  • Phenytoin has been used globally as an effective anticonvulsant. Among its adverse effects, peripheral neuropathy including polyneuropathy has sometimes been reported. We report a case of sensorimotor polyneuropathy associated with high serum level and long-term phenytoin therapy. A 29-year-old male presented with motor weakness in all extremities. He was treated with phenytoin (400 mg/day) for about eight years because of generalized tonic clonic seizure. During none conduction assessment, sensorimotor polyneuropathy was discovered.

사르코이드증에 동반된 다발신경병증 (Sensorimotor Polyneuropathy Associated with Sarcoidosis)

  • Sarcoidosis is a multisystemic granulomatous disease of unknown etiology ogenesis, and most frequently presented with bilateral hilar lymphadenopathy, pulmonary infiltration, skin and eye lesion. However, neurological involvement including peripheral neuropathy is relatively rare. We report a patient who had sensorimotor polyneuropathy without other systemic symptoms or organ involvements frequently reported in sarcoidosis. Laboratory investigation suggestive of sarcoidosis lead to sural nerve biopsy for confirmation, which demonstrated noncaseating granulomatous changes. Sarcoidosis shoud be included in the differential diagnosis in subacute polyneuropathy even if there is no usual symptoms or signs suggestive of the systemic disease.
