A Case of Tuberculosis-associated Hemophagocytic Syndrome during Antituberculosis Medication for Tuberculous Pericarditis (결핵성 심막염으로 항결핵약을 복용하던 중 발생한 혈구 탐식증후군 1예)

  • A 63-year old woman was admitted to our hospital for an evaluation of thrombocytopenia. She had been diagnosed with tuberculous pericarditis three months earlier in a local clinic and treated with anti-tuberculosis medication. Two months later, thrombocytopenia developed. The medication was subsequently stopped because it was suspected that the anti-tuberculosis medication, particularly rifampin, might have caused the severe platelet reduction. However, the thrombocytopenia was more aggravated. A bone marrow biopsy was performed, which showed moderate amounts of histiocytes with active hemophagocytosis. This finding strongly suggested that the critical thrombocytopenia had been caused by hemophagocytic syndrome, not by the side effects of the anti-tuberculosis medication. Furthermore, the development of hemophagocytosis might have been due to an uncontrolled tuberculosis infection and its associated aberrant immunity. Therefore, she was started with both standard anti-tuberculosis medication and chemotherapy using etoposide plus steroid. One month after the initiation of treatment, the thrombocytopenia had gradually improved and she was discharged in a tolerable condition. At the third month of the follow-up, her platelet level and ferritin, the activity marker of hemophagocytic syndrome, was within the normal range.

Survey on the gross lesions of slaughtered pigs in Jeonbuk area, Korea (전북지역 도축 출하돈 병변 조사)

  • Respiratory disease in pigs is common in modern pork production worldwide and is often referred to as porcine respiratory disease complex (PRDC). PRDC is polymicrobial in nature, and results from infection with various combinations of primary and secondary respiratory pathogens. The control of swine respiratory disease requires an understanding of the interactions between the organisms that can cause this illness, the pig and management of the environment. This study was carried out to investigate the lesion of red internal organs in slaughtered pigs and provided assistant data for pig farms. A total of 900 lung samples, 45 farms were collected randomly from slaughtered pigs in Jeonbuk province from April to December in 2014. Gross lesions such as swine enzootic pneumonia (SEP), pleuritis, pleuropneumonia, pericarditis, liver white spots were examined for the pigs. Overall prevalence of SEP was 70.8%. According to season, the incidence occurred higher in summer than winter, fall and spring. The mean SEP score was 1.4, the highest incidence occurred in fall. The prevalence of pleuropneumonia, pleuritis, pericarditis, and milk spot was 26.1%, 71.4%, 2.8%, 21.6%, respectively. In the detection of pathogens, PRRS was not detected, PCV2 was positive in 87.6%.

Cardiac Hematoma with Pericardial Effusion in a Dog (개에서의 심낭삼출물을 동반한 심장혈종 1예)

  • A 12-year-old spayed female, Golden Retriever presented with dyspnea and lethargy of 3 months duration. Pericardial effusion and cardiac mass were identified on echocardiography. Pericardiectomy and mass resection were performed for treatment of pericardial effusion and histopathological analysis was conducted for the definitive diagnosis. The mass was diagnosed as hematoma with mainly erythrocyte, fibrin and many of neutrophils based on microscopic description. Severe purulent pericarditis thickened by inflammatory cells and reactive fibrous tissue were identified, but not diagnosed as neoplasm. This is the first case report in veterinary literature of a dog with pericardial effusion caused by cardiac hematoma.

Acute Suppurative Pericarditis Caused by Liver Abscess -1 case report- (간농양에 속발한 급성화농성 심 낭염 -1례 보고-)

  • Pericardial abscess due to liver abscess is a very rare disease, the result of which is usually fatal when untreated. But a combination of antibiotics therapy and pericardial drainage has broug t good result. A 32-year-old man was admitted to the emergency room because of fever and chilling sensation. Chest X-ray showed cardiomegaly and a mass lesion In the left lobe of liver on ultrasonography. The echocardiography revealed impending cardiac tamponade. Pus drainage of the liver abscess through per- foration of right diaphram and pericardiotomy were performed. The pericardium was thickened and 600 ml of purulent fluid and necrotic debris were drained. Fibrin clots were firmly attached to the undelying myocradium. Histological examination of the pericardium showed acute inflammation with microabsscess and Escherichia soli was cultured from pericardial pus. He was discharged after 4 weeks on antibiotics, but Doppler echocardiograpy revealed a slightly thickened pericardium with a mild constrictive physiology.

