Structural damage to periodontal tissues at varying rate of anesthetic injection

  • Background: Incorrect administration of an anesthetic during local anesthesia is one of the most important causes of pain symptoms in patients scheduled for dental procedures. The current study assessed the severity of damage to periodontal tissue following different rates of anesthetic administration. Methods: The research was conducted on 50 outbred male rats with a body mass of 180-240 g. The anesthetic used was 1% articaine. Results: The results showed that administration of the anesthetic at a rapid pace caused structural damage to the periodontal tissue. Further, signs of impaired microcirculation were noted at all rates of administration. Biochemical studies demonstrated changes in the level of glucose and enzymes with the rapid introduction of the anesthetic, indicating severe systemic stress response of the body. Conclusions: Injection of local anesthetic at any rate of introduction induces vascular congestion in the microcirculatory bloodstream and exudative reactions. Rapid introduction of an anesthetic causes progression of structural changes in the gingival tissue.

A Study on the Effects of the Active Pulmonary Tuberculosis to the Several Oral Environmental Factors

  • The authors had studied the oral environmental changes by salivary salivary pH, amount, periodontal Index in patient with active pulmonary tuberculosis patients. Among the subjects, The experimental group was consisted of 100 patients (50 males and 50 females) of 20-29 years and 100 persons of control group (50 males and 50 females) of 20-29 years. The measurement of salivary amount was performed with wide mouthed plastic (2 Inches) bottle for avoidance of Ionization of SiO2 by using of glass bottle and salivary pH was checked by pH meter 27 radiometer Copenhagen. The results are as follows : 1. The slivary pH does not appear to be characteristic of tuberculous paticents. 2. The obtained salivary amount indicated no significant to the tuberculous patients as compared to the obtained supposedely wealthg individuals. 3. The Russel Index wasfound higher and there had found more periodontal involvement(3 times than normal) in the experimental group.

Intraosseous anesthesia using a computer-controlled system during non-surgical periodontal therapy (root planing): Two case reports

  • Local anesthesia is administered to control pain, but it may induce fear and anxiety. Root planing is a non-surgical periodontal therapy; however, when it is performed in an extensive manner, some tissue removal is inevitable. Notably, this removal may be so painful that local anesthesia is required to be administered to the area scheduled for the treatment. Although patients tend to accept root planing easily, they frequently express a fear of local anesthesia. Intraosseous anesthesia (IA) is an intraosseous injection technique, whereby local anesthetic is injected into the cancellous bone supporting the teeth. A computer-controlled IA system (CIAS) exhibits multiple benefits, such as less painful anesthesia, reduced soft tissue numbness, and the provision of palatal or lingual, as well as buccal, anesthesia via single needle penetration. In this report, we present two cases of root planing that were performed under local anesthesia, using a CIAS.

  • Mucous membrane pemphigoid (MMP) is a heterogeneous group of chronic, autoimmune subepithelial blistering diseases, predominantly involving oral mucosal membrane. Because of its pathophysiology of autoimmune reaction, MMP-related gingivitis would not respond to conventional periodontal treatments. We present a case of a 65-year-old female with a chief complaint of chronic generalized buccal gingivitis, unimproved after periodontal treatment for four months. Based on the clinical findings, histological examination, and direct immunofluorescence microscopy, it was diagnosed as MMP. The symptoms were relieved with immunomodulatory therapy using corticosteroids and the supportive management of professional plaque control. MMP can cause pathological involvement throughout the oral and other mucosae of the body, thus leading to functional impairment through repeated inflammatory cascades. Therefore, accurate diagnosis is essential to properly manage local and possible systemic complications of MMP.

