Musculocutaneus Island Flap Based on the Distal Vascular Pedicle of Gracilis Muscle (박근의 원위혈관경을 이용한 도서형 근피판술)

  • The gracilis that is frequently used as a donor of free muscle trasfer is appropriate in the muscular shape and vascular position. This muscle is belonged to the second type of muscle group by the classification of the pattern of muscular nutrient vessel. The adductor branch or first perforating branch of deep femoral artery which supplies the proximal 1/3 of this muscle is a dominant one and this is used for the microscopic anastomosis of muscle or musculocutaneous flap. The minor vascular pedicles which enter the distal 1/3 of this of this muscle are branches of the superficial femoral artery and it is 0.5mm in diameter, 2cm in length with two venae comitantes. These minor pedicles supplies distal half of the gracilis muscle. This island musculocutaneous flap using distal vascular pedicle can be used to cover the defect of soft tissue around the distal femoral supra-condylar area, knee joint and proximal tibial condyle area which cause limitation of motion of knee joint, or in the cases that usual skin graft is impossible. The important operative procedure is as follows; The dissection is carried proximally and distally and the entire gracilis muscle including proximal and distal pedicle is completely dissected. After temporary blocking of the proximal vascular pedicle, the adequate muscle perfusion by the distal pedicle is identified and it is rotated to the recipient site around knee joint. The advantages of this procedure are simple, no need of microscopic vascular anastomoses and no significant functional loss of donor site. Especially in the cases of poor condition of the recipient vessel, this procedure can be used effectively. From 1991 to 1996, we performed 4 cases; complete survival of flap in 3 cases and partial survival of flap with partial necrosis in 1 case. This procedure is though to be useful in the small sized soft tissue defect of distal femoral supra-condylar area, knee joint and proximal tibial condylar area, especially in the defect of anterior aspect which expected to cause limitation of motion of knee joint due to scar contracture. But the problems of this procedure are the diameter of distal vascular pedicle is small and the location of distal vascular pedicle is not constant. To reduce the failure rate, identify the muscular perfusion of distal vascular pedicle after blocking the proximal pedicle, or strategic delay will be helpful.

Study on the textuality of Haedongyeongeon[해동영언] in Mansebo[만세보] ("만세보(萬歲報)" 소재(所載) <해동영언(海東永言)>의 텍스트성 연구)

  • Mansebo[만세보] contains a total of 111 old shijos under the title of Haedongyeongeon[해동영언]. This dissertation presumes Haedongyeongeon[해동영언] as early 20th century shijo text and surveys its literary characteristic and its significance in relation with anthological compilation. Haedongyeongeon can be seen as both newspaper serials and a short anthology. The basic pattern of the serials shows an organization of 'title. musical designation, author. text. and a brief review. Of these, the review is what most clearly shows the characteristic of the serials. The review is written in Chinese followed by Korean letters to designate the sound of the Chinese. which is presumably designed to attract more readers for the newspaper. On the other hand, Haedongyeongeon[해동영언], when seen as a collection of works printed in serials, clearly shows an intention of compiling an anthology, particularly in its way of overall classification of works or arranging works according to their authors, and thus may well be defined as a short anthology. This anthology somewhat excessively pursues perfection in formality, and is characterized by its strong intent to be read as popular literature, and therefore could be said to manifest the general characteristic of 20th century anthologies. The planner of the serial Haedongyeongeon[해동영언], or the compiler of the anthology is thought to be one of the core figures of Mansebo[만세보], that is, O Sechang[오세창], Lee Injik[이인직], Choi Yeongnyeon[최영년], Shin Gwanghui[신광희], but of them all, considering all circumstances, Choi Yeongnyeon[최영년] is most likely to be the one. Lastly, it is presently unknown what anthology was used as the basis of Haedongyeongeon[해동영언] and accordingly any judgement on that head has been deferred.

Characteristics for Heavy Metal Pollution in Road Dust from Daebul Industrial Complex: Classification by Particle Size and Magnetic Separation (대불산업단지 도로먼지 내 중금속류 오염 특성: 입도와 자성에 따른 구분)

