중도척수장애인의 삶의 질에 미치는 영향 요인 (Factors Influencing Quality of Life of People with Noncongenital Spinal Cord Injury)

  • Purpose: The purpose of the study was to examine the relationships among pain belief, perceived social support, coping strategies, and quality of life of people with noncongenital spinal cord injury and to identify factors influencing quality of life. Methods: A correlational predictive design was used. The data were collected from 197 people with noncongenital spinal cord injury with questionnaires in 2012 in Korea. The data were analyzed using descriptive statistics, t-tests, one-way ANOVA, Pearson's correlation coefficients, and stepwise multiple regression using SPSS/WIN 18.0. Results: Pain belief, perceived social support, and coping strategies were correlated significantly with the quality of life. As a result of stepwise multiple regression analysis, pain belief, perceived social support, coping strategies, damaged area, and time since injury were discovered to account for 59.1% variance of the quality of life. The variable that most affected the quality of life was pain belief followed by perceived social support and coping strategies. Conclusion: The results of the study clearly demonstrate the importance of pain control, social support, and coping skills in order to improve quality of life among people with noncongenital spinal cord injury.

  • 본 연구는 척수장애인의 사회통합에 미치는 요인을 연구함으로써 사회복지실천 현장에서 척수장애인의 사회통합을 위한 대안 마련에 기초자료를 제공하고자 시도되었다. 본 연구의 목적을 달성하고자 한국척수장애인협회의 산하조직 13개 지역협회의 협조를 얻어 각 지역 협회에 배포한 390부 중 회수된 설문지 335부 중에서 답변이 불성실한 21부를 제외하고 304부를 사용하였다. 척수장애인의 사회통합에 영향을 미치는 요인에 관한 구조방정식 최종모형의 경로별 효과분석과 유의성 검증을 토대로 신체 심리적 요인, 경제적 요인, 사회적 지지가 척수장애인의 사회통합에 미치는 영향을 비교한 결과 삶의 만족도는 신체 심리적 요인, 사회적 지지와 사회통합을 부분매개 하는 것으로 나타났다. 또한 경제적 요인과 사회통합에는 완전매개 하는 것으로 나타났다. 이러한 결과는 척수장애인의 삶의 만족도를 높이면 척수장애인의 사회통합의 목적을 달성할 수 있을 것이라는 것을 시사하고 있다.

척수손상 환자의 희망, 자아존중감과 삶의 질에 관한 연구 (Hope, Self-esteem and Quality of Life in People with Spinal Cord Injury)

  • Purpose: The purpose of the study was to identify levels of hope, self-esteem, and quality of life, and to find correlations among these variables in people with spinal cord injury. Methods: This study was a cross-sectional descriptive survey. The data were collected by survey interview in 2010 from 120 people with spinal cord injury living in the community. To measure hope, self-esteem and quality of life, the Dispositional Hope Scale, Self-Esteem Scale, and World Health Organization's Quality of Life Instrument were utilized respectively. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation and stepwise multiple regression analysis using SPSS version 12.0. Results: Mean score of hope was 2.56 and that of self-esteem was 3.23. Mean score of quality of life was 3.01. Statistically significant relationships were found between quality of life and hope (r=.73, p<.001), and between quality of life and self-esteem (r=.67, p<.001). Multiple regression analyses showed that hope and self-esteem were statistically significant in predicting quality of life with the explanatory power of 59.1%. Conclusion: The results of this study indicate that nursing interventions fostering hope and self-esteem should be integrated in developing rehabilitation programs to improve quality of life for people with spinal cord injury.

  • The failure of injured axons to regenerate in the mature central nervous system (CNS) has devastating consequences for victims of spinal cord injury (SCI). Traditional strategies to treat spinal cord injured people by using drug therapy and assisting devices that can not help them to recover fully various vital functions of the spinal cord. Many researches have been focused on accomplishing re-growth and reconnection of the severed axons in the injured region. Using cell transplantation to promote neural survival or growth has had modest success in allowing injured neurons to re-grow through the area of the lesion. Strategies for successful regeneration will require tissue engineering approach. In order to persuade sufficient axons to regenerate across the lesion to bring back substantial neurological function, it is necessary to construct an efficient biocompatible bridge (cell-free or implanted with different cell lines as hybrid implant) through the injured area over which axons can grow. Therefore, in this paper, spinal cord and its injury, different strategies to help regeneration of an injured spinal cord are reviewed. In addition, different aspects of designing a biocompatible bridge and its applications and challenges surrounding these issues are also addressed. This knowledge is very important for the development and optimalization of therapies to repair the injured spinal cord.

