Effects of Rituximab Including Long-term Maintenance Therapy in Children with Nephrotic Syndrome in a Single Center of Korea

  • Rituximab (RTX) is a chimeric monoclonal antibody that inhibits CD20-mediated B-cell proliferation and differentiation. Several studies have examined its use in intractable nephrotic syndrome (NS) with some positive results. However, those studies examined such effects for a short-term period of 1 year, and some patients continued to relapse after a lapse in RTX treatment. Our use of RTX as a maintenance therapy (RTX injection when the CD19 cell count exceeded $100-200/{\mu}L$ before relapse) showed some noticeable efficacy. We used RTX in 19 patients with steroid-dependent NS (SDNS). In 12 patients treated with RTX maintenance therapy, only one relapse occurred. The mean treatment period was $23.4{\pm}12.7months$, and the mean number of RTX administrations was $3.9{\pm}1.6$. The relapse rates were decreased (from 2.68/year to 0.04/year), and the drug-free period also increased (from 22.5 days/year to 357.1 days/year) during maintenance therapy. The other seven patients were treated with one cycle of RTX or additional cycles in case of relapse (non-maintenance therapy). Relapse rates were significantly decreased after RTX treatment (from 1.76/year to 0.96/year, P=0.017). The relapse-free period was $15.55{\pm}7.38$ (range, 5.3-30.7) months. No severe side effects of RTX were found except for a hypersensitivity reaction such as fever and chills during its infusion. In conclusion, RTX is considered an effective and safe option to reduce the relapse rate by a single- or maintenance-interval therapy in SDNS.

  • The configuration such as series, parallel and k-out-of-n of a repairable system directly affects its reliability. The maintenance strategy can also affect the overall performance of the system. The objective of this work is to investigate the possible trade-off between the configuration of a repairable k-out-of-n system and its maintenance strategy. The redundancy is considered to be the design decision variables, whereas the preventive maintenance period is considered to be the maintenance decision variables. The optimization model is used to minimize the overall life cycle cost associated with the system, considering constraint on reliability. Finally, genetic algorithm is used to find the optimal values for the decision variables. The result is compared with optimal values for considering redundancy and maintenance respectively.

  • Seoul subway is an safety, quick, and clean public transportation to secure citizens safety. One of the most important elements of train is optimizing railroad function and need to conform railroad inspection for function maintenance. Railroad is the most biggest part of the railroad facility and it is a structure which requires sustainable maintenance. The purpose of study is to improve domestic metropolitan railroad facility inspection method and period. For this, we refer documents about present roving method, roving members and period of railroad roving. In addition, analyzed problems of railroad roving investigation method and period by examining the type and standards of railroad investigation and interviewing experts. By this, we analyzed improving plan and expectations of railroad maintenance and repair operation.

터널 유지관리 계측의 국내설계기준 개선 및 최신 기술동향 (Improvement of Domestic Design Criteria of Tunnel Maintenance Monitoring and Latest Technology Trend)

  • 공용 중인 터널 구조물의 구조적 취약부위를 대상으로 시행되는 유지관리 계측은 효과적인 관리를 위해서 터널의 굴착에 따른 지반 및 주변 구조물의 거동을 파악하기 위한 공사 중 계측과 연계하여 판단되어져야 하나 공사시기 및 유지관리 시점을 기준으로 공사 중 계측과 유지관리 계측을 별도로 시행하고 있어 연계성 및 관련 자료의 체계적 관리가 대부분 미흡한 상태이다. 본 논문에서 국내 설계기준의 터널계측 문제점을 분석하여 개선방향을 제시하였으며, 향후 유지관리에는 첨단센서 및 무선통신 기술의 활용이 활발히 추진될 것으로 전망되므로 이와 관련된 유지관리 계측시스템의 개발 적용 및 국내 설계기준 및 시방기준의 개정이 필요하다.

