Purpose: The purpose of this study is to identify the factors affecting the role of nurses with regard to the perception of good death, knowledge of advance directives, and stress and attitude toward withdrawing life-sustaining treatment. Methods: Data were collected from 102 nurses working at a general hospital. Data were analyzed by descriptive statistics, independent t-test, one-way ANOVA, Scheffé test, Pearson correlation coefficient, and multiple linear regression using the SPSS 28.0.1.0 program. Results: Educational level (β=.37, p=.001 for bachelor; β=.33, p=.005 for master or higher) and good death perception (β=.22, p=.024) were significant factors of the role of nurses associated with withdrawing life-sustaining treatment. Conclusion: Based on the result of this study, information about awareness of good death should be included in development of an educational program in order to strengthen the role of nurses in withdrawing life-sustaining treatment of patients.
Purpose: This study was conducted to examine awareness of biomedical ethics, and to identify affecting factors of the awareness of biomedical ethics in nursing students. Methods: The subjects consisted of 266 nursing students their third and fourth years of study. The data were collected from October to December, 2015 by self-report using questionnaires. Data analysis was performed using SPSS/WIN 18.0, descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficient, and multiple regression analysis. Results: The mean score of the awareness of biomedical ethics was $2.81{\pm}0.22$, perception of death was $3.15{\pm}0.36$, and knowledge of brain death, organ donation, and organ transplant was $12.12{\pm}3.02$. The prediction factors of awareness of biomedical ethics were gender (${\beta}=.29$, p<.001), participation in religious activity (${\beta}=.23$, p=.015), and perception of death (${\beta}=.20$, p=.016). The explanation power was 17.1%. Conclusion: These results showed that education about biomedical ethics is necessary for nursing students, and the development of biomedical ethics educational programs should reflect affecting factors.
Purpose: This pilot study aimed to examine the influence of death counseling on perceptions, preparedness, and anxiety regarding death and dying among family caregivers of hospice patients. Methods: Death counseling developed based on the SPIKES model was provided to 37 family caregivers in a hospice and palliative care unit. Perceptions, preparedness, and anxiety regarding death were assessed with a self-administered structured questionnaire, and participants' scores before and after counseling were compared using the paired t-test. Results: Significant changes were found in perceptions, preparedness, and anxiety regarding death after counseling. Compared to before counseling, the scores for perceptions of death (t=-4.90, P<0.001) and preparedness for death and dying (t=-16.23, P<0.001) improved, while anxiety (t=3.72, P=0.001) decreased after counseling. Some changes were also found in the types of support that family caregivers needed to prepare for the death of their family members in the hospice care unit. Conclusion: The findings demonstrate that death counseling could help family caregivers prepare for the death of their loved ones. Hospice and palliative care providers should play a key role in supporting family caregivers of hospice patients by developing strategies for counseling.
목적: 본 연구는 우리나라 사람들이 인지하고 있는 존엄한 죽음의 의미를 참여자의 관점에서 있는 그대로 탐구하여 확인하고 기술함으로써 그 구조와 본질을 규명하기 위한 것이다. 방법: 현상학적 분석방법을 적용한 질적 연구로 본 연구의 참여자들은 연령대와 성별을 고려한 일반인으로 총 13명이었다. 자료수집은 참여자와 개별적 심층면담을 통해 이루어졌으며, 자료분석은 Colaizzi(1978)의 현상학적 분석방법을 이용하였다. 결과: 우리나라 사람들이 인지하는 존엄한 죽음은 4개의 범주와 9개의 주제모음 및 18개의 주제로 구조화할 수 있었다. 4개의 범주는 '편안한 죽음', '보기 좋은 죽음', '해결하고 죽는 죽음', '남긴 죽음'이었고, 9개의 주제모음은 '고통 없는 죽음', '자연에 순응하는 죽음', '흉하지 않은 죽음', '좋은 회상을 남기는 죽음', '원하는 방식으로 죽는 죽음', '주변을 정리한 죽음', '응어리를 풀고 가는 죽음', '잘 살고 죽는 죽음', '인정받는 죽음'이었다. 결론: 우리나라 사람들이 의미하는 존엄한 죽음은 고통 없이 편안하게 죽으면서 다른 사람에게 피해를 주지 않고 남아 있는 삶 동안 모든 일을 해결하고 가족을 포함한 다른 사람과의 관계 속에서 좋은 이미지로 남아야 하는 것으로 나타나 우리나라 사람들의 존엄한 죽음을 위하여 임종을 앞둔 대상자의 호스피스 간호에 적용할 수 있을 것이다.
