The humeral suspension technique: a novel operation for deltoid paralysis

  • Isolated deltoid paralysis is a rare pathology that can occur after axillary nerve injury due to shoulder trauma or infection. This condition leads to loss of deltoid function that can cause glenohumeral instability and inferior subluxation, resulting in rotator cuff muscle fatigue and pain. To establish dynamic glenohumeral stability, a novel technique was invented. Humeral suspension is achieved using a double button implant with non-resorbable high strength cords between the acromion and humeral head. This novel technique was used in two patients with isolated deltoid paralysis due to axillary nerve injury. The results indicate that the humeral suspension technique is a method that supports centralizing the humeral head and simultaneously dynamically stabilizes the glenohumeral joint. This approach yielded high patient satisfaction and reduced pain. Glenohumeral alignment was improved and remained intact 5 years postoperative. The humeral suspension technique is a promising surgical method for subluxated glenohumeral joint instability due to isolated deltoid paralysis.

Literature Review on the Development of Cognitive Function Improvement Program for the Elderly in Community (지역사회 노인의 인지기능 향상 프로그램 개발에 대한 문헌적 고찰)

  • Objective: This study was to compares and analyzes programs applied to improve cognitive function in patients with mild cognitive impairment and early dementia in the community to find out their effectiveness. Methods: In this study, 12 papers were finalized by searching for "elderly", "cognitive", "community", and "program" using the database of the Research Information System (RISS), National Assembly Library, and Korean Studies Information (KISS). Results: Programs for cognitive function were in the order of cognitive stimulation program, arts and crafts, and exercise program. In the program, rather than applying the cognitive stimulation program alone, the program was operated by combining leisure or exercise, music, art, and handicraft. The time was shown to be 30 minutes. The most frequently used evaluation tool was MMSE, followed by GDS and BBS. By cognitive domain, cognitive stimulation program and memory, satisfaction in psychology, and balance ability in exercise were evaluated the most. In the cognitive area, various cognitive stimulation areas were included, and in the exercise area, basic exercise, muscle strength exercise, joint exercise, and balance exercise were applied. Conclusion: Therefore, developing a program to improve cognitive function for mild cognitive impairment, it will be possible to prepare guidelines to establish and development.

Measuring and Evaluating the Work-Related Stress of Nurses in Saudi Arabia during the Covid-19 Pandemic

  • Prior to the emergence of Covid-19, Saudi Arabia (SA) had never faced the challenge of dealing with a global pandemic. Significantly, the current crisis has impacted all industries and sectors in the country, including the healthcare system, and has led to an emphasis on human life being more precious and valuable than economic profit. This study focuses on the impact of Covid-19 on the health of nurses, including their quality of life, during 2020. Understanding the position of the nursing profession during the pandemic, including the most effective methods of preventing work-related stress is important. Information was acquired through an online survey method (i.e. self-completion), known as the Expanded Nursing Stress Scale (ENSS), which was distributed to nurses in all regions of SA. It was found that the main aspects impacting nurses' work-related stress include gender, employment type, training, and dealing with infected patients. In addition, they highlight that such stress plays a substantial role in patient safety and nurses' satisfaction at work, as well as the future survival of organizations. The emergence of Covid-19 as a novel infectious disease has increased nurses' uncertainty and work-related stress. The results of this research will provide insights into the views of both nurses and their managers, in order to identify the main indicators of stress.

Predictive Factors of Nurses' Turnover Intention during the COVID-19 Pandemic (COVID-19 유행 동안 간호사의 이직의도 예측요인)

  • Purpose: Nurses are exposed to stressful circumstances during the COVID-19 outbreak, which increases their turnover intention. This study aimed to identify predictive factors of turnover intention among nurses during the COVID-19 outbreak. Methods: This study was a cross-sectional design using self-report questionnaires. A total of 164 nurses working at three hospitals participated voluntarily in the study. Questionnaires included nursing professionalism, job satisfaction, control belief and COVID-19 stress. Data were collected from February 1, 2021, to March 15, 2021. Data were analyzed with SPSS WIN 23.0 program using descriptive statistics, x2-test, independent t-test and logistic regression. Results: The turnover intention rate of nurses was 33.5%. Two significant predictors of nurses' turnover intention were identified, including risk acceptance in caring for patients with COVID-19 (odds ratio [OR] 4.24, 95% confidence interval [CI]=1.76~10.49) and nursing professionalism (OR=1.06, 95% CI=1.02~1.10). Conclusion: Hospital administrations should provide an improved work environment, such as individual protective equipment, compensation and protective system to nurses to avoid turnover intention in COVID-19 outbreak.

Effects on Unplanned Extubation, Oral Mucosa, and Facial Skin Integrity of New Method to Secure Endotracheal Tube (새로운 기관튜브 고정기구 적용이 비계획적 발관과 구강·안면 피부 통합성에 미치는 효과)

  • Purpose: Examination was done of the effects on unplanned extubation, oral mucosa, and facial skin integrity in patients with oral intubation secured with a new method using Multifix Endo II. Methods: A prospective quasai-experimental design was used. Data were collected during a 3-week EICU admission period between Jun. 1 and Nov. 30. Two hundreds seventeen (conventional taping group: 105, Multifix Endo II group: 112) orally intubated adults were enrolled in the study at the EICU of the C University Hospital. Results: The incidence of unplanned extubation (${\chi}^2=11.580$, p=0.03) and scores for the facial skin integrity impairment of the Mutifix Endo II group (t=5.28, p=.000) were significantly lower than those of the conventional taping group even though the scores for oral mucosa impairment were not different. The nurse's evaluation of the clinical effectiveness of the two methods including convenience, comfort, safety, and satisfaction were also higher for the new securement method compared to the conventional taping group. Conclusion: The securement method using Mutifix Endo II was more effective than the conventional method in the prevention of unplanned extubation and maintenance facial skin integrity.