데이터마이닝을 이용한 공군 무기정비병의 조기 숙달을 위한 배속방안 연구 (An Effective Recruits' Assignment Method for Early Job Adaptation of Air-munition Maintenance Airmen Using Datamining Technique)

  • Recently, the military service period has been shortened continuously. Meanwhile, more skilled airmen are needed as the complexity of weapon systems increase. This phenomenon could lead to a disastrous result such as deteriorating the level of the readiness and the fighting power. We suggest a method to improve recruit's maintenance capability rapidly by assigning airmen to jobs appropriate to their characteristics using Datamining methods (K-menas and CART). We focus on the assigning method for air force's air-munition maintenance airmen since they are requested more skilled than other airmen. Grouping airmen with k-means method and devising classification rule with CART algorithm, we found that airmen's proficiency arrival period could be shortened by 1.79 months when they are assigned in the suggested way.

  • 철도차량은 사용기간이 길고 철로 위를 달리기 때문에 마모, 진동이 심하여 다른 교통수단에 비해 정비비용이 많이 소요된다. 이에 따라 정비최적화는 매우 중요하다. 본 연구에서는 코레일에서 운용하고 있는 철도차량 정비현황을 분석해 보고 개선방안을 찾고자 하였다. 특히, 차량정비인력이 많이 소요되는 일상검수에 대해 가장 효율적인 정비 주기와 방법을 적용하고자 하였다. 철도차량은 컴퓨터시스템 적용으로 자가진단이 가능하고 과거에 비해 차량의 품질이 많이 향상되었으나, 현행 일상검수는 관례적으로 짧은 기간 시행해야 한다는 측면에서 현재 비효율적인 측면을 시급히 개선해야 함은 당연하다. 따라서 본 연구에서는 차량의 상태를 반영한 개선된 일상검수 주기를 적용하여 차량의 신뢰성 확보와 정비비용을 최소화하고자 한다.

  • The existing maintenance program is focused on time-based maintenance to inspect and repair components according to maintenance period, rather than condition-based maintenance or predictive maintenance. The preventive maintenance template of the steam turbine has been developed for optimizing maintenance procedure and improving reliability and availability of the steam turbine of nuclear power plants based on EPRI PM template methodology and EPRI technical reports about preventive maintenance.

노후 고츨아파트의 유지관리 및 개선실태 조사 (A Study on the maintenance situation of the deteriorted high-rise apartments)

  • The deteriorted high-rise apartments make a social problem in now days. As a result, and new alternative plan for deteriorted high-rise apartments such as the remodeling concept must be considered. This study aims to develop the remodeling items which is the guide lines when remodeling of the apartment will be conducted. Thorough the case studies of maintenance and repair status of the deteriorted apartments in seoul, reasonable guidelines for selection of remodeling items were developed. Physical maintenance items includes those that need major repair activity like wall painting, water proof roof, change/repairs/implement of heating system, boiler, city gas pipelines, cold and hot water pipes, elevators, cleaning of water tanks. The period of repair is shorter than the durable year of law or codes. As a result of the study, basic maintenance items and actual repair period were proposed. This result will be the basic guidelines for remodeling items.

The Analysis of User Cost according to Timing of National Highway Pavement Maintenance-Focusing on the Maesang Bidge Section in Korea

  • The traffic volume on the road shows various trends reflecting regional characteristics for monthly and hourly, and economic loss affecting users varies according to the selected period for the maintenance of road pavements. Therefore, in this study, the user cost (or delay cost) for monthly and hourly on the work zone near the Maesang Bridge Section on Poseung-Gu, Pyeongtaek-si, Gyeonggi-do was calculated using the time series models and VISSIM, and based on the result, the effect of user cost reduction according to the selection of best maintenance period was examined. The analysis result showed that the month of the lowest user cost occurred due to the maintenance of target section was January (10,293,258 KRW/Day×1km) and the month of the greatest user cost occurred was November (13,337,495 KRW/Day×1km).

  • The technology of preventive maintenance and corrective maintenance is widely applied to ships in order to maintain the good voyageable condition. One of the most important fields of marine engineering is to seek the maximum availability and to solve the stochastic maintenance problem such that the cost for corrective maintenance is minimized. Accordingly, for the purpose of making the most suitable maintenance schedule which minimizes the expected cost function, this paper suggests the method to grasp the failure characteristics by the ship's maintenance data that are collected from the past. And, suggests the method to estimate the optimal maintenance interval by using the dynamic programming and the cumulative hazard rate function attained from the maintenance data.