Effect of Postoperative Constrictive Physiology on Early Outcomes after Off-Pump Coronary Artery Bypass Grafting

  • Background: Constrictive pericarditis after coronary artery bypass surgery has been known to affect cardiac output by limiting diastolic ventricular filling. We aimed to assess the influence of postoperative constrictive physiology on the early outcomes of patients undergoing off-pump coronary artery bypass grafting (OPCAB). Materials and Methods: Between January 2008 and July 2011, 903 patients underwent an isolated OPCAB and postoperative transthoracic-echocardiography. The patient cohort was classified into two groups: group A, constrictive physiology and group B, control group without constrictive physiology. Early outcomes were analyzed between the two groups. Results: Of the total 903 patients, group A consisted of 153 patients (16.9%). The amount of blood loss in group A during the postoperative 24 hours was greater than that of group B, but this was not statistically significant (p=0.20). No significant differences were found in the mortality rates (group A, 0.6%; group B, 1.4%; p=0.40) and 30-day major adverse cardiac and cerebrovascular events (MACCEs; group A, 3.3%; group B, 6.1%; p=0.42). Conclusion: Postoperative constrictive physiology does not affect 30-day MACCEs or other major complications after OPCAB. The results of this study suggest that patients with early postoperative constrictive physiology do not need medical or surgical treatment, and that conservative care is sufficient.

  • Left atrial myxoma is a rare disease and its recurrence is reported to be always possible whenever primary excision is incomplete. Cardiac Myxoma is rare disease of the heart, and it has a little chance of recurrence. We experienced a patient of recurrent left atrial myxoma who was 31 years old, had been gotten excision of Lt. atrial myxoma when she was 28 years of her age. She was gotten mitral valve replacement simultaneously during 2nd operation with difficulty. So we report this case with the review of the literatures. [KTCS 1981;3:260-267]Surgical Treatment of Acute Pyogenic Pericarditis followed the Sepsis Ki Woo Shin, M.D.,* Ho Wan Lee, M.D.* and Dong Jun Lee, M.D.* Two cases of acute pyogenic pericarditis are, one case, 12-year-old male patient, followed the bacteremia of pneumonia and other case, 9 year old female patient, followed the bacteremia of osteomyelitis. After the confirmed diagnosis by pericardial aspiration, the emergency pericardial window was made to relief the severe cardiogenic symptoms. The general symptoms were improved immediately, but 40 days and 15 days after pericardiostomy, in each case, the sign and symptoms of cardiac compression were seen with recurrent cardiac tamponade. Pericardiectomy with median sternotomy was performed in each case and thereafter the patients were discharged without any problems.

Seasonal survey on the respiratory diseases of slaughtered pigs in Jeonbuk, Korea (전북지역 도축돈 호흡기질병 조사)