  • In this study, we investigated physical and chemical properties such as grain size, heavy metal pollution, magnetic properties, and their environmental impacts of road dusts (RD) collected from 14 sampling points in Daebul industrial Complex. Heavy metal concentrations in RD were in the order of Fe>Zn>Cu>Pb>Cr>Ni>As>Cd>Hg, and this pollution pattern was related to major industries and traffic activities in this area. The results of the correlation analysis between heavy metal elements and particle size in RD showed that Fe and all of analyzed heavy metals had a significant correlation with each other and metal concentrations had a significantly negative correlation (p<0.05). However, due to the input of large metal particles some heavy metal concentrations in the particle fraction of >1000 ㎛ were highest. Pollution load per unit area of this fraction was the highest among the grain size fractions. Cr, Ni, Cu, Zn, Cd, Pb levels in RD decreased and the levels of Cr, Ni, Cu, Zn, Cd, and Pb were reduced to 85 (As) -22 (Ni)% of the whole after removal of MFs fraction from RD. The mean heavy metal levels in the study area did not exceed the soil contamination guide value of Korea, indicating that heavy metal levels in RD were not a concern. However, at some sampling points, Zn concentrations were exceeded the soil contamination guide value for the 3rd areas of Korea and this result indicated that further studies of the impact of RD on the surrounding environment through re-suspension or non-point pollution, and of effective management methods are required.

  • Acanthnmoebn sp. YM-4 is simitar to A. culbertsoni based upon morphological characteristics of trophozoites and cysts. However, based on other characteristics, pathogenicity to mice, in uitro cytotoxicity and isoenzyme patterns, Acanthomoebo sp. YM- 4 was quite different from A. culbertsoni. Restriction fragment length polymorphism (RFLP) analysis of mtDNA is useful in the classification of members belonging to the genus Acanthcmoebn. Therefore, in this study, RFLP analysis of Acnnthcmoeba mtDNAs was accomplished using five restriction enzymes: Hnelll, Hinull, Clcl, Pudl and ScE. Each restriction enzyme produced approximately 3-15 fragments (range: from 0:6 kip to 34.4 kbp) . The mtDNA genome size, calculated by the summation of restriction fragments, averaged 46.4 kbp in Acnnthamoeba sp. YM-4,48.3 kbp in A. culbertsoni and 48.8 kbp in A. polyphaic, respectively. Digested mtDNA fragments of Accnthcmoeba sp. YM-4 contained nine and seven same size fragments, respectively, from a total of 67 and 69 fragments observed in A. culbertsoni and A. polyphcgn. An estimate of the genetic divergence was 10.1% between Acanthamoebc sp. YM-4 and A. culbertsoni, and 9.9% between Acanthamoebn sp. YM-4 and A. polyphcga.

A Study on the Regional Function of Health Care by the Disease Pattern of the Inpatients (입원환자 질병유형의 구성에 의한 지역별 진료기능에 관한 연구)

  • The objectives of the study were to provide the basic informations needed in the development of balanced medical services throughout the nation. As the national health care system was expanding rapidly along with the economic growth, quantitative re-evaluation of the system is of great need. For that reason, characteristics of the admitted patients were analyzed for the case-mix and patients' flow within and through regions. Materials were 421,530 cases of inpatients, who were reported through Korea Medical Insurance Corporation(KMIC) for insurance claim, during the period of March 1, 1985 through February 28, 1987. Korean Diagnosis Related Groups(K-DRGs) classification system was adopted for the study of case-mix and 189 cities and counties were classified into 5 district groups by factor analysis results of K-DRGS. The major findings of this study were as follows ; 1) Factor analysis of case-mix, employing K-DRG system, revealed 5 distinct funtional district groups. Group A(18 districts) was prominent for tertiary medical care. In group B(36 districts), rather simple procedures were prevalent. Group C(26 districts) was distinctive for the medical care of well organized internal medicine practices with qualified clinical laboratories. Group D(17 districts) was characterized by relatively high balanced medical care. Group E (92 districts) was with very low level of medical care. 2) Analysis of the case-flow through the districts showed 3 types of flow patterns : inflow, outflow, and balanced types. Inflow type of case-flow was found in Group A, C and D while Group B and E showed outflow type. Inflow was most prominent in Group A and Group E was of typical outflow type. Group B was consistently the outflow type except for Major Diagnostic Category XX regardless of the disease treaters, but Group C and D were inflow or outflow types according to the disease tracers.

A study on correlation between electrical resistivity obtained from electrical resistivity logging and rock mass rating in-situ tunnelling site (전기비저항 검층으로 얻은 전기비저항과 터널 현장 암반등급의 상관관계에 관한 연구)

  • Rock mass rating (RMR) is the key factor when designing the appropriate support pattern of tunnel projects. Borehole drilling is usually performed along the tunnel route in order to determine the rock mass rating to be used for tunnel design. The rock mass rating at the non-boring region between boreholes is usually assessed through geophysical surveys such as electrical prospecting, seismic prospecting, etc. Many studies were carried out to find out the correlation between electrical resistivity and rock mass rating. However, most researches were aimed at obtaining the relationship between the two parameters utilizing experimental results obtained from laboratory tests or electrical prospectings. In this paper, efforts were made to analyze and obtain relationships between the electrical resistivity obtained from in-situ electrical resistivity logging data and the rock mass rating. Correlation studies using field data showed that the electrical resistivity is highly correlated with the rock mass rating with the determination coefficient more than 90%. The correlation analysis was also carried out between RMR classification parameters and the electrical resistivity. It was shown that the correlation between the condition of discontinuities and the electrical resistivity was very high with the determination coefficient more than 80%; that between the groundwater condition and the electrical resistivity was very low with the determination coefficient less than 57%.