본 연구는 일반인들 가운데에서 죽음을 어떻게 인식하고 있느냐에 따라 삶의 태도나 방식이 달라지기 때문에 행복에 따른 죽음인식에 차이가 있는지 알아보고, 행복지수가 높은 그룹과 행복지수가 낮은 그룹간의 호스피스 관심이 차이가 있는지 연구하고자 한다. 2017년 4월 22일 D 광역시에서 열린 걷기대회 행사에 참가한 20세 이상 성인 남녀 200명을 대상으로 설문조사를 실시하였다. 설문구성은 일반적 특성이 성별, 연령, 학력, 직업, 결혼상태, 가족 동거여부, 종교, 건강상태 8항목이었다. 행복에 관한문항은 총 29항목, 죽음 인식은 9항목으로 구성하였다. 통계 검정법으로는 설문지의 항목에 대하여 행복지수를 기준으로 카이제곱 검정, 독립표본 t-검정, 상관분석, 정준상관분석을 실시하였다. 연구결과, 임종하기 적합한 장소에 관한 문항에서 행복지수가 낮은 그룹은 "의료기관"이 46.3%로 가장 높았으며, 행복지수가 높은 그룹에서는 "살던 집"이 59.4%로 가장 높게 나타나 유의한 차이를 보였다. 병원에서 임종하는 경우가 훨씬 많은 우리나라의 현실에서 환자들이 원하는 편안하고 익숙한 장소인 집에서 임종을 맞이할 수 있는 방법인 가정형 호스피스의 이용률을 높일 수 있도록 전문 인력의 확보 및 교육 등 관리체계를 보완해야 할 것이라고 보여 진다. 그리고 대상자의 일반적 특성을 고려한 개별적이고 다양한 접근을 통하여 죽음인식을 높이고 호스피스를 효과적으로 이용할 수 있도록 하여야 할 것으로 사료된다.
목적: 본 연구는 웰다잉 교육프로그램이 간호대학생의 죽음준비와 죽음 및 웰다잉에 대한 인식에 미치는 효과를 확인하기 위하여 실시되었다. 방법: 연구설계는 비동등성 대조군 사전-사후 설계의 유사실험 연구였다. 대상자는 간호대학생으로서 실험군 32명, 대조군 36명이었다. 웰다잉 교육프로그램은 1회에 90~150분씩, 프로그램 소개와 죽음의 의미 탐색, 사망전 정리해야 할 일, 장례준비, 유언장 작성 등 5회에 걸쳐 워크숍으로 진행되었다. 수집된 자료는 SPSS Win 18.0 프로그램을 이용하여 t-test, ${\chi}^2$ test, paired t-test와 ANCOVA로 분석하였다. 결과: 웰다잉 교육프로그램을 적용한 중재 전 후에 실험군의 죽음준비인식이 통계적으로 유의하게 증가하였고(t=2.61, P=0.014), 중재 후 실험군의 죽음에 대한 인식은 대조군보다 유의하게 증가하였다(F=154.15, P<0.001). 그러나 웰다잉 인식에서는 유의한 차이가 없었다. 결론: 이상의 결과로 볼 때 죽음에 대한 체험을 바탕으로 한 워크숍 형태의 웰다잉 교육프로그램은 간호대학생의 웰다잉 인식에는 효과가 없었으나 죽음준비인식과 죽음에 대한 인식을 긍정적으로 변화시키는 것으로 나타나 죽음과 관련된 인식을 높이는 데에 도움이 되는 중재로 활용될 수 있다고 판단된다.