  • The present study was conducted to investigate the lesion of red internal organs in slaughtered pigs and provided assistant data for pig farms. During March to December 2015, a total of 1,160 lung samples out of 58 herds were collected randomly from pigs slaughtered in Jeonbuk province. In addition, 290 hilar lymph nodes from pig with pneumonic lung lesion (5 samples per herd) were screened for selected viral and bacterial pathogens. Gross lesions of lungs such as swine enzootic pneumonia (SEP), pleuritis, pleuropneumonia, pericarditis and liver white spots were examined. The overall prevalence of SEP was 64.3% (746/1,160). In the analysis of seasonal prevalence, there was an increase of occurrence during the spring months (287/400, 71.8%) and decrease during the fall months (93/200, 46.5%) among the whole herds. The mean number of SEP score per pig was $1.20{\pm}1.28$. The prevalence of pleuropneumonia, pleuritis, pericarditis, and milk spot was 25.5% (296/1,160), 44.1% (512/1,160), 3.8% (44/1,160) and 17.6% (204/1,160), respectively. The most frequent region with lung lesion was diaphragmatic lobes (left 17.1%, right 17.3%). In the detection of viral pathogens by PCR, porcine circovirus type2 (PCV2) was positive in 86.9% (252/290), while porcine reproductive and respiratory syndrome virus (PRRSV) was not detected, In the case of bacterial pathogens, 50 microorganisms were isolated by PCR and/or microbiological test. The most frequently isolated bacteria was Streptococcus suis (20, 34.4%), followed by Pasteurella multocida (17, 29.3%), Streptococcus spp. (11, 3.4%), Actinobacillus pleuropneumoniae (2, 8.9%).

  • In Behcet syndrome, cardiac involvements are rare and have been reported pericarditis, myocarditis, right heart endocardial fibrosis, right ventricle mural thrombus with pulmonary embolism, active endocarditis, granulomatous endocarditis, conduction disturbance, acute aortic insufficiency, mitral valve prolapse. Our three patients underwent AVR because of aortic insufficiency and ascending aorta enlargement combined with Behcet syndrome. Two patients had mitral regurgitation too. So one underwent MAP and the other underwent MVR concomitantly. One who underwent AVR have been well for 50 months. Another who underwent AVR+MAP and redo AVR due to aortic paravalvular leakage was died of congestive heart failure. The other who underwent AVR+MVR and repeated AVR three times because of aortic paravalvular leakage is in condition of aortic paravalvular leakage. Paravalvular leakage is considered to recur due to progressive dilatation and fragility of aortic root that is the result of pathologic change of Behcet syndrome in it. If Open heart surgery is needed in Behcet`s syndrome during inflammatory reaction is active, postoperative complications such as paravalvular leakage or suture line rupture may be prevented with pre- and postoperative anti-inflammatory management.

  • Epizootic streptococal infction is one of the common disease in guinea pig. We have confirmed 54 case of epizootic streptococal infection in guinea pig by histopathology and epidemiology. Grossly infection is characterized by the presense one to several enlarged lymph nodes particularly of the mandible cervical lymph nodes and accompanied by pleuropneumonia. Histologically there were lymphadenitis with abscesses surrounded by loose connective tissue fibrinopurulent pneumonia pleuritis myocarditis pericarditis and peritonitis. Epidemiological survey revealed that mortality was high in young adult female(15.1%) and changeable weather season such as spring and fall. From findings it was suggested that epizootic streptococal infection of guinea pig begin to infect in breeding colony around weaning period and died from that infection at animal facility during the quarantine period.

Mesalizine-Induced Acute Pancreatitis and Interstitial Pneumonitis in a Patient with Ulcerative Colitis

  • Ulcerative colitis (UC) is a chronic idiopathic inflammatory bowel disease. Mesalizine for the first-line therapy of UC has adverse effects include pancreatitis, pneumonia and pericarditis. UC complicated by two coexisting conditions, however, is very rare. Moreover, drug-related pulmonary toxicity is particularly rare. An 11-year-old male patient was hospitalized for recurring upper abdominal pain after meals with vomiting, hematochezia and exertional dyspnea developing at 2 weeks of mesalizine therapy for UC. The serum level of lipase was elevated. Chest X-ray and thorax computed tomography showed interstitial pneumonitis. Mesalizine was discontinued and steroid therapy was initiated. Five days after admission, symptoms were resolved and mesalizine was resumed after a drop in amylase and lipase level. Symptoms returned the following day, however, accompanied by increased the serum levels of amylase and lipase. Mesalizine was discontinued again and recurring symptoms rapidly improved.