척수장애 성인남성의 신체측정치 분석 (Analysis of Body Size of Disabled Men with Spinal Cord Injuries)

  • Today the number of the disabled people with a spinal cord injury is increasing by the acquired factor. The purpose of the study is to furnish basic data for the study of satisfactory clothing construction data by considering the physical variation by the time passed the since the injury. Also the study includes the actual measuring of the disabled men with spinal cord injuries which is compared with the data of the 5th national anthropometric survey of Korea and makes it possible to grasp the distinguished physical feature of the disabled men with the spinal cord injury. As a result, first, the measured data are smaller than those of the 5th national anthropometric survey of Korea in the items of height and length of the upper half of the body related to stature. Second, the measured data are bigger than those of the 5th national anthropometric survey of Korea in the items of breadth, depth and circumference of the upper half of the body related to the exercise. Third, there are differences in the items of depth of the lower half of the body, breadth and circumference in the analysis of the time passed since the injury.

가상현실 시스템(Virtual Reality System)을 이용한 훈련이 척수손상환자의 앉기 균형 및 일상생활동작에 미치는 영향 (The Effect of Training Using Virtual Reality System on Sitting Balance and Activities of Daily Living for the Patient with Spinal Cord Injury)

  • Purpose: This study was examined the effect of training using a Virtual Reality System on the sitting balance and Activities of Daily Living for patients with a spinal cord injury. Methods: The subjects were divided into an experimental (6 persons) and control group (5 persons). The experimental group trained the 5 programs, three times per week for 6 weeks using the Virtual Reality System and five days for week using conventional physical therapy. The control group trained five days for a week using conventional physical therapy. Results: The difference in the mean Spinal Cord Independence Measurement (SCIM) score in the experimental and control groups was increased to 8.33 and 6.60 (p=0.79), respectively. The difference in the mean functional reaching test in experimental and control group increased to 4.21 and 1.09 (p=0.25), respectively. The difference in the mean sitting time in experimental and control group increased to 41.05 and 10.33 (p=0.66), respectively. There was a difference in the mean of all variances but these differences were not statistically significant. Conclusion: These results suggest that training using the Virtual Reality System increased the SCIM, functional reaching test and sitting time in people with a spinal cord injury.

척수장애인의 성적적응과 관련요인에 관한 모형구축 (Model Construction of Sexual Adjustment of Patients with Spinal Cord Injury)

  • The purpose of this study was to construct model of sexual adjustment in people with spinal cord injury and to determine factors that relate to sexual adjustment using methodological triangulation. A total of 134 persons who were registered members of spinal cord injury organization and admitted rehabilitation unit in the hospital were included in the study. Participants answered questionnaire concerning importance of life events, sexual concern, sexual adjustment. Qualitative data were collected through semi-structured indepth interviews from 10 individuals with spinal cord injury who were previously included in the quantitative study. Constant compatative method was used to analyze the data. The results were as follows : 1) With respect to eleven other areas of life, sex life ranked the sixth and economic status ranked the highest in terms of importance. However social life ranked the lowest among the 11areas. 2) Among seven topics related to sexuality were methods and techniques to achieve sexual satisfaction, and helping a partner cope emotionally with limitation on sexual dysfunction was the second greastest. 3) The mean score for sexual adjustment was 19.47 which can be considered. 4) A process on how individuals with spinal cord injury adjust to their changed sexual life immerged from the qualitative data. It includs 4 stages: 'stage of loss' 'stage of endeavoring' 'stage of effort' and 'stage of adjustment'. Categories showing context for the action/interaction strategies were 'steadiness' and 'rediscovery as a sexual being'. There were three factors which may stimulate the adjustment process while the others may interrupt it. Those factors included personal matters, family matters and social matters. The individuals may follow each stage step by step but may go back to the previous step depending on the outcomes of their adjustment. 5) There were three factors which may stimulate the adjustment process while the others may interrupt it. 척수장애인의 성적관심과 성적적응에 관한 연구 (Sexual Interest and Adjustment for Spinal Cord Injury Patients)