Nonodontogenic toothache

  • Toothache is one of the most common discomforts experienced by patients in dental clinic. If clinical and radiographic examinations do not reveal any pathologic findings, we can suspect nonodontogenic toothache. Nonodontogenic toothache can be caused by a variety of causes such as muscle disorders, sinus and nasal mucosal problems, neuropathic pain, neurovascular pain, psychogenic problems, and cardiogenic disease. A thorough history and clinical examination should be performed to confirm the cause of the pain, and more accurate diagnosis can be established through local anesthetic injection. If the nonodontogenic toothache is misdiagnosed, unnecessary dental treatment such as root canal treatment, periodontal treatment, and extraction can be performed, and the patient's pain is not alleviated through such treatment. Therefore, the cause of toothache must be diagnosed correctly before dental treatment is performed, and clinicians should be fully aware of the diseases that may cause these symptoms.

몰약, 라타니아, 카모밀레 등의 구강 내 병원균에 대한 항균작용 (Effect of antibacterial effects of myrrh, rhatany, chamomomilla against to oral microorganisms)

  • 여러 식물의 추출물에 대한 항균효과와 항균작용은 이미 연구되어 있으나, 구강 내 병원균에 대한 연구는 아직 미미한 편이다. 이에 타액에 수 종의 식물 추출물, myrrh, rhatany, chamomomilla를 첨가하여 치주질환과 구취 발생에 중요한 역할을 하는 원인균들에 대한 억제효과를 파악하고자 하였다. 실험에 사용한 생약 추출물은 Myrrh tincture (100 mg/ml), Ratanhia tincture (100 mg/ml), Chamomile tincture (100mg/ml)를 사용하였으며, 충치원인균인 S. mutans GS5, S. sobrinus 6715와 치주염원인균주인 P. gingivalis 2561 및 P. intermedia ATCC 25611, Candida albicans ATCC 18804, E. feacalis ATCC 4083 균주를 대상으로 항균실험하여 다음과 같은 결과를 얻었다. 본 연구에서 사용한 식물 추출물인 myrrh, rhatany, chamomomilla는 수렴작용과 살균작용, 항염증작용을 가지고 있는 물질로서, P. gingivalis, S. mutans, C. albicans 등의 구강 내 병원균에 대한 항균효과나 항균작용이 구명되었다. 또한 가장강한 항균효과를 보인 rhatany를 처리한 이들 병원균을 투과전자현미경 하에서 관찰하였을 때, 심한 형태적 변형이 관찰되었다. 이는 식물 추출물에 의하여 구강 내 유해균을 억제시킴으로써 치주질환과 충치, 구취 및 구강염의 발생을 예방할 수 있다는 것을 의미하므로, 향후 구강 환경 개선을 위한 임상적 근거가 마련될 수 있을 것으로 생각된다.

Non-Odontogenic Toothache Caused by Acute Maxillary Sinusitis: A Case Report

  • Non-odontogenic toothaches are frequently present and can be challenge to the dental clinician. A 41-year-old male patient with sharp and spontaneous pain on the right maxillary posterior dentition, which developed as like localized toothache 3 months ago, was finally treated with endoscope assisted sinus surgery on right maxillary sinus. Although the initial clinical characteristics are similar to odontogenic toothache in this patient, previous several treatment with the affected teeth did not alleviate the pain. Sinusitis around the facial structure is one of the common causes to make referred pain to maximally teeth and the sinus toothache resembles the pulpal or the periodontal toothache. The clinician should be well aware of various causes of the non-odontogenic toothache and be able to differentiate them.

치주수술후 치주포대 비부착시 클로르헥시딘과 리스트린 구강 양치의 효과 (THE EFFECTS OF CHLORHEXIDINE AND LISTERINE MOUTHRINSE WITHOUT PERIODONTAL DRESSING AFTER PERIODONTAL SURGERY)