  • Decision Support Systems (DSS) is an Information Systems (IS) application that aids in decision-making processes for many business concepts and Customer Relationship Management (CRM) is one of them and it depends on the firm's tasks for developing and retaining customers while achieving their satisfaction and enhancing the sense of belongingness for their products and services. Profit maximization, the process of customer value, and building strategic values for the firm are the three empirical benefits of CRM that are achieved through analytical, operational, and direction (AOD) capabilities respectively. This research focuses on the application of DSS models of what-if analysis (WIA) for CRM at (AOD) and also shows the dependence on the Information Success model (ISM). Hypothetical data are analyzed for (AOD) by three types of (WIA) to attain CRM and profit maximization and this analytical method can be used by any customer-oriented firm as a general model and for the purpose of the study we have compared the CRM between patients and hospital management.

  • The purpose of this study was to investigate the effects on skin temperature, pain, muscle tone, and ROM after applying the massage robot "PIRO-ZERO" to 6 men and 4 women in their 20s who complained of non-specific pain in the shoulder or back for more than 12 weeks. As a result of the study, there was a significant increase in skin temperature not only in the area where the massage was applied, but also on the opposite side and throughout the body due to increased blood flow. Pain in the upper trapezius, rhomboid, and erector spinae muscle was decreased, and muscle tone in the erector spinae muscles was significantly decreased. There was a significant increase in ROM of neck and trunk flexion, which is thought to be because the massage reduced muscle tone around the spine, increasing flexibility. In the future, as the safety and effectiveness of massage robots are further verified and the pressure, speed, and path become more diverse, satisfaction is expected to increase.

  • AI or Artificial Intelligence has been a significant tool used in the organisational backgrounds for an effective improvement in the management methods. The processing of the information and the analysis of the data for the further achievement of heightened efficiency can be performed by AI through its data analytics measures. In the medical field, AI has been integrated for an improvement within the management of the medical services and to note a rise in the levels of customer satisfaction. With the benefits of reasoning and problem solving, AI has been able to initiate a range of benefits for both the consumers and the medical personnel. The main benefits which have been noted in the integration of AI would be integrated into the study. The issues which are noted with the integrated AI usage for the medical sector would also be identified in the study. Medical Image Processing has been seen to integrate 3D image datasets with the medical industry, in terms of Computed Tomography (CT) or Magnetic Resonance Imaging (MRI). The usage of such medical devices have occurred in the diagnosis of the patients, the development of guidance towards medical intervention and an overall increase in the medical efficiency. The study would focus on such different tools, adhered with AI for increased medical improvement.

Silicone radial head prostheses revisited: do they have a role in today's practice? A systematic review of literature on clinical outcomes

  • Background: Silicone radial head prostheses (SRHP) are considered obsolete due to reports of frequent failure and destructive silicone-induced synovitis. Considering the good outcomes of modern non-radial silicone joint implants, the extent of scientific evidence for this negative view is unclear. The aim of this research was to systematically analyze the clinical evidence on complications and outcomes of SRHP and how SRHP compare to both non-SRHP and silicone prostheses of other joints. Methods: A systematic literature review was conducted through the Cochrane, PubMed, and Embase databases. Results: Eight cohort studies were included, consisting of 142 patients and follow-up periods ranging from 23 months to 8 years. Average patient satisfaction was 86%, range of 71%-100%, and 58 complications were seen, but no cases of synovitis. These outcomes were in line with non-SRHP. Four case series with 11 cases of synovitis were found, all due to implant fractures years to decades after implantation. Six systematic reviews of currently used non-radial silicone joint implants showed excellent outcomes with low complication rates. Conclusions: Since SRHP have satisfactory clinical results and an acceptable complication rate when selecting a patient group in suitable condition for surgical indications, it is considered that SRHP can still be chosen as a potential surgical treatment method in current clinical practice.

IMMUNIES, a unique polyherbal extract, exhibits antiproliferative activity and improves tumor-bearing canine patients: a pilot study

  • Dog owners seek treatment when their pets develop cancer. IMMUNIES is traditional herbal medicine-based figment made of 10 natural herbs, designed to maintain host immune function. The major component of IMMUNIES is Dendropanax morbiferus. This clinical pilot study monitored the toxicity and efficacy of IMMUNIES. Four senile dogs with spontaneously occurring mammary and liver cancers were enrolled in this study and treated orally daily for 3 months, and their blood/urine biochemical profiles were examined each month. IMMUNIES was well tolerated during the treatment period. Blood urea nitrogen, creatinine, alanine aminotransferase, alkaline phosphatase, and C-reactive protein levels decreased in all four dogs, whereas red blood cells and hematocrit were within the normal range. IMMUNIES also changed the expression of several molecular targets in the anticancer pathway, such as pro-NAG-1, p53, and cyclin D1. Although the tumors did not completely respond to IMMUNIES, the biochemical profiles and clinical examination showed a stabilized cancer status for 3 months. Thus, IMMUNIES was found to be safe and well-tolerated in the dosage range tested and exhibited cancer antiproliferative activity in canine cancer. Future studies should address other potential benefits of IMMUNIES, including correlative assessments of immune function, quality of life, and owner satisfaction.