  • The purposes of this study were to identify sexual interest and adjustment in patients with spinal cord injury and to determine the factors that relate to sexual adjustment. A total of 134 persons in this study was the members of spinal cord injury organization and the impatients in rehabilitation unit between February and June 1999. Five questionnaires were answered concerning importance of life events, sexual concern, sexual adjustment, relationship with a sex partner, and depression. The collected data were analyzed by Frequency, t-test, ANOVA and pearson correlation. The results were as follows : 1) Considering the importance among 11 areas of life, economic status occupied the top, sex life held the sixth rank, and the mean rating for social life being 3.78 was the lowest of all. 2) As regard to 7 topics related to sexuality, the most attention was drawn to methods and techniques achieving sexual satisfaction, and the next interest was for helping partners cope emotionally with sexual dysfunction. 3) The mean for sexual adjustment and depression was average, and the mean for sex partner relationship was high. 4) Sexual adjustment in spinal cord injury patients correlated with economic status and sex partner relationship. While It was inversely correlated with age and depression. It was found that lower age and less depression play a positive role on sexual adjustment by SCI people. Also, higher economic status and favorable sex partner relationship increase sexual adjustment. In conclusion, a sex partner relationship, depression, economic status, and age seemed to influence on their sexual adjustment after SCI. Also, psychosocial factors would be more important for satisfying sexual life and relation ship rather than physical factors.

Isoflurane Preconditioning Induces Neuroprotection by Up-Regulation of TREK1 in a Rat Model of Spinal Cord Ischemic Injury

  • This study aimed to explore the neuroprotection and mechanism of isoflurane on rats with spinal cord ischemic injury. Total 40 adult male Sprague-Dawley rats were divided into the four groups (n=10). Group A was sham-operation group; group B was ischemia group; group C was isoflurane preconditioning group; group D was isoflurane preconditioning followed by ischemia treatment group. Then the expressions of TWIK-related $K^+$ channel 1 (TREK1) in the four groups were detected by immunofluorescent assay, real time-polymerase chain reactions (RT-PCR) and western blot. The primary neurons of rats were isolated and cultured under normal and hypoxic conditions. Besides, the neurons under two conditions were transfected with green fluorescent protein (GFP)-TREK1 and lentivirual to overexpress and silence TREK1. Additionally, the neurons were treated with isoflurane or not. Then caspase-3 activity and cell cycle of neurons under normal and hypoxic conditions were detected. Furthermore, nicotinamide adenine dinucleotide hydrate (NADH) was detected using NAD+/NADH quantification colorimetric kit. Results showed that the mRNA and protein expressions of TREK1 increased significantly in group C and D. In neurons, when TREK1 silenced, isoflurane treatment improved the caspase-3 activity. In hypoxic condition, the caspase-3 activity and sub-G1 cell percentage significantly increased, however, when TREK1 overexpressed the caspase-3 activity and sub-G1 cell percentage decreased significantly. Furthermore, both isoflurane treatment and overexpression of TREK1 significantly decreased NADH. In conclusion, isoflurane-induced neuroprotection in spinal cord ischemic injury may be associated with the up-regulation of TREK1.

Ginseng extract and ginsenosides improve neurological function and promote antioxidant effects in rats with spinal cord injury: A meta-analysis and systematic review

  • Spinal cord injury (SCI) is defined as damage to the spinal cord that temporarily or permanently changes its function. There is no definite treatment established for neurological complete injury patients. This study investigated the effect of ginseng extract and ginsenosides on neurological recovery and antioxidant efficacies in rat models following SCI and explore the appropriate dosage. Searches were done on PubMed, Embase, and Chinese databases, and animal studies matches the inclusion criteria were selected. Pair-wise meta-analysis and subgroup analysis were performed. Ten studies were included, and the overall methodological qualities were low quality. The result showed ginseng extract and ginsenosides significantly improve neurological function, through the Basso, Beattie, and Bresnahan (BBB) locomotor rating scale (pooled MD = 4.40; 95% CI = 3.92 to 4.88; p < 0.00001), significantly decrease malondialdehyde (MDA) (n = 290; pooled MD = -2.19; 95% CI = -3.16 to 1.22; p < 0.0001) and increase superoxide dismutase (SOD) levels (n = 290; pooled MD = 2.14; 95% CI = 1.45 to 2.83; p < 0.00001). Both low (<25 mg/kg) and high dosage (25 mg/kg) showed significant improvement in the motor function recovery in SCI rats. Collectively, this review suggests ginseng extract and ginsenosides has a protective effect on SCI, with good safety and a clear mechanism of action and may be suitable for future clinical trials and applications.