본 연구는 간호학과 1학년을 대상으로 호스피스 인식에 미치는 영향을 파악하여 간호대학생의 호스피스 인식과 죽음에 대한 태도를 향상시킬 수 있는 호스피스완화 간호중재 및 전략을 위한 기초자료를 제공하고자 시도되었다. G도에 소재한 J대학 간호학과 185명을 대상으로 자료수집 하였다. IBM SPSS WIN/25.0을 이용하여 t-test, ANOVA, Pearson correlation coefficient, Multiple regression을 사용하였다. 호스피스 인식은 도덕적 행동(r=.22, p=.002), 생명의료윤리 의식(r=.29, p<.001)과 상관관계가 있었다. 대상자의 호스피스 인식에 가장 영향을 미치는 요인은 생명의료윤리 의식(β=.224, p=.012), 그 다음으로, 부모님의 경제력(상)(β=.187, p=.027) 순이었다. 설명력은 11.5%이었다. 본 연구결과를 토대로 호스피스 인식 제고를 위한 대학생활 동안 도덕적 행동과 생명의료윤리 의식을 향상시킬 수 있도록 체계적인 프로그램이 필요하며, 호스피스 교육 프로그램 개발에 도움이 되는 기초자료로 활용될 수 있을 것이다.
Purpose: The purpose of this study was to understand the meaning of death experienced by medical and nursing students through end-of-life care practice. Methods: Data were collected by in-depth interviews with twelve (six nursing and six medical) students. Conventional qualitative content analysis was used to analyze the data. Results: This findings were analyzed in three areas: 'feeling from the word of death', 'color association of death', and 'relation between life and death'. Results were three major themes and sixteen categories from the analysis. Three major themes include 'reality of uncertain death', 'have to leave, and 'new perception about death'. Sixteen categories include 'being well', 'fear', 'unknown', 'boundless', 'being with', 'out of sight', 'new start', 'go back to', 'place going by itself', 'place to meet with', 'being transformed', 'a sense of futility', 'the same point', 'a different point', 'continuous line', and 'a crossroad'. Conclusion: The findings suggest a number of themes that nursing and medical students reported about the end of life experiences that could be explored as a way of improving end of life care.
End-of-life care competencies have been perceived as important and essential, so it has been suggested that end-of-life care be studied in undergraduate medical education. However, end-of-life care curriculum has mostly focused on acquisition of knowledge and skills rather than attitudes. Therefore, we aimed to explore whether education about death affects medical students' attitudes towards care for dying patients and perception of death anxiety, meaning in life, and self-esteem. A total of 15 first- or second-year medical students were surveyed with questionnaires before and after completing a 6-week death education course. Paired data analysis showed that participants' attitudes towards caring for terminally ill patients and their caregivers improved significantly (t=-2.84, p=0.013) with an effect size of 0.73. In contrast, no significant changes were found in death anxiety, meaning in life, or self-esteem. All participants agreed that formal teaching about death and dying must be encouraged in medical schools. Our results suggest that death education may positively influence attitudes towards end-of-life care. Although replication with larger samples is necessary, this preliminary finding may support the importance of developmentally appropriate end-of-life care education in medical schools.
본 연구는 좋은 죽음에 관한 연구 동향을 분석하고 종합하여 향후 좋은 죽음에 관한 연구의 방향을 제시하고자 시도되었다. 이를 위해 2011년부터 2021년까지 국내학술지에 발표된 좋은 죽음 관련 66편의 논문을 분석준거틀에 근거하여 분석하였다. 그 결과 좋은 죽음 연구가 가장 많이 이루어진 년도는 2019년(18.2%)이었다. 연구설계별 분석에서는 양적 연구가 52편(78.8%)으로 가장 많았고, 그 중 조사연구가 36편(54.5%)으로 나타났다. 연구분야에서는 간호학 논문이 47편(71.2%)으로 가장 많이 연구되었고, 다학제간 연구는 3편(4.5%)이었다. 좋은 죽음 연구대상자는 주로 지역사회 노인과 간호대학생으로 이 각각 13편(19.7%)으로 나타났고, 환자 대상연구는 4편(6.1%)으로 나타났다. 연구도구는 좋은 죽음 인식도구가 38편(73.1%)에서 사용되었다. 주요 연구 변인은 좋은 죽음 인식과호스피스완화의료 및 임종간호였고, 가족지지는 매개변인으로 사용되었다. 따라서 좋은 죽음을 종합적으로 이해하기 위해 다학제 연구와 다양하게 좋은 죽음을 측정할 수 있는 도구 개발을 제언한다.