  • The purpose of this study was to compare the effects of Chlorhexidine, Listerine and hypertonic saline mouthrinse without periodontal pack during 2 weeks following periodontal surgery. Twelve patients were treated with modified Widman flap procedures. Each fourth patients assigned to chlorhexidine mouthrinse group, Listerine mouthrinse group and control group. Each group wasn't significant difference in clinical and microbiologic parameters at preoperation. Examination regarding plaque index, gingival index, pocket depth, change of gingival color postsurgically, pain index and contrast phase microscope were performed. Evaluations were made at the first, the second and the fourth weeks postsurgically. The results were as follows: 1. Chlorhexidine mouthrinse group and Listerine mouthrinse group were significantly different in plaque index at the first, the second and the fourth weeks 2. The pocket depth of three groups were significantly reduced at the fourth weeks. 3. Chlorhexidine mouthrinse group and Listerine mouthrinse group were significantly different in change of gingival color at the first week. 4. Cocci and non-motile rods were increased at the first and the second weeks. But, motile rods and spirochete were increased at the fourth weeks.

수종의 구취처치법의 구치감소효과에 관한 연구 (Effect of Tongue Scraping, ZnCl2 Mouth Rinse, and Periodontal Treatment on the Reduction of Oral Malodor)

  • This study was performed to investigate the effect of three modalities commonly used for reduction of oral malodor and to review the contributing factors for halitosis. For this study, 45 subjects of good general health in their third decades were selected, and they were divided into three groups by each modality, that is, tongue scraping, $ZnCl_2$ mouth rinse, and periodontal treatment. Subjective sense of halitosis, diets, Oral hygiene index based on tooth brushing and tongue brushing habit, resting salivary flow rate, gingival index, degree of tongue coating were recored in the Halito-Chart designed by the author. Oral malodor converted from volatile sulfur compound was measured by the Halimeter$^{(R)}$(Interscan Co., USA), and the correlation between the oral factors and pre-treatment halimeter score were analysed. Data obtained were analysed by the SPSS windows program and the results of this study were as follows : 1. There were no significant correlation between oral factors such as oral hygiene index, salivary flow rate, gingival index, degree of tongue coating and pre-treatment halimeter score. 2. Difference between pre- and post-treatment halimeter score by the three modalities were significant, but correlation between pre- and post-treatment score within group was most significant in the periodontal treatment group followed by the tongue scraping group and lowest in the $ZnCl_2$ mouth rinse group. 3. Reduction of halimeter score was significant only in subjects with relatively more tongue coating in the tongue scraping group, but in the $ZnCl_2$ mouth rinse group and in the periodontal treatment group, the modality applied to each group had same significant effects without regard to the degree of oral hygiene index or gingival index. 4. Men had more tongue coating than women, and reduction of halimeter score was significant in men, but not in women.

Effectiveness of anterior middle superior alveolar injection using a computer-controlled local anesthetic delivery system for maxillary periodontal flap surgery

  • Background: Profound anesthesia with adequate duration is required in periodontal flap surgery, which involves the manipulation of both hard and soft tissues. The anterior middle superior alveolar (AMSA) injection may be an alternative to multiple injections required for this purpose in the maxilla. The present study aimed to assess the effectiveness of AMSA injection using computer-controlled local anesthetic delivery (CCLAD) system to anesthetize buccal hard tissue (BHT), buccal soft tissue (BST), palatal hard tissue (PHT), and palatal soft tissue (PST) around the maxillary teeth. Methods: Thirty-five patients who were indicated for open flap debridement in a whole maxillary quadrant were given AMSA injection using the CCLAD. The effectiveness of anesthesia was evaluated using subjective and objective parameters around each tooth. Supraperiosteal infiltrations were administered to complete the surgery wherever the AMSA injection was ineffective. Results: The AMSA injection was more effective on the palatal tissues than on the buccal tissues, as 94.14% of PST and 87.89% of PHT sites were anesthetized compared to 49.22% and 43.75% of BHT and BST sites, respectively. There was no significant difference in the frequency of anesthesia around the anterior and posterior teeth. The PHT was significantly more anesthetized (P = 0.003) in males than in females. Conclusions: The AMSA injection using CCLAD is highly effective on palatal tissues and could be used as a first-line anesthesia for periodontal flap surgery. However, its effect on buccal tissues is less predictable, with supraperiosteal infiltration often required to supplement the AMSA injection.