Study on inhibition effects on metastasis and recurrence of Traditional Herb Medicine(THM) (한약의 전이재발억제 효과에 관한 연구)

  • Recently, the occurrence and death rates of cancer have increased rapidly. Metastasis and recurrence is the most important factor of death from cancer. So inhibition of recurrence and metastasis can increase the cure rate of cancer disease. At the basic chinese traditional medicine(TCM), there are a lot of theory related to neoplasm treatment. The metastasis and recurrence of neoplasm was the basis of yudu(餘毒) on remained neoplasm cell and stagnation of blood, thermotoxo, phlegm, asthenia of healthy enerngy and stagnation of vital energy. The principles therapy of neoplasm on metastasis and recurrence was based on knowledge of the method of support the healthy energy and strengthen the body resistance, promote blood circulation to remove blood stasis, clear away heat and toxic materials, dissipate phlegm and disperse the accumulation of evils. But the major clinical features of neoplasm was to be considered in developing a treatment plan include (1) distinguish between clinical and pathologic staging - acute and chronic, (2) classification of pathologic pattern, and (3) distingction of body situation : for examples asthenia - sthenia etc. It was most important to distinguish between supporting the healthy and eliminating the evil factors and to treat differently at the root and the branch cause of a neoplasm. This paper's results indicate that identification and effective use of THM can inhibite netastasis and recurrence and then it will help increasing survival rate. Also as BioTechnology(BT) has developed rapidly nowadays, as genes related to cancer have revealed. So it will give rise to the development of searching the mechanism of herb medicines which have inhibition effects of metastasis and recurrence.

Classification and Characterization of Bacteriophages of Lectobacillus casei (Lactobacillus casei Bacperiophage의 분류 및 특성에 관한 연구)

  • Phages of Lactobaciilus casei (PLC) isolated from plant drainage were classified and characterized. The results are as follows : 1. On the basis of host range pattern, phages could be divided into 2 groups (PLC-B and PLC-C). PLC-B group phages could be further divided into 5 sub-groups $(B_1, \;B_2, \;B_3, \;B_4, \;and\;B_5)$. Although PLC-C group phages had the same host range, they could be also divided into 2 sub-groups $(C_1\;and\;C_2)$ by morphlogical type. 2. It was $B_3$ group phages that represented a major proportion (44.4%) of phages tested. However, $B_1$ group phages were shown to have the widest host range. 3. Electron micrographs revealed that the phages fell into three different morphological types. $(B_1, \;B_2, \;and\;B_3)$ group phages hd a hexagonal head (52nm in diameter) and a sheathless noncontractile (245 nm in length). $B_4\;and\;C_2$ group phages had a hexagonal head (56 nm) and a short flexible tail (169nm) having no sheath. $B_5\;and\;C_1$ group phages were shown to have a hexagonal head (81 nm) and a contractile tail (140 nm) having a sheath, a base plate and tail fibers. 4. The inactivation of the phages by antisera indicated that serological relationships correlated completely with morphological types. 5. $B_1, \;C_1\;and\;C_2$ group phages produced a large (1, 2 mm in diameter) plaque with a clear ring. The morphology of plaques of $B_3\;and\;B_5$ group phages was the same as those produced by the above, but the average plaque sizes for $B_3\;and\;B_5$ were 0.8 mm abd 0.5 mm, respectively. $B_2\;and\;B_4$ group phages produced a small (0.5 mm) turbid plaque with an irregular edge. 6. The latent period and the average burst size of $B_1\;and\;B_3$ group phages were 90 min and 100, respectively. These phages reuqired calcium ions for their miltiplication. 7. $B_3$ group phages could not be absrobed to R-variant $KC_1$. 8. The order of resistance of phages to heat was $B_2\;>\;B_1, B_4\;and\;B_5\;>\;B_3\;and\;C_2, \;B_5$ group phages were more stable than $B_3$ in various pH values. $C_2$ group phages were more sensitive to UV irradiation than $B_1\;and\;B_3$ group phages. 9. Strains YIT9018 and IAM 1043 were induced by mitomycin C treatment. Phage particles detected in the lysates had a hexagonal head (38 and 49 nm, respectively), but no tail. Any sensitive indicator strain could not be isolated in spite of repaeated trials.

Hydrolytic and Metabolic Capacities of Thermophilic Geobacillus Isolated from Litter Deposit of a Lakeshore (수변 낙엽퇴적층에서 분리한 호열성 Geobacillus의 물질 분해 특성)

  • To understand contribution of thermophilic microorganisms in decomposition of litter deposits on shore of lakes, we surveyed a lakeshore litter deposit for bacteria growing at $60^{\circ}C$. Ten thermophilic isolates were selected for in-depth characterization, based on their high capacity to degrade high molecular weight organic compounds. Based on phylogenetic analysis on their 16S rRNA gene sequences, all isolates were identified as Geobacillus. The optimal growth temperature and pH of the strains ranged $55{\sim}60^{\circ}C$ and 6.0${\sim}$8.0, respectively. Salinity was inhibitory to the growth of the isolates, showing marked decrease of growth rates at 3% salinity. Based on activities of hydrolytic enzymes and profiles of carbohydrate utilization (determined by API 50 CHB kit), three G. stearothermophilus strains showed patterns clearly distinctive from other isolates. Two G. kaustophilus strains also demonstrated distinctiveness in their metabolic pattern and ecological parameters. However, ecological and metabolic profiles of the other five isolates were more variable and showed some degree of digression from their phylogenetic classification. Therefore, it could be concluded that endospore-forming thermophilic bacteria in lakeshore litter deposits contribute to degradation of organic materials with diverse ecological niches while having successions similar to microbial flora in compost. We propose that the thermophilic isolates and/or their thermo-tolerant enzymes can be applied to industrial processes as appropriate mixtures.

A STUDY FOR THE CHANGES OF THE MASTICATORY MUSCLES AND THE MANDIBULER MOVEMENT EFFECTED BY INTENTIONAL INCREASE OF ANTERIOR GUIDANCE ANGLE (전치 유도각의 인위적 증가에 의한 저작근과 하악 운동 양상의 변화에 대한 연구)

  • This study was performed to measure the changes of the mandibular movement and the masticatory muscular activities - anterior temporal and masseter muscle of both side - reflected by intentional increase of anterior guidance angie. For this study, 5 volunteers (3 males and 2 females with average age of 24.0) were selected. Each volunteer had Angle's classification I and did not have any missing tooth except third molar and any extensive restorations. Metallic guide plate was made at volunteer's working model fabricated by improved dental stone and cemented to the palatal surface of maxillary central incisor using resin cement(Panavia $21^{(R)}$) and then adjusted not to give any occlusal interferences at intercuspal position. The activity of masticatory muscles and the changes of mandibular movement were recorded by EMG and Sirognathograph in Biopak analysing system(Bioresearch Inc., Milwaukee, Wisconsin, USA). Measurement was done at before experiment, immediatley after placement, 1 week after placement, immediately after removal, and 1 week after removal. The results were as follows: 1. Moderate phonetic disturbance and mild headache were occured to 3 volunteers for 2 days after setting and 1 volunteer had positive reaction to percussion and slight midline diastema. But all of these clinical signs were diappeared 1 week after removal and the other volunteer did not have any special clinical sign. 2. In the EMG of the mandibular rest position, the mean value of anterior tempotal muscle was increased immediately after placement(p<0.01) and then decreased 1 week after placement(p<0.05) and increased 1 week after removal(p<0.05) but not recovered as before experiment. The mean value of masseter muscle was decreased during the experiment period. 3. In the EMG during mandibular protrusive movement, all muscular activity was decreased during the experiment period. Reduced activity was not recovered 1 week after removal(p<0.03). 4. During the habitual opening, anteroposterior movement of mandible was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not statistically significant(p>0.1). Vertical movement was not shown significant difference during the experiment period(p>0.1). Lateral movement was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not recovered as before experiment. The opening and closing velocity of mandible was shown minor changes but not statistically significant. 5. During the habitual opening, anteroposterior movement of mandible was decreased 1 week after placement(p<0.05) and then increased immediately after removal and recovered 1 week after removal as before experiment. Vertical movement was not shown significant changes. Lateral displacement of mandible was increased continuously and recovered 1 week after removal. Opening velocity was temporarily increased immediately after removal but recovered and closing velocity was not shown significant changes. 6. During the right side chewing, anteroposterior movement of mandible was increased immediately after removal but recovered and vertical movement was not shown statistically significant results. Lateral displacement and velocity of mandible were not shown significant results. 7. During the left side chewing, the changes of mandibular movement pattern were not shown statistically significant results.
